Current conditions of physical disabilities evaluations in clinical practice during the coronavirus pandemic - A cross-sectional study -
Get PDFapplication/pdf目的は、コロナ禍での総合臨床実習中の身体障害領域検査（血圧、徒手筋力検査、関節可動域測定、脳卒中の運動麻痺検査）に関する実態を明らかにすることであった。研究デザインはwebアンケートによる横断研究とし，2021年度に総合臨床実習を履修した本学作業療法学科4年次生を対象に検査を行った頻度、実行度、検査を通じて難しいと感じたことを聴取した。分析時には Spearman の順位相関係数の算出と質的帰納的分析を行った。結果、ROM測定の頻度と実行度間には有意に中程度の相関があった（r=.57, 95%信頼区間[.24,.84], p＝.011）。また、質的データからは学生が検査の準備から実施。分析という検査の一連の過程で難しさを感じており、身体障害者への適切な検査と、患者に行う検査説明が特に難しかったとわかった。本研究により、総合臨床実習前の臨床実習時に検査を経験すること、作業療法参加型臨床実習に基づいて学生に検査経験を提供することが重要であると考えられた。This study aimed to investigate the current conditions of physical disability evaluations, such as blood pressure, manual muscle testing, range of motion (ROM) measurement, and Brunnstrom Recovery Stage, in clinical practice during the coronavirus pandemic. This was a cross-sectional study that used a web-based questionnaire. Fourth-year students at the university's Department of Occupational Therapy who underwent clinical training in 2021 answered a questionnaire about the frequency with which they performed these examinations, the degree to which they thought they performed them well, and what they found difficult. The Spearman's rank correlation coefficient was calculated and qualitative inductive analysis was performed. Results showed a moderate correlation between frequency of ROM measurement and degree of performance (r= 57, 95% confidence interval [.24, .84], p = .011). Qualitative analysis indicated that students found difficulty in the sequence of tests, from preparation to execution and analysis, and that the appropriate tests for persons with physical disabilities and explanation of the tests administered were particularly difficult. The findings showed that it is important to provide students with opportunities to experience testing during clinical clerkship prior to clinical practice.departmental bulletin pape
A case study of the influence of "Koko-kara classroom" on life satisfaction, motivation for living, and social participation -Occupational record and dialogue focusing on the activities and participation of older adults living in a community -
Get PDFapplication/pdf我々は、地域在住高齢者を対象とした独自の介護予防教室（ココから教室）を開催した。ココから教室の特徴は、活動記録と対話を用いて目標とする作業を特定し、その意味や取り組みを振り返ること、そして参加者自身がその作業がうまくいく方法に気付くよう支援することである。参加した事例A（女性、80歳代後半）は次のような変化があった。カナダ作業遂行測定の遂行スコアは2.3から6.6へ、満足スコアは2.0から6.3へ向上した。高齢者向け生きがい感スケールは6点から 20点へ、社会活動に関する過ごし方満足度尺度は35点から41点へ、生活満足度は5点から8点へ向上した。そしてA氏は水墨画教室へ再び通うようになった。活動と参加に焦点をあてたココから教室は、地域在住高齢者の作業との結びつき、生活満足度，生きがい感を高めることが推察された。また社会参加を促進させる効果が期待され、介護予防に貢献する可能性が示唆された。We conducted a unique class aimed at preventing the need for long-term care (Koko-kara classroom) for older aduls living in the community. The "Koko-kara classroom'" is characterized by the use of occupational records and dialogue to identify target occupation and to reflect on their meaning and approach as well as help the participants themselves become aware of how to engage with the occupation. Participating case A (female, late 80s) had many changes. On the Canadian Occupational Performance Measure, her performance score increased from 2.3 to 6.6 and her satisfaction score increased from 2.0 to 6.3. Her score on the Scale for the Feeling That Life is Worth Living went from 6 to 20 and the satisfaction scale for spending time on social activities increased from 35 to 41. Life satisfaction improved from 5 to 8. In addition, case A started to attend ink painting classes again. The "Koko-kara classroom," which focuses on activities and participation, appears to increase the connection to work, life satisfaction, and a sense of purpose in life among the older adults in the community. It is also expected to promote social participation, suggesting that it may preventing the need for long-term care.departmental bulletin pape
