    TERAPI IMUNOSUPRESAN PADA PASIEN ANEMIA APLASTIK DENGAN PERDARAHAN GUSI: MENYEMBUHKAN ATAU MEMPERPARAH?

    Background: Aplastic anemia (AA) is a hematologic disorder characterized by hypoplastic bone marrow and peripheral pancytopenia. The main symptoms are fatigue and bleeding. In the oral cavity, gingival bleeding is the common complaint.Case Management: A 18-year-old female patient referred from internal medicine department due to swelling of palate since 2 days before that interrupted eating activity. She routinely consumes Sandimun®100 mg tablets. A 2 x 1 cm black blood clot in anterior hard palate and ulcerative lesion at dorsal tongue regio 45 surrounded by necrotic area were found on intra oral examination. Patient was diagnosed with necrotizing ulcer-related neutropenia accompanied by AA-associated hemorrhagic bullae in the hard palate. Patient treated with mouthwash containing 0.1% hyaluronicacid and 1% feracrylum.Discussion: Immunosuppressant is the AA gold standard therapy. Cyclosporine (CsA) as imunosupressant is common drug to prevent T cells from attacking stem cells in bone marrow. The side effects of CsA is gingival hiperplasia.Conclusions: Gingival bleeding and gingival hiperplasia due to thrombocytopenia and the use of cyclosporine are vicious circles that need to be aware. With comprehensive management, good oral conditions will be obtained, therefore the quality of life can increase

    Background: Odontogram is one part of the dental record. The odontogram form isfilled with diagnoses and treatment done by the dentist. Odontogram can be used for the further treatment plan and help the forensic department to do the identification. The purpose of this study was to identify the completeness of odontogram filling out by the dentist in Semarang.Method: The research was descriptive using a cross-sectional study design. The subject of the research was graduated dentist in year 2015 until 2017. Subjects were chosen through information from PDGI.Result: The result of this study showed that none of the subjects filled odontogramcompletely, 5 subjects filled less complete and 39 others were categorized incomplete according to medical record guidance book published in 2015.Conclusion: Based on the study, concluded that the need to pay attention aboutthe importance of filling odontogram by the dentist was still low

    According to Riskesdas 2018 the prevalence of dental and oral diseases in Indonesia is still high (57.6%) and the most cases are caries and periodontal disease. Periodontal tissue infection is a local oral infection that can be a focal oral infection, and has a risk of affecting systemic conditions. Microorganisms, toxins and inflammatory reactions in infected periodontal tissues can spread systemically to other parts of the body, thatcan cause infections in the area or worsen existing conditions. Systemic conditions that can be affected by periodontal tissue infections including coronary heart disease / arterosclerosis, stroke, diabetes mellitus, pregnancy, chronic pulmonary obstruction, acute respiratory infections. The purpose of this literature review is to refresh about oral focal infection, as well as more convincing that periodontal infection was a risk factor for systemic conditions, in terms of inflammatory reactions.Influencing inflammatory reactions are inflammatory mediators, including interleukin (IL-1α, IL-β, IL-6, IL-8), tumor necotic factors (TNFα) and prostaglandin E2 (PGE2). Biological mechanisms support the role of periodontal infection as a potential risk factor for a number of systemic conditions.Future multidisciplinary research should better illustrate the role of periodontal infections in systemic health, so that the influence between periodontal infections and systemic conditions such as premature LBW, diabetes, cardiovascular and cerebrovascular diseases, and respiratory diseases will be increasingly prove

    Background: Gelatin has been used as a hemostatic agent to stop bleeding and it can be absorbed by human body within 6 hours. The patin fish (Pangasius djambal) gelatin has a high amount of amino acid, especially glutamine which has an important role in increasing the proliferation of fibroblast in wounds. The purpose of this study was to determine the effect of patin fish gelatin to the number of fibroblast in wound after tooth extraction.Method: Rat’s tooth extraction is done on the lower left incisive tooth. Thirty white rats were divided into two groups randomly with 3 time series. Control group was the rats without given of patin fish gelatin and treatment group was the rats given 1cc of patin fish gelatin with pipette. Socket taken for histologically processed by Hematoxylin Eosin staining.Result: It showed there were significant differences in the number of fibroblasts between groups. The Post Hoc Tukey test result showed there was a significant difference between the control group and the treatment group.Conclusion: Patin fish gelatin has effect increasing the number of fibroblast inwound after white rat’s (Rattus norvegicus) tooth extractio

    SURFACE DETERIORATION OF GIC TYPE II BASED ON ITS EXPIRATION DATE AFTER IMMERSION IN CARBONATED DRINK

    Background: Inside the oral cavity, restoration always in contact with wet condition, including carbonated drink with low pH level and erosive potential. Sometimes, materials are stored for prolonged time exceeding their expiration date. This research aimed to determine the effect of expiration date (ED) of GIC type II toward its surface deterioration (surface hardness and roughness) when immersed in carbonated drink.Method: This was an experimental laboratory using GIC type II with ED of 2014,2016 and 2021. Ten cylindrical specimens per group were made (d=10mm, h=2mm). Surface hardness and roughness measurements were done prior andafter specimens were immersed for seven days in carbonated drink. Carbonateddrinks were changed daily to maintain its pH level.Result: There were reduction in surface roughness and increased in surfaceroughness after immersed for seven days. The greatest deterioration occurred ingroup expired in 2014. Based on the Kruskal Wallis test result, there was a signifcant difference in surface hardness alteration (p=0.000) and surface roughness alteration (p=0.000) based on its ED.Conclusion: ED has great impact on surface characteristics (hardness androughness). Using expired GIC type II deal with great risk of surface deterioration when in contact with low pH level beverage

