    Changes in plasma lipid and in-hospital deaths in patients with sepsis

    Background: Lipid profiles are infrequently measured in clinical management of sepsis patients. Sepsis leads to significant alterations in the metabolism of lipids. The aim of the present study was to determine whether changes in plasma lipid concentrations during sepsis treatment were associated with clinical outcome. Methods: In this study, 74 adult septic patients were included in this prospective observational study from January to December 2017. Patients taking lipid lowering agents were excluded. A detailed medical history was obtained and clinical examination was performed. Serum total cholesterol (STC) and its fractions [low-and high-density lipoprotein] and triglyceride levels were measured in the morning of the first day after admission and then once weekly. The primary outcomes of the study were in-hospital mortality, and hospital stay and hypocholesterolemia were defined as STC levels < 50 mg/dL. Manne-Whitney U and chi-squared tests were used for data analysis, and significance level was set at p<0.05. Results: In this study, 78.4% (CI 95%: 67.3-87.1) of patients had hypocholesterolemia. During the study period, 21.6% (CI 95%: 12.9- 32.7) of patients died. All lipid (except TG) concentrations continuously decreased in deceased sepsis patients but increased in recovering patient (p value for STC (p=0.004), LDL (p=0.006), HDL (p=0.010), and TG (p=0.052)). The serum lipids concentration was not associated with length of hospital stay (p value for STC (p=0.524), LDL (p=0.813), HDL (p=0.799) and TG (p=0.581)). Conclusion: In this study it was found that the additional decline of lipid profile was significantly associated with increased mortality rate of sepsis patients. Thus, the clinically termed 'the lipaemia of sepsis' is not true in all situations

    Colorimetric-based detection of Ureaplasma urealyticum using gold nanoparticles

    Ureaplasma urealyticum (uu) is one of the most common agents of urogenital infections and is associated with complications such as infertility, spontaneous abortion and other sexually transmitted diseases. Here, a DNA sensor based on oligonucleotide target-specific gold nanoparticles (AuNPs) was developed, in which the dispersed and aggregated states of oligonucleotide-functionalised AuNPs were optimised for the colorimetric detection of a polymerase chain reaction (PCR) amplicon of U. urealyticum DNA. A non-cross-linking approach utilising a single Au-nanoprobe specific of the urease gene was utilised and the effect of a PCR product concentration gradient evaluated. Results from both visual and spectral analyses showed that target–Au-nanoprobe hybrids were stable against aggregation after adding the inducer. Furthermore, when a nontarget PCR product was used, the peak position shifted and salt-induced aggregation occurred. The assay's limit of detection of the assay was 10 ng with a dynamic range of 10–60 ng. This procedure provides a rapid, facile and low-cost detection format, compared to methods currently used for the identification of U. urealyticum

    The Effect of Remineralizing Agents With/Without CO2 Laser Irradiation on Structural and Mechanical Properties of Enamel and its Shear Bond Strength to Orthodontic Brackets

    Introduction: Remineralizing agents may be used for the treatment of white spot lesions (WSLs) prior to bracket bonding. However, some concerns exist regarding their possible interference with the etching and bonding process, negatively affecting the bond strength. This study aimed to assess the effect of two remineralizing agents with/without CO2 laser irradiation on the mechanical properties and shear bond strength (SBS) of demineralized enamel to the orthodontic bracket. Methods: This study evaluated 60 premolar teeth in 6 groups (n=10) as follows: (I) sound enamel, (II) demineralized enamel, (III) Nupro remineralizing agent (N), (IV) Nupro and CO2 laser (N/L), (V) Teethmate remineralizing agent (T), and (VI) Teethmate and CO2 laser (T/L). The remineralizing agents were applied to the enamel surfaces after their immersion in a demineralizing solution for 5 days. In groups IV and VI, the CO2 laser with a 10.6 μm wavelength, 10 ms pulse duration, a 50 Hz repetition rate, 0.3 mm beam diameter and 0.7 W power was irradiated after applying the remineralizing agents. Brackets were bonded to the enamel surfaces and SBS was measured by a universal testing machine. For the assessment of enamel microhardness, 20 sections of molar teeth were divided into 4 groups (n=5; N, N/L, T, T/L) and their microhardness was measured before demineralization, after demineralization and after remineralization. X-ray diffraction (XRD) analysis, field-emission scanning electron microscopy (FESEM) and energy-dispersive spectrometry (EDS) were carried out to assess the formation of hydroxyapatite. The atomic percentages of the C, O, P, Ca, Na, Si, F and Ca/P ratio were determined by EDS analysis. Results: The SBS significantly decreased in group II (P < 0.001). There was no significant difference among the groups I, III, IV, V and VI (P < 0.05). This finding was similar to the microhardness results, which showed an increase in microhardness after remineralization (P < 0.05), with no difference among the remineralizing agents. The Ca/P ratio was the highest in the Nupro group and the lowest in the demineralized group. Conclusion: Remineralizing agents can significantly improve the microhardness and structural properties of demineralized enamel to a level similar to that of sound enamel with no adverse effect on SBS to orthodontic brackets

    Spiritual Intelligence and Self-Efficacy among Operational Staff of Qazvin's Medical Centers: A Path Analysis

