    Obstacles of providing exclusive breastfeeding among working mothers using social-ecological theoryBackground: The coverage of exclusive breastfeeding (EBF) in Indonesia was increased to 38% but it was lower than the Millennium Development Goals (MDGs) target (80%). One of the influence factors is to return to work. Society assumed that a mother who stays at home more successful to provide EBF than working mothers. A previous study indicated that some obstacles become early cessation factors among working mothers.Objective: To explore the obstacle of breastfeeding using social-ecological theory.Methods: Qualitative research inquiry based on phenomenology design. Informants were chosen through purposive sampling. Inclusion criteria were working mothers who succeeded provide exclusive breastfeeding, had children age 6-24 months, lived, and formally worked at Manado. Data triangulation was conducted through in-depth interviews with the manager/supervisor/colleague. Data collected using in-depth interviews and observation.Results: A total of twelve informants have interviewed consists of six working mothers and six triangulation informants. The result indicated that lack of confidence was related to providing exclusive breastfeeding. This is caused by family influence, culture, health provider, and lack of awareness from the working place. A support group was the main factors that influence the mother to overcome all obstacles.Conclusions: The main obstacles for working mothers to provide EBF come from interpersonal factors that determined by other factors: intrapersonal, organizational, community. Therefore working mothers need breastfeeding preparation since early pregnancy to decrease breastfeeding obstacles

    The importance of maternal parenting to energy and protein intake in children in a low-income familyBackground: Energy and protein insufficiency was still a major problem for Indonesian children, especially for those who live in rural areas. Objective: This study aimed to find the important role of mothers on their children's intakes in low-income families in the village of Tasikmalaya, West Java. Methods: The study was an observational research with a cross-sectional design. 120 children (6 months to 5 years old) were randomly selected from the list of children in the area. Children's intake was measured using a 2x24 hour food recall questionnaire and food frequency questionnaire (FFQ). Data were analyzed using correlation test (Spearman rho), and multiple linear regression test significance was set at p-value <0,005. Results: The majority of the children (83.3%) consumed less than four types of food (staple foods, side dishes, vegetables, and fruits), 58.3% ate the main menu <3 times/day, 13.3% of the children had low energy intake, and 7.5% had low protein intake. Although a majority of the children had normal nutritional status, 5.8% were categorized as wasting, and 2.5% overweight. There was a positive correlation between frequency of eating with energy and protein intakes of children (p=0.006; p=0.035). Types of food did not correlate to the energy and protein intakes of the children. Mother's education, children's age, and history of illness are factors that influence the energy and protein intake of the children. Conclusions: Maternal parenting (frequency of eating) correlated to the energy and protein intake of the children

    Achievement of the nutrition minimum service standard at Manokwari District Hospital in West PapuaBackground: There are three indicators of nutrition services that are regulated in the hospital Minimum Service Standards (MSS), namely the timeliness of food distribution, patient's plate waste, and the accuracy of the patient’s diet. The achievement of minimum service standards as the indicator of quality for nutrition services at Manokwari District Hospital has not yet been known.Objective: To find out the implementation of minimum service standards for nutrition services,i.e. the promptness of food distribution, the proportion of plate waste, and accuracy of the patient’s diet at Manokwari District Hospital. Methods: This was a mixed-method study conducted at Manokwari District Hospital, West Papua, from January until March of 2018. A quantitative approach with patient samples was carried out to quantify the achievement of MSS for nutrition service. Punctuality of food distribution and accuracy of the diet was measured using observational sheets while the patient’s plate waste was determined using food weighing. A qualitative approach through in-depth interviews with patients, nutritionists, cooks, food service workers, and nurses was performed to explore the influencing factors. Results: The percentage of timeliness of patient food distribution was only 37.1%, the waste of patients’ food reached 34.5% and the accuracy of the patient's diet was only 83.87%. These were affected by poor human resource management, inadequate hospital facilities, and foods brought from outside of the hospital. Conclusions: Promptness of patient’s food distribution, plate waste and diet accuracy in Manokwari District Hospital has not reached the minimum service standard for nutrition service

    Effect of Secang (Caesalpinia sappan L.) drink toward plasma-malondialdehyde level and physical fitness in prehypertensive government office workerBackground: Prehypertension is associated with a higher risk of hypertension and cardiovascular disease. A government office worker has lower physical activity, thus increasing the risk of hypertension, low physical fitness, and a higher level of oxidative stress. Free radical which is the product of oxidative stress can be reduced by antioxidants. Secang (Caesalpinia sappan L.) is one of the ingredients in a traditional herbal drink (jamu) and a natural source of antioxidants in the form of polyphenol.Objective: The purpose of this study is to discover the effect of secang drink supplementation, and efficacy towards plasma MDA level and physical fitness on a government office worker.Methods: This study conducted with true experimental by pre and post-test control group design. The subject is 35 prehypertensive government office workers aged between 25-45 years old in Yogyakarta, divided into treatment and control groups. Secang is given in 3.8g teabag which brewed in 200cc hot water, consumed for 4 weeks. Plasma MDA level is measured using TBARS, and physical fitness is measured using the Harvard step test. Results are analyzed using student paired t-test, Pearson and Spearman correlation test, regression test, and ANOVA.Results: There’s insignificant decrease of plasma MDA level between intervention group (from 4,28±3,16 to 3,66±1,21) and control group (4,85±2,35 to 3,65±1,33). There’s difference on Harvard step test results, intervention group increase from 35,54±20,18 to 48,09±21,86 in male, while 11,56±4,94 to 13,87±6,25 in female. In control group, male subject increase 29,20±10,06 to 38,18±23,62, but decrease 13,30±7,12 to 13,26±2,92 in female.Conclusions: 4 weeks drinks of 3.8 g Secang drink brewed in 200ml hot water, shows no evidence of lowering oxidative stress (MDA) in plasma, but it may increase physical fitness

