Family members’ experiences with intensive care unit diaries
Background: The admission of a family member to intensive care represents an emotionally complex experience, often characterised by anxiety, stress and uncertainty. ICU diaries, compiled by nurses and family members, have been proposed as a useful tool to support caregivers’ psychological well-being, improve communication and humanise the care environment. The aim of the study was to describe the content of ICU diaries filled out by family members to explore the experiences and meanings attributed to the diaries. Methods: The study used a qualitative approach based on thematic analysis of diaries completed by 16 family members of patients admitted to intensive care units. The data were coded and analysed to identify recurrent themes and to understand the emotional and psychological experience of the family members. Results: Three main themes emerged from the analysis: time, the family context (including maintaining contact with the patient, the relatives‘emotions, fear of suffering, spirituality, the person at the centre of the relatives’ lives and connection with the outside world) and the usefulness of the diary in understanding the care process. The diaries facilitated the continuity of the affective bond with the patient, offered a space to express emotions and improved communication with healthcare professionals. Conclusion: The results highlight the value of ICU diaries in supporting family members during the patient’s admission, reducing stress and strengthening the relationship with the healthcare team. The practical implications suggest the importance of promoting the structured use of diaries to foster more empathetic and family-centred care. Clinical trial number: Not applicable
Identifying the location-dependent adipose tissue bacterial DNA signatures in obese patients that predict body weight loss
Recent sets of evidence have described profiles of 16S rDNA sequences in host tissues, notably in fat pads that are significantly overrepresented and can serve as signatures of metabolic disease. However, these recent and original observations need to be further detailed and functionally defined. Here, using state-of-the-art targeted DNA sequencing and discriminant predictive approaches, we describe, from the longitudinal FLORINASH cohort of patients who underwent bariatric surgery, visceral, and subcutaneous fat pad-specific bacterial 16SrRNA signatures. The corresponding Porphyromonadaceae, Campylobacteraceae, Prevotellaceae, Actimomycetaceae, Veillonellaceae, Anaerivoracaceae, Fusobacteriaceae, and the Clostridium family XI 16SrRNA DNA segment profiles are signatures of the subcutaneous adipose depot while Pseudomonadaceae and Micrococcacecae, 16SrRNA DNA sequence profiles characterize the visceral adipose depot. In addition, we have further identified that a specific pre-bariatric surgery adipose tissue bacterial DNA signature predicts the efficacy of body weight loss in obese patients 5–10 years after the surgery. 16SrRNA signatures discriminate (ROC ~ 1) the patients who did not maintain bodyweight loss and those who did. Second, from the 16SrRNA sequences we infer potential pathways suggestive of catabolic biochemical activities that could be signatures of subcutaneous adipose depots that predict body weight loss

Cardiometabolic Effects of Nigella sativa in Postmenopausal Women with Hypertension: A Prospective, Observational, Pilot Study
Background: Hypertension is a prevalent condition, impacting a significant amount of general population and contributing prominently to global mortality. Increasing attention has been directed towards phytotherapy products as potential complementary or alternative therapies for hypertension prevention and treatment. Among these, Nigella sativa (NS) has shown encouraging effects in improving cardiovascular parameters. This study aimed to evaluate the efficacy of NS supplementation in reducing seated office systolic blood pressure (BP) in postmenopausal women. We also explored the dose-dependent effects of this intervention on BP levels and metabolic parameters. Materials and Methods: We conducted an observational pilot study including 52 women, who were stratified into two active groups (n = 32) receiving two different dosages of NS (n = 16, age 54.2 ± 2.3 at 400 mg/day and n = 16, age 52.3 ± 2.4 at 800 mg/day) and a control group (n = 20, age 53.9 ± 3.0). Participants were evaluated at baseline (T0), at 4 (T1) and 8 weeks (T2) for office brachial and central BP, heart rate (HR), lipid profile, body weight, and menopausal symptoms. Results: NS supplementation significantly reduced office systolic and diastolic BP in a dose-dependent manner (p < 0.01), with more pronounced reductions at 800 mg/day. Improvements in climacteric symptoms and reduced HR were observed as early as T1, while metabolic parameters, including lipid profile and weight, showed significant changes at T2. Notably, the 800 mg/day dosage group also experienced significant reductions in weight and body mass index. Younger age, more recent menopausal transition, and elevated baseline HR were identified as predictors of a better response to treatment. Conclusions: NS supplementation demonstrates significant dose-dependent benefits in reducing office BP and improving metabolic parameters. These findings support the role of NS as an effective complementary therapy in hypertension management in postmenopausal women

