Frequency of Multi-Drug Resistance and Molecular Characteristics of Resistance to Colistin in Acinetobacter baumannii Collected from Patients in Intensive Care Units with Ventilator-Associated Pneumonia
Background: Acinetobacter baumannii is one of the most common causes of
ventilator-associated pneumonia (VAP) in patients hospitalized in ICU.
Multiple resistance has resulted in excessive use of Colistin antibiotic, which is
the latest treatment option for this bacterium. Therefore, the purpose of this
study was to determine the abundance of multi-resistance and molecular
characteristics of resistance to colistin among A. baumannii isolated from
patients that are infected with VAP and hospitalized in ICU of “Qazvin” and
“Masih Daneshvari” hospitals.
Materials and Methods: In this study, 200 A. baumannii isolates related to VAP
were collected from ICU of “Masih Daneshvari” (2012-2018) and “Qazvin”
(2017-2018) hospitals, from bronchoalveolar lavage & tracheal aspirate
specimens. Isolates were detected as A. baumannii by PCR with specific primers
of the blaOXA-51-like gene. Antibacterial susceptibility of isolates to colistin was
determined by the MIC method, and other antibiotics were examined by the
disk diffusion method, according to the CLSI criteria. Multi-drug resistance
(MDR) and extended–drug resistance (XDR) isolates were determined
according to standard definitions of the CLSI.
Results: All the isolates were susceptible to colistin. Moreover, they were
resistant to piperacillin, piperacillin-tazobactam, ceftazidime, cefotaxime,
ceftriaxone, amikacin, gentamycin, levofloxacin, co-trimoxazole, and
ciprofloxacin. Antimicrobial resistance rates for tetracycline and ampicillinsulbactam were 8.5% and 20%, respectively. All isolates were MDR and XDR.
All isolates were susceptible to colistin (MIC50=1 and MIC90=2 µg/ml). The
sequencing results did not show any point mutation in pmr CAB genes, and
mcr-1 gene was not detected in any isolates.
Conclusion: In this study, all A. baumannii isolates collected from VAP patients
were MDR and XDR. Although all isolates were susceptible to colistin, and this
agent seems the most appropriate antibiotic for treatment of VAP, colistin
resistance can become endemic in the world rapidly due to plasmid-mediated
mobile colistin resistance mcr genes
A case report of rhino‑facial mucormycosis in a non‑diabetic patient with COVID‑19: a systematic review of literature and current update
Background: COVID-19 disease may be associated with a wide range of bacterial and fungal infections. We report
a patient with COVID-19 infection who developed rhino-facial mucormycosis during treatment with corticosteroids.
Case presentation: A 59-year-old non-diabetic male patient was admitted with a diagnosis of COVID-19 based on
positive RT-PCR and CT of the lungs. Due to sever lung involvement, he was treated with methylprednisolone. The
patient was re-admitted to hospital, due to nasal obstruction and left side facial and orbital swelling, several days after discharge. In sinus endoscopic surgery, debridement was performed and the specimens were sent to pathology and mycology laboratories. A nasal biopsy showed wide hyphae without septa. The sequenced PCR product revealed Rhizopus oryzae. Despite all medical and surgical treatment, the patient died. In addition, the characteristics of patients with COVID-19-associated mucormycosis were reviewed in 44 available literatures. In most studies, diabetes mellitus was the most common predisposing factor for mucormycosis.
Conclusion: Our report highlights the need for assessing the presence of mucormycosis in patients with COVID-19
and also it shows that physicians should consider the potential for secondary invasive fungal infections in COVID 19case
Determinants of sleep habits in toddlers: Application of planned behavior theory
BACKGROUND: Sleep health is a prerequisite for mental and physical health. The family affects the sleeping habits of the
child.
OBJECTIVE: The purpose of this study was to investigate the theory of mothers’ planned behavior in predicting sleeping habits
of toddlers.
METHODS: The participants of this descriptive-analytical study were 120 mothers of 12- to 36-month-old children living in
Razan, Hamadan province, Iran. Data were collected in three stages in order to develop a theory-based tool. Semi-structured
qualitative interviews with mothers were conducted to investigate the application of theory in predicting mothers’ intention and
behavior (n = 25). The face and content validity of the questionnaire was confirmed by a panel of experts. Test-retest method
was used to determine the reliability of the questionnaire at 3 weeks interval (n = 25). Validity of Structural Convergence and
Divergence with Factor Analysis, Internal Consent with Cronbach’s Alpha and predictive validity was determined by multiple
regression coefficients (n = 120).
