    Polimorfisme gen ferroportin (FPN1) -1355 G/C sebagai faktor risiko anemia defisiensi besi pada ibu hamil

    Background: According to WHO data, prevalence of anemia pregnancy in Indonesia is 44.3%, it’s higher than world prevalence (41.8%). Ferroportin (FPN1) is one of important iron exsporter for iron absorption, release, and recycle inside the body. The varian of FPN1-1355 G/C in promoter region, leads to increased of ferroportin expression and iron export, increased cellular iron needs, overexpression of soluble transferrin receptor (sTfR), decrease hemoglobin (Hb) and erythrocyte indices that manifest to iron deficiency anemia (IDA).Objective: This research will study the frequency of FPN1-1355 G/C polymorphism as a risk factor of IDA in pregnant women in Indonesia.Method: The research design was a case and control study. Blood samples were taken from 26 pregnant women with anemia and 48 pregnant women without anemia. FPN1-1355G/C polymorphism were determined using PCR-RFLP method. sTfR and ferritin level were measured with ELISA. Hemoglobin, erythrocyte indices, and sTfR level were compared among genotype group, then statistically analyzed using independent sample t-test and one way ANOVA. Bivariat analysis of Pearson test was conducted to analyze correlation between level of blood Hb and ferritin in pregnant women (p0.05). The sTfR and hepcidin level in subjects carrying C allele were higher than subjects carrying only G allele (p<0.05). Conclusion: In this study the FPN1 gene promoter -1355 G/C polymorphism was not a risk factor for anemia, but it was a risk factor for iron deficiency anemia in pregnant women

    Background: Malnutrition is most commonly found in gastrointestinal cancer that affects the result of an operation. Albumin serum is an effective and simple way of assessing risk which is associated with malnutrition intensity so that it is generally used as the prognostic index for the development of the incidence of colorectal cancer postoperative complications.Objective: To identify the effect of preoperative hypoalbumin to the incidence of postoperative complications and length of stay of colorectal cancer in patients.Method: The study used the quantitative approach with a nonconcurrent cohort (retrospective analysis) design. The population was patients diagnosed having colorectal cancer who would undergo an operation at Kasih Ibu, Dr. Oen, and Panti Waluyo Hospital of Surakarta. Data analysis used Chi-Square test and logistic regression.Results: Hypoalbumin, either based on admission or pre-operation, was significant risk factor for the incidence of post operative complications in colorectal cancer patients as well as age and American Society of Anesthesiologist Physical Status Classification (ASA) status. The result of multivariate analysis showed age of 20-59 years (OR=2), hypoalbumin based on admission (2.1 times) and preoperation (1.9 times) were risk factors for the incidence of postoperative complication (infection). ASA status III-IV had risk 2.8 times for the incidence of postoperative sepsis complications and 3.7 times for mortality than ASA status I-II. Status of hypoalbumin did not influence total length of stay or postoperative length of stay but age influenced postoperative length of stay.Conclusion: Hypoalbumin influenced the incidence of postoperative complications in colorectal cancer patients, particularly infection complication; whereas factors of age and ASA status were external factors strongly influenced. Hypoalbumin did not influence length of stay. Age was an external factor that strongly influenced postoperative length of stay

