    Prevalence of musculoskeletal disorders in firefighters and its association with insomnia

    Musculoskeletal disorders (MSDs) are among the main underlying reasons for decreased productivity at workplace. Awkward working posture, forceful movements, and unexpected events are main reasons for high prevalence of MSDs in firefighters. According to previous research, there is a significant contribution between MSDs and other co-morbidities such as insomnia. The present study aimed to evaluate the prevalence of MSDs in firefighters and its relation with severity of insomnia. A cross-sectional study was conducted on firefighters in Qazvin. Demographic information was collected for the participants. A self-administered questionnaire including Nordic question, body map, and insomnia severity index were completed by the firefighters. Descriptive statistics, Student’s t-test, and Chi-square test were used for data analysis. One hundred and eighteen firefighters participated in this study. The prevalence of MSDs was 54.2% amongst the firefighters, and low back pain was the most common complaint with 30.5%. The highest intensity of pain was in the middle of lumbar region. About 20.3% of firefighters had subclinical insomnia, and 30% of them had moderate to severe insomnia. Severity of insomnia was significantly more in subjects with MSDs (12.4 ± 7.9) compared to those without MSDs (7.1 ± 6.6) (p<.05). Among different parts of body, significant association was solely found between back pain and severity of insomnia. The results showed that MSDs, especially around the lumbar region is associated with the severity of insomnia in firefighters. In addition, the prevalence of MSDs and insomnia was significantly higher in firefighters compared to similar studies in general population which calls for preventive activities

    Predicting Inpatient Length of Stay in Iranian Hospital: Conceptualization and Validation

    Objective: The length of stay is an important indicator of hospital performance and efficiency. Regarding the importance of the length of stay, this study aimed to design a structural model of the inpatients’ length of stay in the educational and therapeutic health care facilities of Iran in order to identify the influencing dimensions. Methods:The present study was an analytical and applied study. The face validity of the data gathering tool was investigated by the expert judgment and the construct validity was examined by using the exploratory factor analysis. In order to verify the reliability of the tool, the internal consistency was also trialed by using the Cronbach’s alpha. For ranking the influencing dimensions and factors and also in order to examine the causal relationships between the variables in a coherent manner and presenting the final model, the structural equation modeling technique was used in AMOS software at a significant level of 0.05. Results: The mentioned structural model consists of 4 dimensions and 29 factors influencing the length of stay of hospitalized patients. The independent variables are based on priority and importance as follows: patients’ conditions, the underlying factors, the clinical staff performance, and hospitals’ service delivery, which were examined by second-order factor analysis in order to study the relationship between them and the inpatients’ length of stay. Conclusion: Considering the importance of each one of the proposed dimensions from the point of view of service providers in some therapeutic centers of the country by paying attention to the role of each one of them in preventing prolonged hospitalization can be essential in the effectiveness of the treatment and cost reduction

    Study of jack bean urease interaction with luteolin by the extended solvation model and docking simulation

    Abstract: In this study, the interaction between Luteolin and urease was made at 300 K in aqueous buffer solutions using isothermal titration calorimetry. The extended solvation model was used to calculate the solvation parameters. Moreover, to determine the interaction of Luteolin with Jack Bean Urease (JBU), a molecular docking process was performed. The purpose of this investigation was to measure the inhibitory effects of Luteolin on the activity and structure of urease. Molecular docking analysis confirmed the extended solvation model

    Effect of Assertiveness Training on Adaptive and Aggressive Assertiveness in Nurses

    Background Assertiveness is one of the most important skills needed by nurses to reduce interpersonal conflict and build an effective teamwork in complex hospital environments. Objective The aim of this study was to determine the effect of an assertiveness training program on nurses’ aggressive and adaptive assertiveness. Methods In this interventional study, participants were 64 nurses working in hospitals of Abhar city in Iran, randomly divided into two groups of intervention (n=32) and control (n=32). Assertive training program was presented as a two-day training workshop (10 hours) to the intervention group, while the control group received no training. Thompson and Burnham’s Adaptive and Aggressive Assertiveness Scale were completed by the participants in both groups at baseline, immediately, one and three months after the intervention. After collecting the data, Friedman, Mann–Whitney, Chi-squared, and Fisher tests were used for their analysis. Findings The mean of adaptive assertiveness score was significantly different between the the two groups immediately (P=0.002) and 1 month after the intervention (P=0.025). The aggressive assertiveness scores were not significantly different immediately, 1 and 3 months after the intervention (P>0.05), but the results of Friedman test showed a statistically significant decrease in the aggressive assertiveness scores compared to its baseline score (P=0.002). Conclusion The assertiveness training program increased adaptive assertiveness and decreased aggressive assertiveness in nurses. Relevant authorities can design continuous and useful assertiveness training programs to promot this valuable skill in nurses

    Natural Selection at the NHLH2 Core Promoter Exceptionally Long CA-Repeat in Human and Disease-Only Genotypes in Late-Onset Neurocognitive Disorder

