    Risk Factor Analysis of Work Fatigue on Online Motorcycle Taxi Drivers in Jember Region

    Fatigue is part of the main problem in workers that can reduce health status resulting in accidents to death. Work fatigue is still the main cause of work accidents, especially for drivers. Online motorcycle taxi drivers are a group at high risk of fatigue because most of their working time is on the road. This study aims to analyze the effect of risk factors on work fatigue on online motorcycle taxi drivers in the Jember Region. The type of this research is observational analytic with cross-sectional design. The sample is 100 respondents. The sampling technique used consecutive sampling method. The research was conducted from February 2021 to April 2021 using a questionnaire. Data analysis using Spearman Correlation test and Logistic Regression test. The results of Spearman's analysis were age (p = 0.000), smoking habits (p = 0.000), work motivation (p = 0.000), years of service (p = 0.000), and length of work (p = 0.002). The results of the Logistics Regression analysis are the length of service (p = 0.000). The conclusion of this study is that age, smoking habits, work motivation, years of service, and length of work have an influence on work fatigue and tenure is the most influential risk factor for online motorcycle taxi drivers in Jember Region. The suggestion of this research is that it is necessary to research other risk factors and other fatigue measurement methods and the driver is expected to reduce the level of risk of work fatigue.  Keywords: risk factors, work fatigue, online motorcycle taxi driver

    Caregiving Burden in Family Caregivers Of Patients With Schizophrenia Post-Pasung in Jember: An Interpretative Phenomenological Analysis

    Pasung (restrain) towards people with schizophrenia has yet to recede from being a complex problem in Jember, East Java. One of many aspects of pasung that has not been widely studied is the caregiving burden in the post-pasung period. This research aimed to comprehend the meaning-making of the caregiving phenomenon as experienced by the families of people with schizophrenia during the patients’ post-pasung period in Jember. This research utilizing the interpretive phenomenological analysis (IPA) recruited 11 participants (7 caregivers of people with schizophrenia and 4 community mental health nurses) who were purposively sampled and underwent semistructured interviews from November to December 2019. Two themes emerged in this study namely the experience of the post-pasung caregivers and triangulation with the community mental health nurses which consisted of 8 and 3 subthemes, respectively. Post-pasung caregiving burden experienced by families in this study consisted of subjective, objective, and iatrogenic burdens. Compared with the pasung period, the burden was lighter both for objective and subjective caregiving burden. The most dominating burden is the subjective burdens including apprehensiveness, stigma from the family and the society, and advice from external parties that were against the process of recovery and the objective burden came in the form of the caregiver’s significant health problems. Caregivers made various efforts supported by the provision of accessible and holistic health services as well as interdisciplinary coordination to ease the burden. This study indicates the diversity of the caregiving burden of post-pasung people with schizophrenia. This diversity suggests that interventions aimed at lightening the burden should be carefully selected based on the actual needs of the caregivers.     The Effect of Neem (Azadirachta indica) Gum Administration to Malondialdehyde Level of Wistar Rats’ Kidney Induced by Diazinon

    Diazinon is the most commonly used pesticide in Indonesia. It may increase the production of free radicals that triggers the lipid peroxidation process which will produce the final product, MDA, which can damage cells and tissues, especially the kidneys. Neem gum with its high polysaccharide is a good antioxidant agent to neutralize free radicals. The purpose of this study was to determine the effect of neem gum administration to prevent the increase in kidney MDA levels of wistar rats induced by diazinon. Thirty wistar rats were divided into 6 groups, K0 (given cornoil), K1 (induced DZN 40 mg/kgBW), groups P1, P2, P3, and P4, administered with neem gum as a drinking water at a dose 3.75 g/kgBW, 7.5 g/kgBW, 15 g/kgBW, and 30 g/kgBW and induced by DZN 40 mg/kgBW. The treatment was carried out for 8 days, in which diazinon induction and neem gum solution were administered simultaneously. The average renal MDA levels are K0=20.85±1.10; K1=26.98±5.87; P1=26.84±3.75; P2=21.43±3.44; P3=20.23±3.27; P4=21.99±1.70. One Way ANOVA test showed a significant difference (p<0.05). The LSD posthoc test showed a significant difference in the treatment groups P2, P3, and P4 compared to the K1 group (p<0.05). This shows that the neem gum solution at a dose of 7.5 g/kgBW, 15 g/kgBW, and 30 g/kgBW can prevent the increase of kidney MDA level in wistar rats induced by diazinon.     Relationship between Ferritin Levels with Calcium Levels in Patients Thalassemia β Major in Hospital in Jember