    Background: Self-efficacy and spiritual beliefs can be considered as strong motives for improving the quality of working. Due to the different job description and environment than other service sectors, medical centers need to pay attention to these factors. So, the present study is aimed at identifying the effect of spiritual intelligence components on self-efficacy among operational staff of Qazvin medical centers. Methods: The present research is a descriptive-analytical and crosssectional study conducted in 2019. About 238 people were selected by stratified sampling out of the medical centers staff in Qazvin, Iran. Spiritual intelligence was measured by King’s questionnaire including four dimensions, and self-efficacy was measured by Sherer’s questionnaires including three dimensions. Primary data analysis was done by Pearson’s correlation test in SPSS20 software, and path analysis was done by structural equation modeling in AMOS software with the significance level of 0.05. Results: There was a significant direct relationship between all dimensions of self-efficacy and spiritual intelligence except persistence in the face of adversity and consciousness expansion (P-value ˂ 0.01). All the relationships were significant in path analysis of the final model. Also, the fit indices including NFI = 0.901, RMSEA = 0.055, GFI = 0.923, and CFI = 0.913 suggest the good fitness of the final model. Conclusion: The managers of the studied hospitals can promote self-efficacy in clinical staff and improve their performance and behavior in service provision by psychological enrichment of the staff and raising their spiritual intelligence

    Effects of atmospheric cold plasma on microbial growth of Listeria innocua and Staphylococcus aureus in ready-to-eat fish products

    Food-borne pathogens, including Staphylococcus aureus and Listeria monocytogenes, in fish products account for death to a great extent. To increase shelf-life of foods and also preserve their sensory properties, plasma treatments have attracted more attention. The present study was intended to investigate the efficacy of atmospheric cold plasma (ACP) using argon and helium gas flows for ready-to-eat (RTE) fish products contaminated with L. innocua and S. aureus at short treatment durations of 4, 6 and 10 min. The results showed that argon plasma at each time point (4, 6, and 10 min) had a significantly inhibitory effect on the growth of L. innocua. Also, it reduced over a 10-minute treatment with helium plasma. There was a direct correlation between exposure time and antimicrobial efficiency of argon and helium ACP. The survival of S. aureus decreased by 0.19-1.04 Log cycles after argon plasma whereas helium plasma had a lower potential for growth reduction within the range of 0.03 and 0.55 Log cycles. It could be concluded that ACP is effective for lowering growth of different micro-organisms on RTE fish samples. L. innouca was considerably more sensitive than S. aureus to atmospheric pressure plasma (APP)

    A theory-based intervention to promote medication adherence in patients with rheumatoid arthritis: A randomized controlled trial

    Introduction/objectives Adherence to prescribed medication regimens is fundamental to the improvement and maintenance of thehealthofpatientswithrheumatoidarthritis.Itisthereforeimportantthatinterventionsaredevelopedtoaddressthisimportant health behavior issue. The aim of the present study was to design and evaluate a theory-based intervention to improve the medication adherence (primary outcome) among rheumatoid arthritis patients. Methods The study adopted a pre-registered randomized controlled trial design. Rheumatoid arthritis patients were recruited from two University teaching hospitals in Qazvin, Iran from June 2018 to May 2019 and randomly assigned to either an interventiongroup(n=100)oratreatment-as-usualgroup(n=100).Theinterventiongroupreceivedatheory-basedintervention designed based on the theoretical underpinnings of the health action process approach (HAPA). More specifically, action planning (making detailed plans to follow medication regimen), coping planning (constructing plans to overcome potential obstacles that may arise in medication adherence), and self-monitoring (using a calendar to record medication adherence) of the HAPA has been used for the treatment. The treatment-as-usual group received standard care. Results Dataanalysiswasconductedbasedontheprincipleofintentiontotreat.Usingalinearmixed-effectsmodel(adjustedfor age, sex, medication prescribed, and body mass index), the results showed improved medication adherence scores in the intervention group (loss to follow-up=16) compared to the treatment-as-usual group (loss to follow-up=12) at the 3-month (coefficient=3.9; SE=0.8) and 6-month (coefficient=4.5; SE=0.8) follow-up.     Psychometric Testing of Three COVID-19-Related Scales Among People with Mental Illness

    Fear of novel coronavirus 2019 (COVID-19) may result in psychological health problems among different populations. Moreover, believing COVID-19 information and preventive COVID-19 infection behaviors are relevant constructs associated with fear of COVID-19. Therefore,thepresent studyvalidatedthreeinstrumentsassessingfear,beliefs,andpreventive behaviors related to COVID-19 among individuals with mental illness. Moreover, relationships between the three constructs were examined. Individuals with mental illness (N=400; 178 females; mean age=46.91 years) completed the Fear of COVID-19 Scale (FCV-19S), Believing COVID-19 Information Scale (BCIS), Preventive COVID-19 Infection Behaviors Scale (PCIBS), and Depression Anxiety Stress Scale-21 (DASS-21). The FCV-19S, BCIS, and PCIBS demonstrated a single-factor structure with satisfactory fit indices. MoreoverbelievingCOVID-19informationpositivelyandsignificantlyassociatedwithfearofCOVID19, and fear of COVID-19 negatively and significantly associated with preventive behaviors and positively and significantly associated with psychological distress. The FCV-19S, BCIS, and PCIBS may assist healthcare providers in assessing COVID-19-related information among individuals with mental illness. Consequently, relevant programs may be designed to help individuals with mental illness going through the period of crisis
