    Professional Narrative Identities of Regional LGBTQ certifiers : Accountability and Resistance in Change Work

    This dissertation explores the professional narrative identity work of the emerging profession of regional LGBTQ certifiers tasked with changing heteronormative norms and values within healthcare organizations. It  focuses on how these change agents  understand, negotiate and justify themselves as certain kinds of professionals. Previous research has demonstrated how the professional identity work of change agents is shaped by dilemmas, paradoxes and contradictions (Carollo & Guerci, 2017). The dilemmas appear in change agents’ negotiations of societal and organizational norms, values and interests, and in their personal values and morals regarding change and how to challenge status quo within organizations (Blanchard, 2022).  As a continued exploration into these research areas, this dissertation builds on a focused ethnographic approach (Knoblauch, 2005) including interviews with regional LGBTQ certifiers and observations of these actors’ professional network meetings. The analysis draws on a narrative-as-practice approach (De Fina, 2020) to professional narrative identity, conceptualizing identity as emergent, situational and relational (Mishler, 1999) and focuses on identity positioning in narrative interaction (Bamberg, 2011).  The results demonstrate how the regional LGBTQ certifiers’ professional narrative identity work is constructed from an activist professional ethos drawing on master narratives of LGBTQ rights as a battle, and on heteronormativity and neoliberalism as a threat. The certifiers make these relevant in relation to healthcare organizations and professionals who are constructed as accountable both for the certification process and for committing to the promotion of  LGBTQ rights. The results show how the certifiers engage in various educational processes aimed to ensure the commitment of healthcare organizations and professionals to the political cause of promoting LGBTQ rights. This sheds light on a moral dimension of the certifiers’ professional narrative identity work which is conducted in relation to the dilemmas of accountability and resistance appearing in the certifiers’ narration of their professional practice. Moreover, the results show how the regional LGBTQ certifiers are constructing and co-constructing their professional narrative identities drawing on a norm critical discourse offering the certifiers guidance in their moral assessments of healthcare organizations and professionals as being in need of guidance and of each other as agents of change.

    Conceptualisations and analyses of distrust and mistrust in news media : Reviewing research from a decade of distrust

    This article provides a comprehensive review of research on distrust towards news media and journalism, a topic that has gained significant attention in recent years due to declining institutional trust and the rise of political disinformation. We analyse existing literature focusing on how media distrust has been studied and conceptualised, its explanations, and its consequences. We highlight the conceptual diversity and methodological plurality in the field, noting the varied definitions and operationalisations of distrust, as well as the dominance of quantitative approaches alongside significant qualitative contributions. The complex relationship between distrust in the media and democracy is also explored, revealing conflicting evidence on whether distrust leads to news avoidance and reduced political engagement or fosters critical media consumption and political engagement. The review underscores the need for clearer definitions and typologies of distrust, supplementing dichotomous operationalisations of trust/distrust with ones that capture distrust as a multidimensional phenomenon, with each dimension having different antecedents and consequences

    Imaging Mofette Structures in the Ohře Rift System, Czech Republic, Using Radio-Magnetotelluric Data

    The pathways of fluids and mantle-originated carbon dioxide in the seismically active Ohře (Eger) Rift system appearing as mofettes at the surface are currently subject to investigation, especially by the International Continental Scientific Drilling Program “Drilling the Eger Rift”. If the aquifers show significant contrast in electrical resistivity to the host rocks, they can be investigated with geo-electromagnetic methods. However, imaging complex fluid and CO2 pathways in detail in near-surface structures is challenging, because, in contrast to the background stratigraphy, they are often oriented in near-vertical directions. Therefore, we aim to investigate how the shallow aquifer structures can be examined best with an inductive electromagnetic method. For this purpose, we collected radio-magnetotelluric data in the Hartoušov mofette field and evaluated them by two- and three-dimensional inversions. Data from a nearby magnetotelluric station, drill hole data, gas flux measurements and electrical resistivity tomography models were used to assess the reliability and robustness of our inversion results. We concluded that the near-surface fluid reservoirs are adequately depictable, while the migration paths of gaseous CO2 cannot be traced properly due to a lack of resistivity contrast. Our model analyses suggest that imaging the given geological setting with fluids and gases ascending in anastomosing pathways benefits from a fine-scale three-dimensional inversion approach because the fluids mostly appear as local conductive reservoir-like anomalies, which can be falsely projected onto the profiles during inversion in two dimensions. The resistivity models contribute with detailed images of the near-surface aquifers to the geodynamic model of the Ohře Rift

    Hur kvinnor upplever sin kroppsuppfattning efter en hysterektomi : En litteraturöversikt