Plasmonic Silver Nanoparticles Facilitate Electron Emission from Diamond upon Sun‐like Excitation
The development of a stable, non-toxic material that emits electrons following absorption of visible light may have a major impact on the solar photocatalysis of difficult reactions such as CO2 and N2 reduction, as well as for targeted chemical transformations in general. Diamond is a good candidate, however it is a wide bandgap material requiring deep UV photons (λ < 227 nm) to promote electrons from the valence band into the conduction band. Embedding silver nanoparticles under the diamond surface allows the photoconductivity of the diamond in the spectral region of the surface plasmon resonance to be increased, while also leading to an enhancement of visible light photoemission. Considering the low intensity of the light sources used in this work and the spectral properties of the enhanced photoconductivity and photoemission a mechanism based on plasmonically enhanced photoconductivity which in turn allows surface states emptied by photoemission to be recharged thus leading to enhanced photoemission in the visible range is proposed
Symptom perception in adults with chronic physical disease: A systematic review of insular impairments
Background: to perform self-care, individuals with a chronic illness must be able to perceive bodily changes (ie.,
interoception) so they can respond to symptoms when they arise. Interoception is regulated by the insular cortex
of the brain. symptom perception is poor in various physical diseases, which may be associated with impairments
in the insular cortex.
objective: the purpose of this study was to explore whether patterns of insular impairment exist among adults
with chronic physical diseases and to analyze the relationship with disease-related symptoms.
methods: we identified studies that assessed the structure and/or activity of the insula through MRI and/or (f)
MRI in adults with chronic physical diseases (vs. healthy controls) by searching five databases. results are re
ported as a narrative synthesis.
results: fifty studies were conducted to investigate the structure or activity of the insula among adults with
diabetes, cancer, heart failure, or chronic pulmonary disease. In 19 studies investigators found that patients with
a chronic disease had lower/damaged insular volume/density/thickness than healthy controls or reduced insular
blood flow. when insular activity was explored in 22 studies, most investigators reported higher insular activity
and lower neural connectivity. five studies explored the association between insular volume/activity and
symptom severity: four reported a positive trend.
conclusion: people with chronic physical diseases have lower insular grey matter volume/density/thickness and
abnormal insular activity when compared to healthy people. Insular activity may be related to symptom severity.
these results suggest that insular structure and/or activity may explain poor symptom perception
Adalimumab Monotherapy or Combination Therapy With Methotrexate in Paediatric Uveitis: Data From the AIDA Network Uveitis Registry
Background: The study objective was to compare the effectiveness of adalimumab (ADA) in monotherapy and in combination with methotrexate (MTX) for paediatric noninfectious uveitis (NIU). Methods: Registry-based observational study. Children receiving ADA for active uveitis were divided into the ADA monotherapy group (group 1) and the ADA plus MTX combination group (group 2). Results: Eighty four children were enrolled (146 eyes): 22 in group 1 (26.2%) and 62 in group 2 (73.8%). ADA effectiveness was complete in 48 children (57.1%), partial in 23 (27.4%) and absent in 4 (5.3%), without any differences across the groups (p = 0.89). Fewer relapses per 100 PY occurred after ADA treatment both in group 1 (280.0 vs. 23.0, p = 0.005) and in group 2 (297.9 vs. 86.0, p < 0.001). The final BCVA was similar between groups 1 and 2 [median 1.0 (IQR 0.3) and 1.0 (IQR 0.3), respectively, p = 0.55]. A statistically significant steroid-sparing effect was observed in the entire cohort and in group 2 at the 6-month (p = 0.01 and p = 0.01), 12-month (p = 0.02 and p = 0.02), and last follow-up (p = 0.045 and p = 0.045). The estimated ADA retention rate was 97.1% at 12 months, 87.7% at 24 months, and 82.6% at 36 months, without a statistically significant difference among the groups (p = 0.77). Conclusions: ADA monotherapy could be equally effective as its combination with MTX in both preventing uveitis relapses and preserving visual acuity in paediatric NIU, with comparable retention rates over 36 months of treatment. The steroid-sparing effect of ADA monotherapy warrants further extensive evaluation to define its optimal placement in the therapeutic strategy for paediatric NIU
