    Investigation of Nano Spray-Dried, Hyaluronic Acid-Modified Polymeric Micelles for Nasal Administration

    Background/Objectives: The combination of nanomedicine with nasal administration is of paramount importance in current research and development. Polymeric micelles coated with hyaluronic acid may be a suitable solution to enhance drug release and permeation whilst properly adhering to the nasal mucosa, increasing residence time. Methods: Solid state characterization included morphology and laser diffraction-based size analysis and X-ray powder diffraction. The characterization of dispersed polymeric micelles in aqueous media was performed based on dynamic light scattering and determining the solubility enhancement related factors such as encapsulation efficiency and thermodynamic solubility. In vitro nasal drug release and permeability studies were also conducted to characterize the different hyaluronic acid-modified polymeric micelles. Quantitative measurements were carried out via liquid chromatography. Results: Concentration dependence on hyaluronic acid was found during all measurements, with one formulation candidate overcoming the others. With a high yield above 80%, monodispersed particles were formulated with an approximately 4 µm particle size in uniform distribution and spherical morphology. The small micelle size (107.3 nm) in uniform manner led to a high encapsulation efficiency above 80% and released the drug amount above 70% in 15 min. High drug permeation was also achieved compared with the initial active substance by itself. Conclusions: A value-added polymeric micelle formulation was developed with rapid drug release and permeation kinetics alongside its high mucoadhesion

    Impact of Collagen Membrane in Vertical Ridge Augmentation Using Ti-Reinforced PTFE Mesh: A Randomised Controlled Trial

    AimsThis non-inferiority randomised clinical trial aimed to compare vertical bone gain (VBG), volumetric bone changes and incidence of complications after vertical ridge augmentation (VRA) using perforated titanium-reinforced dense-polytetrafluoroethylene (PTFE) mesh covered by a collagen membrane (CM) or used alone.Materials and MethodsThirty patients with vertical bone defects were randomly assigned to receive VRA with either PTFE + CM or PTFE alone. Meshes were removed after 9 months. Clinical assessments included complication rates, pseudo-periosteum type and bone density. VBG, effective regeneration rate and the need for additional augmentation were evaluated using CBCT reconstructions.ResultsNon-inferiority of PTFE alone compared with PTFE + CM was not demonstrated for absolute and relative VBG (4.5 +/- 2.1 mm vs. 4.1 +/- 2.7 mm, 79.2% +/- 16.6% vs. 85.8% +/- 10.6%, respectively), effective regeneration rates (69.3% +/- 17.9% vs. 72.3% +/- 16.4%, respectively) or complication rates (6.7% in both groups). A higher incidence of type 1 pseudo-periosteum was observed in the PTFE + CM group.ConclusionThe non-inferiority of PTFE alone compared with PTFE + CM for absolute VBG was not established. However, both techniques led to comparable outcomes for VBG, complication rates and bone density.     Magyarország Eurotransplant teljes jogú tagságának első 10 éve [The first 10 years of Hungary's full membership of Eurotransplant] : Nemzetközi szervcsere 2013 és 2023 között [International organ exchange between 2013 and 2023]

    Bevezetés: Magyarország Eurotransplanthoz történő csatlakozásának célja volt, hogy javítsuk a magyar betegek szerv- hez jutási esélyeit, valamint a graft- és a betegtúlélést. Célkitűzés: Az Eurotransplant-együttműködés eredményeit magyar szempontból vizsgáltuk, ideértve a csatlakozáskor megfogalmazott célok megvalósulását is. Módszer: Adatelemzés történt a teljes jogú csatlakozást követő 10 év (2013-tól) abszolút és az egyes éves átlagok adataival, valamint a vizsgált periódus adatainak összevetése történt az Eurotransplant-csatlakozást megelőző 5 év, illetve amennyiben az adatok elérhetők voltak, 10 év adataival. A vizsgált időszak vonatkozásában elemeztük a Magyarországon elhunyt szervdonorok, többszervi donorok, adományozott szervek és transzplantációk számát és körülményeit, valamint a várólisták és a nemzetközi szervcserék fejlődését. Eredmények: Az Eurotransplanthoz való csatlakozást követő 10 évben a donorjelentések száma 17%-kal csökkent, azonban a koronavírus-világjárvánnyal érintett évek adatait nem számolva a megvalósult donációk száma 12,53%-kal nőtt. A legjelentősebb javulás a többszervi donációk arányában mutatkozott (40,38% vs. 71,23%). A vizsgált időszak- ban 18,95%-kal több transzplantáció történt (3325 vs. 3955). A hazai transzplantációk 23,49%-ában külföldi szervet használtak fel. Az új páciensek száma 25,16%-kal nőtt a várólistán (évi átlag 461 vs. 577), miközben csökkent a vára- kozási idő. Vesék esetében a 0-tól 3 HLA-mismatch-ig terjedő egyezési arány 10,7%-kal növekedett, miközben a nagyobb mismatch-arányok 10,9%-kal csökkentek. Az Eurotransplant-együttműködés első 10 évében 801 extra élet- évet adtunk vesetranszplantált betegeinknek. A megduplázott létszámú akut listáról végrehajtott transzplantációk száma csaknem megháromszorozódott. Megbeszélés: Az Eurotransplanthoz történő csatlakozás hatékony és eredményes döntésnek bizonyult, amely a magyar várólistán lévő páciensek számára növelte a szervhez jutási esélyeket és javította a transzplantációt követő életkilátá- sokat. Következtetés: Magyarországon a szervhez jutási esélyek javultak a szervadományozás elérhetőségének tekintetében a kórházi koordinátorhálózat kialakítása és az Eurotransplanthoz való csatlakozás révén. A megvalósulási és transzplan- tációs arányok javulása is arra utal, hogy a nemzetközi szervcsere segíti a donorszervek hatékonyabb allokációját an- nak érdekében, hogy ne veszítsünk el transzplantációra alkalmas szerveket

