LESI ORAL TERKAIT LEUKEMIA MIELOID KRONIK: LAPORAN KASUS
Background: Oral lesion is one the clinical signs found in chronic myeloid leukemia (CML). CML is a myeloid cell neoplasm that have oral manifestations such as gingival hyperplasia, spontaneous haemorrhage, petechiae, oral infection, mucosal pallor, and ulceration. The purpose is to find out how to diagnose and oral lesions associated CML to occure, so the dentist could provide the right therapyCase Management: 46 years old male was referred from Internal Medicine Department with diagnosis CML. The patient complained of pain at the right upper gum. Cheilitis exfoliative was found at the extra oral examination, while intra oral examination found ulcer at the palate. Blood test result showed decreases of Haemoglobin, Hematocrite, erythrocytes which showed iron deficiency anemia causes disruption of oral mucosal integrity, thereby facilitating oral ulcerations. Based on history, clinical examination and laboratory investigation, intra-oral abnormality was diagnosed as oral lesion associated CML. Triamcinolone acetonide 0.1% in orabase as theraphy oral lesion, vaseline album for management exfoliative cheilitis, and Surbex Z® as multivitamin. Oral ulcer improved within 6 weeks.Conclusion: The diagnosis oral lesions associated CML can be diagnosed based on history, clinical examination and laboratory investigation. Oral ulcers in this patient caused by iron deficiency anemia
PENGARUH KONSENTRASI GEL THEOBROMINE TERHADAP KETAHANAN KEKERASAN PERMUKAAN EMAIL GIGI DESIDUI
Background: Primary tooth enamel are susceptible to damage because it has a low thickness so it requires preventing treatments to maintain. Theobromine, an alkaloid of the methylxanthine group, has been shown to improve crystallinity and resistance of enamel apatite solubility to acids. The enamel resistance to acid requires a series of test of physical properties, by measuring at microhardness. This study analyzes the effect of theobromine gel consentration on the hardness resistance of primary incisors enamel surfaces.Method: Twenty-fve specimens were divided into 5 groups of topical application of theobromine gel with concentrations of 1%, 2%, 3%, 4%, and 5%, and then immersed in acidic solution. Results were analyzed using one-way ANOVAResult: One-way ANOVA test showed signifcant differences in the increase and decrease in enamel microhardness after the application of theobromine gel and acid immersion (p <0.05). The highest values of increase and decrease in enamel microhardness (764,74±187,08 VHN and 457,18±54,37 VHN, respectively) was shown in 5% group of theobromine gel.Conclusion: The higher theobromine gel concentration, the higher the microhardness value but unable to maintain the enamel resistance against acid. The 5% theobromine gel concentration was the most effective one to increase the microhardness of primary teeth enamel
MANAGEMENT OF BURNING MOUTH SYNDROME: A LITERATURE REVIEW
Background: Burning mouth syndrome (BMS) is a chronic idiopathic oral dysaesthesia that presents as a burning sensation in the oral cavity usually without any specific oral lesions or laboratory findings. Burning sensations may appear on the buccal mucosa, hard and soft palate, tongue and lips. BMS appears to be more prevalent in postmenopausal women. The term menopause is often used for a condition in which the permanent cessation of the primary function of the ovaries in humans that occurs especially in middle-aged women. Available evidence suggested that BMS is a multifactorial disorder with physiological basis. Pathophysiology of BMS remains unclear. The etiopathogenesis in most patients who complain of burning sensations have interactions with several factors such as local, systemic, and/ or psychogenic factors. The aim of this literature review was to assess and evaluate the management of BMS comprehensively.Discussion: The therapy of BMS including hormone replacement therapy,systemically therapy, such as antidepressants, clonazepam and topical medication (clonidine and capsaicin) and reassurance as the stress management.Conclusion: Proper management of the BMS involves the combination of pharmacologic treatment and psychotherapy, as well as reassurance is an important thin
TANTANGAN FARMAKOLOGI PENGGUNAAN STEROID TOPIKAL PADA PASIEN ORAL LICHEN PLANUS
