    L'Health Technology Assessment è una logica al servizio della governance sanitaria: un modello condiviso di valutazione del valore che le tecnologie sanitarie offrono alla prevenzione, diagnosi, cura e riabilitazione sanitaria. Il testo ripercorre i modelli proposti dagli scholar, descrivendo le dimensioni, riconosciute in letteratura, da presidiare nella valutazione delle innovazioni tecnologiche, sia essa rapida, quick and dirty, o completa, proponendo anche il supporto finale alla presa di decisione, derivato dalla Multi Criteria Decision Analysis. Il Centro sull'Economia e il Management nella Sanità e nel Sociale di Liuc Business School ha sempre operato con tale approccio, distinguendosi nel panorama italiano, unendo tecniche di ingegneria gestionale a pragmatismo della valutazione economica e prospettiva manageriale del payer istituzionale. È proprio questa flessibilità e sintesi multidisciplinare il valore dell'opera, ossia l'integrazione di competenze e prospettive tipiche dei bisogni di professionisti e organizzazioni sanitarie moderne.     An organization-as-platform and a strategy-as-practice for an electronically supported booking service in healthcare

    In Chapter 6, Resca starts from the consideration, suggested in the organizational and management literature, that bureaucratic structures have failed to manage businesses in turbulent markets or those that are subject to rapid technological innovation. Drawing from the case of the introduction of the first e-booking centres to access secondary care in Bologna in the late 1980s and early 1990s, this chapter proposes the concept of platform organization as a meta-organization able to deal with these changing markets. The author suggests that, although the platform organization concept offers a macro perspective, it falls short of shedding light on micro aspects related to such endeavours as the introduction of e-booking centres. Strategy-as-practice (or strategizing) is introduced to outline a model that includes both the macro and micro level

    Diabetic macular edema, innovative technologies and economic impact: new opportunities for the Lombardy Region healthcare system?

    Purpose: Diabetic macular edema (DME) is a leading cause of vision loss and blindness. The aim of this study was to evaluate the economic benefits of introducing additional alternative technologies (Dexamethasone intravitreal implant - DEX - and Aflibercept injections), compared with the historical scenario of Ranibizumab intravitreal injections. Methods: A 3-year budget impact model was developed, taking into consideration the perspective of the Lombardy Region Healthcare Service (LRHS). Total administration costs (real-life data retrieved from clinical practice at three Departments of Ophthalmology) as well as costs related to the management of potential adverse events (information collected from the literature) were analysed. Results: Over a 36-month horizon, the results showed that a higher consumption of DEX could lead to significant economic savings for the Regional Healthcare Service, ranging from a minimum of -4.35% (if DEX were used only in the second-line of treatment) to a maximum of -12.97% (if DEX were used in both the first-line and second-line), including the potential impact of adverse events. Therapy costs with Aflibercept and Ranibizumab were similar. Conclusions: This study demonstrates that concentrating all eligible patients within the Ranibizumab regimen is unlikely to represent a cost-effective strategy. Indeed, significant economic advantages would be achieved by introducing the other licensed alternatives, Dexamethasone implant and Aflibercept, thus optimising DME Italian healthcare expenditure. The results demonstrate DEX as an advantageous technological alternative for the target population affected by DME, both as a first- and second-line treatment option, reducing the economic burden of the pathology for the Regional/National Health Service

