    Social situation, health, and health behavior of children and adolescents in one-parent households at the end of the COVID-19 pandemic: Results of the KIDA study 2022–2023

    Einleitung: In der COVID-19-Pandemie waren Alleinerziehende und ihre Kinder durch die Eindämmungsmaßnahmen und aufgrund oftmals geringer Ressourcen in besonderem Maße Belastungen ausgesetzt. Es wird analysiert, inwieweit sich zum Ende der Pandemie Unterschiede in der sozialen und gesundheitlichen Lage von Kindern und Jugendlichen in Ein-Eltern- und Zwei‑Eltern-Haushalten zeigen. Methoden: Die Analyse basiert auf Daten der KIDA-Studie, in der 2022/2023 Eltern von 3‑ bis 15-Jährigen und 16- bis 17-Jährige befragt wurden (telefonisch: n = 6992; online: n = 2896). Für die Indikatoren psychosoziale Belastungen, soziale Unterstützung, Gesundheit und Gesundheitsverhalten wurden nach Familienform stratifizierte Prävalenzen berechnet. In Poisson-Regressionen wurde für Geschlecht, Alter, Bildung und Haushaltseinkommen adjustiert. Ergebnisse: Heranwachsende aus Ein-Eltern-Haushalten sind häufiger durch finanzielle Einschränkungen, familiäre Konflikte und beengte Wohnverhältnisse belastet und erfahren weniger schulische Unterstützung als Gleichaltrige aus Zwei‑Eltern-Haushalten. Sie haben häufiger gesundheitliche Beeinträchtigungen sowie einen erhöhten Versorgungsbedarf und nehmen häufiger psychosoziale Angebote in Anspruch. Sie sind zwar seltener in Sportvereinen aktiv, nehmen jedoch gleich häufig an Sport-AGs in Schulen teil wie Gleichaltrige aus Zwei-Eltern-Haushalten. Die Unterschiede zeigen sich auch bei Kontrolle für Einkommen und Bildung. Diskussion: Kinder und Jugendliche aus Ein-Eltern-Haushalten können über Bewegungsangebote im schulischen Setting gut erreicht werden. Niedrigschwellige Angebote in Kita, Schule und Kommune sollten daher weiter ausgebaut werden. Weiterhin bedarf es Maßnahmen zur Verbesserung der sozioökonomischen Lage von Alleinerziehenden und ihren Kindern.Introduction: During the COVID-19 pandemic, single parents and their children were particularly exposed to stress due to the containment measures and to limited resources. We analyzed differences in the social and health situation of children and adolescents in one-parent households and two-parent households at the end of the pandemic. Methods: The analysis is based on data from the KIDA study, in which parents of 3‑ to 15-year-old children as well as 16- to 17-year-old adolescents were surveyed in 2022/2023 (telephone: n = 6992; online: n = 2896). Prevalences stratified by family type were calculated for the indicators psychosocial stress, social support, health, and health behavior. Poisson regressions were adjusted for gender, age, level of education, and household income. Results: Children and adolescents from one-parent households are more likely to be burdened by financial restrictions, family conflicts, and poor living conditions and receive less school support than peers from two-parent households. They are more likely to have impairments in health as well as increased healthcare needs, and they use psychosocial services more frequently. Furthermore, they are less likely to be active in sports clubs, but they take part in sporting activities at schools as often as minors from two-parent households. The differences are also evident when controlling for income and education. Discussion: Children and adolescents from one-parent households can be reached well through exercise programs in a school setting. Low-threshold offers in daycare centers, schools, and the community should therefore be further expanded. Furthermore, interventions are needed to improve the socioeconomic situation of single parents and their children.Peer Reviewe

    Prevalence of HIV in people with potential HIV-indicator conditions in Germany: an analysis of data from statutory health insurances

