    Relationships between teachers' perceptions of positive psychological capital and organizational happiness: Mediating role of organizational silence

    Positive moods such as happiness that encourage interaction with the social environment were expected to be more favorable for organizational contribution and creativity than passive moods such as silence. Positive relationships between positive psychological capital and organizational happiness were expected in educational organizations where positive communication was the most desired. However, negative moods also had negative effects on creativity, and the negative work environments caused organizational silence. In this study, it was aimed to investigate the relationships among teachers' perceptions of positive psychological capital, organizational silence and organizational happiness. The research was designed as a relational screening model and the structural equation modelling technique was used to test the hypothesized model. Participants of the study consisted of 420 teachers working in public and private schools. The results showed that high levels of positive psychological capital were associated with both higher organizational happiness and lower organizational silence. In contrast, higher organizational silence was associated with lower levels of organizational happiness. Organizational silence mediated the relationship between positive psychological capital and organizational happiness. This study supported the idea that the effect of positive psychological capital on creating organizational happiness may decrease in school environments that cause organizational silence

    Permanent impairment of language functions in an adolescent case of autoimmune encephalitis

    Abstract Introduction: In pediatric and adolescent population, autoimmune encephalitis (AE) may present with a wide variety of symptoms including cognitive regression accompanied with loss of language skills. Despite its high prevalence in AE, linguistic functions have not been investigated in extensive detail. Case: A 12-year-old girl with no significant premorbid history and normal school performance presented with fever, hypersomnia, nocturnal myoclonus and behavioral changes. Although neurological examination was normal, psychiatric evaluation revealed euphoria, mild irritability and visual hallucinations. Cranial MRI was normal, whereas cerebrospinal fluid (CSF) analysis showed elevated protein concentration and lymphocyte count, electroencephalogram (EEG) showed diffuse slow waves. A panel for anti-neuronal antibodies demonstrated glutamic acid decarboxylase (GAD) antibodies in the serum. Following immunotherapy, all neurological and behavioral symptoms vanished. However, the patient suffered from significant worsening of school performance. Psychiatric evaluation revealed severe depression. Assessment of intelligence done on the 10th and 18th month of follow-up yielded significantly low scores at mental retardation level. Linguistic assessment showed significant impairment in all domains but especially in semantics. Conclusion: Our case emphasizes the fact that AE may cause permanent cognitive dysfunction and language impairment even in patients with normal MRI/neurological examination findings and relatively mild treatment-responsive disease course

