Type and efficacy of online training for informal carers: a narrative review
Background: The need to provide training and support for unpaid informal carers of people with various health conditions is well established. This narrative literature review was conducted to explore recent evidence of informal carer eLearning education and training programs, and to guide the design of an online training program for Australian carers, including those living in rural and geographically remote settings.Method: Different combinations of relevant search terms were used across three databases: Google Scholar Advanced, MEDLINE with full text incorporating PUBMED, and CINAHL to search for literature published since 2014. Articles within peer-reviewed journals were chosen based on their topic relevance and strength of evidence. Results: The recent systematic reviews highlight that there is a lack of good quality research evidence for the range and efficacy of eLearning programs designed for carers of veterans and adults with disabilities, or mental health conditions, with most evidence related to carers of older people and people with dementia.Conclusion: Online educational programs for carers of older people and/or older people with dementia, and those with mental health conditions can improve carer well-being. Future programs should be co-designed with carers, evaluate care recipient outcomes, and address recruitment, retention, and information technology skills.</p

Improving the Sustainability of Public Health Interventions: The INNOVATE Model
Abstract:-Introduction: Improving the sustainability of public health interventions is a priority for policymakers and program implementers. However, existing theories, models, and frameworks focus on describing constructs, processes, and measures of sustainability. Models to guide the improvement of sustainability of public health interventions are currently lacking.Objective: To develop an evidence-based model that can systematically guide the improvement of the sustainability of public health interventions.Methods: We used a systematic and multi-stage approach to iteratively develop the model. A narrative review of the sustainability literature identified key constructs relevant to the improvement mechanisms of sustainability. Identified constructs were then developed into stages and activities. We applied the resulting model to guide development of improvement strategies in four public health interventions in Southeast Asia, verifying the applicability of the model to real-life contexts.Findings: Evidence from 8 systematic reviews; 60 theories, models, and frameworks of sustainability; and 24 measures of sustainability, were used to develop the model. Expert review and real-world evaluation further refined the model. The resulting INNOVATE model has eight stages: Identify the mechanisms; Narrate determinants; Navigate strategies; Optimize the acquisition of resources; Validate strategies; Apply strategies; Tailor evaluation of the strategies; and Enhance the applied strategies and their implementation. Within each stage, there are five activities (IDEAL): Inform with quality evidence; Dedicate efforts to nurture local leaders/champions; Engage relevant stakeholders; Adapt to the local context; and Leverage tacit and local knowledge. The initial model was used to identify practical strategies for improving the sustainability of four public health interventions in Southeast Asia.Conclusion: By applying the five activities within each of the eight stages, the INNOVATE model can systematically guide the improvement of the sustainability of public health interventions.</p

Sensitive and modular amplicon sequencing of Plasmodium falciparum diversity and resistance for research and public health
Targeted amplicon sequencing is a powerful and efficient tool for interrogating the Plasmodium falciparum genome, generating actionable data from infections to complement traditional malaria epidemiology. For maximum impact, genomic tools should be multi-purpose, robust, sensitive, and reproducible. We developed, characterized, and implemented MAD4HatTeR, an amplicon sequencing panel based on Multiplex Amplicons for Drug, Diagnostic, Diversity, and Differentiation Haplotypes using Targeted Resequencing, along with a bioinformatic pipeline for data analysis. Additionally, we introduce an analytical approach to detect gene duplications and deletions from amplicon sequencing data. Laboratory control and field samples were used to demonstrate the panel’s high sensitivity and robustness. MAD4HatTeR targets 165 highly diverse loci, focusing on multiallelic microhaplotypes, key markers for drug and diagnostic resistance (including duplications and deletions), and CSP and potential vaccine targets. The panel can also detect non-falciparum Plasmodium species. MAD4HatTeR successfully generated data from low-parasite-density dried blood spot and mosquito midgut samples and detected minor alleles at within-sample allele frequencies as low as 1% with high specificity in high-parasite-density dried blood spot samples. Gene deletions and duplications were reliably detected in mono- and polyclonal controls. Data generated by MAD4HatTeR were highly reproducible across multiple laboratories. The successful implementation of MAD4HatTeR in five laboratories, including three in malaria-endemic African countries, showcases its feasibility and reproducibility in diverse settings. MAD4HatTeR is thus a powerful tool for research and a robust resource for malaria public health surveillance and control.</p
Support needs and preferences for people with multiple sclerosis: a rapid review protocol
INTRODUCTION: People with multiple sclerosis (MS) live with a progressive condition that can result in complex disability support needs. Consequently, they may require specialised care and assistive technology to maintain independence at home. It is essential to develop an understanding of the support needs and preferences of people with MS to maintain independence and enhance their quality of life. METHODS AND ANALYSIS: The principles of a systematic review framework are incorporated into our rapid review. As such, a reference group was recruited at the project's outset, shaping the research question and study design, followed by developing a thorough listing of search terms. We then adapted the methodology in line with recommendations for conducting a rapid review whereby only three databases were included: MEDLINE (Ovid), Embase (Ovid) and PsycINFO (Ovid). Data from qualitative and mixed-method studies will be extracted, coded and classified using content analysis. In addition to including peer-reviewed articles, we will examine grey literature sourced from MS-specific organisations and research centres. Grey literature content that has the potential to inform practice will be extracted and categorised. Collectively, themes will be identified from the analysed literature, with a subsequent evaluation conducted by our reference group. The inclusion criteria assert that all documentation must be published in English from 2010 onwards, specifically focusing on people with MS. ETHICS AND DISSEMINATION: This rapid review does not require ethics approval. Findings will be circulated to our reference group and then published in peer-reviewed journals or presented at conferences.</p
