    Clarification of a marathon-induced muscle soreness

    マラソンの壁の特徴であるマラソン終盤における速度低下が大きいランナーは,マラソン直後の血清クレアチンキナーゼ等の筋損傷指標が大きかった.これらのランナーは,序盤に比べて終盤のストライド長の短縮,ストライド頻度の減少,マラソン中のステップ数の増大を示していたが,酸化ストレス等とは関連性を認めなかった.また,マラソン序盤に比べて,終盤で走フォームの変化が大きいランナーは終盤に速度低下が大きかったが,筋損傷指標と明確な関連性は認められなかった.これらから,マラソン誘発性の筋損傷には,酸化ストレス等のメタボリックストレスよりもステップ数の増加によるメカニカルストレスの影響が大きいことが示唆された.The purpose of this study was to investigate the relationships between the marathon induced muscle damage and metabolic and/or mechanical factors. Twenty-three healthy male non-professional runners ran at the full marathon with GPS watch to obtain the running parameters. We set video camera at the 8- and 38-kilometers points to record their running form. Runners with reduced pace exhibited high muscle damage markers values, but not oxidative stress markers immediately after the marathon. Also, they were significantly decreased stride length and stride rate, hence they needed more steps than that maintained runners during whole the marathon. In addition, they had significant a large change of running form in 38 km point compared to that in the 8 km point, however, there were no relations between the change ratio and muscle damage values.     Object: In this study, we conducted a fact-finding investigation to determine the reason for the gap between medical services provided by family pharmacists and the medical needs of patients, and examined factors that affect the rate of signing consent forms.Methods: This study implemented an Internet questionnaire survey of patients who regularly use pharmacies owing to chronic diseases, and family pharmacists, surveying information on the services provided by family pharmacists.Results: The survey extracted “inventory of leftover drugs”, “providing consultations outside pharmacy hours”, “introducing local medical/nursing facilities”, and “home visits”, as characteristic services that patients expected to be provided by family pharmacists. In addition, family pharmacists provided these four services at rates of 87.4, 75.9, 51.7 and 48.5%.Conclusion: These results suggest a possible discrepancy between the functions of family pharmacies required by patients and the medical services provided by family pharmacists. Steps should be taken to match services provided by family pharmacists with the needs of the patients, to increase the rate of signing consent forms

    Lifestyle-mediated nitric oxide boost to prevent SARS-CoV-2 infection: A perspective

    The severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has spread worldwide and has seriously threatened public health by causing significant morbidity and mortality. Patients with coronavirus disease (COVID-19) with preexisting endothelial dysfunction caused by aging, diabetes, hypertension, and obesity are at high risk for life-threatening thromboembolic complications. This suggests a possibility that reduced endothelial nitric oxide (NO) production and NO bioavailability could be a common underlying pathology for the progression of COVID-19. Increasingly, evidence from experimental and clinical studies of SARS-CoV-2 infection shows that NO inhibits the pathogenesis of COVID-19, including virus entry into host cells, viral replication, host immune response, and subsequent thromboembolic complications. Restoring NO bioavailability may have the potential to be a preventive or early-treatment option for COVID-19. This review aims to provide in-depth discussion of NO bioavailability to prevent SARS-CoV-2 infection, particularly by focusing on lifestyle factors such as nitrate-rich diets, physical exercise, and nasal breathing, which could be easily performed on a daily basis to boost NO bioavailability