    Background: Caries is common problem found in children. Caries prevalences increased mainly in developing countries such as Indonesia. Theobromine is an alkaloid compound in metilxantin group which can be found in the cocoa bean. Theobromine is believed to prevent caries by restoring the rough surface of email as the result of demineralisation. This research is aimed to analyze the effect of theobromine gel on the surface roughness of deciduous teeth after demineralization.Method: It was laboratory experiments. Sample of research were 20 incisive cariesfree deciduous teeth. Samples divided into fve groups of treatments (concentration of gel theobromine 1%, 2%, 3%, 4%, and 5%). Each group consists of 4 samples. All samples were demineralized with 37% phosphoric acid for 60 seconds. Samples were immersed in artifcial saliva for 60 minutes in temperature 37â°. Roughness examination before and after treatment was conducted using Surface Roughness Tester.Result: This experiment showed that surface roughness is decreasing after treated by fve different concentrations of theobromine. There is a signifcant difference compared to other treatments (p<0,05).Conclusion: Theobromine can be used as one of the caries preventive products.     POTENSI KOMBINASI EKSTRAK AKAR SIDAGURI (Sida rhombifolia L.) DAN GETAH JARAK (Jatropha curcas L.) SEBAGAI BAHAN DEVITALISASI

    Background: Arsenic paste was used to devitalize dental pulp in some problems before surgical, but it has been proven to have many hazardous effects. So, other alternative drugs are needed that are equally effective but safer. Jatropha (Jatropha curcas L.) can cause lysis blood vessels in the pulp, whereas the sidaguri (Sidarhombifolia L.) are effective in relieving inflammation. The purpose of this study was to determine the devitalization effect of jatropha and sidaguri on rabbit animal model.Method: Sidaguri roots were extracted using reflux method while jatropha sap was dried using lyophilization method. For easy application, the extract made into a paste (1; 1) and inserted into the cavity. After 7 days of exposure, the teeth were removed, calcified and followed to histopathological staining and COX-2 expression (IHC).Result: Histopathological examination showed that the paste was able to cause necrosis of pulp nerve. Higher dose of the drug increased the necrosis area, but the paste is not able to reduce COX-2 expression.Conclusion: It can be concluded that the paste has a potential to developed as devitalization agent although it is necessary to study to the reduce the inflammation process

    EFFECTIVITY ANTIBACTERIAL ZINC OXIDE EUGENOL WITH ZINC OXIDE PROPOLIS FOR ENDODONTIC TREATMENT IN PRIMARY TEETH

    Background: Enterococcus faecalis is generally found on the failure of root canal treatments. Zinc oxide propolis is believed to have an antibacterial effect on that bacteria. This research aimed to compare bacteriostatic effect of zinc oxide eugenol (ZOE) and zinc oxide propolis (ZOP) as the sealer materials of root canal.Method: This was an experimental research with post-test only control group design with two different groups (ZOE and ZOP). Culture of Enterococcus faecalis bacteria was smeared on Blood Agar Plate media with six times replication per group and kept inside incubator for 24 hours. The result was obtained from the inhibition zone formed around the pasta.Result: The average result of ZOE and ZOP was 27.7 mm and 13.45 mm respectively. Normality test using Shapiro-Wilks showed that data was normal (p>0.05). Then, the data was analysed using Independent Samples T-test. The result showed that there was different inhibition zone between ZOE group and ZOP group (p<0.05).Conclusion: Based on the result, it can be concluded that ZOP has lower antibacterial effectiveness of the Enterococcus faecalis than ZOE

    Background: Dysplasia is malignant transformation of stratified squamous epithelium characterized by cellular atypia and loss of stratification and maturation. It can caused by cigarette smoke. Measuring the activity of cell proliferation as the beginning of dysplasia is very useful to know the development of malignancy. One of proliferation marker is Ki67 that expressed in all cell cycle phase except G0.Methods: 30 male wistar rats divided into: control grup (n=10), exposed to cigarette smoke for 4 weeks (n=10), 8 weeks (n=10) as much as 20 cigarettes/group/day/exposure used smoking pump. Specimen were stained HE to see dysplasia and Ki67 expression seen immunohistochemically. Statistical analyzed by Spearman correlation.Result: There was an increase in mean of Ki67 expression and significant differences between groups (p=0,000) and also significant difference of degree of dysplasia between groups (p=0,000). There was a strong correlation between Ki67 score to degree of dysplasia (p=0.001).Conclusion: Increased of Ki67 expression proportional to the Increase in degrees of dysplasi

    Background: Chronic osteomyelitis mandibula is one of the complications from dental extraction. Inadequate wound handling can have an impact on the spread of infection in the surrounding tissue like nerve which results in facial nerve paralysis. The purpose is to present a rare case that facilitative nerve paralysis as a result of the spread of osteomyelitis infectionCase Management: A 69 years old woman with chief complains numbness onher lips accompanied by pus out beside the lower teeth. No sistemic disease. Panoramic radiograph showed abnormal bone-like sequester. Extraoral examination appeared the bluish color on the right cheek and there was right facial muscle paralysis. Debridement, sequesterectomy by general anesthesia and medication using ceftriaxone intravenous, ketorolac injection, multivitamin, and corticosteroid, physiotherapy for facial nerve paralyze, also.Discussion: Pathogenesis mandibular osteomyelitis involves contiguous spreadfrom an odontogenic focus infection. The bacteria produce an exotoxin, which, while unable to cross the blood-brain barrier, can have deleterious effects on thePeripheral Nerve System (Fasialis Nerve) in up to 75% of cases, with the severity of presentation correlating with the severity of the infection.Conclusion: Chronic mandibular osteomyelitis can spread the infection to around another anatomy oral cavity like facials nerves
