Adopting Risk Based Maintenance Strategy of Export Gas Pipeline Using House of Risk: A Proposed Framework
42” SNP-BEM is the biggest buried gas export pipeline belong to PT XYZ and has been operated joinly with PT POMA for duration of 20 years. Until today, this pipe has survived very well from the worst potential failure or risk events such as rupture and leak since all strategic of maintenances are considered as sufficient facing the sources of risk agent, despite still different perceptions are presence among the operators in real application. Nevertheless, following the operation versus times, there is an increasing of potential external threat or risk agents due to the growth of housing, the overlapped operational permit of industrial of non-oil/gas, illegal road crossing which may cause the occurrence of risk events/failures. In 2016, there was a critical incident of the soil movement caused by the excessive load of coal’s stock pile nearby ROW and impacted to the pipe movement about 6 meters in horizontal and 2 meters in vertical and may lead to disastrous. Therefore, a more effective maintenance strategy is to be developed to reduce these risks. Combination of Fish Bone diagram (FBD) and House of Risk (HOR) is selected to identify the sources of risk and to develop more effective risk-based maintenance strategy. Based on the assessment, it is identified (twelve) 12 credible risk agents which contributing to pipeline failures/risk events and (eight) 8 proposed preventive actions to reduce the risk. These proposed actions shall be agreed by all operators as a reference for developing risk-based maintenance strategy
Place Attachment of Entrepreneurs: A Proposed Analytical Framework
In recent years, home-based enterprises have been giving significant implications for the economic improvement of the middle-low income community in Surabaya. With all of the limitations that exist, space, complex activities, and community acceptance become predictors that influence the formation of people-place relationship. Previous studies that tend to focus on the use of space are less concerned with the interaction undertaken by entrepreneurs which make them choose to continue their business where they have started. The dynamics of daily life in shaping social relations and functionality of place will affect how entrepreneurs perceive and assess their home-environment. Thus, it is important to highlight the process of the emerging attachment related to the use of a home for generating additional income. As the nature of place attachment which is multidisciplinary and socially constructed, this paper provides reviews on different paradigms, conceptualizations, and attachment types that should be used as analytical frameworks in the context of productive housing. A rich literature study aims to give further insight without ignoring the natural settings of the research context. By emphasizing the role of life experience of entrepreneurs, a more fruitful understanding towards accumulative assessment and perceived environment that triggers place attachment could be obtained in the future research

Product Quality Improvement in the Assembly Process of the 5.56 mm Ammunitions (Variant MU5-TJ) at PT.X Using the Six Sigma Method
Quality is one of the essential indicators for companies to exist amid strong competition. The 5.56 mm ammunition, especially variant MU5-TJ, is the primary commodity of PT. X. The process in ammunition production includes making cartridge cases, making bullets, making primers, and assembling ammunitions. The highest cost of defective products is led by defects in the preparation of the 5.56 mm ammunition, reaching 32.3%. In the process of preparing this kind of ammunition, most of the defects came from variant MU5-TJ, which accounted for 64%. This study aimed to improve the quality of the MU5-TJ ammunition preparation process by increasing its level of sigma. The method used in this study was the Six Sigma method through the Define, Measure, Analyze, Improve, and Control (DMAIC) phases. The quality identification results showed that the weighing and gauging process was the most contributing factor to the defects in the preparation of MU5-TJ ammunitions. The main defect found was the improper weight of the munitions due to the underfilling of propellant powder. The results of the chi-square test showed that the defect-causing factors with significant effects included the presence of sensor filling, varnish composition, tool material, and inspection methods. To reduce the defects, further improvements were made by installing the filling sensor on the ammunition assembly machine, changing the composition of the cartridge case mouth varnish, the use of tools made of hartmetall, and auto-control by the operator every 15 minutes. The results of the implementation of the improvements made resulted in an increase in sigma level from 3.69 to 3.79
Design Reduirement of Pasar Besar Malang Through Site Analysis Method
In discussing traditional markets, not a few are found that the market experiences a setback in existence in the community. The decline in the existence of traditional markets, one of which occurred in the Great Market of Malang with the increasing lifestyle of Malang people in harmony with technological sophistication. Pasar Besar Malang has urgency, which is experiencing 3 fires and is increasingly doubting the ability of Pasar Besar Malang to guarantee safety and comfort on the move. In addition to buyers, traders also experienced a decrease in the number due to the uneven distribution of trade transactions that caused traders to continue to suffer losses. Through the site analysis study method and the sense of place approach that is applied to the questionnaire, the potential and the possibility of utilization are explored by producing design requirements. Implementation of design requirements in the form of design aspects that discuss such as optimizing access and circulation through the transition zone. This is done in order to create a quality of space that is comfortable and able to provide sustainability for the existence of Pasar Besar Malang as a provider of needs and public space
