    Role of service quality, trust and loyalty in building patient-based brand equity: Modeling for public hospitals

    In the current competitive situation, hospitals are seeking to keep their position among patients. This study aimed to find factors influencing on the brand equity of hospitals.in the cross sectional study, Firstly, the factors affecting the brand equity were identified Subsequently, by holding the panel of experts, the effective factors were taken into account After standardization, the questionnaire was given to 450 admitted patients in Qazvin’s hospitals. Finally, the causal relationship between the variables was evaluated using the Structural Equation Model (SEM) on AMOS22 software, with a significant level of 0.05. Analysis of findings revealed all fit indices were in an acceptable level. The path coefficient between brand trust and brand loyalty with brand equity was positive (t > 1.96). Study result is necessary for hospitals to prioritize attention to dimensions and factors affecting brand equity in order to maintain their place in society and provide effective service

    Guillain-Barré Syndrome as a Neurological Complication of Novel COVID-19 Infection: A Case Report and Review of the Literature

    INTRODUCTION: The novel coronavirus (COVID-19) is a global pandemic. Although the main clinical manifestation of COVID-19 is respiratory involvement, there is evidence suggesting the neuroinvasive potential of COVID-19. There are limited reports of neurological complications of COVID-19 infection in the literature. Herein, we aim to describe 2 members of a family affected by COVID-19, presenting with ascending paresthesia with the final diagnosis of Guillain-Barré syndrome. CASE REPORT: A 38-year-old man presented with a history of ascending paresthesia and bilateral facial droop since 5 days before admission. The medical history was positive for flulike symptoms affecting all the members of his family. The neurological examination was notable for bilateral peripheral facial paralysis, generalized areflexia, and derceased sensation in distal limbs. The cerebrospinal fluid analysis revealed an albuminocytologic dissociation. In addition, the electromyography-nerve conduction study findings were suggestive of acute axonaldemyelinating polyneuropathy. Meanwhile the patient was treated with a diagnosis of GuillainBarré syndrome, his 14-year-old daughter presented with a history of progressive paresthesia and weakness. Similar to her father, the paraclinical evaluations were consistent with Guillain-Barré syndrome. Taking into account clinical findings and the outbreak of COVID-19, the suspicion of COVID-19 was proposed. Eventually, on the basis of throat swab samples stand on polymerase chain reaction, the patients were diagnosed with COVID-19. CONCLUSION: Our cases revealed the familial occurrence of Guillain-Barré syndrome after COVID-19 infection. The authors emphasize neurological complications of COVID-19

    Melatonin synergistically enhances protective effect of atorvastatin against busulfan-induced spermatogenesis injuries in a rat model

    Busulfan, a chemotherapy drug, causes a variety of adverse effects, such as damage to the reproductive system of male cancer patients. The aim of this study was to investigate the protective effect of intra-peritoneal (IP) injection of exogenous melatonin and atorvastatin alone and in combination on busulfan-induced spermatogenesis injuries and oxidative stress activity in a rat model. A total of 36 male Wistar rats (220–250 g) (n = 6) were randomly divided into six different groups, including atorvastatin, melatonine, sham (received DMSO or busulfan solvent), busulfan, atorvastatin + melatonine, and control groups. Busulfan was diluted in DMSO and intra-peritoneally (IP) administrated in two doses of 25 and 10 mg/kg with a 14-day interval in all groups except the control and sham groups. Administration of atorvastatin and melatonin started 1 day after injection of busulfan. After 28 days, the animals were euthanized and the semen was collected from epididymis and the testicular tissues were removed for sperm parameter assessment and biochemical analysis, including super oxide dismutase (SOD), SOD/total protein (TP), catalase, catalase/TP, and MDA. The highest and lowest values of sperm parameters belonged to the control and busulfan groups, respectively, followed by sham, atorvastatin + melatonine, melatonin, and atorvastatin groups as well as antioxidant enzyme activity. However, MDA activity in the busulfan group was significantly increased compared to that in the other groups. In conclusion, the results of the present study indicated that co-administration of melatonin and atorvastatin and monotherapy can attenuate busulfan-induced spermatogenesis injuries in rats via reducing oxidative stress

