Reliability of Using the Scabies Detection Form (DESKAB) in Islamic Boarding Schools

Scabies is an infectious skin disease caused by the Sarcoptes scabiei. This mite infestations not only cause skin damage, but can develop into local, even systemic skin infections. Therefore, instruments are needed that help early detection of scabies, especially through diagnosis in communities such as Islamic boarding schools. This study aimed to analyze the reliability of the scabies detection form (DESKAB) between Madrasah Aliyah female student rater and dermatovenerologist rater. Based on the results of the examination and Cohen's kappa test, a coefficient of 0.84 was obtained, which means that the level of reliability of the DESKAB form is strong, and a p value of 0.019 means that there is agreement in the perception of the diagnosis of scabies by the two raters. The DESKAB form can be recommended as an early detection instrument for non-medical personnel before being confirmed by medical personnel Keywords: DESKAB; diagnosis; non-medic; scabie

    No full text
Seborrhoeic dermatitis is a skin disease with erythropopulosquamous morphology in areas of the skin that are rich in sebaceous glands such as the folds, scalp and face. The prevalence of seborrheic dermatitis worldwide is only around 5%, but the incidence of the non-inflammatory type of this disease (dandruff) reaches 50%. Seborrheic dermatitis can appear together with other types of dermatitis such as contact dermatitis, such as allergic contact dermatitis or irritant contact dermatitis. In this case, we will describe the incidence of seborrheic dermatitis accompanied by allergic contact dermatitis. Diagnosis in cases of seborrheic dermatitis accompanied by contact dermatitis must be carried out appropriately according to the subjective data and efflorescence obtained. The correct diagnosis influences appropriate management and a better prognosis for the patient. Keywords: Seborrhoeic dermatitis, allergic contact dermatiti

Intestinal Evisceration and Hemorrhagic Shock Due to Penetrating Abdominal Trauma in Elderly Patient at Rural Area: : A Case Report

Penetrating trauma is the most frequent cause of death in patients aged 65 years and over. Penetrating trauma is often caused by armed violence, both firearms and sharp weapons, and occurs in urban civilian populations. In this case, penetrating trauma occurred in an elderly patient due to an abdominal stab wound caused by a palm sickle when harvesting palm fruit. The location of the incident was in a rural oil palm plantation area and far from the hospital. Limited facilities and resources at the community health center where the patient was first treated, resulted in initial treatment being less than optimal and inadequate. The penetrating abdominal trauma that occurred in this patient was accompanied by intestinal evisceration and hypovolemic shock which was an absolute indication for immediate laparotomy exploration. The patient's comorbidities in the form of chronic heart failure and the patient's elderly age need to be taken into consideration and attention to prevent initial and subsequent complications. After laparotomy, the patient was able to survive and recover well without any complications. Keywords: Penetrating abdominal trauma, intestinal evisceration, laparotom

Malondialdehyde Expression in HT29 Cells with Blastocystis sp. Exposure

Blastocystis sp. is one of the intestinal protozoa that is frequently found in stool specimens of colorectal cancer patients. This protozoan infection will trigger the host cell defense response to produce Reactive Oxygen Species (ROS) which causes oxidative stress. Oxidative stress can cause and aggravate colorectal cancer. Lipid peroxidation is one of the best indicators of ROS levels. Malondialdhyde is its end product so it is used as a marker of oxidative stress. This study aims to determine the exposure of Blastocystis sp. on malondialdehyde expression in colorectal cancer cell line HT29. This research is an experimental study in vitro. HT29 cells were treated with various concentrations of Blastocystis sp. antigen (0.005 μg/ml, 0.01 μg/ml, 0.05 μg/ml, 0.1 μg/ml and 0.5 μg/ml) for 72 hours. Malondialdehyde expression was examined using spectrophotometric method. Data analysis was performed using One Way Anova test.  The results of the study are the mean expression of malondialdehyde in nmol/ml. The mean malondialdehyde levels of the control group, treatment with concentrations of 0.005 μg/ml, 0.01 μg/ml, 0.05 μg/ml, 0.1 μg/ml and 0.5 μg/ml were 3.134 nmol/ml, 2.682 nmol/ml, 2.556 nmol/ml, 2.744 nmol/ml, 2.84 nmol/ml and 3.286 nmol/ml respectively. The conclusion is that there is an effect of exposure to Blastocystis sp. on malondialdehyde expression of HT29 cells. Keywords: Blastocystis sp., HT29, malondialdehyd

Anti-lipase Activity of Kemuning (Murraya paniculata) Leaves Extract and Its Fractions