    Thalassemia β major is a hereditary disorder caused by mutations in the β-globin gene, which regulates the formation of one of the components that make up hemoglobin. This disorder results in the production of β-globin chains being reduced or not formed. The prevalence of thalassemia carriers in Indonesia reached 3.8% of the entire population based on data from the Ministry of Health in 2019. Therapy for thalassemia β major patients is lifelong transfusion, which results in iron overload and triggers hypocalcemia and osteoporosis. The aim of this study was to determine the relationship between ferritin levels and calcium levels in thalassemia β major patients. This study uses an observational analytic research type and a cross-sectional research design. This research was conducted at the RSD dr. Soebandi Jember, Jember Clinic Plantation Hospital, and in December 2019-January 2020. The total sample was 13 patients suffering from thalassemia β major aged 1-18 years. The results of the Pearson analysis showed that p=0.858  showed that there was no  ignificant correlation between ferritin levels and calcium levels in Thalassemia β major in Jember.    Keywords: Thalassemia β major, ferritin levels, calcium levels, hypocalcemi

    Immunization Stress-Related Response (ISSR) from Covid-19 Vaccination First-Dose among Medical Clerkship Students in dr. Soebandi Hospital Jember

    Vaccination is one of the recommended way to eradicate Covid-19 pandemic. Unfortunately, there are still many people who refuse to get vaccinated. One of the causes is anxiety about the side effects that occur after vaccination. With this misinformation about vaccines, public anxiety is getting higher, so that more and more people are hesitant to get vaccinated. The purpose of this study was to determine the level of anxiety related to vaccines in medical students who have more knowledge about vaccines than the general public society. The anxiety assessment method used was the Hamilton Rating Scale for Anxiety (HARS) questionnaire. There were 108 medical clerkship students in this study, consisting of 25 male respondents and 83 female respondents, aged between 21-27 years. Based on the HARS-score obtained, it showed that all respondents were in the low or minimal anxiety range. Aspects of anxiety predominately appeared in the pre-vaccination phase. The differences between men and women were influenced by hormonal fluctuations and conditions before vaccination, such as stress, workload, knowledge capacity, and information obtained related to vaccination.     Increased of IL-4 and Ig E Level in Infection Hookworm in Plantation Workers in Jember

    Hookworm infection is a disease that is included in the neglected tropical disease. Hookworm infection is suffered by many plantation workers who are closely related to the soil in daily life. Immune response in hookworm-infected individuals will experience disorders, especially in eosinophilia, mastocytosis, and IgE stimulation. It is therefore important to know the immune response to hookworm infections in plantation workers. In this study, eosinophilia was calculated on peripheral blood smears and counts of IL-4 and Ig-E levels by ELISA. The results showed an increase in eosinophil levels from blood samples of patients and a significant increase in IL-4 and IgE levels based on the results of statistical analysis.     Cardiology Services During The Second Year Of The Covid-19 Pandemic

    Cardiology services have been affected by the COVID-19 pandemic. The consequences of the disease on cardiology services have not been studied in Indonesia, especially in the second year of this pandemic, as the surge in infections is currently declining. It is necessary to quantify the impact of the COVID-19 pandemic on cardiology services to evaluate whether or not cardiac services have returned to normal during this second year of the pandemic. The basis of research used in this study is a survey using a Google Form application. The technique to determine the research subjects carried out in this study is non-random sampling with the Purposive Sampling technique.One hundred and fifty-seven subjects were included in this study, with the majority gender of men (61.1%) as a clinical cardiologist (50.3%).  Data were collected from several provinces in Indonesia from Jakarta to West Nusa Tenggara, with National Cardiovascular Center Harapan Kita as a majority hospital (n=15, 9.55%). There were generally sustained reductions in all aspects of cardiology service. However, the number of outpatients who underwent cardiac device implantation procedures did not increase. Telemedicine has been applied to 52.9% in the hospital where the subject works, yet the proportion of patients receiving telemedicine are less than 25%. During the second year of the COVID-19 pandemic, cardiology services need to establish new strategies to aid people with cardiovascular disease.     The Correlation between Fatigue and Quality of Life on Knee Osteoarthritis Patient at RSD dr. Soebandi Jember

    Osteoarthritis (OA) is a form of chronic arthritis that mainly attacks the knee joint. The prevalence of knee OA in Indonesia, especially in East Java, is around 27% which is relatively high. Therefore, this study aims to determine the correlation between fatigue and quality of life from knee OA patients at RSD dr. Soebandi Jember. The methodology uses in this study is the analytical observational method with a cross-sectional survey as the research design. For the data, this study uses primary and secondary data. The primary data was obtained through online interviews by phone using a questionnaire instrument, namely Fatigue Assessment Scale (FAS) and World Health Organization Quality of Life (WHOQOL) – BREF. Then, the secondary data was obtained from the medical records of knee OA patients who were undergoing treatment at the RSD dr. Soebandi Jember in the period January 2019 to December 2020. The results of this study showed that the p-value of fatigue with quality of life domain 1 (physical health) was 0.042, domain 2 (psychological) was 0.087, domain 3 (social) was 0.221, and domain 4 (environmental) was 0.426. Therefore, this study concludes that there is a weak relationship between fatigue and quality of life on domain 1 (physical health), with a correlation coefficient of 0.373. Also, there is no relationship between fatigue and quality of life on domain 2 (psychological), domain 3 (social), and domain 4 (environmental) in knee OA patients at RSD dr. Soebandi Jember.
