    What are South Asian parents' experiences of their child's Autistic Spectrum Disorder diagnosis process

    Aim: A wealth of literature has contributed to our understanding of Autistic Spectrum Disorder (ASD). However most of this research has been carried out with white families and there is a paucity of research documenting ethnic minority families' experiences of ASD. The current study attempted to address this limitation in the research. The aim of the study was to explore South Asian parents' experiences of their child's Autistic Spectrum Disorder (ASD) diagnosis process. Method: Interpretative Phenomenological Analysis was used to analyse data from interviews with 8 parents (5 families) who had a child diagnosed with ASD. Parents were recruited from schools, child and adolescent mental health services, local authority child services, mosques and support groups. Results: Four super-ordinate themes were developed from the analysis. These included the early search for support, meeting the family: the extended family's rejection of ASD, a tug of war: finding a safe person and place in services and the struggle to maintain cultural links. Conclusions: The findings suggest that in addition to common challenges reported by all families during their child's ASD diagnosis process, South Asian families face unique challenges that arise due to specific cultural factors (e.g. language, cultural and religious beliefs). One of the key findings of the research is the loss of culture South Asian parents experience as a result of the ASD diagnosis.     An investigation into the effects of the initial hours of therapy in psychological services

    The systematic review explored the effect of the initial hours of psychological therapy; a critical phase of therapy, which some studies have suggested sets the course of therapy as a whole. The 24 studies included in the review were selected because of their relevance to outpatient adult mental health services. Methodological concerns regarding the nature of the sample, diversity of outcome measures, interventions and timing of follow-up measurement are discussed. Overall, the review suggests that a significant proportion of patients report benefits, including symptom change, following initial hours of therapy and this is especially clear when compared to no-treatment controls. Furthermore, these positive effects appear to be maintained at follow-up. These conclusions are tempered by the methodological concerns outlined in the review and a recommendation is made for more robustly designed research, which could confirm the tentative conclusions of the current review. An empirical study explored whether patients attending secondary care psychology services made early gains following an extended initial 'trial therapy' session of Experiential Dynamic Therapy and what mechanisms of change facilitated these gains. Video-recordings of 31 initial sessions were rated using the Achievement of Therapeutic Objectives Scale, a process scale that assesses patients' degree of absorption and assimilation of therapeutic objectives. Outcome was assessed through comparison of pre- and post scores using the Clinical Outcome Routine Evaluation-Outcome Measure, Brief Symptom Inventory, Inventory of Interpersonal Problems, Self- Compassion Questionnaire and Remoralisation Scale. The patients achieved an initial level of therapeutic objectives, in particular insight into and motivation to relinquish their maladaptive patterns and made significant positive changes in symptoms, remoralisation and self-compassion. Ratings of some achievement of therapeutic objectives were associated with improvement on the outcome measures. The results highlight the potential clinical benefits of extended experiential dynamic therapy sessions in the initial phase of treatment in secondary care NHS psychological therapy services.EThOS - Electronic Theses Online ServiceGBUnited Kingdo