    Background: Approximately 2% of the human core promoter short tandem repeats (STRs) reach lengths of ≥6 repeats, which may in part be a result of adaptive evolutionary processes and natural selection. A single-exon transcript of the human nescient helix loop helix 2 (NHLH2) gene is flanked by the longest CA-repeat detected in a human protein-coding gene core promoter (Ensembl transcript ID: ENST00000369506.1). NHLH2 is involved in several biological and pathological pathways, such as motivated exercise, obesity, and diabetes. Methods: The allele and genotype distribution of the NHLH2 CA-repeat were investigated by sequencing in 655 Iranian subjects, consisting of late-onset neurocognitive disorder (NCD) as a clinical entity (n = 290) and matched controls (n = 365). The evolutionary trend of the CA-repeat was also studied across vertebrates. Results: The allele range was between 9 and 25 repeats in the NCD cases, and 12 and 24 repeats in the controls. At the frequency of 0.56, the 21-repeat allele was the predominant allele in the controls. While the 21-repeat was also the predominant allele in the NCD patients, we detected significant decline of the frequency (p < 0.0001) and homozygosity (p < 0.006) of this allele in this group. Furthermore, 12 genotypes were detected across 16 patients (5.5% of the entire NCD sample) and not in the controls (disease-only genotypes; p < 0.0003), consisting of at least one extreme allele. The extreme alleles were at 9, 12, 13, 18, and 19 repeats (extreme short end), and 23, 24, and 25 repeats (extreme long end), and their frequencies ranged between 0.001 and 0.04. Evolutionarily, while the maximum length of the NHLH2 CA-repeat was 11 repeats in non-primates, this CArepeat was ≥14 repeats in primates and reached maximum length in human. Conclusion: We propose a novel locus for late-onset NCD at the NHLH2 core promoter exceptionally long CA-STR and natural selection at this locus. Furthermore, there was indication of genotypes at this locus that unambiguously linked to late-onset NCD. This is the first instance of natural selection in favor of a predominantly abundant STR allele in human and its differential distribution in late-onset NCD

    Photocatalytic degradation of 2,4,6-trichlorophenol in aqueous solutions using synthesized Fe-doped TiO2 nanoparticles via response surface methodology

    In this study, the photocatalytic degradation of 2,4,6-trichlorophenol (2,4,6-TCP) was evaluated under UV radiation by Fe-doped titanium dioxide (Fe-doped TiO2 ) nanoparticles (NPs) which were synthesized by a sol-gel method. Diffuse reflectance spectroscopy (DRS), X-ray diffraction (XRD), and scanning electron microscopy (SEM) were applied to identify the synthesized nanoparticles. According to the SEM image, the synthesized nanoparticles had fine and irregular shapes with relatively smooth surfaces, as well as XRD spectrum showed that the crystalline size of Fe-doped TiO2 NPs was 10.42 nm, furthermore, according to the DRS analysis, the bandgap energy of Fe-doped TiO2 NPs was determined about 2.9 eV. The effects of operating parameters, including initial 2,4,6- TCP concentration, pH, contact time and Fe-doped TiO2 NPs dosage on 2,4,6-TCP degradation were studied and optimized based on the response surface methodology with Box–Behnken method. The analysis of 2,4,6-TCP degradation showed that under optimum conditions, the removal efficiency reached 95.9% which is consistent with the model prediction. The optimum degradation conditions were as follows: pH, 3.29; initial 2,4,6-TCP concentration, 50.5 mg L–1; Fe-doped TiO2 NPs dosage, 0.59 g L–1; and contact time, 55.7 min. The present results showed that Fe-doped TiO2 NPs have great potential for removing 2,4,6-TCP from aqueous solution

    Probiotic Saccharomyces cerevisiae var. boulardii supernatant inhibits survivin gene expression and induces apoptosis in human gastric cancer cells

    Natural anticancer drug and compounds with other great benefits are of interest recently due to lower side effects than chemotherapy for cancer treatment and prevention. Different natural and synthetic drugs have been suggested to be used for treatment of gastric cancers, the second deadly cancer worldwide. The aim of this study was to investigate anticancer activity of SBS including inducing apoptosis and inhibition of survivin gene expression in gastric cancer cells. We evaluated cell viability, inducing apoptosis and change in survivin gene expression of EPG85-257P (EPG) and EPG85-257RDB (resistant to Daunorubicin, RDB) cell lines under exposure of SBS after 24, 48, and 72 hr. We found that SBS decreased cell viability, induced apoptosis, and reduced survivin gene expression in treated EPG and RDB cells (with the significant IC50 values of 387 and 575 μg/ml after 72 and 48 hr for EPG and RDB cells respectively). However, we observed SBS was more efficient to induce apoptosis in EPG than RDB cells. We strongly suggest SBS be considered as a prospective anticancer agent or in formulation of complementary medication to treat and prevent gastric cancers
