Going Beyond Counting First Authors in Author Co-citation Analysis
The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation
counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings
are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that
only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into
account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed
Dispelling the Myths Behind First-author Citation Counts
We conducted a full-scale evaluative citation analysis study of scholars in the XML research field to explore just how different from each other author rankings resulting from different citation counting methods actually are, and to demonstrate the capability of emerging data and tools on the Web in supporting more realistic citation counting methods. Our results contest some common arguments for the continued
use of first-author citation counts in the evaluation of scholars, such as high correlations between author rankings by first-author citation counts and other citation
counting methods, and high costs of using more realistic citation counting methods that are not well-supported by the ISI databases. It is argued that increasingly available digital full text research papers make it possible for citation analysis studies to go beyond what the ISI databases have directly supported and to employ more

Life expectancy in patients treated for osteoporosis: observational cohort study using national Danish prescription data
Osteoporosis is a chronic disease, carrying an elevated risk of fractures, morbidity and death. Long term treatment may be required but the long term risks with osteoporosis drugs remain incompletely understood. The competing risk of death may be a barrier to treating the oldest, yet this may not be rational if the risk of death is reduced by treatment. It is difficult to devise goal directed long term strategies for managing osteoporosis without firm information about residual lifetime expectancy in treated patients. We conducted an observational study in Danish national registries tracking prescriptions for osteoporosis drugs, comorbid conditions and deaths. We included 58,637 patients and 225,084 age- and gender matched control subjects. Information on deaths until the end of 2013 was retrieved, providing a follow-up period of 10–17 years.In men below age 80 and women below age 60, the relative risk of dying declined from being strongly increased in the first year to a stable but elevated level in subsequent years. In women older than 65–70 years of age there was only a small elevation in risk in the first year of treatment followed by lower than background mortality. The residual life expectancy of a 50-year old man beginning osteoporosis treatment was estimated to be 18.2 years and 7.5 years in a 75-year old man. Estimates in women were 26.4 years and 13.5 years.This study shows an excess mortality in men and in women below age 70 who are treated for osteoporosis, compared with the background population. This excess risk is more pronounced in the first few years on treatment. The average life expectancy of osteoporosis patients is in excess of fifteen years in women below the age of 75 and in men below the age of 60, highlighting the importance of developing tools for long term management
Early Permian Pangea 'B' to Late Permian Pangea 'A"
The pre-drift Wegenerian model of Pangea is almost universally accepted, but debate exists on its pre-Jurassic configuration since Ted Irving introduced Pangea 'B' by placing Gondwana farther to the east by ∼3000 km with respect to Laurasia on the basis of paleomagnetic data. New paleomagnetic data from radiometrically dated Early Permian volcanic rocks from parts of Adria that are tectonically coherent with Africa (Gondwana), integrated with published coeval data from Gondwana and Laurasia, again only from igneous rocks, fully support a Pangea 'B' configuration in the Early Permian. The use of paleomagnetic data strictly from igneous rocks excludes artifacts from sedimentary inclination error as a contributing explanation for Pangea 'B'. The ultimate option to reject Pangea 'B' is to abandon the geocentric axial dipole hypothesis by introducing a significant non-dipole (zonal octupole) component in the Late Paleozoic time-averaged geomagnetic field. We demonstrate, however, by using a dataset consisting entirely of paleomagnetic directions with low inclinations from sampling sites confined to one hemisphere from Gondwana as well as Laurasia that the effects of a zonal octupole field contribution would not explain away the paleomagnetic evidence for Pangea 'B' in the Early Permian. We therefore regard the paleomagnetic evidence for an Early Permian Pangea 'B' as robust. The transformation from Pangea 'B' to Pangea 'A' took place during the Permian because Late Permian paleomagnetic data allow a Pangea 'A' configuration. We therefore review geological evidence from the literature in support of an intra-Pangea dextral megashear system. The transformation occurred after the cooling of the Variscan mega-suture and lasted ∼20 Myr. In this interval, the Neotethys Ocean opened between India/Arabia and the Cimmerian microcontinents in the east, while widespread lithospheric wrenching and magmatism took place in the west around the Adriatic promontory. The general distribution of plate boundaries and resulting driving forces are qualitatively consistent with a right-lateral shear couple between Gondwana and Laurasia during the Permian. Transcurrent plate boundaries associated with the Pangea transformation reactivated Variscan shear zones and were subsequently exploited by the opening of western Neotethyan seaways in the Jurassic

Pragmatic Case Studies as a Source of Unity in Applied Psychology
To unify or not to unify applied psychology: that is the question. In this article we review pendulum swings in the historical efforts to answer this question—from a comprehensive, positivist, “top-down,” deductive yes between the 1930s and the early 60s, to a postmodern no since then. A rationale and proposal for a limited, “bottom-up,” inductive yes in applied psychology is then presented, employing a case-based paradigm that integrates both positivist and postmodern themes and components. This paradigm is labeled “pragmatic psychology” and, its specific use of case studies, the “Pragmatic Case Study Method” (“PCS Method”). We call for the creation of peer-reviewed journal-databases of pragmatic case studies as a foundational source of unifying applied knowledge in our discipline. As one example, the potential of the PCS Method for unifying different angles of theoretical regard is illustrated in an area of applied psychology, psychotherapy, via the case of Mrs. B. The article then turns to the broader historical and epistemological arguments for the unifying nature of the PCS Method in both applied and basic psychology.Peer reviewe
Utility of PINP to monitor osteoporosis treatment in primary care, the POSE study (PINP and Osteoporosis in Sheffield Evaluation)
Purpose: In Sheffield (UK), we introduced the PINP monitoring algorithm for the management of osteoporosis treatment delivered in primary care. Our aims were to evaluate whether this algorithm was associated with better osteoporosis outcomes and was cost-effective compared to standard care. Methods: Inclusion criteria were referral from Sheffield GPs, BMD scans performed between 2012 and 2013 and a report advising initiation of oral bisphosphonate and PINP monitoring. 906 patients were identified and retrospectively divided into Group A (intention to monitor, with baseline PINP, n = 588) and Group B (no intention to monitor, without baseline PINP, n = 318). The model described by Davis and colleagues was used to extrapolate life-time costs and quality-adjusted life-years (QALYs). Results: No differences were found in baseline characteristics between groups (age, gender, BMI, BMD and major risk factors for fractures). More patients in Group A started oral treatment (77.4% vs 49.1%; p < 0.001), but there were no differences between groups in the presence of a gap in treatment >3 months or in treatment duration. Patients in Group A were more likely to have follow-up DXA scan at 4–6 years from baseline (46.9% vs 29.2%; p < 0.000) and had a greater increase in total hip BMD (+2.74% vs + 0.42%; p value = 0.003). Fewer new fractures occurred in Group A but this was not statistically significant, but the numbers of fractures were small. Patients in Group A were more likely to change management (p = 0.005) including switching to zoledronate (p = 0.03). The PINP measurement and increased prescribing in Group A resulted in increases in both costs (£30.19) and QALYs (0.0039) relative to Group B, giving an incremental cost effectiveness ratio (ICER) of £7660 in the probabilistic sensitivity analysis. Conclusions: Patients monitored with PINP are more likely to start oral bisphosphonate treatment, switch to zoledronate, have follow-up DXA scans and a greater increase of hip BMD. PINP monitoring has the potential to be cost-effective in a UK NHS setting given that interventions with an ICER under £20,000 are generally considered to be cost-effective