    Background: Oral health status and dietary intake contribute to nutritional status in elderly. Missing teeth cause chewing disorder that reduces quality and quantity of food intake, which finally makes the elderly have underweight nutritional status.Objective: To determine the risk factors for underweight in the elderly at Yogyakarta Municipality.Method: The study was observational with case control design on elderly at Yogyakarta Municipality. Subjects consisted of 210 elderly matched in age and gender. Sampling was done by multistage random sampling. Oral health status was assessed through dental health status (index of missing teeth) and periodontal status (gingival index, periodontal index and oral hygiene index), dietary intake was collected by using semi quantitative food frequency questionnaire (SQ-FFQ) and underweight nutritional status was based on body mass armspan (BMA). Data were analyzed by using tests of X2 Mc.Nemar, X2 Stuart Maxwell, and conditional logistic regression.Results: Bivariate analysis showed the number of missing teeth ≥ 21 (OR=3.67, p<0.05) and 16-20 (OR=3.53, p<0.05) as risk factors of underweight, whereas the gingival index, periodontal index and oral hygiene index were not. Less intake of energy (OR=6.3), protein (OR=7.83), fat (OR=5.67) and carbohydrates (OR=7.5) were risk factors of underweight (p<0.01). Income less than Rp 808.000,00 was also risk factor for underweight (OR=4.5; p<0.01). Multivariate analysis showed the significant risk factors for underweight were the missing teeth ≥ 21 (OR=8.76) and 16-20 (OR=6.04) which increased by income less than Rp 808.000,00 (OR=5.94), less fat intake (OR=4.88), and less carbohydrate intake (OR=5.48). Income was confounding factor in the risk of missing teeth and protein intake for becoming underweight.Conclusion: Significant risk factors of underweight in elderly were missing teeth ≥ 16, less intake of fat and carbohydrate, and income less than Rp 808.000,00

    Background: Obesity in children is associated with impairment of pulmonary function and increased risk of asthma. Obesity in asthmatic children may reduce lung function, that can be assessed by peak flow meter, a practical and an inexpensive tool.Objectives: To compare the peak expiratory flow (PEF) between obese and non-obese asthmatic children.Method: We conducted a cross sectional study in Yogyakarta during March 2010-September 2012. Fifty obese asthmatic patients and 50 non obese asthmatic control subjects participated in this study. Inclusion criteria were asthmatic patient, according to Pedoman Nasional Asma Anak (PNAA), and 6-18 years of age. Exclusion criteria were asthmatic attack, respiratory disease, heart disease and congenital chest malformation. Obesity is defined as body mass index (BMI) for age more than +3 SD WHO growth chart standards BMI for age 2007 z-score. Z-score is calculated with WHO AnthroPlus for Personal Computers. Data PEF is taken with electrical peak flow meter when the patient was not suffering from asthma attack. Normal PEF was defined as PEF ≥80% average (predicted) value for height.Results: The mean of age of asthmatic children in this study was 9.38 years and 9.50 years for non obese and obese respectively. The PFR was not different between obese asthmatic children and non obese asthmatic children (p=0,83). Pearson correlation of PFR and z-score BMI for age was positive weak correlation (r=0.12). There was significant difference of PFR between z-score BMI for age <3,20 and z-score BMI for age ≥3.20 (p=0.03). Significant difference of PFR also appears in duration of illness (p<0.001).Conclusion: There is no PFR difference between obese asthmatic children and non-obese asthmatic children. The difference of PFR emerges when statistic analysis performed using z-score BMI ≥3.20

    Background: Some studies state that fish consumption of at least 200-400 g/week can reduce the risk of coronary heart disease. Omega 3 polyunsaturated fatty acids in fish have cardioprotective effects. However, the cardiac effect of fish consumption are not primarily related to the amount of fish consumed but also its cooking method. The cooking method (eg, frying) may alter the fatty acid content of a fish meal by absorbing fatty acids from cooking oils and adding trans fatty acids. Non-fried fish consumption (eg, boiled/steamed) was associated with lower risk of coronary heart disease but fried fish was not.Objective: To identify the quantity of fish consumed and the cooking method with the incidence of dyslipidemia.Method: The study was observational with case control design in patients that checked themselves at Prodia laboratory of Samarinda. Respondents consisted of 152 patients, comprising 76 cases and 76 control. Cases were patients with dyslipidemia and control were those without dyslipidemia matched according to gender and age. Analysis used Chi-Square statistical test at confidence interval 95%.Results: There was significant difference in average of fish consumed between case and control group (164.86±63.61 g vs 218.55±62.21 g; p0,05), but the influence is in the cooking method process (OR=10,84; p<0,05).Conclusion: There was no influence between quantity of fish consumption. Risk for the prevalence of dyslipidemia was 10,84 times greater in the subject who the cooking method does not suitable the recommendation

