    Effect of Vaginal Progesterone and Dydrogesterone on Pregnancy Outcomes in patients with Threatened Abortion: A Randomized Clinical Trial

    Introduction: Despite the positive evidence on the effect of progesterone on protection of pregnancy in patients with threatened abortion, the results of studies regarding its drug type have been controversial. This study was performed with aim to compare the effect of vaginal progesterone and dydrogesterone on pregnancy outcome in cases with threatened abortion. Methods: In this single-blind randomized clinical trial, 160 pregnant women with threatened abortion who referred to Qazvin Kowsar Hospital in 2018 were randomly assigned to receive dydrogesterone (Duphaston) 10 mg twice daily or vaginal progesterone (Cyclogest) 400 mg daily. Finally, pregnancy outcomes were compared between the two groups. Data were analyzed by SPSS software (version 18) and Chi-square, independent t-test and Mann-Whitney tests. P<0.05 was considered statistically significant. Results: Frequency of cesarean section was 27 (33.7%) in the dydrogesterone group and 25 (31.2%) in the vaginal progesterone group (P = 0.736). In general, the incidence of preterm labor was 97 (60.6%) and there was no significant difference between the two groups (P >0.05). In addition, incidence of preeclampsia, gestational diabetes mellitus, placenta previa and intrauterine fetal death as well as neonatal weight were not significantly different between the two groups (P >0.05). Finally, maternal and neonatal complications showed no significant difference between the two groups (P = 0.675). Conclusion: Pregnancy outcomes after administration of dydrogesterone are not different with vaginal progesterone in the treatment of threatened abortion

    Sources of academic stress among Iranian adolescents: a multilevel study from Qazvin City, Iran

    Background: Academic stress can cause mental and physical problems and affect adolescents’ healthy development. This study aimed to estimate academic stress and explore its sources at the individual- and school levels among school-going adolescents in the city of Qazvin, Iran. Results: This cross-sectional study used a stratified cluster sampling to recruit 1724 students aged 12–19 years from 53 schools in Qazvin City. Data were collected using a validated self-administered questionnaire. The mean academic stress score was 45.7 (95% CI 45.2, 46.3). The stress level was statistically higher among older 47.5 (95% CI 46.7, 48.3) than younger 44.1 (95% CI 43.4, 44.9) adolescents. The main academic stressors included: future uncertainty 69.7 (95% CI 68.8, 70.7), academic competition 58.5 (95% CI 57.3, 59.6), and interaction with teachers 56.1 (95% CI 55.3, 56.9). Gender, educational period, school type, family socioeconomic status, and father’s education were associated with academic stress. Conclusions: We conducted a multilevel study using a random sample of male and female students in the city of Qazvin, Iran. Results indicated moderate levels of stress among Iranian adolescents. The academic stress was associated with several individual and school-level variables. Students and their families and teachers need education on stress prevention methods and coping mechanisms. Future research should focus on developing and testing multilevel policies and interventions to improve students’ mental health and academic performance.     The effect of Ellagic acid on sleep quality in patients with type 2 diabetes: a randomized double blind clinical trial

    Background Oxidative stress can reduce the quality of sleep in patients with type 2 diabetes. Antioxidants such as polyphenols may increase sleep quality by improving oxidative stress conditions. Objective Considering the antioxidant properties of Ellagic acid (EA), this study was designed to evaluate the effect of EA on sleep quality in diabetic patients. Methods In this study, 44 diabetic patients were recruited. Patients who met the inclusion criteria that were randomly allocated consumed a capsule containing 180 mg of EA per day (n = 22) or placebo (n = 22) for 8 weeks. Anthropometric factors, physical activity, food intake, and Petersburg’s Sleep Quality (PSQI) questionnaire were assessed at the beginning and end of the study. Kolmogorov-Smirnov test, paired sample t test and independent sample t test were used to analyze the data. Results At the end of the study, the mean scores of PSQI and sleep subgroups in the intervention group were significantly lower than in the placebo group (p 0.05). Conclusion According to these findings, intake of EA may help to improve the sleep quality in patients with type 2 diabetes. These effects may be due to the antioxidant effects of this polyphenol

    فراوانی و ویژگی های مولکولی مقاومت به کلیستین با واسطه ژن پلاسمید mcr-1,2 در ایزوله های کلبسیلا و انتروباکتر جداشده از نمونه های کلینیکی

