A literature review on the support for older people with dementia to continue to live at home
Get PDFapplication/pdf在宅生活を継続している認知症高齢者が必要とする支援の内容を明らかにすることを目的とし，文献検討を行った。医中誌webを用い，検索式は「認知症高齢者 and 在宅」等，期間は 2013 ～ 18 年とし，抽出された 15 文献を対象とした。在宅において実施されている支援は【本人・家族の力を引き出すための支援】が最も多く，【心 身状態の安定化を図る支援】【近隣住民や知人からのインフォーマルなサポートを得るための支援】などが挙がっ た。認知症高齢者と家族の自助力の適切なアセスメントとサポート，インフォーマル・サービス活用に向けた環 境整備，アクシデントへの早期対応などが行われていた。また多職種協働による実践内容では【タイムリーに対 応する】【関連する職種をつなげ合わせる】などが挙がった。チームメンバーが，今ある課題の解決にはどの職 種の力が必要かをアセスメントし，多職種に繋げながら認知症高齢者の在宅生活を支援していた。This study conducted a literature review to investigate the support provided to older people with dementia who live at home. Using “older people with dementia and living at home” and some other expressions as key words, we searched articles published between 2013 and 2018 on Ichushi-Web, and 15 articles were extracted. As the support for older people with dementia to continue to live at home, “support to bring out the strength of older people with dementia and their family” was most frequently reported, and “support to stabilize the mind and body” and “support to promote informal support from their neighbors and friends” were also reported. The assessment and promotion of self-care ability of older people with dementia and their family, and the improvement of the environment to promote use of informal services were also observed. In addition, “responding at an appropriate time” and “connecting between the relevant professions” were reported as types of support provided by multi-professional collaboration. Older people with dementia living at home were supported by multiple professionals collaborating to resolve their existing problems.資料departmental bulletin pape