    Background: Enteral nutrition is nutrition used to fulfill the needs of nutrition entirely and as the supplement for malnutrition patient. In a certain condition of a patient, this nutrition is usually given in the form of liquid. Local material foods such as tempeh, rice, mung bean, and ganyong have adequate nutrition, therefore they are suitable for being used as main raw materials in the making of enteral nutrition.Objective: To know the influence of feeding enteral nutrition formulated with local food material toward malnutrition white rats (Rattus norvegicus) of which the parameters are iron (Fe), hemoglobin (Hb) level and weight.Method: This research used Completely Random Design (CRD). Twenty-seven of malnutrition male white rats were divided into 3 groups of treatment with 9 repetitions for each group of the treatment. Group A was given enteral nutrition diet of formula A (tempeh, rice, and mung bean as the main raw material), group B was given enteral nutrition diet of formula B (tempeh, rice, mung bean, and ganyong as the main raw material), and group C (as the positive control) was given commercial enteral nutrition. The daily giving of enteral nutrition is 20 g/day during 30 days. The analysis of Fe and Hb level and the measurement of weight firstly were done before the treatment is given. The next measurement was conducted in 15th day and 31st day. Statistical analysis used ANOVA test dan DMRT of significance 5%.Results: The result showed that the treatment of the enteral nutrition feeding of formula B was more optimal than formula A in terms of the way to increase the level of Hb and Fe. Those two components will give positive effect toward the increasing of the weight of malnutrition white rats (Rattus norvegicus).Conclusion: The enteral nutrition of formula B is more proper to be developed as the main material of making enteral food in order to treat the malnutrition

    Pemanfaatan internet untuk edukasi gizi bagi penyandang diabetes mellitus

    Background: The pattern of disease incidence has changed nowadays as indicated from epidemiological transition, from infection to noninfection diseases or noncommunicable diseases. The quantity of diabetes mellitus (DM) patients in Indonesia is increasing every year. Integrated management of DM requires synergy from different parties. According to Act No. 36/2009 on Health, article 17 paragraph 1 states that the government is responsible for the availability of access to information, education, and health facilities to improve and maintain highest health status.Objective: The study aimed to analyze the utilization of the internet or online media in the dissemination of recommended eating pattern and physical activities of DM patients as prevention against complication.Method: The study used cross-sectional design and was carried out in January 2013 at Surabaya Municipality. It used e-survey, with secondary data from hospitals at Surabaya Municipality. Samples were DM patients of 20-50 years old registered in hospitals and agreed with informed consent. Samples were taken purposively involving 66 respondents.Results: There was a difference in eating pattern among DM patients utilizing online and those using nononline media (OR=3.33; CI 95%=1.06-10.43; p=0.03); and in physical activities (OR=0.09; CI 95%=0.01-0.75; p=0.008).Conclusion: The utilization of online media affected eating pattern and physical activity pattern in DM patient as much as 3.33 times and 0.09 times greater than in those not using online media in looking for communication, information, and education resources in supporting prevention against DM complication. Thus online media could be one of the effective and efficient methods for the delivery of communication, information, and education for DM patients by health staff

    Background: Poverty and lack of income will cause inability of the family to provide enough and nutritious food for all the family members. Fulfillment of nutritious food will bring direct impact to nutrition status of children. Malnutrition will affect brain development and intelligence that in the long run disrupts academic achievement.Objective: To identify association between food security of the household and nutrition status with academic achievement of elementary school students at Yogyakarta Municipality.Method: The study was observational with cross sectional design. Samples were students of grade V of elementary school at Subdistrict of Gedongtengen, Yogyakarta Municipality. Data of food security of the house were obtained through interview based on questionnaire of Radimer/Cornell, nutritional status data used height for age collected by measurement, and academic achievement through secondary data, i.e. original score of final semester examination. Statistical analysis was performed by using Chi-Square and Fisher’s Exact test.Results: As much as 67.6% of the household were food insecure and 13.3% of students were stunted. Food security of the household was significantly associated with nutrition status (p=0.033) and achievement in mathematics (p=0.045). There was association between nutrition status and achievement in mathematics (p=0.035); Indonesia language (p=0.000); and combined achievement in Mathematics, Indonesia language and science (p=0.004).Conclusion: There was association between food security of the household and nutritional status with academic achievement of student. There was association between food security of the household and nutrition status.