    Carcinogen Risk Assessment of Mutagen X in Chlorinated Drinking Water in West of Tehran, Using Probabilistic Approaches

    The present study aims at evaluating the risk of Mutagen X (MX) (3- chloro-4-(dichloromethyl)-5-hydroxy-2 (5H)-furanone) and adverse health effects, associated with direct ingestion of chlorinated drinking water in west of Tehran, supplied by chlorinated drinking water from surface and underground water sources. For one year, MX concentrations in tap water samples has been measured for consumers in four different zones in western Tehran. It has been found that average MX concentration in the whole study area is 24.16 ng/L, with the highest concentration being in Zone 1 with a value of 38 ng/L. Also, the role of water sources, seasonal changes, and effective factors such as Total Organic Carbon (TOC) have been evaluated on MX formation. The highest of excess lifetime cancer risk (ELCR), estimated as 0.0037E-05, belongs to Zone 1, which uses surface water to supply drinking water, while the lowest can be seen in Zone 4, being 0.0021E-05. This latter zone utilizes underground water as the water source. In all zones, the highest risk of excessive cancer is related to winter, ranging from 0.0045E-5 in Zone 1 to 0.0023E-5 in Zone 4. The estimated number of cancer cases for Zones 1 to 4 have been 0.012, 0.016, 0.016, and 0.004, respectively, based on their population. The estimated average risk and the number of ELCR, caused by exposure to MX, through direct ingestion of drinking water have been 0.0030E-5 and 0.047, respectively, in the entire studied area for the duration of one year

    Comparison of the effect of receiving three different stresses during pregnancy on anxiety in male rats

    Introduction: Evidence shows that pregnancy conditions have a lasting effect on the fetus. This study aimed to investigate the effect of prenatal three common stresses, namely electromagnetic field, social stress, and movement restriction on the anxiety in male rats. Methods: In this experimental study, pregnant female Wistar rats were randomly divided into 4 groups, including 1. Control (with normal pregnancy) 2. Group exposed to electromagnetic field 2. The group receiving immobility stress and 3. Social stress group. The 3 treatment groups received one of the name different stresses daily from the 8 to the 18th day of pregnancy. 3 months after birth, the anxiety level of the male offspring was assessed using plus-maze. Results: The mean percentage of open arms entrance and their presence duration was significantly decreased in offerings that were under all three types of fetal stress compared to the control (P<0.05). Conclusion: According to the findings, fetal stress increases anxiety disorders in children. Also, immobility stress has the greatest effect and social stress has the least effect in this regard

    Evaluation of Bacterial Species to Determine Antimicrobial Resistance in Patients with Chronic Rhinosinusitis after Surgery of Paranasal Sinuses Referring to Amiralmomenin Hospital in Rasht, 2018

    Background and Aim: Bacterial resistance to antibiotics has made treatment difficult. The purpose of this study was to investigate bacterial species in patients with chronic rhinosinusitis after surgery of paranasal sinuses to determine antimicrobial resistance patterns of them. Materials and Methods: The data of 70 patients after paranasal sinuses surgery in Amiralmomenin hospital in Rasht city, in 2018 were evaluated. The identification of bacteria by microbiological laboratory methods and microbial susceptibility test was performed by disk diffusion method. For data analysis, SPSS version 22 software and chi -square test were used (p ≤0.05). Results: 62 (88.57%) positive bacterial culture samples were identified. The most abundant strains was Staphylococcus epidermidis (38.70%). Staphylococcus aureus had the highest antibiotic resistance to penicillin and oxacillin (52.94%) and Staphylococcus epidermidis to penicillin (62 .50%). Highest antibiotic resistance in Pseudomonas aeruginosa was to ceftazidime (90.90%). Escherichia coli was resistant to ceftazidime and ampicillin (100%) and Hafnia alvei was resistant to ceftazidime (100%). Klebsiella aerogenes had higher resistant to ceftazidime and cefixime (100%). With increasing of patient’s age, resistance to antibiotics increased (p ≤0.05) . Conclusion: Antibiotic resistance was observed in bacterial samples isolated from patients after surgery. Given that antibiotic resistance may cause failure in the treatment. Monitoring of the antibiotic -resistant pattern is necessary to select the appropriate antibiotic.