    In March 2011, the Fukushima Daiichi Nuclear Power Station Accident occurred in Japan. After the accident, parts of information on the accident has been distributed as grey literature and they cause some issues in terms of permanent accessibility to the information. This paper introduces the activity of the Japan Atomic Energy Agency Library as an example of efforts to improve access of Internet information using the DSpace.     Il est assez simple de reconnaître les principales espèces de félins sauvages en les regardant évoluer dans leur environnement. Cependant, il peut s'avérer plus difficile d'identifier l'espèce de l'individu étudié sans une observation directe. La liste des indices retrouvés sur le terrain peut parfois être trop mince pour conclure à la nature de l'animal recherché. Le but de ce travail est donc de fournir aux vétérinaires et aux biologistes un outil de travail permettant de tenter d'établir la diagnose des espèces félines rencontrées fréquemment en parcs zoologiques. Ce manuscrit reprend leurs techniques de chasse spécifiques, leurs principales caractéristiques morphologiques, avant de présenter de manière plus approfondie les détails macroscopiques de leur pelage à l'aide d'examens simples.It is quite easy to recognize the main wild cat species by watching them in their environment. However, it could be more difficult to identify the species of an individual without direct observation. The clues found on the field may be too poor to conclude with the kind of animal that is looked for. Therefore, the aim of this work is to provide veterinarians and biologists with a tool that could allow to diagnose the feline species frequently found in zoological parks.     Peu d études traitent spécifiquement du temps d attente aux urgences. L objectif principal de l'étude a consisté en l'évaluation de la satisfaction du délai de prise en charge médicale.Patients et Méthodes293 patients des Urgences du CHU d Angers ont été inclus du 18 au 24 novembre 2013 après avoir répondu à un premier questionnaire avant de voir un médecin. Un deuxième questionnaire était complété par téléphone à J+7 de la sortie des urgences par le même Interne. 181 patients ont répondu au deuxième questionnaire. Le critère de jugement principal a été évalué sur la notation (de 0 à 10) de la satisfaction concernant le délai de prise en charge médicale.Résultats Les satisfactions moyennes concernant le délai de prise en charge médicale et la durée de prise en charge sont respectivement de 6,77 et 6,08/10. Les délais de prise en charge moyens envisagé, acceptable, réel et ressenti étaient respectivement de 63, 62, 74 et 86 minutes. 67 % des patients auraient mieux vécu leur attente si on leur avait annoncé le délai prévisionnel de prise en charge médicale dès l arrivée aux Urgences. Seulement 10 % des patients interrogés ont correctement estimé leurs délais de prise en charge médicale. Les examens biologiques, la réalisation de scanner ou la demande d'avis spécialisés allongent la durée de prise en charge de manière significative. Conclusion L obtention de l information concernant le délai de prise en charge médicale prévisionnel après évaluation par l infirmière d'accueil pourrait être un moyen d'améliorer la satisfaction des patients consultant aux urgences.Only few studies deal specifically with waiting time in emergencies departments. The main objective of the study consisted in evaluating the physician waiting time satisfaction. Patients and Methods We conducted this study from November 18th to 24th at Angers university hospital center. 293 Emergency department patients were included. They answered a first questionnaire from an Intern before seeing a doctor. The second questionnaire was completed by the same Intern on the phone seven days after the patient went out from the Emergency department. 181 patients answered the second questionnaire. The main assessment criterion was the rating (from 0 to 10) of the physician waiting time satisfaction Results The average satisfactions concerning the physician waiting time and total waiting time are respectively 6,77 and 6,08/10. The average physician waiting time imagined, acceptable, real and felt were respectively 63, 62, 74 and 86 minutes. Were they announced the projected waiting time at the moment of admission, 67 % of the patients would have had a better experience of their hospitalization. Only 10 % of the patients estimated correctly their physician waiting time. The biological examinations, the realization of scanner or the specialized requests lengthen significantly the duration of Emergency total waiting time.     Étude des COV issus de la dégradation thermique et oxydative des matériaux polymères

