    Association between vitamin D status and secondary infections in patients with severe COVID-19admitted in the intensive care unit of a tertiary-level hospital in Turkey

    There are several studies showing that the vitamin D status can determine risk of COVID-19 infections, severity and mortality from coronavirus disease 2019 (COVID-19). However, the association between vitamin D (25(OH)D) and secondary infections in the prognosis of COVID-19 patients has not been reported yet. The aim was to investigate whether the vitamin D status affects the rates of secondary infections in patients with severe COVID-19 hospitalized in the intensive care unit (ICU) of a tertiary-level hospital in Turkey. The data of 194 patients with diagnosis of severe COVID-19 who were admitted to the ICU from March 2020 to June 2021 and older than 18 years were evaluated in this retrospective study. The patients were divided into two groups according to total serum 25(OH)D level as normal group (≥20 ng/mL) and low group (<20 ng/mL). The 25(OH)D level was low in 118 (60.8%) and normal in 76 (39.2%) patients. The mean age of the low group was significantly higher than that of the normal group (67.02 ± 14.47 vs. 61.70 ± 14.38; p = 0.013). The systolic and diastolic blood pressure as well as the Glasgow coma scale score of the low group were significantly lower than that of the normal group (p = 0.004, 0.002 and 0.001, respectively). The intubation rate and APACHE (Acute Physiology and Chronic Health Evaluation) score of the low group was significantly higher than that of the normal group (p = 0.001). The platelets number and blood pH decreased, and the neutrophil/lymphocyte ratio, procalcitonin, lactate, urea, creatinine and lactate dehydrogenase concentrations increased significantly in the low group (p < 0.05). The mortality rate was 79.7% in the low group and 22.4% in the normal group (p < 0.001). Microbiological growth was observed in 68.6% of the normal group and 52.6% of the normal group (p = 0.025). The number of cultures with resistant bacteria was significantly higher in the low group (25.9%) than that in the normal group (17.5%) (p = 0.035). The severe COVID-19 patients hospitalized with vitamin D deficiency may have increased risks of poor prognosis and mortality due to secondary infections in the ICU

    Öz Multisistem İnflamatuar Sendromu (Multisystem Inflammatory Syndrome in Children / MIS-C), COVID-19’un nadir görülen komplikasyonu olarak tanımlanan, Nisan 2020 tarihinden itibaren COVID-19 geçirildikten 2-4 hafta sonra ortaya çıktığı varsayılan bir sendromdur. MIS-C’de; sıklıkla ateş, döküntü, bilateral konjonktivit, gastrointestinal sisteme ait semptomlar, aşırı yorgunluk hissi veya miyokardit gibi bulgular görülmektedir. Hemşireler pediatrik hastaların fiziksel, mental ve spiritüel ihtiyaçlarını anlayarak ve karşılayarak bütüncül bir yaklaşımla kaliteli, bireyselleştirilmiş ve aile merkezli bakım vermeye özen göstermelidir. Bakımda izolasyon önlemleri alınmalı ve uygun kişisel koruyucu ekipmanlar kullanılmalı, kişisel koruyucu ekipmanlara yönelik çocuk ve ailenin eğitimi bakımın bir parçası olmalıdır. MIS-C yeni bir sendrom olduğundan sağlık ekibi üyeleri, pediatrik hastaların ve ailelerinin birçok sorusu ile karşı karşıya kalacaklarından bu süreçte literatürü takip etmeleri oldukça önemlidir. Bu derlemede, 2021 Ocak – Şubat aylarında PubMed veritabanında yer alan COVID-19 ile ilişkili MIS-C tanılı hastaların olgu sunumlarındaki hastalığın seyri ve buna yönelik hemşirelik yaklaşımı ele alınmıştır.Abstract Multisystem Inflammatory Syndrome (MIS-C) is a syndrome defined as a rare complication of COVID-19, which is assumed to occur 2-4 weeks after COVID-19 as of April 2020. In MIS-C, symptoms such as fever, rash, bilateral conjunctivitis, symptoms of the gastrointestinal system, feeling of extreme tiredness or myocarditis are frequently observed. Nurses should take care to provide quality, individualized and familycentered care in a holistic approach by understanding and meeting the physical, mental, and spiritual needs of pediatric patients. Isolation measures should be taken care of, appropriate personal protective equipment should be used, and education of the child and family should be a part of the care for personal protective equipment. It is very important for healthcare members to follow the literature in this process, as they will face many questions from patients and their families. In this review, the course of the disease and the nursing approach to this in the case reports of patients with a diagnosis of MIS-C associated with COVID-19 in the PubMed database between January and February 2021 were discussed

    Optical solitonic structures with singular and non‑singular kernel for nonlinear fractional model in quantum mechanics