    ÖZET Galaktagoglar; anne sütü salınımının başlamasını ve devamlılığını stimüle eden besin veya takviyelerdir. Çalışma laktasyon dönemindeki 72 anneyle gerçekleştirilen bir anket çalışmasıdır. Anneler galaktogog kullanan (n:36) ve kullanmayan (n:36) olmak üzere iki gruba ayrılmıştır. Çalışma; 14 Mart-20 Nisan 2022 tarihleri arasında Biruni Üniversite Hastanesi Çocuk Sağlığı ve Hastalıkları Polikliniği’ne başvuran sağlıklı bebeklerden ve annelerinden oluşmaktadır. Annelerin antropometrik verileri kıyaslandığında, galaktogog kullanan annelerin vücut ağırlığı ve BKİ değerleri daha yüksek bulunmuş ve bu değerlerin istatistiksel olarak önemli olduğu saptanmıştır (sırasıyla; p=0,042 ve p=0,048). Galaktogog kullanmayan annelerin gebelik döneminde demir takviyesi kullanım oranı daha yüksek bulunmuş ve bu farkın istatistiksel olarak önemli olduğu saptanmıştır (p=0,047). Emzirme döneminde D vitamini kullanımı da iki grup arasında önemli derecede farklı bulunmuş (p=0,039) ve galaktogog kullanmayan annelerin laktasyon döneminde D vitamini kullanımının daha yüksek olduğu görülmüştür. Galaktogog olarak en sık kullanılan besin ve maddelerin; ticari amaçlı galaktogog bitki çaylar (%50), rezene çayı (%36,1), bira mayası ekstresi (%13,8), malt içeceği (%11,1), ticari amaçlı satılan galaktogog damla (%8,3), hurma (%8,3) ve aktardan alınan anne sütü çayı (%5,5) olduğu tespit edilmiştir. Galaktogog kullanan gruptaki bebeklerin çoğunluğu (%80,6) hiç formula almazken galaktogog kullanmayan gruptaki bebeklerin yarısından fazlasının (%58,3) formula aldığı görülmüş ve bu fark istatistiksel olarak önemli bulunmuştur (p=0,041). Annelerin yaptıkları doğum sayısı ile galaktogog kullanımının ters orantılı olduğu saptanmış ve bu durum istatistiksel olarak önemli bulunmuştur (p=0,009). LATCH Emzirme Tanılama Aracı ve Postpartum Emzirme Öz Yeterlilik Skorları kıyaslandığında ise iki grup arasında önemli bir fark bulunmamıştır (p>0,05). Galaktogog kullanımının etkilerini araştırmak amacıyla benzer çalışmaların daha büyük örneklem gruplarıyla yapılması önerilmektedir.ABSTRACT Galactagogues; foods or supplements that stimulate the initiation and maintenance of breast milk secretion. The study is a survey study conducted with 72 mothers in the lactation period. Mothers were divided into two groups as galactogogue users (n: 36) and non-users (n: 36). Study; It consists of healthy babies and their mothers who applied to Biruni University Hospital Child Health and Diseases Polyclinic between 14 March-20 April 2022. When the anthropometric data of the mothers were compared, the body weight and BMI values of the mothers using galactogogue were found to be higher and these values were found to be statistically significant (p=0.042 and p=0.048, respectively). The rate of iron supplement use during pregnancy was found to be higher in mothers who did not use galactogogue, and this difference was found to be statistically significant (p=0.047). The use of vitamin D during breastfeeding was also found to be significantly different between the two groups (p=0.039) and it was observed that the use of vitamin D during lactation was higher in mothers who did not use galactogogue. The most commonly used foods and substances as galactogogues; commercial galactogogue herbal teas (50%), fennel tea (36.1%), brewer's yeast extract (13.8%), malt beverage (11.1%), commercial galactogogue drops (8.3%), date (8.3%) and breast milk tea (5.5%) taken from herbalists. While the majority of infants (80.6%) in the galactogogue group did not receive any formula, more than half (58.3%) of the infants in the galactogogue group received formula, and this difference was found to be statistically significant (p=0.041). It was found that the number of births and the use of galactogogues were inversely proportional to the mothers, and this was statistically significant (p=0.009). When the LATCH Breastfeeding Diagnostic Tool and Postpartum Breastfeeding Self-Efficacy Scores were compared, no significant difference was found between the two groups (p>0.05).     Students’ self-directed online learning skills in distance higher education: Students’ voice and faculty members’ supports