Design and Development of Data Management System to Support The Preparation Process of Accreditation form for Program Study
In order to realize public accountability, the program study must actively build an internal quality assurance system. To prove that the internal quality assurance system has been implemented properly and correctly, the program study must be accredited by an external quality assurance institution. Criteria for evaluating commitments in accreditation are outlined in number of standards and are presented by programs study in instruments of forms. The problem that arises is that data/ documents for the process of supporting accreditation forms are not all available in the Program Study so they still have to involve other units to obtain them. The purpose of this study is to design a data management system to prepare the availability of data / documents to support the process of preparing accreditation forms. The System Development Life Cycle (SDLC) method is used with the waterfall model in designing this system. This research has produced an application system called Dokumen Akreditasi Online which is oriented towards standards of accreditation form. Overall the results of testing the system functionality shows that all features in the application can be run. The results of measuring the level of user satisfaction amounted to 75.27% which means that users are satisfied with this system. Thus, the need to prepare for accreditation is faster and easier and useful for other administrative needs

A Case Study of Applying Customer Segmentation in A Medical Equipment Industry
The purpose of this paper is to apply LRFM (length, recency, frequency, monetary) for customers in the medical equipment industry and identify differences in each customer segment. This study uses LRFM and clustering to segment its customers. This research uses transaction data of the medical device industry in Indonesia. This data will be extracted for the length, recency, frequency, and monetary (LRFM). The optimal cluster obtained from the validation process is four which will be used as a basis for customer segmentation. This study uses the K-Means algorithm as a clustering method and Decision Tree as a classification method and the application of IF-THEN rules. The segmentation process will be identified based on LRFM criteria in each segment that has been formed and will form a marketing strategy that is appropriate for the company. The results obtained from this study are four customer segments based on LRFM with each segment given a profile name as: Best, Frequent, Low and Uncertain. This study provides guidance on customer identification based on LRFM that can be used by medical equipment companies to develop strategies that are in accordance with the criteria of each segment that has been obtained to improve customer relationships management system and new ways of marketing products
Work Efficiency Improvement in the Bidding Process
A higher of competition level requires each company to increase its competitive advantage to be a market leader. One of the options to improve the competitive advantage is to increase company performance efficiency since using this option, operational cost can be reduced. In this particular study, Lean Service concept is applied to eliminate waste that occurs throughout the value stream of bidding process in order to improve procurement performance in terms of its efficiency in PT XYZ. Lean concept implementation started from creating the Value Stream Mapping (VSM) to map current value stream along the bidding process continued with Process Activity Mapping (PAM) to classify waste that occurs throughout the value stream, Borda Count Method (BCM) to determine the critical waste and Root Cause Analysis (RCA) to find the root cause with risk analysis approach which can be used as a reference to determine priority of improvement. The goal of this study is work efficiency improvement in the bidding process by identifying its waste and its root cause since by recognizing those waste, improvement plan can be formulated. This study generates recommendations for improvement plan namely designing SOP which not only standardize its processes moreover set its time limit and improvement in the management system by increasing coordination with end user as well as upgrading procurement knowledge. Those result might be utilized by organization to increase procurement’s work efficiency
Risk Evaluation of The Use of The Umbrella Contract for The Construction Project for Medium Voltage Distribution in South Surabaya Region Using The Expected Monetary Value
In the program for accelerating the distribution of electricity to users, also known as the getting electricity program by PT. PLN Persero. Specifically, the distribution of the South Surabaya region has accelerated programs with vendors with a new contract system, namely the umbrella contract system. Where is based on the performance of the umbrella contract system in 2018 it has the potential risks that influence during the implementation process of this umbrella contract takes place. Using the stages of risk identification, risk assessment, and calculating the EMV (expected monetary value) analysis method to analyze each important risk event and its risk response. First, a preliminary survey was conducted to identify the risks that significantly influence the umbrella contract with the respondents who played an important role in the implementation of the umbrella contract. In the assessment of each risk, it is found that the risks that have an important influence during the umbrella contract process take place. Expected monetary value is used to analyze the contingency costs that need to be prepared from each risk variable based on the risk response or risk mitigation carried out during the implementation of the umbrella contract The result obtained in this study there are 4 risks need to be concern during implementation of umbrella agreement cause had great impact for the project cost
