    ONE-PIECE DENTAL IMPLANT UNTUK REHABILITASI RUANG KANINUS YANG SEMPIT

    Background: Tooth loss has serious functional, esthetic disabilities, and compromising the patients’ quality of life. Dental implant treatment has become increasingly common for the management tooth loss. Different clinical situations, such as narrow edentulous space, are often encountered in which modifications have to be considered in order to achieve successful rehabilitation. One piece dental implan provided a solution for rehabilitation of narrow edentulous space. The purpose of this paper is to report a rehabilitation management of narrow edentulous space. Case Management: A 22-year-old woman had an upper right canine #13 malposition so there was a narrow space between upper right lateral insisiv #12 and upper right first premolar #14. The spacing between upper right lateral insisivus #12 and upper right first premolar #14 was found to be about 5 mm. Radiograph was assessed for type of bone and for the presence of any pathology. A patient given an initial dental treatment such as scalling root planning and extraction of caninus #13. Following the clinical evaluation, the procedure and complication of implant therapy were explained and consent taken for proposed treatment. One piece dental implant site was anesthetized. Flaps on the buccal aspect were raised. One Piece dental implant inserted using dental implant kit. On completion of treatment, the patient’s esthetics and funcions improved.     Background: Several processes in a teeth treatment often inflict wound ingingiva. Collagen is the key component of the wound healing. The collagen roles in advancing wound healing particularly, triggering the synthesis to induces collagen and cellular migration, and also inducing clotting cascades. Wound treatment using natural product is potential in accelerating wound healing, in particular duck egg membrane which contains collagen, glucosamine, and hyaluronic acid. This study aims to find out the influence of gel extract application of duck egg shell membrane 10 % towards collagen solidity on gingiva wound recovery process (study of Rattus norvegicus).Method: The subjects of the study consist of 20 male Rattus norvegicus aged 2-3 months which were given wound on labial gingiva of the maxillary central incisors with punch biopsy. The samples were divided into 2 groups, they were positive control group (Aloclair Gel®) and experimental group (gel extract of duck egg shell membrane 10%), each of them was treated topically twice a day for one minute.The assessment of collagen fiber was done by giving score over the collagen fiber solidity of the histological tissue through 6 points of view. The observer done by two person (blind interpretation) score of collagen fiber solidity.Result: The analysis result showed that there was a significant difference on collagen fiber solidity (p>0,05) between positive control groups and experimental group on the 3rd, 7th, and 14th day.Conclusion: This study concludes that the application of topical gel extract of duck egg shell membrane 10% affects the improvement of collagen fiber solidity on the process of healing gingiva wound on Rattus norvegicu

    Background: Cleft palate leads to oroantral communication and also malformations of the palate and agenese of teeth adjacent to the defect. Abnormal defect closure was done by replacing the hard, soft tissues and missing teeth using an intraoral maxillofacial prosthesis called obturator. Purpose: Assess the use of prosthetic rehabilitation using frame obturator combination with acrylic resin on patient’s palatum defect to restore esthetic, speech, swallowing, mastication functions.Case: 23 years old male patient with chief complaint palatum defect that cause nasal voice. Intraoral examination revealed a large palate defect in the right of palatum durum segment, also defect on left labial gingiva, and missing teeth in 13, 15, 16, 21, 22, 23, 24, 25, 26, 35, 36, 45, 46.Case Management: Obturator was made to cover defect of palate and replace missing teeth. Impression was taken with hydrocoloid irreversible and gauze that cover the defect to avoid alginate entering nasal cavity. Insertion of obturator showed cleft palate was covered well by frame combination with acrylic resin base. The retention, stabilization and occlusion were good, nasal voice was reduced, defect on left labial gingiva were covered by labial wing.Discussion: Patient was satisfed because nasal voice was reduced, estethic,mastication and swallowing function were restored, also. Patient can clean and use obturator easily.Conclusion: Frame obturator in patient’s palate defect can reduce nasal voice, restore esthetic, swallowing and mastication functions