    Introduktion: Under en hysterektomi avlägsnas hela eller delar av livmodern. Operationen kan leda till fysiska besvär, ändrad kroppsuppfattning och emotionell påverkan. En bredare förståelse för dessa patienter kan göra det lättare för vården att identifiera omvårdnadsbehoven samt hjälpa kvinnor att hantera omvårdnadsbehoven kring den förändrade kroppsuppfattningen efter en hysterektomi.   Syfte: Syftet var att beskriva kvinnors upplevelse av en förändrad kroppsuppfattning efter en hysterektomi.   Metod: Studien har en beskrivande design med litteraturöversikt som datainsamlingsmetod. Databaser (PubMed, CINAHL och PsycInfo) har använts för datainsamling. Tio originalartiklar användes och dessa granskades med hjälp av bedömningsmallen för “bedömning av studier med kvalitativ metodik”, framtagen av Statens beredning för medicinsk och social utvärdering.   Resultat: Utifrån de tio valda artiklarna identifierades två huvudkategorier vilka var Förändrad kroppsuppfattning i relation till kvinnlig identitet och Sexuella hälsans påverkan på kroppsuppfattningen. Kvinnorna i studien talade om förlusten av ett organ som definierade deras kvinnlighet, vilket efteråt påverkade hur de upplevde sin kropp. Infertiliteten efter hysterektomin skapade även en osäkerhet hos många kvinnor som därför värderade sin kropp sämre. Partnern kunde också påverka kroppsuppfattningen beroende på hur de bemötte kvinnorna efter operationen och de flesta upplevde relaterat till detta en förändrad sexuell hälsa.   Slutsats: Majoriteten av kvinnor som genomgått en hysterektomi upplevde förändringar gällande kvinnligheten, den sexuella hälsan samt stödet från partnern. Detta kunde leda till nedvärdering av sig själva och deras kroppar.Introduction: During a hysterectomy, the whole uterus or parts of it will be removed. The procedure could cause both physical and emotional troubles but also a changed body image. A bigger understanding for these patients could improve the way health care identifies their healthcare needs and help them handle their changed body image after a hysterectomy.   Aim: The aim was to describe how women experience their body image after a hysterectomy.   Method: The study has a descriptive design with literature review as the method for collecting data. Databases (PubMed, CINAHL and PsycInfo) have been used for data collection. Ten original articles were used and assessed with the template “assessment of studies with qualitative methodology” produced by Swedish Agency for Health Technology Assessment and Assessment of Social Services.   Results: Based on the ten chosen articles two main categories were identified; Change in body image in relation to female identity and The impact of sexual health on body image. The women in the studies described losing an organ that defined their femininity, and that had an impact on their body image. The infertility following the hysterectomy led to insecurity amongst some women, which in turn made them devalue their bodies. A partner could affect the women's body image depending on how they treated her after the surgery, and a lot of women experienced deteriorated sexual health because of it.   Conclusion: The majority of women who had undergone hysterectomy noticed a change in femininity, sexual health and support from their partner. This could lead to women devaluing themselves and their bodies

    Initial Validation of the Self Experiences Questionnaire-2 in People with Chronic Pain

    Purpose: The struggle with sense of self has a fundamental impact on health and wellbeing in people with chronic pain. Acceptance and Commitment Therapy (ACT) particularly includes a process related to sense of self, called self-as-context (SAC). A measure of SAC, called the Self Experiences Questionnaire-8 (SEQ-8) has been developed in people with chronic pain. A brief version of the SEQ can facilitate more efficient assessment in clinical settings, and further research using intensive assessment to produce situation and time-sensitive information on SAC. The aim of the study was to establish a two-item version of the SEQ. Methods: This study used secondary analyses of data from 453 people with chronic pain. Results: The newly validated SEQ-2 demonstrated acceptable reliability, Cronbach's α=0.77, and performed well in assessing SAC, with 88.6% of the variance in the SEQ-8 explained by the SEQ-2. The construct validity of the SEQ-2 was supported by its associations with other theoretically-specified processes, r=.25-0.51, p <.001. and measures of functioning, |r|=0.18-0.40, p <.001. The incremental validity of the SEQ-2 was relatively limited, |β|=0.08-0.34. Conclusions: The SEQ-2 appears to be an acceptable measure of SAC. The brief version of the SEQ can facilitate the application of individualised research methods and personalised pain treatments, informed by intensive assessment

    Configuration, Performance, and Commissioning of the ATLAS b-jet Triggers for the 2022 and 2023 LHC data-taking periods

    In 2022 and 2023, the Large Hadron Collider produced approximately two billion hadronic interactions each second from bunches of protons that collide at a rate of 40 MHz. The ATLAS trigger system is used to reduce this rate to a few kHz for recording. Selections based on hadronic jets, their energy, and event topology reduce the rate to O(10) kHz while maintaining high efficiencies for important signatures resulting in b-quarks, but to reach the desired recording rate of hundreds of Hz, additional real-time selections based on the identification of jets containing b-hadrons (b-jets) are employed to achieve low thresholds on the jet transverse momentum at the High-Level Trigger. The configuration, commissioning, and performance of the real-time ATLAS b-jet identification algorithms for the early LHC Run 3 collision data are presented. These recent developments provide substantial gains in signal efficiency for critical signatures; for the Standard Model production of Higgs boson pairs, a 50% improvement in selection efficiency is observed in final states with four b-quarks or two b -quarks and two hadronically decaying.. -leptons.For complete list of authors see http://dx.doi.org/10.1088/1748-0221/20/03/P03002</p

