Factor structure and construct validity of the Adult Social Care Outcomes Toolkit for Carers (ASCOT-Carer)
Background: The ASCOT-Carer is a self-report instrument designed to measure social care-related quality of life (SCRQoL). This article presents the psychometric testing and validation of the ASCOT-Carer four response-level interview (INT4) in a sample of unpaid carers of adults who receive publicly-funded social care services in England.
Methods: Unpaid carers were identified through a survey of users of publicly-funded social care services in England. 387 carers completed a face-to-face or telephone interview. Data on variables hypothesised to be related to SCRQoL (for example, characteristics of the carer, cared-for person and care situation) and measures of carer experience, strain, health-related quality of life and overall QoL were collected. Relationships between these variables and overall SCRQoL score were evaluated through correlation, ANOVA and regression analysis to test the construct validity of the scale. Internal reliability was assessed using Cronbach’s alpha and feasibility by the number of missing responses.
Results: The construct validity was supported by statistically significant relationships between SCRQoL and scores on instruments of related constructs, as well as with characteristics of the carer and care recipient in univariate and multivariate analyses. A Cronbach’s alpha of 0.87 (7 items) indicates that the internal reliability of the instrument is satisfactory and a low number of missing responses (<1%) indicates a high level of acceptance.
Conclusions: The results provide evidence to support the construct validity, factor structure, internal reliability and feasibility of the ASCOT-Carer INT4 as an instrument for measuring social care-related quality of life of unpaid carers who care for adults with a variety of long-term conditions, disability or problems related to old age
Developing an Easy Read version of the Adult Social Care Outcomes Toolkit (ASCOT)
Background: This paper reports the experiences of developing and pre-testing an Easy Read version of the Adult Social Care Outcomes Toolkit (ASCOT) for self-report by people with intellectual disabilities. Method: The study has combined survey development and pre-testing methods with approaches to create accessible information for people with intellectual disabilities. A working group assisted researchers in identifying appropriate question formats, pictures and wording. Focus groups and cognitive interviews were conducted to test various iterations of the instrument. Results: Substantial changes were made to the questionnaire, which included changes to illustrations, the wording of question stems and response options. Conclusions: The process demonstrated the benefits of involving people with intellectual disabilities in the design and testing of data collection instruments. Adequately adapted questionnaires can be useful tools to collect information from people with intellectual disabilities in survey research; however its limitations must be recognised

The ASCOT trial was a breakthrough observation where the newer hypotensive drugs proved superior to the older generation drugs in reducing cardio-vascular complications. Large clinical trials results, despite their influence on everyday practice, do not answer the question on long term benefits of a certain therapeutic approach. This question was raised in ASOCT-Legacy trial where the authors searched the answer if the initially started therapy influences the long term prognosis and if discovered amlodipine/perindopril combination advantage will persist for the long time and result in reducing patients’ mortality. The mortality analysis in ASCOT-Legacy trial patients, after on average of 16 years of observation, indicated to 29% reduction in number of stroke deaths in newer drugs group. At the same time the patients not included in the LLA arm benefited with 21% reduction in cardiovascular deaths and 24% reduction in coronary disease mortality.Badanie ASCOT było przełomowym badaniem, w którym połączenie nowych leków hipotensyjnych skuteczniej niż terapia lekami starszej generacji chroniło pacjentów przed występowaniem powikłań sercowo- -naczyniowych. Wyniki dużych badań klinicznych, choć zmieniają codzienną praktykę lekarską, jednak nie przynoszą jednoznacznych danych dotyczących odległych korzyści ze stosowania określonych schematów postepowania. Takie zagadnienie podjęto w badaniu ASCOT-Legacy, w którym autorzy zadali sobie pytanie, czy wybór schematu terapeutycznego na początkowym etapie leczenia ma znaczenie dla ro- kowania odległego oraz czy w wieloletniej obserwacji utrzyma się przewaga korzystnego działania schema- tu amlodypina/peryndopryl w postaci zmniejszenia śmiertelności wśród pacjentów. Analiza śmiertelności pacjentów uczestniczących w pierwotnym badaniu ASCOT po średnio 16 latach obserwacji wykazała 29-procentową redukcję liczby zgonów z powodu uda- rów w grupie leczonej nowymi lekami hipotensyjnymi, natomiast wśród pacjentów niezakwalifikowanych do ramienia LLA — zmniejszenie liczby zgonów z przyczyn sercowo-naczyniowych o 21% oraz obniżenie o 24% śmiertelności z powodu choroby wieńcowej
Exploring the structural characteristics of the Adult Social Care Outcomes Toolkit (ASCOT) and ASCOT-Carer
Background: Measurement models inform the approach to assess a measure’s validity and also how a measure is understood, applied and interpreted. With preference-based measures, it is generally accepted that they are formative; however, if they are applied without preferences, they may be reflective, formative or mixed. In this study, we sought to empirically test whether the reflective, formative or mixed measurement model best describes PBMs of social care-related quality of life (ASCOT, ASCOT-Carer). We also explored the network approach, as an alternative. Methods: ASCOT and ASCOT-Carer data were analyzed using confirmatory factor analysis and Multiple Indicators Multiple Causes models to test reflective, formative or mixed measurement models, respectively. Network analysis of partial correlations using the Gaussian graphical model was also conducted. Results: The results indicated that the reflective measurement model is the worst fit for ASCOT and ASCOT-Carer. The formative or mixed models may apply to ASCOT. The mixed model was the best fit for ASCOT-Carer. The network analysis indicated that the most important or influential items were Occupation and Personal cleanliness and comfort (ASCOT) and Time and space and Self-care (ASCOTCarer). Conclusions: The ASCOT and ASCOT-Carer are best described as formative/mixed or mixed models, respectively. These findings may guide the approach to the validation of cross-culturally adapted and translated versions. Specifically, we recommend that EFA be applied to establish structural characteristics, especially if the measure will be applied as a PBM and as a measure of SCRQoL. Network analysis may also provide further useful insights into structural characteristics

El nom de Jomí García Ascot porta temps ressonant en articles, ponències i congressos que tenen com a finalitat aquesta tasa de rescat i restauració literària. La present tesi proposa un anàlisi de l'obra poètica de l'autor a partir de la revisió dels seus poemes. S'examina l'estil, la temàtica i es tracen línies d'intertextualitad amb altres autors com Baudelaire i Emilio Prados. Així mateix, es dialoga amb altres escrits de García Ascot com argument de les valoracions a propòsit de la seva poesia.Paulatinamente, están proliferando los estudios de algunos de los escritores en lengua hispana que condensaron su creación literaria en países extranjeros dada su condición de exilio. Los llamados "niños de la guerra" empiezan a resurgir con interés en el panorama de la crítica literaria. The so-called "children of war" begin to re-emerge with interest in the landscape of literary criticism. The name of Jomí García Ascot has long resonated in articles, papers and congresses that aim at this task of rescue and literary restoration. This thesis proposes an analysis of the poetic work of the author from the revision of his poems. The style, the theme and lines of intertextuality are drawn with other authors such as Baudelaire and Emilio Prados.