Kemuning (Murraya paniculata) belongs to the Rutaceae family, it grows a lot in tropical areas such as Indonesia. Traditionally, Kemuning leaves are used for weight loss preparations and are believed to contain compounds that can inhibit pancreatic lipase, including flavonoids. This study aimed to reveal the antilipase activity of Kemuning leaves extract and its fractions. The extraction of Kemuning leaves was carried out by maceration with 70% ethanol as solvent. The vacuum liquid chromatography method was used to separate the secondary metabolites in the extract with the stationary phase of silica gel and the eluent mixture of n-hexane, ethyl acetate, and methanol in a gradient manner. Meanwhile, the antilipase activity was tested by colorimetric method, with p-nitrophenyl butyrate as a substrate and orlistat as a positive control. The extract obtained was 19.94% of the weight of the simplicia. The separated extract was collected in 20 flasks and then combined based on the Rf value of the spots observed on thin layer chromatography (TLC) producing 8 fractions (fractions A-H). The percentage yields of fractions A-H were 1.20, 1.33, 1.80, 3.92, 3.57, 4.20, 16.85, and 14.47%, respectively. Antilipase activity was indicated by IC50 values, which were 17.603 ± 1.660 and 108.554 ± 1.660 µg/mL for orlistat and extract, respectively. The antilipase activities of fractions A-H were 130.19 ± 2.90, 119.52 ± 1.95, 96.95 ± 1.81, 91.65 ± 0.78, 95.30 ± 1.46, 114.76 ± 2.87, 127.07 ± 3.56, and 134,04 ± 3.16 µg/mL, respectively. Based on these results, it can be concluded that the extract and its fractions exhibit very strong antilipase activity. Keywords: Murraya paniculata; extract; fraction; antilipas

Enhanced Recovery After Cesarean Surgery (ERACS) Method Accelerates Mobilization and Duration of Hospitalization After Cesarean Section

ERACS is a caesarean section method that supports recovery more quickly. The principle of ERACS is the use of multidrug anesthesia, namely bupivacaine at a reduced dose (<10 mg) and combined with fentanyl and morphine to maintain the quality of anesthesia so that patients can mobilize more quickly with minimal pain and reduce the duration of hospitalization. This research used an observational analytical design with a cross sectional approach in September – November 2023 at the Bina Sehat Jember Hospital. This study used 100 samples medical records with sampling using a purposive sampling technique and calculating using Slovin formula. The data was analyzed using Mann-Whitney test with a confidence level of 95%. The research results showed that the average walking mobility ability after ERACS was 10,10 hours, while conventional was 14.76 hours and the duration of hospitalization after ERACS was 2.22 days, while after conventional was 2.98 days. This is supported by the results of statistical tests which show that the significance value (p) for both variables is 0.000 (p < 0.05), which means there is a significant difference between the speed of walking mobilization and the duration of hospitalization with the SC method used. The conclusion of this study was that ERACS can speed up the ability to mobilize after surgery by up to 4.66 hours and the duration of hospitalization is 0.76 days shorter than conventional. Keywords: ERACS, mobilization, duration of hospitalizatio

The prevalence of anemia in pregnant women in Indonesia is still high. Anemia in pregnancy has an impact on maternal and fetuses. We conducted a study on factors related to the incidence of anemia in pregnancy in pregnant women who had their first antenatal visit. The study was conducted on 218 pregnant women who had their first antenatal visit at the Maternal and Child Health Polyclinic at the Sooko Public Health Center, Mojokerto District in January – March 2021. We collected data on risk factors related to the incidence of anemia in pregnancy, namely maternal age during pregnancy, gestational week, distance between current and previous pregnancies, history of parity and body mass index. Chi-square analysis was performed to assess the relationship between variables with a significant value of p<0.05. It was found that 54 (24.8%) of pregnant women had anemia. Gestational age at risk (p=0.970), distance from previous pregnancies (p=0.135), history of parity (p=0.420) and body mass index (p=0.577) had no significant relationship with the incidence of anemia in pregnancy. Gestational age had a significant relationship (p=0.000) to the incidence of anemia in pregnancy. Gestational age has a relationship with the incidence of anemia in pregnant women who make their first antenatal visit. Anemia in pregnancy has a high prevalence. Further treatment is needed, such as giving Fe tablets and educating pregnant women during antenatal care to prevent and treat anemia in pregnancy. Keywords: anemia in pregnancy, gestational week, antenatal car

The Correlation between Thrombocyte, Hematocrit, and Leukocyte Count with Length of Hospitalization in Pediatric Patient with Dengue Hemorrhagic Fever (DHF)

Dengue Hemorrhagic Fever (DHF) is a disease caused by dengue virus with mosquito vectors. Supporting examinations to determine the course of DHF include platelet counts, hematocrit values, and leukocyte counts. Length of hospitalization is the number of days when the patient receives treatment in the hospital, calculated when recorded as an inpatient until discharge from the hospital. This type of research is an analytic observational study with a cross sectional research design. Cases of pediatric DHF at Bina Sehat Hospital Jember for the period 2020-2022 totaled 296 patients. Data were obtained using medical records. Of the 296 cases of pediatric DHF, samples were then taken by purposive sampling according to the researcher's criteria until 44 samples were obtained. The data were then analyzed using Rank Spearman. The results of the data analysis showed that there was a strong opposite relationship (p = 0.000; r = -0.562) between platelet count and length of hospitalization, a strong unidirectional relationship (p = 0.001; r = 0.495) between hematocrit value and length of hospitalization, and a strong opposite relationship (p = 0.005; r = -0.415) between leukocyte count and length of hospitalization. Suggestions for future research are to use other variables such as nutritional status, medical history, and other laboratory examinations.