RESULTS: The mean age of the children was 22.8 ± 7.5 months, 55% (66 children) were boys, the mean score of the children’s
sleep questionnaire was 49 ± 6, and the children’s sleep duration was 9 ± 1 hour. Theoretical constructs have a positive and
direct correlation with each other and with children’s sleep habits, which demonstrates the validity of the theory in predicting
toddler sleep habits. Regression analysis showed that perceived behavioral control was the strongest predictor of sleeping habits
of toddlers
Outcomes of Trauma and Their Relationship With Pre-hospital Time Intervals in Patients Admitted to a Hospital in Qazvin, Iran
Background: Due to the importance of investigating the causes of trauma and functional indicators of pre-hospital emergency care.
Objective: This study aims to evaluate the outcomes of trauma and their relationship with pre-hospital time intervals in patients admitted to Shahid Rajaei Hospital in Qazvin, Iran.
Methods: In this descriptive study conducted in 2018, the medical records of 700 patients transported to Shahid Rajaei Hospital in Qazvin by Emergency Medical Services (EMS) ambulances were examined which were selected from 3200 cases using a systematic sampling method. We used a two-part questionnaire surveying demographic/clinical information of patients and their clinical outcomes.
Findings: Mean age of patients was 33.8 years and 62% were male. Median of on-scene time, transport time, and total prehospital time were 7, 29.3 and 37 minutes, respectively. In overall, 72.4% of transferred patients were discharged from the emergency department, 19.7% admitted to other wards, and 4.4% discharged voluntarily.
Conclusion: Time intervals of EMS in Qazvin city are suitable, and their provided pre-hospital services have acceptable quality
Repeated Transcranial Magnetic Stimulation and Nursing Care: A Review
Repeated Transcranial Magnetic Stimulation (RTMS) therapy is one of the modern treatment methods for various neurological and psychiatric diseases. With the clinical advancement of new therapies, there is a need to increase nursing knowledge, various practical skills, and new ways of organizing care. This study aims to review the rTMS, and the role of psychiatric nurses before, during and after rTMS. The search was conducted in IranMedex, PubMed, Elsevier, Google Scholar, Ovid, and SID databases using the keywords: Repeated transcranial magnetic stimulation, long-term potential, mental nerve, psychiatric nursing both in English and Persian. Initial search yielded 42 articles, of which 24 were selected for the review. Treatment with rTMS can cause significant treatment effects. It has low complications, and is non-invasive and painless. The patient is conscious during the treatment process and is safe from the effects of anesthetic drugs. This method is also effective for people who are resistant to treatment. The psychiatric nurses play an important role in rTMS and teach patients on the formation of realistic expectations and the establishment of systems that ensure the continuity of safety and care. Moreover, they can assess clinical problems and work with the patients to create a therapeutic environment

Development of a SYBR Green Multiplex Real Time PCR for Simultaneous Detection of Mycobacterium Tuberculosis and Nocardia Asteroides in Respiratory Samples
BACKGROUND፡ Nocardia asteroides and Mycobacterium
tuberculosis are worldwide-distributed bacteria. These infectious
agents can cause many infections in humans, especially in
immunocompromised individuals. Pulmonary infections are more
common and have similar clinical symptoms. Proper diagnosis
and treatment of these patients are important for accurate
treatment and could be lifesaving.
METHODS: In this study, a multiplex real-time PCR assay was
established for the simultaneous detection of the N. asteroides
and M. tuberculosis. Both this homemade multiplex real time
PCR and routine commercial tuberculosis tests were performed
on 150 pulmonary specimens collected from individuals suspected
to have tuberculosis.
RESULTS: From 150 specimens, 20 samples were acid fast
positive, 14 positives for M. tuberculosis by singleplex real time
PCR, 10 positives for N. asteroides by singleplex real time PCR
and 2 positives for M. tuberculosis and N. asteroides by multiplex
real time PCR whereas 14 samples were positive for M.
tuberculosis with commercial test. Differential diagnosis of
pulmonary tuberculosis is useful for their proper treatment.
CONCLUSION: Our test had good performance for differential
diagnosis of tuberculosis and nocardiosis. Therefore, it is
recommended to be used to diagnose such patient