    Measuring early vascular aging in youth: an expert consensus document from the Youth Vascular Consortium

    Since the conceptualization of early vascular aging (EVA) in 2008, significant efforts have been made to develop and improve its assessment. Initially lead by the investigation of arterial stiffness through pulse wave velocity (PWV), several additional vascular aging biomarkers have gained prominence in recent years. Despite expanding literature addressing methodological concerns associated with these biomarkers in youth, a standardized approach for clinical evaluation of EVA remains elusive, leaving pertinent gaps in understanding the optimal methodology. This article, resulting from international consensus efforts from the Youth Vascular Consortium, aims to provide an updated overview of methods available to measure EVA in youth and to discuss challenges in translating these methods into clinical practice.     Omega-3 Fatty Acid Consumption Alters Uterine Contraction: A Comparative Study on Different Breeds of Rats

    Polyunsaturated fatty acids (PUFAs) play roles in several physiological and pathophysiological processes, but their effects on reproductive function are controversial. The aim of the study was to investigate the effect of n-3 PUFA-rich fish oil and n-6-rich sunflower oil on sex hormone status, in vivo and in vitro uterine contractility, and endometrial remodeling. Female Sprague Dawley, Lister hooded, and Wistar rats were treated orally for 20 days with 1 mL of tap water, sunflower oil, or fish oil. Blood samples were taken for gonadotropic and sex hormone analysis. In vivo smooth muscle contractions were measured weekly by electromyography. Isolated uterine and cecal contractions were measured after sacrificing the animals. Endometrial remodeling was detected based on the presence of αvβ3 integrin by optical imaging. In Sprague Dawley rats, fish oil increased the LH level and progesterone/estradiol (P4/E2) ratio compared to the sunflower oil-treated group. Uterine contractions were reduced both in vitro and in vivo. Endometrial αvβ3 integrin activity was increased in the fish oil group. In Lister hooded rats, neither sunflower nor fish oil treatments modified the investigated parameters. However, in Wistar rats, both oils increased only the in vivo contractions and reduced the P4/E2 ratio, along with αvβ3 integrin fluorescence. n-3 PUFA-rich fish oil induces a breed-dependent effect on sex hormone status and uterine contractions in rats. The response to PUFA intake may vary significantly within a given species, which may have importance both in animal feeding and human nutrition

    Fatal Outcome of Intravenous Thrombolysis With an Unexpected Finding of Amyloid-β-Related Angiitis-A Case Report Highlighting a Relevant Scenario With Acute Focal Neurological Deficits and Minimal Radiological Presentation

    Cerebral amyloid angiopathy (CAA) has been implicated as a risk for developing lobar intracerebral hemorrhage (ICH) after intravenous thrombolysis (IVT) applied for acute ischemic stroke (AIS). However, there is a paucity of cases reported with histopathological CAA diagnosis in this setting, with a single report to imply the role of CAA-related inflammation (CAA-RI). We report clinical, radiological, and neuropathological observations of a 65-year-old woman who presented with acute left-hemispheric symptoms with an initially unrevealing cranial computed tomography (CT) and received IVT for presumed AIS. The course was rapidly complicated by a huge lobar ICH and a fatal outcome. The autopsy revealed severe CAA, unexpectedly with transmural CAA-RI, a.k.a. amyloid-β-related angiitis (ABRA), and histopathological evidence for vascular amyloid-β phagocytosis. Re-evaluation of initial imaging did not reveal signs of asymmetric confluent white matter edema characteristic of CAA-RI, but raised the suspicion of a tiny left central convexity subarachnoid hemorrhage, a substrate of amyloid spells. The genotype of the apolipoprotein E (ApoE) gene (ApoE) was ε3/ε3. Being the second published thrombolysis-associated fatality with ABRA and among the few with definite CAA, the present case confirms CAA/CAA-RI to be a potential hidden risk for IVT-associated ICHs, urging for awareness of CAA-associated pathologies and clinical-radiological hints in an AIS setting. The findings implicate the relevance of vascular Aβ phagocytosis in the pathogenesis, confirm that CAA-RI may present without prominent edema, highlight that CAA/CAA-RI-related focal neurological deficits (including amyloid spells) can be potential AIS mimics within the IVT time window, and urge for rigorous analysis of pre-IVT CT scans for even subtle sulcal hyperdensities suggesting cSAH/amyloid spell in elderly patients, prompting consideration of magnetic resonance imaging
