    Motivation Affects Self-Efficacy Greater than Age, Sex, and Education in Diabetic Patients in West Coast Area of Java Island

    Self-efficacy and motivation are required for patient with type 2 diabetes in managing their diabetes. This study used observational analytic design with a cross-sectional approach. Samples recruited were 115 respondents selected by using purposive sampling method. The data analysis used logistic regression with significance level of α < 0.05. This study results showed that motivation more affects patient’s self-efficacy (p 0.001) after being controlled by age, sex, and education. Conclusion of this study was that the highest patient motivation is influenced by the patient’s desire to recover from his illness and support from the family. The recommendation of this study is how to develop self-efficacy and motivation to establish the adherence of diabetic patients to carry out their self-care management.     PENGALAMAN ADAPTASI REMAJA PASCA BENCANA GEMPA DI LOMBOK NUSA TENGGARA BARAT

    Pendahuluan: Kejadian bencana dapat menimbulkan kerugian baik dari aspek fisik, psikologis, properti dan lingkungan. Bencana mempengaruhi kesejahteraan psikologis dan kesehatan mental individu, baik orang dewasa maupun anak-anak dan remaja. Diperlukan proses adaptasi pasca bencana untuk mencapai respon yang adaptif bagi remaja sehingga stress pasca trauma tidak menjadi patologis. Tujuan dari penelitian ini adalah untuk mengeksplorasi pengalaman adaptasi remaja pasca bencana gempa di Lombok Nusa Tenggara Barat tahun 2018. Metode:penelitian ini menggunakan metode penelitian kualitatif fenomenologi dengan teknik wawancara mendalam kepada 18 orang remaja terdampak bencana gempa di Lombok Nusa Tenggara Barat, Analisis data yang digunakan mengacu pada sembilan langkah teknik analisis data collaizi. Hasil:hasil dari penelitian ini diperoleh 11 tema: 1) Perubahan cara bersosialisasi, 2) Perubahan peran, 3) Menggali kemampuan lain 4) Respon terhadap gempa, 5) Upaya mengatasi dampak bencana, 6) Sumber Dukungan, 7) Jenis Dukungan, 8) Makna kejadian bencana, 9) Harapan untuk Lombok, 10) Harapan pada diri sendiri, 11) Harapan pada pihak berwenang. Kesimpulan: Upaya menghadapi dampak gempa atau Strategi koping yang digunakan remaja berupa perubahan spiritual dan distraksi. Mendekatkan diri kepada tuhan dengan melakukan berbagai bentuk ibadah sesuai dengan agama yang di anut memiliki peranan penting untuk dapat beradaptasi dengan dampak yang ditimbulkan oleh bencana gempa pada remaja

    PERSEPSI PASIEN HIV/ AIDS MENJADI PENYEBAB LOST TO FOLLOW-UP TERAPI ARV

    Pendahuluan: Penderita HIV / AIDS harus mengonsumsi ARV setiap hari dan rutin kontrol setiap bulannya untuk mengambil ARV di rumah sakit yang membuat penderita beresiko memiliki perilaku lost to follow up. Tujuan dari penelitian ini adalah untuk mengetahui faktor – faktor yang mempengaruhi perilaku lost follow-up pada pasien HIV/AIDS dengan terapi ARV di RSUD Dr Soetomo Surabaya. Metode: Penelitian ini menggunakan desain deskriptif korelasi dengan pendekatan cross-sectional. Populasi sebanyak 135 pasien dan didapatkan 100 pasien dengan consequtive sampling. Variabel independen adalah perceived susceptability, perceived severity, perceived barrier to action, perceived benefit of action, cues to action, dan self-efficacy. Variabel dependen adalah perilaku lost to follow up. Data diperoleh dengan kuesioner dan dianalisis dengan Spearman’s Rho. Hasil: Terdapat hubungan antara perceived susceptability (p=0,002), perceived severity (p=0,025), perceived barrier to action (p=0,022), cues to action (p=0,011) dengan perilaku lost to follow up. Tidak terdapat hubungan antara perceived benefit of action (p=0,196) dan self-efficacy (p=0,071) dengan perilaku lost to follow up. Kesimpulan: Pengetahuan tentang pentingnya manfaat dari ARV perlu ditingkat kepada pasien dengan ARV serta kesadaran diri pasien untuk rutin kontrol dan mengonsumsi obat ARV. Peneliti selanjutnya disarankan untuk melakukan penelitian secara mendalam (penelitian kualitatif) terkait penyebab pasien memilih untuk melakukan lost to follow up, serta dapat menemukan kebaharuan atau aplikasi sebagai pengingat dan monitoring jadwal pasien untuk melakukan kontrol ke rumah sakit