    Abstract The present study examines the nonlinear time-fractional model in the sense of a solitonic structure. A non-linear Schrödinger equation has applications in light scattering, indirect optical pulses as well as planer waves and to Bose-Einstein condensates enclosed in an anisotropic-shaped cigar, in a mean-feld state, etc. A new extended direct algebraic method is utilized to get the soliton solutions with modifed M-truncated and Atangana–Baleanu fractional operators which have Mittag-Lefer kernel. The obtained solutions contain new families of functions such as trigonometric, hyperbolic, rational, and exponential functions. The graphical 2D, 3D, contour, and also 3D spherical presentation pictorial the analysis with the feasible parametric values. On the evidence of the acquired solutions, it can be presumed that this technique is more efective and generalized to obtain solutions of many other non-linear partial diferential equations that appear in diferent scientifc disciplines

To compare the effects of biofeedback sensor and conventional physiotherapist (PT) feedback during core stabilization and activity training in patients with CNLBP. Thirty-eight patients with CNLBP were randomly assigned to Biofeedback (n = 19) or PT feedback (n = 19) groups. Patients continued 12 sessions of combined core stabilization and activity training. An auditory and tactile biofeedback was given using a validated tilt sensor integrated with an application in the Biofeedback group. An experienced PT provided verbal and tactile feedback to maintain the neutral position in the PT Feedback group. The outcomes were; disability (Revised Oswestry Disability Index-RODI), muscle activity (m.transversus abdominis and m.multifidus), pain (Visual Analog Scale-VAS), proprioception error of the trunk, patient beliefs (Fear Avoidance Beliefs Questionnaire-FABQ) and presence of depressive symptoms (Beck Depression Index-BDI), and quality of life (Short Form (SF)-36). The main effect of time were statistically significant on VAS, RODI, m.transversus abdominis and m.multifidus muscle activities, flexion, and extension proprioception error of the trunk, FABQ, BDI, and SF-36 scores in Biofeedback and PT feedback groups (p < 0.05 for all). The time X group interaction was significant on flexion and extension proprioception error of the trunk PT feedback group (consecutively; p = 0.004, p = 0.022).     Optical soliton solutions of the stochastic perturbed Radhakrishnan-Kundu-Lakshmanan equation via Itô Calculus

    Abstract This study presents, for the first time, optical solitons of the stochastic perturbed Radhakrishnan-Kundu-Lakshmanan equation with Kerr law nonlinearity in the presence of chromatic and spatio-temporal dispersions. The stochastic form involves multiplicative white noise in the Ito sense, besides; the Kudryashov and the new Kudryashov methods are picked to analyze. The analysis of the stochastic soliton solutions of the Radhakrishnan-Kundu-Lakshmanan equation and the impact of noise on these solitons are the primary motivations for choosing both of these techniques rather than obtaining many solitons. Therefore, the first goal is to obtain the most basic soliton types, bright and dark solitons, and the second goal is to observe the white noise effect on these solitons. By applying the proposed methods, bright and dark solitons are obtained, and the noise effect on these solitons is illustrated using both 3D and 2D graphic presentations, along with necessary comments. The presentation of the examined model for the first time in this article reflects its originality in terms of contributing both the study and the obtained results to the literature

    The effects of online exercise training on physical functions and quality of life in patients with pediatric-onset multiple sclerosis

    A B S T R A C T Background: Patients with pediatric-onset multiple sclerosis (PwPOMS) frequently experience motor, sensory, and cognitive problems. Although exercise is known to be effective in adult patients with MS, there are still no studies investigating the effectiveness of exercise in PwPOMS. To examine the effectiveness of online exercise training on physical activity, muscle strength, functionality, gait, fatigue, and quality of life in PwPOMS. Methods: Twenty-one individuals were included and randomly divided into two groups. The online exercise training program (OETP) group received exercise training including aerobics, strengthening, and balance training for 8 weeks, and the control group received no intervention. Outcomes were assessed at baseline, 8 weeks, and 32 weeks. Results: Significant improvements were recorded in physical activity, muscle strength, functionality, gait, fatigue, and quality of life in the OETP group after treatment (p<0.05). Between groups, the OETP group was superior to the control group in terms of physical activity, muscle strength, functionality, and quality of life (p<0.05). The OETP group remained superior to the control group in follow-up. Conclusion: OETP performed under the supervision of a physiotherapist is effective in PwPOMS. Even if these patients have no disabilities, it would be beneficial to refer them to rehabilitation from an early perio

    Prediction of students’ perceptions of problem solving skills with a neuro-fuzzy model and hierarchical regression method: A quantitative study