    Background: The number of elderly population growing fastly in Indonesia along with the increase of life expectancy. Oral health of elderly people greatly affect the quality of life. Oral health in the elderly is reflected in the number of missing teeth (tooth loss), dental caries and periodontal disease. Objective: To know the relationship of Elderly oral health behavior with DMFT indexMethods: Cross-sectional survey with WHO’s method was done by multistage random sampling at 124 age 45-64 years old in Kronjo sub-district, Tangerang.The data was collected by DMFT examination and the standard questionnaire of WHO survey. Calibration test for four examiners with Kappa value was 0.75Results: The mean score of DMFT was 15.5 is included in the very high category. There was statistically significant association between elderly selfassessmentand toothpaste use with DMFT index (p=0,016; p= 0,009). There was no significant association between sweet consuming and dental pain with mean DMFT index (p = 0.216; p = 0.324)Conclusion: Oral Health Behavior of the Elderly in Kronjo sub-district is stillpoor with a very high score of DMFT that will require immediate attention fromthe government

    Background: In recent years, the use of tobacco in Indonesia increasingly varied in the forms and methods of consumption, one of which is shisha. Smoking shisha is increasingly popular due to a misperception that smoking shisha is harmless and lack of knowledge about the effects of shisha smoking in oral health.Literature analysis: “PubMed†used as a search tool to identify all empirical studies related to the effects of shisha smoking on health, especially in oral cavity.Discussion: Shisha smoke contained various toxic substances such as Nicotine, Tobacco Specifc Nitrosamines, Polycyclic Aromatic Hydrocarbons, Volatile Organic Compounds, Carbon Monoxide, Tar, and high-temperature metal heating causing shisha smoke contained toxic heavy metals such as arsenic, nickel, cobalt, chromium, lead, and cadmium. The content of these toxic substances showed that smoked shisha is associated with dependence, acute and long-term negative health effects similar to cigarette smoking. Toxic substances may cause various infections of microorganism such as Candida sp, Herpes Simplex Virus (HSV-1), Epstein Barr Virus, Mycobacterium tuberculosis, Human Immunodefciency Virus; Oral mucosal changes such as Hairy Tongue, Smoker’s Melanosis, Nicotine Stomatitis, Frictional Keratosis, Fissured Tongues, gingival or periodontal inflammation, and leukodema; and lead to malignant lesions such as Keratosis, Leukoplakia, Erythroplakia, Oral Submucous Fibrosis and Lichenoid Lesions.Conclusion: Smoking shisha gives bad impact for human health especiallyoral health. Shisha smoking can lead to the development of various infectious diseases and potentially lead to malignancy in the oral mucosa. These foundings breaks the belief that shisha smoking is safe for health

    Background: Estrogen plays a key role in human physiological processes. Polymorphisms of estrogen receptors have been implicated in the development of numerous diseases. The aim of this study was to evaluate the frequency of ERα gene Pvull and Xbal polymorphisms and assessing their association with CD4+ T-cell counts in HIV-infected children on highly active antiretroviral therapy.Methods: CD4+ T cell counts were determined using the FACS count system. ERα PvuII and XbaI polymorphisms were analyzed by PCR-RFLP.Results: This study enrolled 34 HIV-infected children on HAART. The frequencies of the PvuII and XbaI gene polymorphisms were PP 41,2%, Pp 26,5%, pp 32,4% and XX 35,3%, Xx 17,6%, xx 47,1% respectively. CD4+ T-cell counts were significantly associated with XbaI polymorphisms (p0.01).Discussion: Host genetic factor polymorphism is an important determinant of HIV  disease progression and treatment response. The ERα Pvull and Xbal polymorphisms can increase risk for the development of HIV-related complication,including oral diseases.Conclusion: The ERα gene XbaI polymorphism was significantly associated with CD4+ T-cell counts. It may explain the role of estrogen in the regulation of HIV replication. Studying human genetic variation in HIV-infected individuals is important to guide a new therapeutic approach