    Effect of motivational interviewing and nutrition education class based on instagram for change healthy weight loss knowledge and diet quality in obese female studentsBackground: Obesity is a health problem that has a serious impact on women. Instagram is one of the media for nutrition education that can present healthy messages effectively and motivational interviewing can improves intrinsic motivation to changes in behavior. Objective: To analyze the effects of motivational interviewing and nutrition education class on changes in healthy weight loss knowledge and diet quality. Methods: This study used a quasi-experimental design with the pre-post test control group. The sample consists of 40 female students obesity were divided into three groups, control group (K); intervention group 1 with motivational interviewing and nutrition education class (P1); intervention group 2 only nutrition education class (P2). The nutrition education class based on instagram was given for thirty days and motivational interviewing was given 4 times with a duration of 15-45 minutes. Changes in healthy weight loss knowledge were assessed by pre-test and post-test questionnaire, while diet quality was assessed by Diet Quality Index-International (DQI-I). Results: There was a significant difference between healthy weight loss knowledge in the three groups. There were significant differences on sub-components of the adequacy of staple food (p=0.026), fiber (p=0.026), protein (p=0.006), total fat intake (p=0.009), saturated fat intake (p=0.024), sodium intake (p=0.016), energy intake (p=0.000), and carbohydrate intake (p=0.002) in the three groups. Conclusions: Motivational interviewing and nutrition education class based on instagram could increase the healthy weight loss knowledge and reduce staple food intake, total fat intake, saturated fat intake, sodium intake, energy intake, and carbohydrate intake in obese women

    The correlation between vitamin D deficiency and the severity of painful diabetic neuropathy in patients with type 2 diabetes mellitus (T2DM)

    Background: Diabetes and its complications are the major burden health problem worldwide, and diabetic neuropathy is one of the major complications. Vitamin D levels found to be significantly lower in people with painful diabetic peripheral neuropathy compared with healthy people. The data about the vitamin D levels and severity of the neuropathy in Indonesia are very limited.Objective: This study aims to investigate the possible relationship between vitamin D levels and the severity of diabetic peripheral neuropathy.Methods: A cross-sectional study was carried out during the period from October 2019 to December 2019 on 53 subjects with diabetic peripheral neuropathy. The patient’s clinical profile including age, gender, and duration of diabetes, HbA1c, and associated microvascular complications was documented. The treatment history was recorded from electronic prescribing data. The severity of neuropathy was measured with the Toronto Clinical Neuropathy Scoring System. Serum 25-OH vitamin D levels were measured by enzyme immunoassays for the quantitative measurement of total serum 25-OH Vitamin D level in ng/mL.Results: Vitamin D levels based on the severity of neuropathy are divided into mild, moderate, and severe. At mild neuropathy severity, the average patient's vitamin D level was 19±8.85ng/mL, at moderate severity the patient's vitamin D level was 16.25±6.08 ng/mL, and for severe neuropathy, the average vitamin D level was 13.35±6.20 ng/mL. Spearman correlation test obtained r value= -0.439 and p value=0.001.Conclusions: There is a moderate, significant, and negative patterned correlation between vitamin D level and diabetic peripheral neuropathy severit

    Dominant factors of functional constipation among adolescent in JakartaBackground: The impact of functional constipation, including the quality of life decreases, increase the additional health costs that must be incurred, and reduce productivity. Previous research in various places in Indonesia shows that functional constipation is experienced among adolescents, percentages between 24.6-68.5%.Objective: Determine the relationship and dominant factor between gender, fiber intake, fluids intake, consumption of probiotic drinks, physical activity, nutritional status, stress, and nutritional knowledge on the incidence of functional constipation.Methods: The study design was cross-sectional. The number of respondents studied was 150 high-school students from class X and XI. The sample selected by using a total sampling system. Data collected using the Roma III questionnaire of functional constipation, physical activity using the Physical Activity Questionnaire for Adolescent, and stress level using the Holmes and Rahe Stress Scale for students.  Nutritional knowledge was obtained by filling out a questionnaire. Food recall 2x24 hours to assess fiber intake, fluid, and probiotic drinks. Measure body weight and height of students to determine nutritional status. Data analyzed using univariate, Chi-Square test for bivariate, and multiple logistic regression tests for multivariate.Results: 75.3% of students had functional constipation, 64.0% less fiber intake,54.0% less fluid intake, 91.3% less physical activity, 62.0% did not consume probiotic drinks, 38.7% were obese and 30.0% lacking in nutrition knowledge. Dominant factor of functional constipation was physical activity (p-value 0.035; OR 3.57; CI 1.09-11.66).Conclusions: Low physical activity can increase the incidence of functional constipation in adolescents at Jakarta

