    Abstract Psoriasis (PsO) is a phenotypically heterogeneous, immune-mediated skin condition with a relapsing and remitting course, that affects approximately 2% of the general population with equal sex incidence. About 30% of PsO patients are also affected by psoriatic arthritis PsA. Several psoriatic skin presentations have been associated with an higher probability of present PsA. Clinical predictors for developing arthritis will be comprehensively reviewed in this article to evaluate if there are specific skin phenotypes associated with PsA

    Water Vapour Adsorption Measurements on Metal Surfaces

    A method of measuring the water vapour desorbed/adsorbed on electro-polished stainless steel (EP-SS) surface at room temperature is discussed. It relies on the measurement of the outgassing rate, Q, from the metal surface which is supposed to obey a power law of the form Q = Q10 t-. From the outgassing model the amount of desorbed and adsorbed water vapour was estimated in terms of water monolayers. The investigation was performed at different exposure conditions, i.e., at water vapour pressures from about 1 Pa to 133 Pa and exposure times from about 60 s to 6000 s, on baked AISI 316L EP-SS tubing of the same type to be used in a moisture generator. As expected, the amount of adsorbed water vapour is proportional to both the initial vapour pressure and the exposure time. The paper discusses the outgassing model, gives details of the experimental procedures and apparatus and reports about the experimental results

    Psoriasis is histologically characterized by hyperkeratosis and papillomatosis with elongated vessels in the upper dermis. In order to evaluate the role of gelatinases in remodelling psoriatic skin in this study we examined the production of the 72-kDa (gelatinase A), 92-kDa collagenase (gelatinase B) and their tissue inhibitors TIMP-2 and TIMP-1. A total of 19 patients affected by different types of psoriasis were included in this study. An immunohistochemical study on cryosections was performed using antibodies to 72-kDa gelatinase, 92-kDa gelatinase, TIMP-1, TIMP-2, laminin, collagen types I, III, IV, VII. mRNA expression for gelatinases and their inhibitors were also analyzed by reverse transcriptase polymerase chain reaction (RT-PCR). In 14 of 19 patients there was a positivity in 92-kDa protein expression in keratinocytes. The 92-kDa gelatinase protein was also present in the upper dermis with prevalence around blood vessels. In 15 of 19 patients the 72-kDa was localized in the upper dermis, almost exclusively in the papillary dermis but absent in epidermis. TIMP-1 and TIMP-2 were both negative in all cases in immunoperoxidase and RT-PCR. Using RT-PCR we show that the 72-kDa mRNA is expressed exclusively in the dermis, on the contrary the 92-kDa was present in epidermis and dermis. Type I, III, IV and VII collagens did not show any alteration or disruption. Overexpression and production of gelatinases without inhibitory effects suggest a role of these proteins in remodelling the psoriatic skin probably inducing the typical histological pattern of papillomatosis

    Going Beyond Counting First Authors in Author Co-citation Analysis

    The present study examines one of the fundamental aspects of author co-citation analysis (ACA) - the way co-citation counts are defined. Co-citation counting provides the data on which all subsequent statistical analyses and mappings are based, and we compare ACA results based on two different types of co-citation counting - the traditional type that only counts the first one among a cited work's authors on the one hand and a non-traditional type that takes into account the first 5 authors of a cited work on the other hand. Results indicate that the picture produced through this non-traditional author co-citation counting contains more coherent author groups and is therefore considerably clearer. However, this picture represents fewer specialties in the research field being studied than that produced through the traditional first-author co-citation counting when the same number of top-ranked authors is selected and analyzed. Reasons for these effects are discussed

    Variations on the Author

    “Variations on the Author” discusses two of Eduardo Coutinho’s recent films (Um Dia na Vida, from 2010, and Últimas Conversas, posthumously released in 2015) and their contribution to the general question of documentary authorship. The director’s filmography is characterized by a consistent yet self-effacing form of authorial self-inscription: Coutinho often features as an interviewer that rather than express opinions propels discourses; an interviewer that is good at listening. This mode of self-inscription characterizes him as an author who is not expressive but who is nonetheless markedly present on the screen. In Um Dia na Vida, however, Coutinho is completely absent form the image, while Últimas Conversas, on the contrary, includes a confessional prologue that moves the director from the margins to the center of his films. This article examines the ways in which these works stand out in the filmography of a director who offers new insights into the notion of cinematic authorship

    Case series of two different dose regimen Rituximab therapy for severe Pemphigus

    Rituximab is a monoclonal antibody approved for treatment of adults with severe and refractory pemphigus vulgaris. Concerns about side effects and high costs of conventional doses have rised the hypothesis that low-dose rituximab regimen may be cost-effective with a better safety profile. Here we report our experience of seven patients with extensive/recalcitrant pemphigus, who either were steroid dependent, had contraindications or refused conventional treatment. Two patients received conventional rituximab (1000 mg 2 weeks apart) while five received ultra low-dose rituximab (200 mg 2 weeks apart). At 3 months, the two patients treated with high Rituximab regimen showed respectively a complete remission off therapy (CROT) and a complete remission on minimal therapy (CRMT), while among the five patients treated with ultra low-doses, three achieved CROT, one achieved CRMT and one a partial remission off therapy (PROT). All patients treated with ultra low-dose rituximab achieved complete depletion of cd19+ and cd19/45+ B lymphocytes after three months and all patients except one male manteined the zeroing after 6 months. No serious side effect was documented with low dose regimen except for a case of diziness. Our data suggest that ultra low-dose rituximab can be effective even in patients with extensive/recalcitrant pemphigus, with a lower probability of side-effects respect to higher dose regimen and may act as a steroid sparing strategy.