    ORAL HYGIENE BURUK PASIEN RAWAT INAP TIDAK BERKAITAN DENGAN PERTUMBUHAN ORAL CANDIDIASIS

    Background: Nosocomial infections can develop in the oral cavity due to poor oral hygiene. Oral Candidiasis is one of the most frequent nosocomial infection in oral cavity. The objective of this study is to describe the oral hygiene and oral candidiasis in hospitalized patients. Method: This analitical observational study used cross sectional design. Subject of the study were 74 adult patients aged 20-65 who are hospitalized at the Sultan Agung Islamic Hospital Semarang. Oral hygiene assessed from the presence of debris and calculus on the tooth surface using Simplified Oral Hygiene Index (OHIS) that were grouped in three categories (good, moderate, bad). Oral candidiasis diagnosed through clinical examination and swab procedure of suspicious lesions, and identified the presence of its spores and hyphae under a microscope observation. Kendal Tau test is used to analyze the correlation between oral hygiene with oral candidiasis. Results: Results showed patients with oral candidiasis in the poor and moderate oral hygiene were 29.7% and 1.4%, whereas patients without oral candidiasis in the poor and moderate oral hygiene were 60.8% and 8.1%. Kendall Tau correlation test results p value of 0.235 (p> 0.05). Conclusion: Conclusion of this study showed there is no association between oral hygiene with oral candidiasis

    Background: Organosulfur compound, allicin and phenolic are the main compound in garlic responsible for antioxidant activity. Correlation between free radicals and some dental diseases, such as free radicals and Reactive Oxygen Species (ROS) responsible for periodontal inflammation, caries, lesions and oral cancers. The research objective is to get scientifc data about the antioxidant activity from garlic extract (Allium sativum).Method: The research was conducted using three different varieties of garlic:local garlic varieties Ciwidey, single local garlic’s clove, and import garlic which is found at the market in Bandung. Garlic extract is made by maceration using ethanol 96% and antioxidant activity test using DPPH method (1,1-diphenyl-2- picrylhydrazyl). Statistical analysis using One Way ANOVA followed by post hoc analysisResult: three varieties of garlic have a strong antioxidant activity. Value of IC50 = 13.61 mg / ml for local garlic varieties Ciwidey, IC50 = 10.61 mg/ml for single local garlic’s clove and IC50 = 11.32 mg/ml for imports garlic.Conclusion: Three varieties of garlic have different antioxidant strength and the best antioxidant strength is single local garlic’s clove

    Background: Down syndrome (DS) is a congenital disorder caused by abnormalities of chromosome 21, resulting in migration defect of neutrophils, specifically in GCF as a marker of increasing periodontal infection. This study aimed to analyze the difference in neutrophil numbers between down syndrome and healthy children.Method: This research was an analytic observational with cross sectional design, and divided into two groups. The control group is consisted of healthy children and the other is consisted of down syndrome children. GCF was taken using paper point number 45-50 for 30 seconds, then it smeared into object glass and painted with giemsa staining. Observations was done by light microscopy with 1000 times magnification.Result: The result of studies with p<0.05 was indicating a significant difference of neutrophil numbers between children with down syndrome and the healthy group.Conclusion: This study concluded that there was a difference in neutrophil numbers due to the migration defect of neutrophils in children with down syndrome, that can cause proneness to periodontal infections

    Background: Lactobacillus acidophilus, Gram-positive bacterias that enterthe dentinal tissue during the carious process are suspected to influence the immune response in human dental pulp. Odontoblasts situated at the pulp dentin interface are the frst cells encountered by these bacteria and have an important role in this response. Lipoteichoic acid (LTA), a wall component of Gram-positive bacteria, triggered the activation of the odontoblasts. LTA upregulated the expression of its own receptor TLR2, as well as the production of proinflammatory cytokineTNFα. Propolis is a resinous material that holds a great potential as an antiinflammatory agent. Present studies have shown that propolis has a reduction effect towards the proinflammatory cytokines expression and favor pulp healing.Purpose: To reveal the moleculer mechanism of propolis stimulation onodontoblast like cells, induced by inactive Lactobacillus acidophilus.Methods: This review was presented in odontoblasts like cells culture induced by inactive Lactobacillus acidophilus and exposed to propolis extract. Pulp cell culture isolated from human impacted thirds molar that has been extracted. Observation and measurement the expression of TLR2 and TNFα was processed by using immunocytochemistry (ICC) technic.Result: Data analysis with ANOVA test, a signifcant difference in every group(p<0,05) was present. The expression of TLR2 and TNFα were shown at lowlevel percentation on odontoblast like cells, induced by Lactobacillus acidophilus and propolis extract stimulation.Conclusion: Propolis extract stimulations lower the TLR2 and TNFα expression on odontoblast like cells, induced by inactive Lactobacillus acidophilu
