    Endocan (ESM-1) levels in gingival crevicular fluid correlate with ICAM-1 and LFA-1 in periodontitis

    Endocan, a 50 kDa soluble proteoglycan, also called endothelial cell-specific molecule-1 (ESM-1), is involved in many major cellular activities and has been reported to be overexpressed in inflammatory conditions. This study aims to determine ESM-1 levels in gingival crevicular fluid (GCF) samples from individuals with periodontitis to determine the correlation between the levels of lymphocyte-function-associated antigen-1 (LFA-1), intercellular adhesion molecule-1 (ICAM-1), and clinical findings of periodontitis. This study enrolled 27 individuals diagnosed with Stage III-Grade C generalized periodontitis and 16 individuals as healthy controls. Bleeding on probing (BOP), probing pocket depth (PPD), and clinical attachment loss (CAL) were calculated. Enzyme-linked immunosorbent assay (ELISA) test was used for detecting the levels of ESM-1, ICAM-1, and LFA-1 in GCF samples. PPD, BOP, CAL, and GCF volumes were significantly increased in patients with periodontitis in comparison to the control group (p < 0.001). The total amount of ESM-1, ICAM-1, and LFA-1 levels in GCF were increased in the periodontitis group (p < 0.001). ESM-1 level correlated with PPD, BOP, and CAL (p < 0.05). ICAM-1 level correlated with BOP and CAL (p < 0.05). LFA-1 level correlated with PPD and CAL (p < 0.05). Our data indicate that ESM-1, ICAM-1, and LFA-1 levels are increased in GCF of patients with periodontitis. These molecules could be associated with the pathogenesis and progression of periodontal disease.3

    An assessment of emergency department visits for cardiovascular diseases in the early phase of the COVID-19 pandemic in Istanbul

    Objectives: The coronavirus disease (COVID-19) pandemic has had a severe impact with an excess of one hundred million cases and over two million deaths worldwide. Countries have adopted different paths toward `flattening the curve'however, many countries observed a common trend of people avoiding emergency departments (ED) for varying reasons. This study aims to investigate the ED utilization patterns of patients with cardiovascular diseases (CVD) in the early phases of COVID-19 in Istanbul, Turkey. Methods: The data were retrospectively collected in eight hospitals, which were authorized and declared as pandemic hospitals by the Turkish Ministry of Health. The data for these two particular periods of 1st March-30th June were compared between 2019 and 2020. Results: A substantial and statistically significant decrease of 22.7\% was observed in the total number of visits to EDs for cases of CVD from 534 cases in 2019 to 413 cases in 2020. ED visits showed statistically insignificant decreases of 11.6\% for acute coronary syndrome/ACS) and 30.6\% for cerebrovascular events (CVE) between 2019 and 2020. Of the total 422 ACS cases, 67.8\% were male and 32.2\% were femaleand of the 525 CVE cases, 50.3\% were male and 49.7\% were female. The median ages were 61 {[}6-97] for men and 69 {[}19-103] for womenthe median ages were 59 {[}19-97] for patients with ACS and 68 {[}6-103] for patients with CVE. In 2020, the number of ACS cases showed a steady trend between March 1st and June 30th, followed by a prominent increase in June 2020. The monthly trends appeared to be very similar for CVE cases for 2019 and 2020. Of the 379 hospitalizations, 56.2\% were diagnosed with ACS and 43.8\% with CVE. Conclusions: The findings suggest that patients tended to avoid timely medical care during the pandemic compared to pre-COVID-19. Stay-at-home orders and attempts to avoid contact with the virus may explain the dramatic decrease in the number of ED visits.6NOV103-1111

    5-Hydroxyuracil Incision Activity Varies According to the Histological Grade of Non-muscle-invasive Bladder Cancer

    Objective: High levels of endonuclease III-like 1 (NTHL1) DNA glycosylase, which plays a role in the first step of the base excision repair pathway, has been related to cancer initiation and progression. 5-hydroxyuracil (5-OHU) oxidative base damage is a substrate for NTHL1 and endonuclease VIII-like 1 enzyme 1 (NEIL1) DNA glycosylases. This study investigates the association of 5-OHU incision activity with the risk of disease progression in patients with non-muscle-invasive bladder cancer (NMIBC) regarding grade and stage. Materials and Methods: During transurethral resection of 17 NMIBC patients, the papillary tumour before monopolar resection and healthy bladder mucosal tissue from the same person were obtained using cold cup biopsy. Both the normal mucosa and NMIBC tumour were pathologically confirmed. The histological grade and stage were also determined. The 5-OHU incision activity of all tissues was measured using a radiolabelled 5-OHU modified base containing DNA substrate. Results: 5-OHU incision activity was significantly higher in all high-grade NMIBC tissue extracts compared with the corresponding normal tissues (p=0.001). However, we found no significant difference in 5-OHU incision activity in low-grade NMIBC tissues (p=0.89). There was also a significant increase in 5-OHU incision activity at the Ta/T1 stage compared with the corresponding normal tissue (p=0.001). Conclusion: The increase in 5-OHU incision activity according to the histological grade of NMIBC tissue indicates that this activity (mainly performed by NTHL1 and NEIL1 DNA glycosylases) might play a role in NMIBC prognosis. Thus, it could be used as a potential prognostic biomarker for NMIBC.1MAR45-482

