    Vitamin D Treatment Prevents Uremia-Induced Reductions in Aortic microRNA-145 Attenuating Osteogenic Differentiation despite Hyperphosphatemia

    In chronic kidney disease, systemic inflammation and high serum phosphate (P) promote the de-differentiation of vascular smooth muscle cells (VSMC) to osteoblast-like cells, increasing the propensity for medial calcification and cardiovascular mortality. Vascular microRNA-145 (miR-145) content is essential to maintain VSMC contractile phenotype. Because vitamin D induces aortic miR-145, uremia and high serum P reduce it and miR-145 directly targets osteogenic osterix in osteoblasts, this study evaluated a potential causal link between vascular miR-145 reductions and osterix-driven osteogenic differentiation and its counter-regulation by vitamin D. Studies in aortic rings from normal rats and in the rat aortic VSMC line A7r5 exposed to calcifying conditions corroborated that miR-145 reductions were associated with decreases in contractile markers and increases in osteogenic differentiation and calcium (Ca) deposition. Furthermore, miR-145 silencing enhanced Ca deposition in A7r5 cells exposed to calcifying conditions, while miR-145 overexpression attenuated it, partly through increasing α-actin levels and reducing osterix-driven osteogenic differentiation. In mice, 14 weeks after the induction of renal mass reduction, both aortic miR-145 and α-actin mRNA decreased by 80% without significant elevations in osterix or Ca deposition. Vitamin D treatment from week 8 to 14 fully prevented the reductions in aortic miR-145 and attenuated by 50% the decreases in α-actin, despite uremia-induced hyperphosphatemia. In conclusion, vitamin D was able to prevent the reductions in aortic miR-145 and α-actin content induced by uremia, reducing the alterations in vascular contractility and osteogenic differentiation despite hyperphosphatemia

    Valuing Human Impact of Natural Disasters: A Review of Methods

    This paper provides a comprehensive set of methodologies that have been used in the literature to give a monetary value to the human impact in a natural disaster setting. Four databases were searched for relevant published and gray literature documents with a set of inclusion and exclusion criteria. Twenty-seven studies that quantified the value of a statistical life in a disaster setting or discussed methodologies of estimating value of life were included. Analysis highlighted the complexity and variability of methods and estimations of values of statistical life. No single method to estimate the value of a statistical life is universally agreed upon, although stated preference methods seem to be the preferred approach. The value of one life varies significantly ranging from USD 143,000 to 15 million. While an overwhelming majority of studies concern high-income countries, most disaster casualties are observed in low- and middle-income countries. Data on the human impact of disasters are usually available in disasters databases. However, lost lives are not traditionally translated into monetary terms. Therefore, the full financial cost of disasters has rarely been evaluated.     Sensitivity and Specificity of Spanish Prehospital Advanced Triage Method (META)

    Introduction: Mass-casualty incident (MCI) triage systems aim to provide the best possible health care to the greatest number of affected people with the available resources in the context of a mass-casualty event. The Spanish Prehospital Advanced Triage Method (Modelo Extrahospitalario de Triaje Avanzado; META) was designed to improved patient sorting in MCIs. Objective: The aim of this study was to estimate and compare sensitivity and specificity of META and the Manchester Triage System (MTS) in MCIs by retrospectively applying both triage algorithms to real MCI patients from the Emergency and Disaster Research Unit (Unidad de Investigaci´on en Emergencia y Desastres; UIED) MCI database. Method: This was a comparative study of two triage methods using sensitivity and specificity with the Revised Trauma Score (RTS) as the gold standard. A total of 134 MCI patients from the UIED database were included. An MCI in Asturias is defined as an incident that involves four or more victims that require ambulance mobilization. Results: Patients mean age was 39.85 years (95% CI, 35.9-43.8) with an age range from one to 88 years old. In total, 54.4% of the patients were female. The most common types of MCI involved were fires (51.0%), followed by road traffic accident (43.3%) and street fight (3.7%). For MTS, the overall sensitivity was 30.6% (95% CI, 22.9-39.1) and specificity was 66.0% (95% CI, 60.0-71.7). For META triage algorithm, the overall sensitivity was 79.9% (95% CI, 72.1-86.3) and specificity was 89.9% (95% CI, 85.7-93.3).     The current state of preservation of the pyrotechnological structures in the Secano of the Alhambra is not always as clear as could be wished, owing to the aggressive and indiscriminate reconstructions to which they were subjected in the 1960s and 1970s. A better understanding of the structural features of these industrial facilities is essential for the overall understanding of the Nasrid and post-medieval industrial complexes and their integration in the madīna’s urban structure. In order to compile first-hand data about the existing remains, a two-stage programme was implemented (both in trenches 1 and 2 and in the remaining pyrotechnological structures scattered throughout the site). First, the extant structures were examined in detail, paying close attention to such characteristics as construction materials (brick, mortar) and construction techniques to ascertain the actual extent of medieval and post-medieval remains, and record them accordingly. These structures were examined stratigraphically in order to determine phases, reconstructions, and repairs, which can also be indicative of the industrial activities developed within them. Second, the furnaces and kilns were recorded photographically, prior to the production of photogrammetric 3D models, to allow for the remote examination of these structural eatures in great detail. Moreover, the 3D modelling was not limited to they rotechnological structures alone, but extended to the whole trenches, as an extra piece of information with which to examine the complex stratigraphy of both trenches