Background: Oral lichen planus (OLP) is a chronic autoimmune disease sprinkled by T cells on the oral mucosal surface. The goal of OLP treatment is to eliminate erythema, ulceration and relieve symptoms. Corticosteroids are the first line in the treatment of OLP, either systemically or topically. The problem arises when using topical steroids, namely the time of topical steroid attachment to the oral mucosa, especially in cases of OLP with clinical desquamative gingivitis. Some studies suggest that the use of individual gingival tray can overcome this problem. Objective: To provide information on topical steroid use in OLP cases with desquamative gingivitis clinical signs.Case Management: Women, age 31, complain that pain with burning on the lips and mouth is aggravated by spicy food. Intraoral examination shows irregular white plaques on the buccal, labial and dorsal mucosa of the tongue accompanied by diffuse erythema in the anterior gingiva of the upper and lower jaws. Patients diagnosed with OLP. Lesions improve 3 months after topical steroid administration, except lesions on the gingiva. Gingival individual tray is then used to obtain adequate attachment of topical steroids on the gingiva surface. The gingiva showed significant improvement after a month later.Conclusion: The use of topical steroid concoctions applied to the gingival individual tray is effective in treating OLP lesions especially with clinical signs of desquamative gingivitis
HUBUNGAN PENGETAHUAN DOKTER GIGI TERHADAP PENGGUNAAN ALAT PERLINDUNGAN DIRI (APD) DI RUMAH SAKIT ISLAM GIGI DAN MULUT SULTAN AGUNG SEMARANG
Background: A cross infection is the displacement of the infection agents between patients and health ofcers. Knowledge and behaviors of using personal protective equipment (PPE) are needed to prevent and control the infections. The aim of this study is to fnd out the behaviors of the dentists in using PPE at RSIGM Sultan Agung Semarang.Method: This research is descriptive study using cross sectional approach. There were 19 dentists as the respondents. It was using sampling technique. The data collection was done by providing questionnaire concerning the knowledge and observing the behaviors in using PPE. Statistical analysis used Spearman rho.Result: The Spearman rho test obtained the sig value 0,043 (P<0,05) with a correlation coefcient of 0,469. It can be concluded there is signifcant relationship between the dentist knowledge on the use of PPE at RSIGM Sultan Agung Semarang, with the strength of moderate relationships. Observation results of dentist behavior showed: 42.2% had a moderate category.Conclusion: Thus, it can be concluded that the knowledge is related to dentist behaviors in using PPE is in moderate category. Moreover, the dentist behaviors in using PPE can be influenced by several factors such as the lack of isolation awareness, the surveillance of Prevention and Control of Infection (PPI), the PPI education and training
VIRUS AS A CAUSE OF SALIVARY GLAND DISEASES
Background: Enlargement in the extraoral region with the absence of abnormal dental and periodontal structures are sometimes seen in dental practice, sometimes followed by xerostomia. Enlargement of the acute nonsuppurative salivary glands has been associated with several types of viruses. The purpose of this paper is to review salivary gland diseases associated with non-HIV and HIV viral infections.Discussion: Non-HIV viruses which were detected in the salivary glands including Paramyxovirus, cytomegalovirus (CMV), Hepatitis C virus (HCV), human papilloma viruses (HPV), Epstein-Barr virus (EBV), human herpes simplex virus (HHSV-8), and coxsackie virus. HIV-associated salivary gland disease typically presents with xerostomia and/or intraglandular lymph nodes, and diffuse infiltrative lymphocytosis syndrome (DILS). The most common viral infection conditions in salivary gland disorders are mumps and HIV. Enlargement and inflammation of the glandular structures will affects the control of salivary secretion by nerves. Parasympathetic nerves block conducted signals to the salivary glands, so the salivary flow isdecreased.Conclusion: There is association between viral infection and diseases of thesalivary gland. By knowing sequelae viruses on the salivary gland, dentists are expected to understand the clinical condition and therapeutic that should be given to the patients
RECURRENT APHTHOUS STOMATITIS RELATED TO PSYCHOLOGICAL STRESS, FOOD ALLERGY AND GERD
Background: Recurrent Aphthous Stomatitis (RAS) is inflammation in oral mucosa characterized by recurrent single or multiple ulcers that usually affected in non keratinized mucosa. Etiology RAS is unknown but psychological stress, allergy, and gastrointestinal disease can be predisposing factors Case Management: A 23rd years old complained recurrent oral ulcer with free ulcer period for 3-5 days. The patient had a history of food allergy, GERD and psychological stress. Intraoral examination showed recurrent multiple ulcers in variation site of the mouth. DASS 42 screening showed high stress and high anxiety. Skin Prick Test showed positive allergy to kapok, beef, chicken, cow milk, white egg, duck egg, shrimp, cob fish, milkfish, chocolate, and peanut. Ulcers treated with nonsteroid antiinflammation Aloe Vera gel and stress management by reading assignment method.Discussion: Psychological stress altered the immune system so oral mucosa prone to inflammation, and make the history of GERD getting worse. Stress causes cortisol secretion that changes the imbalance of proinflammatory and antiinflammatory cytokines. Oral mucosa becomes more susceptible to hypersensitivity. In addition, stress decreased oral and esophageal mucosa resistance to GERDConclusion: RAS triggered by psychological stress, allergy, and GERD. Treatment of RAS is by elimination predisposing factors to prevent recurrence