    Morcellation allows the removal of a large uterus and fibroids through small incisions with minimally invasive surgery. It helps to prevent the complications associated with large incisions in both hysterectomy and myomectomy operations. Currently, there is much debate regarding the use of power morcellation in laparoscopic hysterectomy and myomectomy, mainly due to the risk of peritoneal dissemination of undiagnosed uterine sarcomas. Unfortunately, there is no valid pre-operative diagnostic method that can differentiate sarcomas from myomas, and the currently available scientific literature regarding morcellation is insufficient. As the Turkish Society of Minimally Invasive Gynecological Oncology, we present our consensus opinion and suggestions for the preoperative evaluation and morcellation of fibroids, in line with the recent literature.1MAR53-572

    Endonasal Dacryocystorhinostomy; the learning curve and our experience

    ABSTRACT Aim: It was aimed to give an insight on the learning curve adopted by an ophthalmic surgeon while performing endonasal dacryocystorhinostomy under the supervision of an otolaryngologist with the results experienced during this survey. Materials and Methods: In this retrospective study, consecutive cases with endonasal dacryocystorhinostomy (DCR) surgery were performed by a single ophthalmic surgeon while a second otolaryngologist was overlooking the procedures. A total of 29 surgeries were performed on 23 patients. Patients that were diagnosed with complete nasolacrimal duct obstruction were then evaluated and proceeded to the surgery. The intraoperative video was recorded in all surgical cases and recording time was noted for each case. All of the complications that occurred during or at the postoperative stages were noted. Results: The mean age of the 23 patients was 55.25±18.6 years (15 y-80 y), of which 20 cases (87%) were female. The left side was involved in 69% (20/29) of cases. At the final follow-up of the mean of 12,4 months, the anatomical and functional success was achieved in 89% (26/29) cases. The mean time spent in the operating theatre was 84±17.2 minutes (range, 40–110 min). It was found that the only 3 failed surgeries were in the first 5 surgeries performed with no surgical failure in the remaining 24 eyes. Conclusions: Endonasal dacryocystorhinostomy is a safe, effective and cosmetically pleasing surgery for the treatment of nasolacrimal duct obstruction. Although it has a somewhat longer learning curve than some surgeries, once handling of the endoscope has been mastered and familiarity with the surgical field is improved, the duration of the surgery will decrease significantly. We believe multidisciplinary coordination plays an important role in decreasing potential complication rates and also in perfecting the technique

    Molecular modelling of the FOXO4-TP53 interaction to design senolytic peptides for the elimination of senescent cancer cells

    Background: Senescent cells accumulate in tissues over time as part of the natural ageing process and the removal of senescent cells has shown promise for alleviating many different age-related diseases in mice. Cancer is an age-associated disease and there are numerous mechanisms driving cellular senescence in cancer that can be detrimental to recovery. Thus, it would be beneficial to develop a senolytic that acts not only on ageing cells but also senescent cancer cells to prevent cancer recurrence or progression. Methods: We used molecular modelling to develop a series of rationally designed peptides to mimic and target FOXO4 disrupting the FOXO4-TP53 interaction and releasing TP53 to induce apoptosis. We then tested these peptides as senolytic agents for the elimination of senescent cells both in cell culture and in vivo. Findings: Here we show that these peptides can act as senolytics for eliminating senescent human cancer cells both in cell culture and in orthotopic mouse models. We then further characterized one peptide, ES2, showing that it disrupts FOXO4-TP53 foci, activates TP53 mediated apoptosis and preferentially binds FOXO4 compared to TP53. Next, we show that intratumoural delivery of ES2 plus a BRAF inhibitor results in a significant increase in apoptosis and a survival advantage in mouse models of melanoma. Finally, we show that repeated systemic delivery of ES2 to older mice results in reduced senescent cell numbers in the liver with minimal toxicity. Interpretation: Taken together, our results reveal that peptides can be generated to specifically target and eliminate FOXO4+ senescent cancer cells, which has implications for eradicating residual disease and as a combination therapy for frontline treatment of cancer.     New Anthropometric Measurements: Relationship to Thyroid Functions in Euthyroid Obese Subjects

    Introduction Body mass index (BMI) is unable to make a distinction between muscle mass and fat mass. Therefore, new anthropometric measurements, such as a body shape index (ABSI), body round index (BRI), and body adiposity index (BAI), have been formulated in recent years. Many studies have reported a correlation between BMI and thyroid function. In this study, we aimed to investigate the relationship between the above-mentioned new anthropometric measurements and thyroid functions in euthyroid obese subjects. Methods We included 675 euthyroid (TSH >= 0.4 and = 30. Thyroid-stimulating hormone (TSH), free T4 (fT4) and free T3 (fT3), anthropometric measurements (weight, height, and waist circumference), and bioelectric impedance analyses {[}percent body fat (PBF) and fat-free mass (FFM)J of individuals were measured and recorded. ABSI, BRI, and BAI were calculated with the data from these measurements. Anthropometric measurements were compared to thyroid function tests. Results Eighty percent of the subjects were female. The mean age and BMI were 38 +/- 17 years and 38 +/- 6 kg/m(2), respectively. TSH was found to be negatively correlated with ABSI (p = 0.006) and positively correlated with BAI (p < 0.001), but a statistically significant relationship with BRI (p = 0.193) was not determined. Free T4 was not associated with any of the anthropometric measurements. While fT3 was determined to be positively correlated with ABSI (p= 0.008) and negatively correlated with PBF and BAI (p= 0.001, p= 0.002, respectively), no statistically significant relationship with fF3 and BRI was determined. Conclusion TSH is positively correlated with measurements of adiposity such as BMI, PBF, BAI while indexes in which abdominal obesity increases, such as waist circumference (WC), waist-hip ratio (WHR), and ABSI, are correlated with fT3 levels.12DEC 151