    This study aims to examine self-directed online learning skills of undergraduate students and the ways of support provided by the faculty members. In this mixed-method study, the participants consisted of 399 undergraduate students studying at a state university in Turkey during the 2019-2020 academic-year spring semester, identified using convenient sampling and 12 faculty members, determined by maximum variation sampling method. Data collection tools included Self-Directed Online Learning Questionnaire and a semi-structured interview form. Independent Sample T-test from parametric tests, One-Way Analysis of Variance for multiple comparisons and LSD test were performed for the quantitative data analysis. In addition, the qualitative data were analyzed via content analysis. Some of the findings show the students have the highest mean score at time management dimension and the lowest one at help-seeking dimension. Female students have higher scores in general, metacognitive skills, persistence, and environmental structuring dimensions. The sophomore students have significantly higher scores than the senior students in terms of metacognitive skills dimension. Besides, the faculty members support the students to get the self-directed online learning skills, especially metacognitive skills and help seeking. However, their supports are limited to some kind of encouragement at time management, environmental structuring and persistence dimensions. The results show the necessity to support students to have self-directed online learning skills and assist faculty members in developing their students skills

    Importance of precise imaging for stone identification during shockwave lithotripsy: a critical evaluation of “OptiVision” as a post-processing radiography imaging modality

    To evaluate the efficacy of a unique imaging software “OptiVision” on the confidence of stone identification using X-ray imaging during shockwave lithotripsy. A total of 57 patients with solitary radio-opaque kidney stones sized < 15 mm were included and evaluated with appropriate radiological imaging and laboratory studies before undergoing shockwave lithotripsy (SWL). The size of the stones was measured on KUB and CT images using maximal stone diameters. All patients were evaluated with radiography on the Dornier Delta III immediately before SWL and at the end of the SWL session with fluoroscopy, a snapshot image, and the OptiVision modality (applied over the acquired snapshot image without additional radiation exposure) for precise identification of the stone. All images obtained with these three different imaging modalities (fluoroscopy, snapshot, and OptiVision) were comparatively evaluated for confidence levels in stone identification using a five-point Likert scale (1 = poor diagnostic confidence, 5 = excellent diagnostic confidence). These confidence levels were also used to differentiate between stones and bony structures using these same imaging modalities. Results were evaluated for statistical significance regarding their impact on the precise identification of renal stones. A total of 57 patients with solitary stones were evaluated using flouroscopy, snapshot, and OptiVision images before and after SWL to ascertain the efficacy of these 3 imaging modalities on the precise identification of the calculi. Stone sizes ranged from 4 to 15 mm, with a mean of 8.30 mm in the longest dimension. Evaluation of the findings demonstrated that while the pre- and post-SWL confidence of fluoroscopic stone identification was from 29.51 to 31.15%, the values using snapshot ranged from 31.15 to 39.34%. Pre- and post-SWL confidence of stone identification with OptiVision post-processing imaging was approximately 89%. In the majority of cases (n = 43), the use of OptiVision image processing enabled urologists to identify the stone with a higher level of confidence rating than the use of both fluoroscopy (n = 0) and snapshot (n = 0) image processing tools. While a rating level of 4 or higher was given in 89% of images obtained by OptiVision during the pre- and post-SWL phase, this value was only 37% (4 or more points) using fluoroscopy and Snapshot image processing techniques. For all the radiography image processing tools, the distribution of the Likert score differed significantly among the OptiVision, fluoroscopy, and snapshot imaging modalities (p < 0.001). Precise and reliable identification of stone(s) located in the kidney is essential in delivering shock waves to the desired focal point and achieving higher stone-free rates. Our findings demonstrated that using the specially designed imaging modality OptiVision was significantly helpful in identifying and localizing stones with high-quality images before SWL for effective stone disintegration during this procedure

    Classification and standardized reporting of percutaneous nephrolithotomy (PCNL): International Alliance of Urolithiasis (IAU) Consensus Statements

    Background: The aim of this study was to reach a consensus in the classification and standardized reporting for the different types of PCNLs. Methods: The RAND/UCLA appropriateness methodology was used to reach a consensus. Thirty-two statements were formulated reviewing the literature on guidelines and consensus on PCNLs, and included procedure specific details, outcome measurements and a classification for PCNLs. Experts were invited to two rounds of input, the first enabled independent modifications of the proposed statements and provided the option to add statements. The second round facilitated scoring of all statements. Each statement was discussed in the third round to decide which statements to include. Any suggestion or disagreement was debated and discussed to reach a consensual agreement. Results: Twenty-five recommendations were identified to provide standardized reporting of procedure and outcomes. Consensual scoring above 80% were strongly agreed upon by the panel. The top treatment related outcomes were size of sheath used (99.1%) and position for PCNL (93.5%). The highest ranked Outcome Measures included definition of postoperative hospital length of stay (94.4%) and estimated blood loss (93.5%). Conclusions: The consensus statements will be useful to clarify operative technique, in the design of clinical trials and standardized reporting, and presentation of results to compare outcomes of different types of PCNLs

    Effect of partially stabilized zirconia thickness on the translucency and microhardness of resin cement

