بررسی تاثیر تحریک الکتریکی نقاط فشاری L14 و L111 بر شدت درد و ناتوانی در زنان مبتلا به کمر درد و لگن در دوران بارداری
مداخله¬ی آموزشی مبتنی بر درمان شناختی-رفتاری بر سندرم پیش از قاعدگی در کارکنان زن مراکز بهداشتی - درمانی شهرستان رودبار Educational intervention based on cognitive-behavioral therapy on premenstrual syndrome in female staff of health centers in Rudbar
چکیده:
زمینه و هدف: تعداد زیادی از زنان در سنین باروری از علائم جسمی و روانی پیش از قاعدگی رنج می-برند، که به طورقابل توجهی بر زندگی روزمره و نقش محوری آنان در تحکیم خانواده و اجتماع تأثیر می گذارد. هدف این مطالعه تعیین تاثیر مداخله آموزشی مبتنی بر درمان شناختی-رفتاری در قالب تئوری خودکارآمدی و با استفاده از شبکه¬های اجتماعی بر سندرم پیش از قاعدگی در کارکنان زن مراکز بهداشتی- درمانی شهرستان رودبار است.
مواد و روش کار: مطالعه حاضر یک کارآزمایی تصادفی شاهد دار می باشد که برروی 120 نفر از کارکنان زن 20 تا 45 ساله مراکز بهداشتی- درمانی شهرستان رودبار در سال 1399انجام شد. نمونهگیری به روش داوطلبانه انجام گرفت که با تصادفی بلوک بندی به دو گروه 60 نفری مداخله و کنترل تقسیم شدند. دادهها توسط پرسشنامههای اطلاعات دموگرافیک، غربالگری سندرم پیش از قاعدگی و تعیین شدت علائم، کیفیت زندگی مرتبط با کار ، خودکارآمدی عمومی، مقابله با علائم سندرم پیش از قاعدگی، اضطراب و افسردگی بیمارستانی و تاثیر سندرم پیش از قاعدگی جمعآوری شد. آموزش در هشت هفته با برگزاری کلاس های آموزشی در واتساپ انجام گرفت. از آزمونهای توصیفی و تحلیلی جهت تجزیهوتحلیل دادهها استفاده شد.
یافتهها: نتایج حاکی از آن بود که مداخله اموزشی مبتنی بر رویکرد درمان شناختی – درمانی بر سندرم پیش از قاعدگی تاثیرگذار بود. قبل و بعد و سه ماه بعد از مداخله آموزشی در گروه مداخله، میانگین نمرهی شدت سندرم پیش از قاعدگی به ترتیب 85/23 ، 81/6 و 81/6 و در گروه کنترل 97/22 ، 87/22 و 28/23 بود. در بررسی شدت سندرم پیش از قاعدگی طبق نتایج آزمون کرویت ماچولی اختلاف معنی داری در زمان های مختلف در دو گروه کنترل و مداخله وجود داشت (000/0 = p 482/116 = F). بین گروه های کنترل و مداخله نیز از نظر متغیر شدت سندرم پیش از قاعدگی اختلاف معنی دار وجود دارد (000/0 = p).
نتیجهگیری: نتایج به دست آمده حاکی از آن بود که آموزش مبتنی بر درمان شناختی – رفتاری بر سندرم پیش از قاعدگی در کارکنان زن تاثیر داشته و منجر به کاهش نشانگان جسمی و روانی سندرم پیش از قاعدگی می گردد.

Abstract:
Educational intervention based on cognitive-behavioral therapy on premenstrual syndrome in female staff of health centers in Rudbar
Background and Aim: A large number of women of childbearing age suffer from premenstrual physical and psychological symptoms, which significantly affect their daily lives and their pivotal role in strengthening the family and society. The purpose of this study was to determine the effect of the intervention. Education based on cognitive-behavioral therapy in the form of self-efficacy theory and using social networks on premenstrual syndrome in female staff of health centers in Rudbar.
Methods: The present study is a randomized controlled trial that was performed on 120 female employees aged 20 to 45 years in Rudbar health centers in 1399. Sampling was done by voluntary method which was randomly divided into two groups of 60 intervention and control. Data were collected through demographic information questionnaires, premenstrual syndrome screening and symptom severity, work-related quality of life, general self-efficacy, coping with premenstrual syndrome symptoms, nosocomial anxiety and depression, and the effect of premenstrual syndrome. The training was conducted in eight weeks by holding training classes on WhatsApp. Descriptive and analytical tests were used to analyze the data.
Results: The results showed that the educational intervention based on the cognitive-therapeutic approach was effective on premenstrual syndrome. Before and after and three months after the educational intervention in the intervention group, the mean scores of premenstrual syndrome were 23.85, 6.81, and 6.81, respectively, and in the control group were 22.97, 22.87 and 23.28. . There was a significant difference in the severity of premenstrual syndrome according to the results of Machuli sphericity test at different times in the control and intervention groups (F = 116,482 p = 0.000).There was a significant difference between control and intervention groups in terms of the severity of premenstrual syndrome (p = 0.000).
Conclusion: The results showed that education based on cognitive-behavioral therapy has an effect on premenstrual syndrome in female employees and leads to a reduction in physical and mental symptoms of premenstrual syndrome.