    Model-based clustering of high dimensional data using copulas

    Finite mixtures are applied to perform model-based clustering of multivariate data. Existing models do not offer great flexibility for modelling the dependence of the data since they rely on potential undesirable correlation restrictions and strict assumptions on the marginal distribution. We proposed recently a model-based clustering method via R-vine copula that allows overcoming the previous restrictions by building flexible dependence models for an arbitrary number of variables using bivariate building blocks. This method shows a disappointing behavior in highdimensional spaces since it leads to over-parametrized models. We propose a more parsimonious version of model-based clustering method via R-vine copula to alleviate the computational burden and the risk of overfitting. The model is based on the selection of the hyper-parameters of sparse model classes using truncated and thresholded R-vine copulas.     Oggi in Italia, una donna su otto si ammala di tumore alla mammella nel corso della sua vita, con un'incidenza di circa 48.000 nuovi casi ogni anno (AIRC, 2013, Ministero della Salute, 2016). In questo contesto, la prevenzione, e tutto ciò che consentirebbe di diagnosticare il tumore in una fase ancora iniziale di sviluppo, è di primaria importanza. A tal proposito sono stati attivati percorsi di screening in tutto il contesto nazionale, spesso diversi a seconda delle disponibilità interne delle aziende sanitarie stesse, dalle differenti visioni proposte dai responsabili di questo processo o dalle forme organizzative peculiari. Pertanto quale sia il percorso organizzativamente e gestionalmente migliore per le pazienti non è un fattore chiaramente determinato e completamente evidente. A tal proposito, oggetto di questo lavoro di ricerca è la valutazione del potenziale ruolo dell'introduzione dello strumento diagnostico ABUS InveniaTM, indicato come una tecnologia aggiuntiva alla sola mammografia per l'effettuazione dello screening del tumore al seno, in grado di rilevare più cancri rispetto alla mammografia tradizionale. Il presente report, dunque, propone, dopo aver definito uno scenario alternativo che prenda in considerazione l'effettuazione di mammografia / di una ecografia e di un controllo con la tecnologia ABUS, i risultati derivanti da una Budget Impact Analysis a tre anni che confronti la situazione AS IS con uno scenario innovativo, che prevede l'aggiunta di ABUS in affiancamento all'esame mammografico, nei programmi di screening organizzato, prendendo in considerazione non solo i costi propri della campagna di screening, ma anche i costi dei percorsi che affrontano successivamente le donne alle quali è stato diagnosticato un tumore. La prima fase ha previsto un modello di studio interventistico, in collaborazione con la Fondazione IRCCS Istituto Nazionale dei Tumori, di modifica del percorso diagnostico di screening tradizionale, prevedendo l'effettuazione di esami con ABUS, al tradizionale controllo ecografico, con l'obiettivo di valutare l'efficacia della procedura innovativa, comprovandone la sua non-inferiorità, rispetto alle altre metodiche di screening. La seconda fase ha previsto la strutturazione di un modello di Budget Impact, assumendo il punto di vista del Servizio Sanitario Regionale Lombardo, considerando un orizzonte temporale di 12, 24 e 36 mesi. La popolazione presa in considerazione proviene da una valutazione dei dati di prevalenza e incidenza della patologia a disposizione all'interno del contesto regionale lombardo. Nella Budget Impact Analysis si sono considerati tre differenti percorsi di screening (AS-IS, IDEALE E TO BE) correlati alle tre tecnologie alternative di mammografia, ecografia e ABUS InveniaTM alle quali è stata aggiunta la parte dei percorsi delle donne che hanno sviluppato tumore. Lo studio interventistico ha dimostrato come ABUS abbia riconosciuto tutti i tumori identificati anche dall'ecografia tradizionale, comprovando la non-inferiorità della metodica. Da un punto di vista economico, invece, l'introduzione di ABUS sia uno scenario ampiamente sostenibile, per il Servizio Sanitario Regionale, e che dovrebbe essere seriamente preso in considerazione dati i limiti esistenti nei programmi di screening attualmente adottati. Nella fase di screening, complessivamente, nell'arco dei tre anni, lo scenario IDEALE è quello che assorbe più risorse (+41% rispetto al caso AS-IS) seguito dal caso TO BE (+39,5% rispetto al caso AS-IS). Nella fase di presa in carico e cura del tumore, considerando globalmente i tre anni, è possibile notare come lo scenario TO BE sia quello economicamente più conveniente che permette un saving del 16% rispetto allo scenario AS-IS pari a più di 120 milioni di euro. Prendendo in considerazione entrambe le fasi, si è potuto osservare come l'assorbimento di risorse globale nei tre anni assume il valore più elevato nello scenario IDEALE (+2% rispetto allo scenario AS-IS), mentre lo scenario TO BE è quello più economico (-13% rispetto allo scenario AS-IS), che permetterebbe un risparmio di più di 105 milioni di euro

    The subjective side of DiDIY: the profile of makers in network marketers communities

    Purpose. The purpose of this paper is to focus on the profile of DiDIYers and the antecedents of digital do it yourself (DiDIY) in Network Marketing Direct Selling Organizations (NMDSO). Design/methodology/approach. A two-step research design allowed the detection of DiDIYers strictu sensu (i.e. digital makers). After collecting data about the attitudes and the actual use of digital technology (DT) by network marketers through an online survey, an open-ended interview technique made it possible to analyze the personal characteristics of DiDIYers, the motivations that drive them and the activities that mainly foster the creation of artifacts in the DT domain. Besides, it allowed to recognize the perceived benefits and the impact of DiDIY output on networkers’ downline and firms’ performance. Findings. The results provide a preliminary profile of the DiDIYer in network marketing communities that is suitable for further comparative studies. More specifically, this exploratory study acknowledges the most important antecedents of DiDIY in digital literacy – including the awareness of being a digital literate and/or a potential appropriateur – and in organizational culture. Research limitations/implications. This is an exploratory study based on a limited sample of DiDIYers, nevertheless it offers a preliminary view of the subjective side of the DiDIY phenomenon in network marketing and represents as well a context-bound study. Practical implications. Considering the ascribed benefits of DiDIY output on the traditional network marketing objectives (effectiveness, efficiency, cohesion, mutual assistance and reinforcement) network marketing communities could benefit from an increase of attention to the topic. Originality/value. The study throws light on the process of DiDIY within direct sales and network marketing activities by defining a preliminary profile of digital makers and thus, underlining a phenomenon neglected so far

    The Marriage of Figaro and the sunset of Ancien Régime legacy on modern legal culture

    The purpose of this abstract is focused on the character of the Count of Almaviva as representative of the transition from the Ancien Régime to the Nouveau Régime. Despite the fact that the opera plot is focused on the adventures of Susanna and Figaro in order to get married, its legal pivot is on the feudal lord who seeks to enforce his status and his rights, first of all his "ius primae noctis" on Susanna, his wife's lady-in-waiting. He tried to seduce her for all the opera but he fails, proving himself ridiculous and surpassed. From the private law perspective, the Count exploits Figaro's promise of marriage to Marcellina in the event of the breach of a debt incurred some time before. Despite his conflict of interest because he is the master, the Count tries to mislead the judgement of his subordinate, unsuccessfully, due to the parent–child relationship between Marcellina and Figaro themselves. However, he does not give up and the final double exchange of role keeps him in a ridiculous situation to obtain forgiveness from his wife, the Countess, quite a proto-feminist in this opera. From the public law perspective, the crisis of the figure of the Count of Almaviva clearly represents the decline of aristocracy itself. None of his subjects are afraid of him. For instance, Cherubino escapes the mandatory enrollment in the Count's regiment by dressing up as a girl. This is not only of scenic value, but it could also represent a crisis of social roles on the eve of the French Revolution. The Marriage of Figaro represents an indispensable inspiration for historical and multidisciplinary reflections to analyze the paradigm shifts which occurred in the late seventeenth century and the legacy they have left on today's legal culture