    Hipoalbuminemia prabedah sebagai faktor prognostik enterokolitis pascabedah penderita megakolon kongenital (Hirschsprung’s disease)

    Background: Hirschsprung’s disease or congenital megacolon is the congenital absence of ganglion cells in the colon. The rectum is always involved and in 90% of patients the abnormality is confined to the rectum and sigmoid. Absence of ganglion cells prevents peristalsis, resulting in functional obstruction. The success of Hirschsprung’s treatment depends on many factors such as age at the time of operation, body weight, hemoglobin levels, albumin levels, length of operation, length of stay and other prognostic factors. Post operative enterocolitis and other complications still represent as the problem wich often are faced by pediatric surgeons.Objective: The aim of this study was to evaluate the effect of albumin levels on postoperative enterocolitis in children presenting with congenital megacolon.Method: We conducted an ambidirectional cohort study involving children presenting with Hirschsprung’s disease in Dr. Sardjito, Panti Rapih and Permata Husada hospitals in Yogyakarta city from January 2005 to December 2010. All children had been operated using ERPT and PSNRHD methods. The subjects were classified into normoalbuminemia (>3.5 g/dl) and hypoalbuminemia (≤3.5 g/dl). The effect of albumin levels on postoperative enterocolitis were indicated by relative risk and 95% confidence interval.Results: Out of 104 children with Hirschsprung’s disease, 53 (51%) were hypoalbuminemic and 51 (49%) were normoalbuminemia. Postoperative enterocolitis was found in 18 (17.3%) children, 11/18 (61.1%) of them were hypoalbuminemia, whereas 7/18 (38.9%) were normoalbuminemia. Albumin levels were not significant prognostic factor for postoperative enterocolitis in children with congenital megacolon (RR=1.51; 95% CI:0.64-3.60; p=0.34).     Conclusion: The albumin levels are not prognostic factor for postoperative enterocolitis in children with congenital megacolon.

    Background: Diabetic is associated with the risk of having lipid peroxidation causing macro and micro vascular diseases. Malondialdehyde (MDA) is a mutagen product of lipid peroxidation.Objective: This research was to identify some factors associated to plasma MDA (P-MDA) level in type 2 diabetes mellitus.Method: Eighty two subjects were randomly recruited. Subjets were members of Persadia, physical training club for diabetic at Mardjoeki Mahdi Hospital in Bogor. Data of fruits, vegetables, and nuts-legumes-seeds consumption were collected by interviewing using semi quantitative food frequency questionnaires. Fasting blood glucose and plasma MDA were assayed using Wills method, based on the reaction of thiobarbituric acid. Other data such as physical training, smoking status, and diseases history were collected by interviewing using structured questionnaires. Nutritional status data was determined as body mass index. All data was analyzed using Odds Ratio (OR) and logistic regression test to identify the dominant factors contributed to P-MDA level.Results: Factors that contribute to P-MDA level is vegetables, nuts-legumes-seeds consumption and physical training. Consuming vegetables less than 300 g/d and nuts-legumes-seeds less than 2.5 servings/d are associated significantly with having high level of P-MDA (≥0.8 nmol/L), respectively OR=5 (95%CI:1.154-22.02) and OR=4.8 (95%CI:1.26-18.57). Not having physical activity routinely is risk for having high level P-MDA (OR=1.3; 95%CI:1.12-1.45). Those factors are associated to P-MDA level independently.Conclusion: Vegetables and nuts-legumes-seeds consumption have a significant contribution to P-MDA level, it is important to know further which type and chemical content in those food that can prevent lipid peroxidation
