    Global incidence of helminthic contamination of vegetables, cucurbits and fruits: A systematic review and meta-analysis

    ABSTRACT Accidental ingestion of infective stages of helminths through consumption of contaminated vegetables and fruits causes a wide range of food-borne diseases in humans. This systematic review and meta-analysis evaluated the global incidence of helminthic contamination of vegetables, cucurbits and fruits. Several databases (Science Direct, Web of Science, PubMed, Scopus, and Google Scholar) were searched for literature published prior to November 2020. Overally, 184 articles (32 countries) met the inclusion criteria. Among these, 137 studies documented helminthic contamination in vegetables, 46 in cucurbits and 9 were in fruits. The pooled incidence (95% confidence interval) was 31% (26%–37%) for vegetables, 20% (14%–27%) for cucurbits and 20% (8%– 37%) for fruits. The highest incidence rate was found in the Western Pacific WHO region (54%; 9%–95%). The most prevalent parasitic agents were Ascaris lumbricoides eggs (12%; 9%–15%) and Strongyloides stercoralis larvae (12%; 8%–16%) in vegetables, S. stercoralis larvae (10%; 5%–15%) and Toxocara spp. eggs (10%; 3%–21%) in cucurbits, and Trichuris trichiura eggs (9%; 1%–22%) in fruits. The highest incidence rate was found to be associated with lower-middle income countries (34%, 28%–41%) and regions with a tropical rainforest climate (50%, 10%–91%). The potential role of vegetables, cucurbits, and fruits in the spread of helminthic parasites was revealed. Utilizing clean water for irrigation, proper washing and cooking of vegetables and improved sanitary practices can decrease the public health hazard regarding the consumption of vegetables, cucurbits, and fruits.     Finasteride in hospitalized adult males with Covid-19: A risk factor for severity of the disease or an adjunct treatment: A randomized controlled clinical trial

    Background: There is controversy about the efficacy of 5-alpha-reductase inhibitors in COVID-19 patients. Some assumed that finasteride might be a risk factor for deterioration and others proposed it as a possible adjunct treatment for moderate to severe COVID-19 infection in the elderly. Methods: We performed a randomized controlled clinical trial (registration ID IRCT20200505047318N1) on 80 hospitalized male patients aged ≥50 years diagnosed with COVID-19 pneumonia in a tertiary hospital in Qazvin (Iran) from April to July 2020. The patients were randomized into one of the 2 treatment groups using simple randomization. Treatment group patients underwent routine drug therapy and 5 mg finasteride once daily for 7 days. The primary endpoint was mortality rate and length of hospital stay (LOS), and secondary endpoints were peripheral capillary oxygen saturation, respiratory rate, and inflammatory markers changes. The study protocol was approved by the medical ethics committee of Qazvin University of Medical Sciences (registration ID IR.QUMS.REC.1399.080). Data were analyzed by statistical tests and SPSS version 25. Also, p<0.05 was considered to be statistically significant. Results: We found a significant difference on O2 saturation among the 2 study groups on fifth day compared with the admission time (p= 0.018). The results did not show significant differences in mortality rate (2.5% vs 10%; p= 0.166) and LOS (p= 0.866) between patients in the finasteride and the control group. Conclusion: A short course of finasteride administration partially improves O2 saturation but does not influence other outcomes in hospitalized male patients aged ≥50 years with COVID-19 pneumonia. Further research in a large scale with longer follow-up is required to help clarify the role of finasteride in this setting.     Role of Socio-demographic Factors in Predicting the Use of Communication Technologies by Older People in Iran

    Background: The use of communication technologies enables the older people to deal with limitations and problems of modern life. Objective: This study aims to determine the socio-demographic factors predicting the use of common communication technologies (computers, internet, ATMs, and mobile phones) by the elderly in Iran. Methods: In this descriptive analytical study conducted in 2017, participants were 165 older adults from Qazvin, Iran who were selected using a cluster sampling method. Data collection tools were a demographic checklist and researcher-made questionnaire for assessing the use of communication technologies. Descriptive tests and multivariate regression analysis were used for data analysis. Findings: The mean age of participants was 68.38±7.89 years. Majority of them reported that they do not use computer (n=127, 77%) and internet services (n=112, 67.9%). The results of multivariate regression analysis showed that age (β=-0.209, P=0.002), gender (β=-0.142, P=0.029), educational level (β=0.501, P=0.000), and financial status (β=0.142, P=0.023) were the predictors of the use of common communication technologies by the older adults. Conclusion: A small percentage of the elderly in Iran use computer and internet. Therefore, it is very important to hold educational programs for the elderly to get familiar with and use new communication technologies