    Santriwati’s bone mass density was associated with the anthropometric profileBackground: One group of female adolescents who had nutritional problems was Islamic boarding school students, namely santriwati. Low intake was supported by a poor diet profile that had a risk of developing osteopenia. The problem of osteopenia in adolescents could affect long-term calcium deposits and had an impact on old age and a high risk of osteoporosis.Objective: This study aims to examine the relationship between diet profiles and anthropometric profiles with bone density in female students.Methods: A cross-sectional study of 76 female students who were selected using a simple random sampling technique. Anthropometric profiles measured were body mass index, body fat percent, and waist circumference. Bone density was measured using Dual Energy X-Ray Absorptiometry (DXA). Nutrient intake profiles were obtained based on intake interviews used food recall form 6x24 hours. Data were analyzed using Spearman correlations.Results: Most female students had normal nutritional status, and 14 female students (18.4%) had low bone density. The intake of energy, macronutrients (protein, fat, and carbohydrate), and micronutrients (phosphorus, calcium, and vitamin D) of female students were insufficient. There was a relationship between body mass index (p=0.036; r=0.241) and percent body fat (p=0.027; r=0.254) with bone density. However, the nutrient intake factor has no relationship with bone density (p>0.05).Conclusions: Body mass index and body fat percent are positively correlated with female bone density

    Differences in food intake among children under five years in urban and rural areas in provinces with the double burden of malnutritionBackground: The double burden of malnutrition is one of the nutritional problems in children at national and global levels. Food intake is the main cause of the double burden of malnutrition. Differences in residence in urban and rural areas will affect food access which will have an impact on different food intake. Objective: To analyze the differences in energy, protein, fat, and carbohydrate intake of children under-fives in urban and rural areas in provinces with the double burden of malnutrition. Methods: This research used secondary data analysis using the Total Diet Study (TDS) 2014. The subjects of this study were 813 children in provinces with a dual burden of malnutrition that fulfilled the inclusion and exclusion criteria. Provinces with the double burden of malnutrition are provinces with a prevalence of underweight children aged ≥24-59 months at >22.36% and a prevalence of overweight children age ≥24-59 months at >1.23%. Total Diet Study (TDS) data will provide information on food intake of children through recall results, differences in residence obtained from district classification data, the double burden of malnutrition data obtained from children nutritional status is overweight and underweight based on weight and age data of children processed using WHO-ANTRO software. Results: The average intake of energy, protein, and fat in the urban area are higher than in a rural area, but the average carbohydrate intake in an urban area is lower than in a rural area. The average intake of energy in urban was 1300.01 kcal, in rural areas was 1223.23 kcal (p=0.0008). The average intake of protein in urban was 55.03 g, in rural areas was 47.67 g (p<0.0001). The average intake of fat in urban was 47.99 g, in rural areas was 37.12 g (p<0.0001). The average intakes of carbohydrate in urban were 163.61 g, in rural areas was 178.88 g (p=0.0042).Conclusions: There are differences in energy, protein, fat, and carbohydrate intake of children under five years in urban and rural areas in provinces with the double burden of malnutrition

    Exclusive feeding methods and their effect on nutritional statusBackground: Exclusive feeding could be given by human milk expression or direct breastfeeding methods. Recent studies indicate expressed human milk given by bottle in early infancy could lead to excess weight on the next stage of life. Objective: Determine the differences in nutritional status between exclusively fed infants with direct breastfeeding methods and a combination of breast pumping. Methods: An analytical observational study with the cross-sectional approach, conducted in Puskesmas Jalan Emas, Tangerang Regency, Banten. The subjects consisted of 68 infants from direct breastfeeding (DB) and the combination of breast pumping (KBP) groups, were selected by purposive sampling. The data obtained from the measurement of nutritional status and infants’ feeding history. Results: In WAZ, HAZ, and WHZ in DB were -0.48±0.7; -1.47±1.2; and 0.64±1.19 respectively. In WAZ, HAZ, and WHZ in KBP were 0.06±0.75; -1.66±0.85; and 1.23±1.05 respectively. The T-independent analytical test showed significant score (p) in WAZ, HAZ and WHZ indicators were 0.003; 0.470; and 0.035 respectively. Conclusions: In WAZ and WHZ indicators show significant differences between infants in DB and KBP groups. Meanwhile, in the HAZ indicator, there’s no significant difference between infants in DB and KBP groups