    Traditionally, students’ various educational characteristics are evaluated according to the grades they get or the results of their answers to the scales. There are some limitations in making an evaluation based on the results. The fuzzy logic approach, which tries to eliminate these limitations, has recently been used in the feld of education. While applying the fuzzy logic method to education, students’ qualifcations are determined qualitatively without using formulas in calculating student performance. However, fuzzy systems lack learning abilities. By combining fuzzy rules and neural networks, the evaluation tool will have greater adaptability to changing conditions. Thus, an educationally robust and easy-to-use assessment tool will be obtained. In this study, in the frst stage, students’ perceptions of problem solving skills, which is one of their educational characteristics, were modeled with the ANFIS approach, which is one of the neuro-fuzzy systems apart from traditional methods, through creative problem solving features. ANFIS is an adaptive network that allows neural network topology to be combined with fuzzy logic. It not only incorporates the benefts of both strategies but also eliminates some of their drawbacks when used alone. The inputs of the research were determined as students’ creative problem-solving characteristics and the output was their perceptions of problem-solving skills. As a second step, statistical methods (correlation and hierarchical regression) were used to examine whether there was a relationship between students’ PoPS skills and CPS characteristics. Afterwards, students’ artifcial PoPS skill scores obtained with ANFIS in the frst step and real PoPS skill scores obtained from their answers to the scale were compared. 360 students from Turkey took part in the study. Depending on the fndings of the study, real PoPS scores and artifcial ANFIS PoPS scores do not statistically difer signifcantly. Therefore, the ANFIS results based on creative problem solving features accurately predict students’ PoPS scores. Additionally, there is a clear relationship between PoPS talents and CPS features. One of the study’s most startling conclusions is that the environment, which is accepted as one of the components afecting creative problem solving in this research, predicts students’ perceptions of problem solving skills. These results also prove that the variable of creative problem solving characteristics, which is used to predict students’ perceptions of problem solving, is an appropriate variable.     ABSTRACT Objectives: This study aimed to determine and classify the reasons for cone-beam computed tomography (CBCT) imaging requests in the Oral and Maxillofacial Radiology Department, Istanbul University Faculty of Dentistry, and to examine which reasons more commonly require CBCT imaging. Material and methods: From the local CBCT database, 956 pediatric patients gave their consent. Subsequently, age, gender, the reason for referral, a field of view (FOV), and referral departments were collected. Patients were grouped according to age distribution as 1-6 years, 7-12 years, and 13-17 years. Descriptive and comparative statistical analysis was performed. Results: When CBCT referral reasons were analyzed, some of the most common requests were bone pathology (26.25%), impacted teeth (19.87%), dental anomalies (14.12%), cleft lip, and palate (13.91%), respectively. When referral reasons are evaluated according to age groups, dentoalveolar trauma (p=0.049), impacted teeth (p=0.000), dental anomalies, surgical applications (p=0.021), bone pathology (p=0.004), cleft lip and palate statistically significant differences were found between the (p=0.000) indications and age groups (p=0.000). Conclusion: This study can guide dental practitioners in referring pediatric patients for three-dimensional imaging.ÖZ Amaç: Bu çalışmada diş hekimliği fakültemizde konik ışınlı bilgisayarlı tomografi (KIBT) görüntüleme isteklerinin nedenlerinin belirlenmesi, sınıflandırılması ve hangi nedenlerin daha sık KIBT görüntüleme gerektirdiğinin incelenmesi amaçlanmıştır. Gereç ve Yöntem: KIBT veri tabanından 956 pediatrik hasta incelenmiştir. Daha sonra yaş, cinsiyet, sevk nedeni, görüş alanı (FOV) ve sevk departmanları toplanmıştır. Hastalar yaş dağılımına göre 1-6 yaş, 7-12 yaş ve 13-17 yaş olarak gruplandırılmıştır. Tanımlayıcı ve karşılaştırmalı istatistiksel analiz yapılmıştır. Bulgular: KIBT sevk nedenleri incelendiğinde en sık başvurulan istekler sırasıyla kemik patolojisi (%26,25), gömülü dişler (%19,87), diş anomalileri (%14,12), yarık dudak ve damak (%13,91) idi. Sevk nedenleri yaş gruplarına göre değerlendirildiğinde, dentoalveolar travma (p=0,049), gömülü dişler (p=0,000), diş anomalileri (p=0,000), cerrahi uygulamalar (p=0,021), kemik patolojisi (p=0,004), dudak damak yarığı, endikasyonlar ve yaş grupları arasında (p=0,000) istatistiksel olarak anlamlı fark bulundu.     Construction of novel bright-dark solitons and breather waves of unstable nonlinear schrodinger equations with applications

    Abstract: The unstable nonlinear Schrödinger equations (UNLSEs) are universal equations of the class of nonlinear integrable systems, which reveal the temporal changing of disruption in slightly stable and unstable media. In current paper, an improved auxiliary equation technique is proposed to obtain the wave results of UNLSE and modified UNLSE. Numerous varieties of results are generated in the mode of some special Jacobi elliptic functions and trigonometric and hyperbolic functions, many of which are distinctive and have significant applications such as pulse propagation in optical fibers. The exact soliton solutions also give information on the soliton interaction in unstable media. Furthermore, with the assistance of the suitable parameter values, various kinds of structures such as bright-dark, multi-wave structures, breather and kink-type solitons, and several periodic solitary waves are depicted that aid in the understanding of the physical interpretation of unstable nonlinear models. The various constructed solutions demonstrate the effectiveness of the suggested approach, which proves that the current technique may be applied to other nonlinear physical problems encountered in mathematical physics
