Generic Cyclosporine in the Treatment of Dry Eye Disease
ABSTRACT
Objectives: To investigate the efficacy and safety of a generic Cyclosporine 0.05% ophthalmic emulsion in chronic dry
eye disease.
Materails and Methods: Thirty patients with dry eye disease were included in this observational, prospective study.
Patients were examined at baseline, at month -1,-3 and 6 of the study. The following were evaluated at each visit: CDVA
(corrected distance visual acuity), corneal and conjunctival staining (Oxford) score, TBUT (tear break-up time), Schirmer-2
score, OSDI (Ocular Surface Disease Index) score, patient’s subjective rating scale of ocular discomfort, daily use of
concomittant artificial tears, the researcher’s assessment of the global treatment response, and IOP (intraocular pressure).
In addition, drug-related side effects were evaluated individually in each examination. When initiating dry eye treatment,
cyclosporine was combined with topical loteprednol etabonate 0.5%.
Results: There was a statistically significant improvement from baseline in corneal and conjunctival staining (decrease in
mean Oxford score), TBUT, Schirmer-2 values, OSDI score, patient’s subjective rating score for ocular discomfort, and mean
physician’s subjective assessment of global response to treatment at all follow-up visits (p<0.001). The mean daily use of
artifical drops decreased statistically at all time points (<0.001). The most commonly reported adverse events were ocular
burning (6.7%), followed by stinging (3.3%), conjunctival hyperemia (3.3%), foreign body sensation (3.3%), and visual
disturbance (3.3%).
Conclusion: Generic cyclosporine 0.05% ophthalmic emulsion with loteprednol etabonate 0.5% on initiation treatment
has well-tolerability and improves subjective and objective measures of dry eye disease
The Effect of Personal Face Mask Use on Hearing Sensitivity Awareness
ABSTRACT
Background and Objectives: The speech information obtained from the acoustic cue alone is not sufficient. The
visual component of speech is crucial for speech perception. This study aims to show that individual hearing sensitivity
awareness has increased due to the blocking of visual components of speech as a result of the use of personal face masks
with the Covid 19 pandemic.
Subjects and Methods: A scale was prepared with questions that measured individuals’ ability to use auditory stimuli
in various conditions before and after the pandemic. The scale prepared consists of two sections and a total of 15 items.
The questions in the first section are about pre-pandemic hearing loss awareness, and the second section is about post pandemic hearing loss awareness. The age average of 1046 people included in the study was 49.47 ± 11.06.
Results: Those who do not feel the need to repeat what was spoken in face-to-face meetings before the pandemic need
to repeat what was spoken significantly after the pandemic (p<0.001). The findings show the increased need to repeat
spoken words in individuals who wear face masks. Also, people have trouble recognizing speech in noisy environments is
due to the absence of visual speech cues, compared to the absence of such trouble before the pandemic.
Conclusion: The use of personal face masks in the post-pandemic period has changed the awareness of individual
hearing sensitivity due to the disappearance of the visual element of speech

MEOX2 homeobox gene promotes growth of malignant gliomas
Background Glioblastoma (GBM) is an aggressive tumor that frequently exhibits gain of chromosome 7, loss of chromosome 10, and aberrantly activated receptor tyrosine kinase signaling pathways. Previously, we identified Mesenchyme Homeobox 2 (MEOX2), a gene located on chromosome 7, as an upregulated transcription factor in GBM. Overexpressed transcription factors can be involved in driving GBM. Here, we aimed to address the role of MEOX2 in GBM. Methods Patient-derived GBM tumorspheres were used to constitutively knockdown or overexpress MEOX2 and subjected to in vitro assays including western blot to assess ERK phosphorylation. Cerebral organoid models were used to investigate the role of MEOX2 in growth initiation. Intracranial mouse implantation models were used to assess the tumorigenic potential of MEOX2. RNA-sequencing, ACT-seq, and CUT\&Tag were used to identify MEOX2 target genes. Results MEOX2 enhanced ERK signaling through a feed-forward mechanism. We identified Ser(155) as a putative ERK-dependent phosphorylation site upstream of the homeobox-domain of MEOX2. S155A substitution had a major effect on MEOX2 protein levels and altered its subnuclear localization. MEOX2 overexpression cooperated with p53 and PTEN loss in cerebral organoid models of human malignant gliomas to induce cell proliferation. Using high-throughput genomics, we identified putative transcriptional target genes of MEOX2 in patient-derived GBM tumorsphere models and a fresh frozen GBM tumor. Conclusions We identified MEOX2 as an oncogenic transcription regulator in GBM. MEOX2 increases proliferation in cerebral organoid models of GBM and feeds into ERK signaling that represents a core signaling pathway in GBM.11NOV 21911-19242