    چکیده فارسی( Abstract) سابقه و هدف : یکی از علل اصلی عفونت های بیمارستانی باکتری های خانواده انتروباکتریاسه هستند. مطالعات کمی در خصوص باکتری های کلبسیلا و انترو باکتر در خصوص مقاومت به کلیستین ، که به عنوان آخرین خط درمانی در باکتری های MDR محسوب می شود، انجام شده است. مطالعه حاضر با هدف تعیین شیوع مقاومت کروموزومی و وابسته به پلاسمید کلیستین در ایزوله های کلبسیلا و انتروباکتر جداشده از بیمارستان های شهر قزوین انجام شد. روش بررسی : 384 ایزوله کلبسیلا و انتروباکتر از بخش های مختلف بیمارستان های شهر قزوین جمع آوری شدند. تمامی ایزوله ها با روش های استاندارد بیوشیمیایی تعیین هویت شدند و جهش های کروموزومی و حضور ژن هایmcr-1 تا mcr-5 مقاوم به کلیستین مورد مطالعه قرار گرفت. یافته ها: در تمامی 280 ایزوله کلبسیلا و 100 ایزوله انتروباکتر به دست آمده، ژن های پلاسمیدی mcr-1 تا mcr-5 مقاوم به کلیستین مشاهده نشد.در مطالعه انجام شده بر روی ژن های کروموزومی مقاومت به کلیستین، از بین 26 ایزوله مقاوم به کلیستین ایزوله های کلبسیلا مطابق MIC بدست آمده ، در 5 ایزوله ادراری ، موتاسیون در ژن mgrB علت ایجاد مقاومت به کلیستین شناخته شد و در 3 ایزوله تنفسی مقاوم به کلیستین، ژن mgrB دست نخورده گزارش شد. بحث و نتیجه گیری: همانند دیگر مطالعات انجام شده در ایران، در هیچ ایزوله کلبسیلا و انتروباکتر، مقاومت به کلیستین با واسطه پلاسمید مشاهده نشد. نتایج ما نشان داد که تغییرات ایجاد شده در ژن mgrB همانند سایر مطالعات، مکانیسم اصلی در مقاومت ایزوله های مطالعه شده بود. کلید واژه ها: کلیستین، مقاومت آنتی بیوتیک، کلبسیلا، انتروباکتر، ژنmcr-1، ژنmgrB ایمیل استاد راهنما : دکتر فرهاد نیکخواهی [email protected]

    The Effect of the Yeast Probiotic Saccharomyces boulardii on Acute Diarrhea in Children

    Background: This study aimed to evaluate the effect of yeast probiotic Saccharomyces boulardii on the frequency and duration of acute diarrhea in children. Methods: Sixty children were randomized to receive yeast probiotic S. boulardii in a daily dose of 250 mg (trial group-30 patients) or placebo (control group-30 patients) for five days. The clinical and demographic characteristics of two groups were recorded before the intervention. After intervention, the two groups were compared in terms of frequency of diarrhea, duration of diarrhea, and hospital stay. Results: The median (interquartile range) of frequency of diarrhea in the trial group was significantly lower than placebo group (4 [2] vs. 5 [2.5]) (P = 0.04). Also, the median (interquartile range) of duration of diarrhea in the trial group was significantly lower than placebo group (3 [2] vs. 4 [2.5]) (P = 0.03). There was no significant difference between groups for hospital stay (P = 0.8). Conclusions: The present study reveals that the use of yeast probiotic S. boulardii can reduce the frequency and duration of acute diarrhea.     Knowledge, Attitude, and Practice Toward SARS COV-2 in Qazvin Province: A Population-Based Study

    Background and Objectives: The spread of COVID-19 in the world has had adverse effects on all aspects of people´ life and social and economic development of the communities. The current study was conducted to determine the knowledge, attitudes, and practices toward COVID-19 in the general population of Qazvin province. Methods: This cross-sectional study conducted on 1223 people above 15 years in Qazvin province between December and January 2020. We used multistage stratified random sampling to select the participants from urban and rural health centers. The questionnaires were completed by the patients as well as through phone interviews. The questions included demographic information and knowledge, attitude, and practice toward COVID-19. Results: In total, 1223 subjects with a mean (standard deviation) age of 35.6 ± 12.2 years participated in this study. The scores of knowledge, attitudes, and practice were 68%, 72% and 91%, respectively. The lowest score of knowledge was related to awareness of common symptoms of the disease (6%). Forty-two subjects believed they observed the principles of disease prevention. A distance of 2 meters was observed by 50.3%. The scores of knowledge (P=0.007) and practice (P=0.028) increased with age, and the scores of knowledge (P<0.001) and attitude (P=0.001) increased and decreased with an increase in the education level, respectively. Males had significantly lower knowledge (P=0.002) and practice (P<0.001) scores. Moreover, rural residents had higher attitude scores compared to urban residents (P=0.002). Conclusion: The results suggest that the general population of Qazvin province had desirable knowledge, attitude and practice towards COVID-19 at the time of the epidemic.     Polysomnographic Findings between Obese and Non-Obese Pediatrics with Obstructive Sleep Apnea

    Background: The prevalence of childhood obesity, which is associated with the health risk of OSA, is increasing. This study aimed to assess the polysomnographic findings of obese and non-obese children and adolescents with OSA. Methods: In this cohort retrospective study, all the obese and non-obese children and adolescents with OSA referring to Sleep Disorders Clinic, Qazvin Children Hospital, during 2014-2019 were included. The participants were 52 pediatrics within the age range of 1-16 years old and mean age of 6.47±3.59 years, 20 (38.5%) of whom were female and 32 (61.5%) were male. The number of samples was determined according to previous studies, and the patients’ case information was applied in this research. Obese children and adolescents were determined according to their BMI. PSG was performed for all the participants and its variables including sleep onset, sleep efficiency, sleep stages (N1, N2, N3, rapid eye movement (REM)), arousal index (AI), apnea hypopnea index (AHI), mean arterial oxygen saturation (SaO2), and total sleep time were determined and compared between the two groups. Results: Mann-Whitney test showed a statistically significant difference in the percentage of REM sleep stage between the obese and non-obese groups (P=0.017). There was no statistically significant difference in the other polysomnographic variables between the two groups. In the obese group, linear regression showed significant correlation between body mass index (BMI) and AHI as well as mean arterial SaO2. Conclusion: the percentage of REM sleep stage in the obese group with OSA was lower than that in the non-obese group. There was a correlation (P≤0.05) between obesity and respiratory events in sleep.