    Statement of problem. Partially stabilized zirconia has been introduced as a more translucent iteration. However, knowledge of the effect of the thickness of partially stabilized zirconia on the microhardness of resin cement is sparse. Purpose. The purpose of this in vitro study was to evaluate the effect of thickness and material type on the translucency of partially stabilized zirconia and the microhardness of the resin cement polymerized beneath. Material and methods. Specimens were prepared from 2 translucent zirconias with different yttrium content (Ceramil Zolid HT+ [HT] and Ceramil Zolid FX [SHT]) and a lithium disilicate glass-ceramic (e.max CAD [EX]) of different thicknesses (0.5, 1, 1.5, and 2 mm) (n=10). Color coordinates were recorded by using a spectrophotometer, and the relative translucency parameter (RTP) was calculated by using the CIEDE2000 formula. The microhardness of the resin cement polymerized under ceramic specimens was measured. Two-way analysis of variance, the Tukey honestly significant difference, and independent samples t tests were used to analyze the data (a=.05). Results. Material type (P<.001), thickness (P<.001), and their interaction significantly affected the RTP (P=.001) and hardness values (P<.001). Regardless of the thickness, EX had the highest RTP (P .027), and the resin cements polymerized under EX showed the highest microhardness (P .002). However, the difference between HT and SHT for RTP (P.082) and resin cement hardness (P.984) was not significant. Specimens of 0.5 mm resulted in higher RTP (P .001) and resin cement hardness (P .006) than the 1.5- and 2-mm specimens of each ceramic. Conclusions.     Öz Bu araştırmanın amacı felsefe temelli Türkçe öğretiminin soru sorma ve işbirlikli öğrenme becerilerine olan etkisini ortaya koymaktır. Belirtilen durumdan hareketle okuma eğitimi derslerinde P4C temelli soru sorma etkinliklerinin ortaokul 5. sınıf öğrencilerinin okuduğunu anlama ve işbirlikli öğrenme becerilerine olan etkisi belirlenmiştir. Mevcut çalışma karma araştırma yönteminin iç içe karma deseni ile desenlenmiştir. Nicel veri toplama aracı olarak işbirlikli öğrenme ölçeği kullanılırken nitel verilerin toplanmasında yarı yapılandırılmış öğrenci görüşme formu kullanılmıştır. İç içe karma desen ile yapılan uygulamalar neticesinde okuma öğretiminde uygulanan P4C temelli soru sorma etkinliklerinin okuduğunu anlamada ve işbirlikli öğrenmenin gelişiminde etkili olduğu görülmüştür. Öğrenci görüşlerinin de nicel verileri destekler nitelikte olduğu belirlenmiştir.Abstract The aim of this research is to determine the effect of philosophy-based Turkish teaching on asking questions and cooperative learning skills. Based on the stated situation, the effect of P4C-based questioning activities on reading comprehension and cooperative learning skills of secondary school 5th grade students was determined. The present study was designed with the nested mixed pattern of the mixed research method. While the cooperative learning scale was used as a quantitative data collection tool, a semi-structured student interview form was used to collect qualitative data. As a result of the applications made with the nested mixed pattern, it was seen that the P4C-based questioning activities applied in reading teaching were effective in the development of reading comprehension and cooperative learning.     ÖZET Dentigeröz kistler, çenelerde görülen tüm odontojenik kistlerin %24'ünü oluşturur. Genellikle asemptomatiktir. Multipl oluşumlar genellikle kleidokraniyal displazi veya Maroteaux-Lamy gibi sendromlarda ortaya çıkabilir. Bu olgu sunumunun amacı, sendromik olmayan bir hastada 4 kadranda multipl dentigeröz kistlerin varlığı son derece nadir görülen bir durum olduğundan literatüre katkıda bulunmaktır. Anamnezinde sistemik bir hastalığı olmayan 21 yaşındaki erkek hasta, sürmemiş 3. azı dişlerini çektirmek için fakültemize müracaat etmiştir. Hastadan alınan panoramik radyografide, 18, 28, 38, 48 no.lu dişlerin etrafında uniloküler bir radyolüsent lezyon gözlenmiştir. Hastadan, konik ışınlı bilgisayarlı tomografi alınmıştır. Mine-sement sınırından başlayarak tüm 3. molar dişlerin kronunu çevreleyen hipodens lezyonlar gözlenmiştir. Dentigeröz kist ön tanısı ile histopatolojik değerlendirme istenmiştir. Alınan biyopsiler sonucu tüm bölgelerdeki lezyonlar histopatolojik olarak dentigeröz kist tanısı konulmuştur. Nadir görülen multipl dentigeröz kistleri olan hastanın lezyonları dişlerle birlikte eksize edilmiştir.ABSTRACT Dentigerous cysts make up 24% of all odontogenic cysts seen in the jaws. It is usually asymptomatic. Mutiple formations can often occur in syndromes such as kleidocranial dysplasia or MaroteauxLamy. The purpose of this case report is to contribute to the literature, as the presence of multiple dentigerous cysts on four quadrants in a non-syndromic patient is an extremely rare condition. A 21-year-old male patient who does not have a systemic disease in his anamnesis has applied to our faculty to have his third molars removed. A unilocular radiolucent lesion was observed around teeth numbered 18, 28, 38, 48 on a panoramic radiograph taken from the patient. Hypodens lesions surrounding the crown of all third molar teeth have been observed, starting at the enamel-cementum border at cone beam computed tomography. As a result of the biopsies taken, lesions in all regions were diagnosed as dentigerous cysts histopathologically.     New analytical solutions by the application of the modified double sub-equation method to the (1+1) - Schamel - KdV equation, the Gardner equation and the Burgers equation

    Abstract In this research we present the application of the modified double sub-equation guess solution together with the analytical solutions of the Riccati equation to obtain new analytical exact solutions to the (1+1)-Schamel-KdV equation, the (1+1)-dimensional Gardner equation (or combined KdVmKdV) and the nonlinear evolution (1+1)-dimensional Burgers equation. Results show some conditions between the allowed values of the interaction coefficients and the parameters of the allowed analytical solutions of the double sub-equation guess solution. The resulting new soliton solutions have by first time the proper real phase value behavior, not seen in previous applications of the double sub-equation method. Additionally, it has been revealed that handle the complexiton process with two different traveling variables have as an important consequence that the mixing between trigonometric and hyperbolic solutions is only observed for the analytical solutions of the nonlinear evolution (1+1)-dimensional Burgers equation, while no mixing between trigonometric and hyperbolic solutions is observed for the solutions of the (1+1)-Schamel-KdV ((1+1)-S-KdV) equation or the (1+1)-dimensional Gardner equation. Numerical simulations are provided in 3-D graphs and 2-D plots representing the general characteristics of the resulting analytical solutions