    Search for +⁢−→⁢⁢0 and ⁢⁢2⁡(1⁢)) at center-of-mass energies from 4.47 to 4.95 GeV

    Utilizing a dataset of 6.7 fb(-1) from electron-positron collisions recorded by the BESIII detector at the BEPCII storage ring, a search is conducted for the processes e(+)e(-) -&gt; phi chi(c0) and phi eta(c2)(1D) across centerof-mass energies from 4.47 to 4.95 GeV. In the absence of any significant signals, upper limits are set. These include limits on the dressed cross sections for e(+)e(-) -&gt; phi chi(c0), as well as the product of the dressed cross section for e(+)e(-) -&gt; phi eta(c2)(1D) and a sum of five branching fractions. Furthermore, the product of the electronic width of Y(4660) and the branching fraction of the Y(4660) -&gt;phi chi(c0), denoted as Gamma Y-e+e-((4660)) B-Y(4660)-&gt;phi chi(c0), is determined to be &lt;0.35 eV at the 90% confidence level.For complete list of authors see http://dx.doi.org/10.1103/PhysRevD.111.012016</p

    Suspected de novo heart failure in outpatient care : the REVOLUTION HF study

    Background and Aims:  Ambulatory patients presenting with signs or symptoms of heart failure (HF) should undergo natriuretic peptide testing. Rates of death, HF hospitalization, and healthcare costs were examined in patients thus identified with suspected de novo HF. Methods: This population-based study (REVOLUTION HF) encompassing two large healthcare regions in Sweden examined patients who presented to outpatient care for the first time between 1 January 2015 and 31 December 2020, who had a recorded sign (peripheral oedema) or symptom (dyspnoea) of HF, and whose N-terminal pro-B-type natriuretic peptide (NT-proBNP) measured &gt;300 ng/L within +/- 30 days of that sign or symptom. Characteristics, outcomes, healthcare patterns, and healthcare costs for these patients were followed for 1 year. Comparisons were made with matched controls without history of HF, its signs, its symptoms, or elevated NT-proBNP. Results: Overall, 5942 patients (median age 78.7 years; 54% women) presented with suspected de novo HF. Within 1 year, 29% had received a HF diagnosis. Patients with suspected de novo HF had higher rates of all-cause death (11.7 vs. 6.5 events/100 person-years) and HF hospitalizations (12.5 vs. 2.2 events/100 person-years) than matched controls (n = 2048), with the highest event rates in the weeks after presentation. Rates were higher with higher NT-proBNP levels. Although some patients already used HF guideline-directed medical therapies for other indications, initiation of new medications was variable. Healthcare costs were higher in patients with suspected de novo HF than in matched controls, driven mostly by HF and chronic kidney disease. Conclusions: Patients with suspected HF and elevated NT-proBNP had high mortality and morbidity in the weeks after presentation, and accrued substantial healthcare costs, highlighting an urgent need for prompt identification, evaluation, and treatment of HF

    A sustainable organizational structure to integrate psycho-social stimulation programme into primary health care services in Bangladesh : protocol for a pragmatic cluster randomized controlled trial on scaling up early childhood development activities

    Background Early childhood development (ECD) in low to middle-income nations has been a pressing concern for the last two to three decades. It is estimated that approximately 250 million children under the age of five are not reaching their full developmental potential due to factors such as poverty, malnutrition, and insufficient home stimulation. It is estimated that in Bangladesh, 44% of the children under 5 years live in poverty and 31% are stunted, and both factors are risks for poor childhood development. We aim to develop a sustainable, scaled ECD programme using an evidence-based curriculum of psychosocial stimulation for disadvantaged Bangladeshi children. Methods Approximately 10,000 malnourished children aged 6-24 months will be identified by mid-upper arm circumference (MUAC) &lt; 13.5 cm, assessed by government health staff in over 500 community clinics (CCs) in 21 sub-districts of Hobiganj, Brahmanbaria, Laxmipur and Narsingdi districts. The children and their mothers will participate in the psychosocial stimulation programme that will be delivered fortnightly for one year to small groups at the CCs by health workers. We will train approximately 1,500 health staff in rural CCs to deliver the ECD sessions. We will follow a cascade training model to train personnel at all levels of the primary health care system. A subsample will be evaluated using a cluster randomized controlled trial in 48 CCs (24 intervention and 24 control). From each CC, 10 children aged 6-24 months will be tested before and after the programme. The primary outcome is children's cognitive development and we will also measure language composite scores on the Bayley Scales of Infant and Toddler Development. Secondary outcomes are children's growth, maternal knowledge and depressive symptoms, and stimulation in the home. Intention-to-treat analysis will be used to assess intervention effects. Discussion There are no existing ECD services for the children under three years delivered by the health care system in Bangladesh. This study will help policy makers to consider incorporating ECD activities into the treatment of malnourished children in rural CCs, under the government's primary health care system, thereby developing a sustainable structure for improving disadvantaged children's development. The findings will also provide information to facilitate scaling ECD in the primary health care systems in other low and middle-income countries.