Objective : to present treatment bucal abscess case thatexpanding to colli, thoraks and complication based on mistherapy about NSAIDwith gaster perforation.Case and management: A 59 years old man with chief complain colli abscessand suspect gaster perforation with diffuse abdominal pain and distensiongaster since three days. Intraoral inspection was multipel dental gangren.Ultrasonography showed gaster perforation. Incicion drainase was done anderadication oral focus infection with repair gaster perforation under generalanaesthesia prosedure. Medication treatment with ceftriaxone injection 2x1 gram, and paracetamol infus 3x 500 mg. Bacteria kulture was negatif andhospitalization patient until 1 week.Discussion: Gold standart abscess therapy is incision drainase, but there wereclinician not aware abiut that and choosed konservative treatment with antibioticand analgesic, so that less maximal theraphy. AUTOTRANSPLANTASI GIGI-GIGI IMPAKSI INSISIVUS MAKSILA KANAN DISERTAI KISTA DENTIGEROUS DENGAN PENAMBAHAN GRAFT KOLAGEN KARBONAT APATIT
Background: Autotransplantation is a surgical movement to reposition teeththat have been erupted, partially erupted or not yet erupted from one place to another in the same individual, either on post-extraction sockets or artifcial sockets made by surgery. Dental folicle from impacted teeth may develope dentigerous cysts that will result in destruction of the jaw bone. There are many factors that influence the success of autotransplantation, one of which is adequate bone support.The purpose of this poster is to present the successful autotransplantation treatment of impacted right maxillary incisor teeth associated with dentigerous cyst and bone support replacement after cyst enucleation using a carbonate apatite collagen graft, GAMACHA®, at RSUP Dr. Sardjito.Case management: A 9-year-old girl, referred to the Oral and MaxillofacialSurgery Department of RSUP Dr. Sardjito with the complaint of right deciduous maxillary incisor persistence. Radiological examination showing persistent of 51 and 52 radices and impaction of teeth 11 and 12 in a horizontal position above radix 53. Also visible radiolucent image with a frm limit around the crown of 11 that supports the image of cyst. This patient performed cyst enucleation under general anesthesia, removal of impacted 11 and 21, followed by an artifcial socket drilling at the alveolar bone that could support all aspects of the root wall although only on the apical part of the tooth root for teeth 11 and 12 placement. Autotransplantation of teeth11 and 12 followed by the application of GAMACHA® and wire fxation, strengthen with acrylic splint.Conclusion: Two years follow up after surgery, patient have no complaintseither from aesthetic or functional and the result of clinical and radiologicalexamination did not found existence of any pathological abnormalities around teeth 11 and 12
PENGARUH PIRANTI FUNGSIONAL LEPASAN TERHADAP PERUBAHAN PANJANG MANDIBULA
Background: Class II malocclusion prevalence is about 70%. Class II malocclusion could be caused by retrognatic mandible and normal maxilla, prognatic maxilla and normal mandible or combination of retrognatic mandible and prognatic maxilla. Class II malocclusion can be treated with fixed or removable functional appliance on advancing mandibular length. Aim: To examine the effect of functional appliance in mandibular length. Method: Age of sample is ranging from 7-13 years old treated using removable functional appliance with proper and complete medical record. In this research, a purposive sampling method was used. The mandibular length changes of samples were evaluated and analyzed with statistical computerized system. Result: The research reveals that total mandibular length was changed by 7.09 mm while mandibular corpus length was changed by 3.27 mm. Based on paired t-test, these results are found to be statistically significant (p< 0.05). Conclusion: The removable functional appliance on samples with Class II malocclusion contributes a significant effect both for total mandibular and corpus mandibular length