    Les matériaux polymères sont aujourd'hui très présents dans notre environnement et deviennent irremplaçables pour de nombreuses applications : emballage, textile, mobilier,... La connaissance du cycle de vie de ces matériaux, de la production à leur destruction, devient importante pour nos sociétés. Par exemple ces matériaux peuvent émettre des Composés Organiques Volatils qui sont souvent toxiques et leur impact sur le milieu doit être évalué. L objectif de ce travail de thèse a été de développer une méthode au laboratoire pour identifier et quantifier ces émissions. La difficulté provient de la grande diversité des composés qui peuvent être émis. Par ailleurs la méthode nécessite une fréquence de mesure élevée (temps réel) souvent incompatible avec les techniques actuelles. Une solution analytique couplant un four et un spectromètre de masse haute résolution associé à une méthode d ionisation chimique contrôlée a été développée. Celle-ci est basée sur un spectromètre FT-ICR compact à bas champ magnétique : BTrap. Les points forts de cette technique sont la très haute résolution en masse qui permet de mesurer la masse exacte des composés et la détection multi composés, simultanée sur toute la gamme de masse. L ionisation chimique contrôlée permet l'ionisation douce et quantitative des molécules d intérêt. Le transfert de proton à partir de l ion H O (PTRMS) a montré son potentiel pour la détection des COV dans de nombreux domaines. Après une présentation du contexte et du besoin, le dispositif expérimental développé est détaillé. La validation de celui-ci pour l'analyse de gaz traces a été effectuée au LPGP, sur un système de dépollution par plasma froid. Les résultats de dégradation de l'acétaldéhyde en fonction des conditions de fonctionnement du réacteur sont présentés.L ionisation par PTR conduit habituellement à la molécule protonée ce qui simplifie l identification. Cependant des fragmentations peuvent être observées. L utilisation d un précurseur d ionisation chimique plus lourd et moins réactif que H O pourrait minimiser ces phénomènes. L utilisation du diflurobenzène protoné a été testée et comparée à celle de l ion H O . Pour cela, des études cinétiques ont été menées sur une série d alcools connus pour fragmenter avec H O , et confirment l'intérêt du nouveau précurseur.Le poly(méthacrylate de méthyle) (PMMA) est un matériau très répandu : plexiglass. Sa dégradation est a priori relativement simple puisqu il s agit très majoritairement d une dépolymérisation, accompagnée de la formation de produits minoritaires. En conséquence, son étude nous a paru intéressante pour la mise au point et la validation de la méthode. La dégradation du PMMA a été étudiée sous atmosphère inerte, puis oxydante. Les résultats obtenus sont présentés : produits émis, bilan en masse, et apport du suivi en temps réel de la dégradation, montrant en particulier que les produits minoritaires sont émis après le monomère.Enfin une dernière partie de conclusion présente les perspectives pour cette nouvelle méthode.Polymeric materials are now ubiquitous in our environment and become irreplaceable for many applications such as packaging, textile or furniture. Knowledge of the life cycle of these materials from production to destruction becomes important for our societies. For instance, plastics may release Volatile Organic Compounds: VOCs are often toxic and their impact on the environment must be evaluated. The aim of this thesis work is the development of a laboratory method for identification and quantification of these emissions. The difficulty arises from the wide variety of compounds being potentially emitted. Moreover the desired method requires a high measurement rate ("real time") often incompatible with existing techniques. An analytical device coupling a furnace and a high-resolution mass spectrometer associated with a controlled chemical ionization method has been developed. This device is based on a low magnetic field FT-ICR compact mass spectrometer. The strengths of this technique are (i) high mass resolution allowing exact mass measurements, (ii) recording of the whole mass range simultaneously, allowing detection of a large variety of compounds. Controlled chemical ionization allows soft and quantitative ionization of molecules of interest. Proton transfer from H O ion (PTRMS) has shown its potentialities for VOC detection in many areas. After presenting the background and need, the developed experimental device is described in detail.The validation of this instrument for trace gas analysis has been performed at LPGP, using a nonthermal plasma depollution device. The results of acetaldehyde degradation according to operating conditions in the discharge reactor are presented.PTR ionization usually leads to the protonated molecule, so that identification is simplified. However fragmentations are sometimes observed. Use of a chemical ionization precursor ion heavier and less reactive than H O may minimize these drawbacks. Protonated difluorobenzene was selected as a precursor and its reactivity was tested and compared with H3O+ reactivity. In this purpose, kinetic studies were performed on a series of alcohols known for their fragmentation behavior with H O . The results confirm the interest of the new precursor.Polymethyl methacrylate (PMMA) is a widespread material: plexiglass. Its degradation process is relatively simple since it consists in predominant depolymerization, along with formation of minor products. Consequently, PMMA study appeared interesting for method development and validation. Thermal degradation of PMMA has been studied under inert, then oxidative atmosphere conditions. The presented results include identification of minor products and mass balance under different temperature conditions. Real-time monitoring of VOC emission showed time differences in emission peaks of MMA monomer and minor products, each of them being emitted slightly later than MMA.     Objectifs: Notre travail a eu 2 objectifs: Améliorer la formation du médecin généraliste sur les difficultés de réinsertion socio-professionnelle des patients jeunes de moins de 65 ans au décours du diagnostic de cancer de la prostate, puis fournir aux médecins généralistes un outil de repérage des patients à risque de difficultés de réintégration. Matériels et Méthodes: Notre point de départ est une étude quantitative descriptive réalisée dans l'Hérault en 2010. 444 patients de moins de 65 ans ont répondu à un auto-questionnaire abordant plusieurs thèmes décrivant leur réintégration socio-professionnelle à 2 ans du diagnostic de cancer de la prostate : séquelles, qualité de vie, relation au système de soins, retour au travail, vie de couple, présence de discriminations, évolution des ressources. Résultats : l'ensemble des réponses obtenues ont permis de mettre en évidence les facteurs de risque associés aux difficultés de réinsertion, puis de construire un indicateur permettant de repérer les hommes à risque de non-réintégration. Discussion: les données de la littérature ont permis d'approfondir l'analyse des difficultés socio-professionnelles de l'après-cancer mises en évidence par notre étude.     Financial modeling with Volterra Lévy processes and applications to options pricing, interest rates and credit risk modeling

    Ce travail étudie des modèles financiers pour les prix d'options, les taux d'intérêts et le risque de crédit, avec des processus stochastiques à mémoire et comportant des discontinuités. Ces modèles sont formulés en termes du mouvement Brownien fractionnaire, du processus de Lévy fractionnaire ou filtré (et doublement stochastique) et de leurs approximations par des semimartingales. Leur calcul stochastique est traité au sens de Malliavin, et des formules d'Itô sont déduites. Nous caractérisons les probabilités risque neutre en termes de ces processus pour des modèles d'évaluation d'options de type de Black-Scholes avec sauts. Nous étudions également des modèles de taux d'intérêts, en particulier les modèles de Vasicek, de Cox-Ingersoll-Ross et de Heath-Jarrow-Morton. These models are formulated in terms of the fractional Brownian motion, the fractional or filtered Lévy process (also doubly stochastic) and their approximations by semimartingales. Their stochastic calculus is treated in the sense of Malliavin and Itô formulas are derived. We characterize the risk-neutral probability measures in terms of these processes for options pricing models of Black-Scholes type with jumps. We also study models of interest rates, in particular the models of Vasicek, Cox-Ingersoll-Ross and Heath-Jarrow-Morton.