    An Effect of Breath Dhikr on the Stress Level of Patients with Pulmonary Tuberculosis

    Introduction: Pulmonary tuberculosis is an infectious chronic disease that can cause a stress condition on patients, such as the symptoms of tuberculosis disease, the long treatment process with a large amount of drugs, daily activity disruption, job loss and death threats. Unresolved stress can lead to irritability, anxiety, negative thinking, despair and helplessness. Some even blame God. Objective: This study aimed to determine the effect of dhikr breath on the stress levels of patients with pulmonary tuberculosis in the public health centres of Tanah Kalikedinding Surabaya. Method: A quasi-experimental design method with pre- and post-control groups was used. There was a treatment group and a control group; each group consisted of 16 respondents. The data analysis was performed using the Wilcoxon Signed Rank test and the Mann Whitney U test with a significance level of p <0.05. results: The result of the Wilcoxon Signed Rank test showed that the stress level of the treatment group had significance with p=0.000. The control group had no significance; p=0.317. The Mann Whitney U test showed the differences in the stress levels post-intervention with p=0,000. Discussion: Dzikir breath was a positive stimulus that affected the cognator process according to Roy’s adaptation theory in the form of merging dhikr and breath, which is aimed at realizing God. Further research is expected to use cortisol measurements to obtain more accurate results. This should be performed at stress levels in other chronic disease patients. Conclusion: Breath Dzikir is a spiritual therapy that can change stress into eustress (positive stress) and decrease the stress level of pulmonary tuberculosis patients overall

    Disease with high stigma, leprosy has a totality effect in patient life.Families are components that a higher risk of contracting leprosy. Purpose of study to get prevention strategies for family caregiver that caring for leprosy patient based on culture in sampang district. A qualitative with phenomenology approach was applied in this study. We conducted the in-depth interview,group discussion forum and expert discussion to explore themes. Using the Collaizi analysis, 15 participants based on the saturation data were selected in this study. The results showed those 2 themes and 4 sub-themes in the study. The first themes such as discipline treatment with sub themes were medication adherence and prohibition during treatment.The second theme such as disease course with sub themes were perceived symptoms and treatment experience.Prevention family caregiver based in culture approach is needed for nurses and family caregiver,improve caring for leprosy patients’ ability

    Playing Cards Using the “Tepuk Nyamuk” Method Improves CognitiveFunction and Social Interaction in the Elderly

    Playing cards using the “tepuk nyamuk” system is a game using a standard 52-card deck to focus between the words spoken on the card that is to be issued while tapping the back of the hand between the players on the card. The study aimed to find out the effect of playing cards using the “tepuknyamuk” method on improving cognitive function and social interaction.This study was quasi-experimental with 30 respondents who were elderly individuals who experienced cognitive decline and social interactions. The independent variable was playing cards in the “tepuknyamuk” method and the dependent variables were cognitive function and social interaction. The results indicate that there was an influence from playing cards on cognitive improvement (p=0.000) and social interaction (p=0.000). Playing cards in the “tepuknyamuk” method has the positive effect of improving cognitive function and social interaction for older adults. Further studies are suggested to determine the effect playing cards on other cognitive component

    Behaviour model for diabetic ulcer prevention

    Purpose Diabetic ulcers are one of the complications that often occur in patients with DM. The aim is to develop a behaviour model for diabetic ulcer prevention by integrating Lawrance Green Theory and the Theory of Planned Behaviour. Methods An explanative observational design was used with a cross-sectional approach. The population consisted of DM patients who had underwent treatment at the internal medicine clinic of Sidoarjo District Hospital. The sample size of 133 respondents was obtained through purposive sampling. The data analysis used Partial Least Square. Results Predisposing factors (knowledge), supporting factors (availability of health facilities and accessibility of health resources) and driving factors (the role of health workers and family support) significantly influence the main factors (attitudes toward behaviour, subjective norms and perceptions of self-control) with a statistical T value>1.96. The main factor influences intention (T = 48.650) and intention influences behaviour (T = 4.891). Conclusion Intention is influenced by the attitudes toward behaviour, subjective norms and self-control perceptions. Good intentions can increase the preventive behaviour related to diabetic ulcers. Increasing the diabetic ulcer prevention behaviour can be done by providing regular education to both the patients and their families about diabetic ulcers and their prevention through the appropriate management of DM, lifestyle modification and regular foot care that requires active involvement from the family and health care workers