    Background: In care of people living with human immunodeficiency virus (HIV), early diagnosis of infection is one of the greatest challenges remaining. A promising approach to increase early diagnosis could be optimized HIV testing in persons with indicator conditions (ICs). ICs are conditions which are AIDS-defining in people living with HIV, conditions that may have significant adverse consequences for the individual's clinical management if the presence of HIV infection is not detected, and conditions with an (undiagnosed) HIV prevalence of ≥0.1%. Methods: In this cohort study, anonymous routine healthcare data of German statutory health insurances from 07/01/2016 to 06/30/2021 based on insured persons with an ICD-10-based diagnosis of selected ICs were analyzed. In a primary analysis, two stratifications (gender and age), and four sensitivity analyses HIV prevalence/incidence were calculated for persons with at least one of 26 IC described in international literature. This study is registered in the German Clinical Trials Register (identifier: DRKS00028743 ). Findings: Routine healthcare data from 513,509 insured persons were selected for analysis. In the primary analysis, only in malignant neoplasm of bronchus and lung a HIV prevalence was observed with a 95%-CI < 0.1%. ICs with particularly high HIV prevalence were pneumocystosis (40.33%), oral hairy leukoplakia (36.71%), and Kaposi's sarcoma (29.86%). When stratified by gender, it was observed that in female patients, the 95%-CI of HIV prevalence fell below 0.1% for seven ICs. No such effect was observed in male patients. Stratified by age, among patients aged 30 to <60 years, the 95%-CI of HIV prevalence were always ≥0.1%, while in the other groups the 95%-CI fell below 0.1% for several ICs. Interpretation: In samples of patients with ICs in Germany, HIV prevalences/incidences were found to be ≥0.1% for all ICs except malignant neoplasm of bronchus and lung. This confirms the classification of these conditions as ICs for the German context and emphasizes the importance of HIV testing in these populations.     Kommentar der Kommission für Krankenhaushygiene und Infektionsprävention (KRINKO) zu Anforderungen an Hautantiseptika zur Prävention postoperativer Wundinfektionen

    Seit dem Erscheinen der KRINKO-Empfehlung zur Prävention von postoperativen Wundinfektionen (surgical site infections, SSI) im Jahr 2018 wurde in verschiedenen Studien darauf hingewiesen, dass durch den Zusatz eines remanent wirkenden Antiseptikums zu alkoholischen Formulierungen die SSI-Rate abhängig von der Operationsart signifikant reduziert werden kann. Die KRINKO bewertet daher ihre bestehende Empfehlung neu und ergänzt diese hinsichtlich der als Hautantiseptika verwendeten Wirkstoffe, ihrer spezifischen Indikation und Applikation.Peer Reviewe

    Stigmatisierung beenden und Würde anerkennen: Von Herausforderungen und Hoffnungen zum Welt-Lepra-Tag 2024

    Der diesjährige internationale Welt-Lepra-Tag steht unter dem Motto „Ending Stigma, Embracing Dignity“, welches an die Notwendigkeit erinnert, sich neben den medizinischen Maßnahmen zur Eliminierung der Lepra auch den sozialen und psychologischen Aspekten der Krankheit zu widmen. Trotz medizinischer Fortschritte, die Lepra zu einer heilbaren und behandelbaren Erkrankung gemacht haben, leiden Betroffene weiterhin unter krankheitsbedingter Stigmatisierung und Diskriminierung. Das soziale Stigma, welches die Krankheit nach wie vor umgibt, führt dazu, dass die körperlichen und psychischen Belastungen der Betroffenen nicht genügend Beachtung finden. Das Motto des Welt-Lepra-Tages 2024 ruft dazu auf, gesellschaftliche Vorurteile zu bekämpfen und die Würde der Betroffenen sicherzustellen.Peer Reviewe