    Capítulo de libroEl catálogo que a continuación se presenta es una muestra de la Colección Cerámica de José Manuel Feito. Estamos ante el fruto de una vida de trabajos, de estudio, de protección y de amor por el patrimonio cerámico. La colección está nutrida por un importante número de cerámicas, más de 450 piezas, en su mayoría elaboradas en el Principado de Asturias, pero también de otros puntos, tanto de la Península Ibérica como de otras partes del mundo. A continuación, mostramos una selección de 52 piezas que abarcan dos mil años de historia de la cerámica en Asturias. Es una selección que ha buscado crear un discurso ceramológico, histórico y arqueológico, a la par que defi ne la colección y a José Manuel Feito como investigador. La estructura del catálogo ha tratado de seguir una organización cronológica ascendente. Las piezas están agrupadas en torno a su centro de producción y se ha analizado cada ejemplar de manera independiente.     Análisis desde la óptica metodológica de Michel Foucault de conceptos como el discurso, la educación y la configuración de las subjetividades de los sujetos y su indudable ligazón con las relaciones de poder y saber.The power relations that operate in all social spheres condition the organization of society through different devices and institutions, legitimising the behaviours and individualities considered appropriate, and therefore accepted, at a given historical moment. On the other hand, knowledge relations are inherent to power relations which they provide with a foundation in order to validate their assumptions, admitting as true a series of knowledge in a specific period. Educational institutions, due to their social character, are also traversed and conditioned by power and knowledge relations and by the discourses and practices that emanate from these relations. The analysis from Michel Foucault's methodological perspective of concepts such as discourse, education, and the configuration of the subjectivities of subjects and their unquestionable link with power and knowledge relations, are the subjects of analysis of this work.Las relaciones de poder que operan en todos los ámbitos sociales condicionan la organización de la sociedad a través de diferentes dispositivos e instituciones legitimando los comportamientos e individualidades consideradas adecuadas, y por lo tanto aceptadas, en un momento histórico determinado.     Estructural sobre Mi3: ejercicios para concretar la afinación interválica en la extensión del clarinete desde el Eje de Si4

    Amongst the skills that musicians who play non-temperate instruments should master are the capacity to adapt their tuning when playing in an ensemble. This article presents an exercise with which clarinet players can develop a solid tuning foundation which will facilitate adapting to other tuning systems. We propose to work extensively on the starting point note of E3 - fundamental note of the B flat clarinet’s tube- so we can extrapolate these proprioceptive sensations. We will then extrapolate these sensations to the note B4 -unison of A4, tuned in 442Hz-. This note will be considered the point of reference from which we will extract other correlative intervals that will form the basics of the clarinet registers, keeping its relative tuning and quality of sound. Key words: clarinet, didactic, tuning of intervals, fingerboard/diapasonEntre las principales competencias que debe alcanzar un músico de instrumento de afinación variable se encuentra la capacidad de adaptar su afinación de forma controlada a cualquier ensemble. El presente artículo plantea un ejercicio con el que los clarinetistas puedan construir una sólida estructura de afinación en toda la extensión de su instrumento desde la que flexibilizar a otros sistemas de afinación. Se propone trabajar en la construcción de un punto de partida sólido consciente sobre el Mi31 – fundamental del tubo del clarinete soprano afinado en Sib–, para después trasladar las sensaciones propioceptivas obtenidas. Sobre ellas, posteriormente se situará la correcta calidad y altura del sonido del Si4 -unísono del La4 del diapasón afinado a 442Hz-, nota que se constituirá en el Eje de referencia del que extraer todos los intervalos correlativos que formarán el esqueleto de los registros del clarinete, manteniendo su afinación relativa y calidad de sonido.