Amniotic Membrane Transplantation for Persistent Epithelial Defects and Ulceration due to Pseudomonas Keratitis in a Rabbit Model
Purpose: The use of amniotic membrane has been suggested in the treatment of
infectious keratitis for its intrinsic anti-infective properties probably mediated by its antiinflammatory effects. The aim of this study was to investigate the effect of amniotic
membrane transplantation (AMT) along with ciprofloxacin to cure the primary stages of
Pseudomonas keratitis.
Methods: In total, 28 rabbits were selected and divided in four groups as follows: group
1 as control, group 2 with amniotic membrane, group 3 with ciprofloxacin, and group
4 with amniotic membrane combined with ciprofloxacin. About 0.05 cc suspension of
Pseudomonas aeruginosa, 27853 ATCC was injected into corneal stroma.
Results: The results showed groups of AMT, AMT + ciprofloxacin, and ciprofloxacin had
0% perforation while the control group had 85.6%. Average infiltration of 5.5 mm was
observed in ciprofloxacin group, 5 mm in AMT + ciprofloxacin group, 24 mm in AMT
group, and finally 23.75 mm for control. Amniotic membrane showed to be effective
in prevention of cornea perforation as well as remission of Pseudomonas keratitis.
There was no significant difference between ciprofloxacin groups in comparison with
ciprofloxacin + AMT group. However, regarding the anti-inflammatory effect, the process
of improvement of inflammation in ciprofloxacin + AMT group was faster.
Conclusion: Transplantation of amniotic membrane in the primary stages of
Pseudomonas keratitis treatment remarkably prevents the disease and it can be used
to control its process.

Phenotypic Identification and Genotypic Characterization of Plasmid‑Mediated AmpC β‑Lactamase‑Producing Escherichia coli and Klebsiella pneumoniae Isolates in Iran
One of the mechanisms of Klebsiella pneumoniae and Escherichia coli resistance to β-lactam antibiotics is the production of
β-lactamase enzymes. Among these are the AmpC β-lactamases, which confer resistance to a class of antibiotics. However,
little is known about the AmpC β-lactamases of K. pneumoniae and E. coli clinical isolates in Qazvin, Iran. This study was
designed to assess the AmpC β-lactamases-producing strains and also identify the prevalence of AmpC β-lactamases genes.
Antimicrobial susceptibility tests were performed on 435 K. pneumoniae and E. coli isolates using disk difusion technique.
Plasmid-mediated AmpC genes were studied using a multiplex PCR assay. The AmpC β-lactamase-producer isolates were
studied by employing cefoxitin disk difusion test, AmpC induction test, AmpC cefoxitin-EDTA test, and boronic acid
disk test. Our results showed that of 46 (18.4%) cefoxitin-insensitive E. coli isolates, 10 (21.7%) were positive for AmpC
β-lactamase genes, among them 4 (8.69%) isolates were positive for blaDHA genes and 6 (13%) for blaCIT genes. Of 57 (30.4%)
cefoxitin-insensitive K. pneumoniae isolates, 10 (17.5%) were positive for AmpC gene with 4 (6.34%) and 6 (9.5%) isolates
positive for blaDHA and blaCIT genes, respectively. However, no MOX, ACC, FOX, or EBC genes were detected in the isolates.
Considering the results of diferent confrmatory phenotypic tests, the AmpC cefoxitin-EDTA test showed a higher discriminatory power for detecting AmpC β-lactamase-producing strains. The specifcity and sensitivity of AmpC cefoxitin-EDTA
were 77%, 100% for K. pneumonia and 70%, 90% for E. coli higher than the other two tests, respectively. Also, the authors
demonstrated high prevalence rate for resistance to certain antibiotics, such as cefuroxime, trimethoprim-sulfamethoxazole,
ampicillin, and cefotaxime. In conclusion, our study provided valuable information regarding the plasmid-mediated AmpC
β-lactamase gene content, antibiotic resistance, and confrmatory phenotypic tests for AmpC β-lactamases in E. coli and K.
pneumoniae isolates from clinical sources