    Effect of trauma-focused cognitive behavior therapy on depression and the quality of life of the elderly in Indonesia

    Purpose – This study aimed to analyze the effect of Trauma-Focused Cognitive Behavior Therapy (TFCBT) on the depression level and quality of life of the elderly in an earthquake-affected district of North Lombok Regency, Indonesia. Design/methodology/approach – A Randomized Controlled Trial (RCT) with a population of two elderly groups living in an earthquake-affected district was used in this study. The intervention comprised giving TF-CBT. There were three research instruments applied to determine the variables, namely, the Clinician-Administered PTSD Scale (CAPS-5), the Geriatric Depression Scale 15 (GDS 15) and the World Health Organization Quality of Life-BREF (WHOQOL-BREF). All instruments were adopted and translated using back translation to Bahasa Indonesia, which is appropriate at the suggestion of the WHO. To investigate the effect of the intervention, we used hierarchical linear models (HLM) with intent-to-treat analysis. The patent parameter effect was tested using the Wald test (t-test) with a confidence Interval of 95 per cent. Findings – The final analysis applied CAPS-5 and showed that there was a decrease in the PTSD of the respondents, which was down to only 8 (17.8 per cent) in the post-test. The same situation happened for the depression variable (2.8 per cent) after it was assessed using GDS-15C. The Quality of life (WHOQoL-BREF) variable was divided into Physical (50.7), Psychological (57.1), Social (53.6) and Environmental (45.7). These components show there to be a significant result in terms of improving the quality of life of the elderly victims of the earthquake. The finding highlights that applying TF-CBT in the elderly population can significantly drop post-traumatic stress disorder and depression level and enhanced quality of life. Social implications – The intervention decreased the depression level and improved the quality of life of the elderly as found in the six- week follow-up. Longer training and integration with the structured local wisdom could be necessary to better address the mental health of the elderly affected by the earthquake. Moreover, strengthening the role of the family as the primary caregiver is required to improve the outcome. Originality/value – This is the first study that has attempted to use TF-CBT as a method of treatment for the elderly to decrease their depression and to increase quality of life among the Indonesian elderly who have experienced an earthquake. This paper provides knowledge on the effectiveness of TF-CBT that can be used by therapists to treat depression problems suffered by the elderly in a post-disaster area

    Indonesian nurses’ perception of disaster management preparedness

    Purpose: Using a quantitative approach, this study aims to assess Indonesian nurses’ perception of their knowledge, skills, and preparedness regarding disaster management. Methods: This study was a descriptive comparison in design. The research samples are Indonesian nurses working in medical services and educational institutions. The variables of nurses’ preparedness to cope with disaster victims were measured using the Disaster Preparedness Evaluation Tool (DPET), which was electronically distributed to all nurses in Indonesia. Data were analyzed using a statistical descriptive one-way Analysis of Variance (ANOVA) and t-test with a significance level of 95%. Results: In total,1341 Indonesian nurses completed this survey. The average scores of preparedness to cope with disasters, the ability to recover from disaster, and evaluation of disaster victims were 3.13, 2.53, and 2.46, respectively. In general, nurses surveyed in this study are less prepared for disaster management, and do not understand their roles either during the phase of disaster preparedness or in coping with a postdisaster situation. Conclusion: Nurses’ preparedness and understanding of their roles in coping with disasters are still low in Indonesia. Therefore, their capacity in preparedness, responses, recovery, and evaluation of disasters needs improvement through continuing education.     ‘It was Hard but it is Satisfying’: The Lived Experience of Older Adults Moving to a Residential Home (Griya Werdha) in Surabaya

    Relocation stress could occur in relocated older adults. The signs and symptoms often manifest as non-specific complaints. This study aimed to elicit the lived experience of relocated older adults in a residential home in Surabaya. Individual interviews were conducted following consent being given, in addition to MMSE, GDS and Family APGAR score tests. Seven residents aged between 60 to 80 years old voluntarily participated in this study. A phenomenological approach was used to develop the narrative findings following verbatim transcription and the work by Colaizzi (1976). Five themes emerged from the study: moving was a hard decision, feeling lonely in the first months, trying to accept changes and the feeling of being health-supported. These themes reflected that moving to a residential home was hard despite the voluntary decision. Eventually, they adapt by virtue of support from the other residents, their visiting families and the health staff.