Perpendicular drill bit alignment provides a practical guidance to determine the appropriate suture anchor insertion angle during acetabular labral repair
The safe acetabular rim angle is an anatomical measurement used to determine the safety margin when inserting suture anchors. The purpose of the present study was to find out whether aligning the drill bit perpendicularly during arthroscopic surgery can provide a reference point for determining an appropriate angle to facilitate the suture anchor insertion and to prevent extra- and intra-articular perforations. One hundred computed tomographic hips were used to reconstruct three-dimensional acetabular hip models. Each model was radially sectioned at the 4 o'clock, 3 o'clock and anterior inferior iliac spine (AIIS) positions (that corresponded mainly to the 2:20 clock position). A perpendicular reference line, representing a perpendicular drill bit alignment, was drawn for each position within the acetabular model, and its relation to the safe acetabular rim angle was measured. The length of the perpendicular reference line and the effect of gender on measurements were also evaluated. The mean safe acetabular rim angle at the 3 o'clock position was significantly smaller compared to other clock positions (P < 0.001). The perpendicular reference line was located out of the safe acetabular rim angle in 28 cases (\%28), mostly in female acetabula at the 3 o'clock position, and relative to the perpendicular reference line the required minimal angle was 4 degrees +/- 2.3 degrees to place the anchor in the safe acetabular rim angle to avoid extra-articular perforation. The perpendicular reference line was shortest at the 3 o'clock position, and its mean length was shorter in female acetabula at all clock positions (P < 0.001). Aligning the drill bit perpendicular to the acetabular opening plane during an arthroscopic anchor placement is a practical way to estimate and target the position of the safe acetabular rim angle to avoid anchor perforations. Based on measurements from a perpendicularly aligned drill bit, the drill bit should be directed towards the joint minimally by 4 degrees to avoid extra-articular perforations and maximally by 30 degrees to avoid intra-articular perforations.2FEB 7185-191
Epidemiologic and clinical characteristics of neonates with late-onset COVID-19: 1-year data of Turkish Neonatal Society
The literature on neonates with SARS-CoV-2 is mainly concerned with perinatal cases, and scanty data are available about environmentally infected neonates. To fill knowledge gaps on the course and prognosis of neonatal cases, we analyzed 1-year data from the Turkish Neonatal Society in this prospective cohort study of neonates with postnatal transmission. Data from 44 neonatal intensive care units (NICUs), of neonates with positive RT-PCR results at days 5-28 of life, were extracted from the online registry system and analyzed. Of 176 cases, most were term infants with normal birth weight. Fever was the most common symptom (64.2\%), followed by feeding intolerance (25.6\%), and cough (21.6\%). The median length of hospitalization was 9 days, with approximately one quarter of infants receiving some type of ventilatory support. Myocarditis (5.7\%) was the most common complication during follow-up. Among the clinical findings, cough (odds ratio {[}OR]: 9.52, 95\% confidence interval {[}CI]: 4.17-21.71), tachypnea (OR: 26.5, 95\% CI: 9.59-73.19), and chest retractions (OR: 27.5, 95\% CI: 5.96-126.96) were associated with more severe clinical disease. Also, there were significant differences in the C-reactive protein level, prothrombin time (PT), partial thromboplastin time, international normalized ratio, and days in the NICU (p = 0.002, p = 0.012, p = 0.034, p = 0.008, and p < 0.001, respectively) between patients with mild-moderate and severe-critical presentations. A PT above 14 s was a significant predictor of severe/critical cases, with a sensitivity of 64\% and specificity of 73\%. Conclusions: Our data showed that late-onset COVID-19 infection in neonates who need hospitalization can be severe, showing associations with high rates of ventilatory support and myocarditis. ABSTRACT Are Family Physicians and Medical Students Aware of Chronic Hepatitis C Infection?