    Die vorliegende Studie soll als epidemiologische Aufarbeitung die tatsächliche Veränderung der bevorzugten Therapieform beim low risk Prostatakarzinom dienen. Um das komplette Bild der Situation in Deutschland zu analysieren, möchten wir die Daten des Krebsregisters anfragen, um die Ergebnisse aller Biopsien sowie Operationspräparate zu korrelieren, welche die Diagnose eines Prostatakarzinoms hergeben. Nur wenn alle Prostatakarzinome aus Deutschland erfasst werden, kann eine adäquate Analyse durchgeführt werden. In der Folge sind die Daten des Krebsregisters nötig. Das nicht-metastasierte Prostatakarzinom bildet eine heterogene Gruppe mit unterschiedlichen histologischen Aggressivitätsmustern. Klassischerweise werden diese Karzinome anhand des Gleason Scores in der Biopsie in verschiedene Risikoformen (low, intermediate und high risk) unterteilt. Für die jeweiligen Risikoformen gibt es eine Vielzahl an Therapieformen wie z.B. die radikale Prostatektomie, die Strahlentherapie oder auch die Aktive Überwachung. Gemäß der Protect-Studie besitzen Patienten aus der Aktiven Überwachung gegenüber einer radikalen Operation die gleichen Überlebensraten nach 15 Jahren in der Gruppe der low-risk Konstellation. In den internationalen Leitlinien ist bereits jetzt die Aktive Überwachung als Therapie der Wahl beim low risk Prostatakarzinom etabliert, um eine definitive Therapie zu vermeiden bzw. hinauszuzögern. Auch die entsprechenden nationalen Fachgesellschaften planen eine Anpassung der bevorzugten Empfehlung auf dieser Ebene. Ziel der Studie ist es, eine epidemiologische Aufarbeitung nach Veränderung der nationalen Leitlinienempfehlung in der Gruppe der low risk Prostatakarzinome durchzuführen, um zu überprüfen, wie viele Patienten wirklich eine leitliniengerechte Therapie erhalten.     When do parents report their child's administrative ADHD diagnosis? A utilisation-based analysis from the consortium project INTEGRATE-ADHD

    Background: This article examines discrepancies in the frequency of diagnoses of attention-deficit/hyperactivity disorder (ADHD) in children and adolescents in Germany using information on health care utilisation from both administrative and parent-reported survey data linked at person level. Methods: 5,461 parents of 0- to 17-year-olds insured with DAK-Gesundheit in 2020 and being registered with a confirmed administrative ADHD diagnosis (ICD-10 F90.0-9) in at least one quarter in 2020 (M1Q criterion) were surveyed online on their child’s ADHD diagnosis, utilisation of specialist care and therapeutic service providers. With regard to the presence of a parental report of the child’s documented ADHD diagnosis, administrative data and survey data were bi- and multivariately analysed. Results: The response rate was 21.5 %. ADHD diagnoses were given more frequently in the context of paediatric care, but in the multivariable model with the administrative data only the diagnosis made by mental health professionals (OR = 2.78), in the model with the survey data only utilisation of mental health professionals (OR = 2.99) positively predicted the parental diagnostic report. With regard to the utilisation of therapeutic service providers, only the utilisation of occupational therapy was associated with the parental report of the diagnosis in both data sources. Conclusions: Parental non-reporting of a child’s administrative ADHD diagnosis in survey studies can be in part be explained by utilisation characteristics.Peer Reviewe

    Wastewater-based Surveillance on SARS-CoV-2

    The SARS-CoV-2 viral load in wastewater is being monitored as part of AMELAG (“Abwassermonitoring für die epidemiologische Lagebewertung”, German for wastewater monitoring for epidemiological situation assessment). Data from 168 sites, i.e. wastewater treatment plants (WWTP), are currently included in the analyses. For the calendar week 31 data from 107 sites were available. Data from other sites will be provided later. After an increase in the aggregated viral load from around mid-May 2024 (20th calendar week), there is currently no relevant change to be observed. Last week’s development may still change due to late notifications, especially due to late notifications from sites with large population coverage