Introduction: Chronic hepatitis C is a public health problem that can result in cirrhosis, liver cancer, and death, which can be cured with current treatments. In our study, we evaluated the level of knowledge and awareness of medical school students and family physicians about chronic hepatitis C.Materials and Methods: Our study was carried out between 01.06.2022 and 01.08.2022, between the 1st and 6th grade medical students, family physicians (family medicine residents, family medicine specialists) using an online questionnaire.Results: In our study, 536 medical faculty students and 162 family physicians were recruited. Two hundred seventy one (50.6\%) of the students were male and the median age was 23.0 (2.0) years. Of the family physicians, 84 (51.2\%) were male, the median age was 30 (24-68) years and the median time in the profession was 14.5 (1-40) years. Students answered 69.2\% of the questions correctly, and family physicians answered 73\% correctly. While the students knew the transmission routes of hepatitis C mostly (97.2\%), they got the less information about the testing (34.9\%) and treatment (21.9\%). Family physicians answered the questions about the transmission routes of hepatitis C with a high rate (99.4\%), and the questions about the groups that should be tested (28\%) and the questions about the treatment of hepatitis C less correctly (29.4\%). In the multivariate analysis, the factors affecting the median knowledge score of the students were male gender (p= 0.019), the student's class (p= 0.002), having Infectious diseases and clinical microbiology committee (p= 0.005) and gastroenterology committee (p= 0.027). The only factor affecting the median knowledge score of family physicians was the use of online presentations by experts in their fields as a source of information (p= 0.003). 67\% of the students and 87.2\% of the family physicians answered the question ``How do you manage the person when the anti-HCV screening test is positive? `` as ``I will refer the patient to the infectious diseases and clinical microbiology or gastroenterology ``.Conclusion: In our study, it was found that awareness of screening and treatment was low in both groups. Considering that the main factors related to the level of knowledge of medical students are having committee of the gastroenterology and infectious diseases and clinical microbiology, and online presentations of experienced teachers in family physicians, preparing a special content for these groups may be useful in raising awareness and level of knowledge.3493-5032
Investigation of ceftazidime-avibactam susceptibility in clinical isolates of gram-negative bacteria
Background/aim: Our study investigated the susceptibility rate of ceftazidime-avibactam and the risk factors associated with its resistance by analyzing gram-negative bacteria isolated from various patient samples. Materials and methods: Between March and November 2020, 1119 gram-negative bacteria strains were isolated from patient samples in Acibadem Healthcare Group hospitalsceftazidime-avibactam susceptibility results were evaluated using a 10/4 mu g (Oxoid, UK) disc and evaluated according to Eucast 2020 recommendations. Patient and isolate characteristics that could be risk factors were retrospectively investigated and statistically analyzed using SPSS 25.0. Results: Male patients made up 52\% (n = 581) of the study's total patient population, and they averaged 55.5 +/- 24.9 years old. Of 1119 gram-negative strains culture and antibiogram, 1023 (91.4\%) were sensitive to ceftazidime-avibactam. An increased risk of resistance was observed with female gender (OR = 2.29CI 95\% {[}1.45-3.61]p < 0.05), Pseudomonas aeruginosa (OR = 1.67, CI 95\% {[}1.03-2.7]p < 0.05), the presence of multidrug-resistance (MDR) (OR = 4.07, CI 95\% {[}2.47-6.7]p < 0.05) pandrug-resistance (PDR) (OR = 12, (CI) 95\% {[}9.9-14.7]]p < 0.05) and admission to intensive care unit (ICU) (OR = 1.89, CI 95\% {[}1.22-2.93]p < 0.05). Conclusion: The resistance rate of ceftazidime-avibactam was found to be 8.6\%, and it was thought that resistant strains produced metallo-beta-lactamase (MBL) type carbapenemase. Risk factors were female gender, Pseudomonas aeruginosa, MDR, PDR, and admission to ICU. Therefore, studying the ceftazidime-avibactam susceptibility test together with gram-negative bacteria identification, especially in groups at risk for resistance, is one of the important factors that can positively affect the success of treatment.61839-18445
Efficacy of PET-CT in the prediction of metastatic adrenal masses that are detected on follow-up of the patients with prior nonadrenal malignancy: A nationwide multicenter case-control study
Metastasis is the second most common type of adrenal gland mass. In patients undergoing follow-up for nonadrenal malignancy, adrenalectomy is performed when metastasis to adrenal gland is suspected on the basis of positron emission tomography-computed tomography (PET-CT) imaging. This study investigated the efficacy of PET-CT in the discrimination of metastatic lesions from nonmetastatic lesions in the adrenal glands. In this multicentric study, data was collected from enrolled centers. Forty-one patients who underwent surgery for suspected adrenal metastases were evaluated retrospectively. The following data types were collected: demographic, primary tumor, maximum standardized uptake value of adrenal mass (a-SUVx) and detectability in computed tomography and/or magnetic resonance imaging, and specimen size and histopathology. Six patients were excluded due to unavailability of PET-CT reports and 4 for being primary adrenal malignancy. The rest were divided into 2 groups (metastatic: n = 17, 55\% and nonmetastatic: n = 14, 45\%) according to histopathology reports. There was no statistical difference between the analyzed values, except the a-SUVx (P .05). It was found that PET-CT was able to discriminate metastatic lesions from primary benign lesions (P = .022). PET-CT can discriminate primary benign lesions and metastatic lesions by cutoff 5.5 value for a-SUVx.34AUG 2610
