    小児前投薬としてbromazepam坐薬の使用経験

    application/pdfIn the present study we investigated the sedative effect of rectally administered Bromazepam as an anesthetic premedicant. In 16 fearful children, aged from 2 to 4 years old, a dose of 0.2mg/kg of Bromazepam was adminitered rectally 2 hours before induction of general anesthesia. The patients underwent dental treatment under enflurane anesthesia. The average duration of anesthesia was 3 hours 25 minutes ±36 minutes. The results were as follows: 1) The sedative effect, especially antianxietic action was satisfactory in the operating room. 2) No remakable changes in respiratory or circulatory function was observed. 3) The induction of anesthesia was smooth due to the lack of the anxiety. 4) No delay of recovery from anestcesia was observed. 5) The adequate sedative effect was maintained after operation. 6) No complication or adverse effects were encountered. These results suggest the usefulness of Bromazepam Suppository for premedicant in fearful children.journal articl

    application/pdfSo-called "foreign body giant cells" often appear in varied lesions. The foreign body giant cells appearing in inflammatory lesions of five cases were studied by histopathologically, histochemically, and immunohistochemically. As the result, giant cells may be classified into two large groups morphologically: spindle and round type. Judging from the nuclei's disposition and the immunohistochemical result, they seems to be made up by the fusion of many cells of the mononuclear phagocyte system. It is thought that foreign body giant cells change their character due to affecting foreign bodies during the maturational process.journal articl

    application/pdfPrecision casting is essential in current dental techniques. It is difficult to eliminate the air bubbles admixed in the investing by polishing after casting. Two practical methods are used to eliminate the air bubbles. These are the vaccuum investing method and the pressure investing method. We have experimented with the latter method, testing the effect of applying pressure right after investing, in an attempt reduce the air bubbles. We used, as investments, Shofu-crystbalite investment, GC ceravest and Whip-mix cerami gold, and we tested vacuum mixing at the standard viscosity for each. In the case of crystbalite, however, we also added hand mixing, changing the length of pressure time (0, 5, 10, 15, 20 minutes) as well as the amount of pressure (0, 2, 4, 6 kg/cm^2). Pressure was applied to the crystbalite samples once they had been left undisturbed after investing. For phosphate bonding investments, pressure was applied to samples that were heated after investing as well as to samples that were left undisturbed. Surface roughness was tested for five samples of each kind by replacing the plastic pattern with metal, and comparing the results with normal values. In conclusion, the above tests showed, in the case of crystbalite, that best results were obtained under 4 kg/cm^2 pressure applied for 15 minutes and that no prominent diffrence was found between hand mixing and vacuum mixing. With ceravest, pressure eliminated the compressive strength and thus adversely affected the surface roughness, indicating that pressure is not suitable for this material. With cerami gold the best result was obtained under 6kg/cm^2 of pressure applied for more than 10 minutes.journal articl

    application/pdfThe district distribution, the number, and the type of malocclusion of orthodontic patients seen during a five year period (1972 to 1976) were studied. The following results were obtained: 1. The number of patients totaled 366 consisting of 146 male and 220 female patients with the ratio of 1:1.5. 2. According to the district distribution, most patients seen were from areas close to the dental school hospital. 3. The number of patients has been increasing yearly and the number of new patients in 1976 was 35.8% of the total patients. 4. The number of patients was higher in June and October during school holidays. 5. The frequency of the type of malocclusions was high in the order of Class III, Class II, Class I malocclusion, and a open bite. 6. Very few of the patients were under 5 years old as 6~11 year olds comprised 70% of the total number of patients. 7. An increased number of adult females over 15 years old patients was seen. 8. The number of patient seen with Class III malocclusion was highest in 6-8 year olds and Class II and Class I malocclusions were most frequently seen in 9~11 year olds.journal articl

    Al_2O_3セラミックインプラント材の臨床応用例に関する経過報告

    application/pdfDuring the last three years we have tested the clinical application of Al_2O_3 ceramic implant (Bioceram^[○!R]) at the Second Prosthodontics Department of Matsumoto Dental College. To examine the progress of 13 such cases, we made observations with the naked eye and x-rays ; in one of the cases, we also examined the progressive change of mas ticatory function. The following results were obtained: 1. Implants were applied in 12 patients (the same number of males and females, aged 20-50), in which 8 out of 20 implants were used for the lower posterior free end region where teeth were missing, the greatest percentage of the whole number, and ll implants of W type were used, the greatest percentage of the number of implants by types. 2. We could not find any case in which implants became detached, and satisfactory results were obtained in all cases on the whole. 3. In the case in which we made continual observations of masticatory function, the following results were obtained: 1) Our observations of masticatory function by standardized x-ray technique 6 months after implantation were satisfactory. 2) We could find nothing unusual in masticatory function examined with Mandibular Kinesiograph before treatment and 6 months after implantation. 3) We examined masticatory function using Lissajous figures from integrated EMGs, Changes in the electrical activitys of the muscles on the side of the implant occurred over time and vaired for different foods. 4) By measuring the individual biting forces 6 months after implantation it was found that the biting force of 76 (the implant denture) was stronger than that of 67.journal articl

    application/pdfTo study the relationship of the origin, development, and enlargement of salivary calculi to bacterial calcification, electron microscopic and bacteriological examinations of a calculus extracted from a child were made. Results obtained were as follows: 1. Cocci-like and filamentous microbiota-like structures, bearing needle crystals, were observed to adhere to the surface of the calculus and/or to exist immediately in the vicinity of the surface of the calculus. They were also found inside the calculus. 2. Two bacterial species, identified as Streptococcus sanguis and Corynebacterium pseudodiphtheriticum, could be isolated from the calculus. 3. Both isolated species formed needle crystals in vitro within the cell from which calcium and phosphate were detected by elementary analyses, demonstrating that the two bacterial species possess calcification activity. 4. These isolated bacteria may contribute to the formation of salivary calculi.journal articl

    application/pdfCemental lesions in the mandibles are as rare than the odontoma, ameloblastoma, and odontogenic fibroma. The rate is 10.73% of all odontogenic tumors. A 63 year old wowan was found to have multiple cemental lesions in the bilateral mendibular molar regions. Roentgenograms revealed sharply marginated, irregularly outlined, and non homogeseous mass lesions, involving 876 5678 root apexes. We reported a case of cemental lesions which had very interesting variable radiographical features, however the patient did not undergo surgical operation and the histopathological diagnosis cannot be recognized.journal articl

    application/pdfWe report a case of chronic alveolar abscess extending into the maxillary sinus of a female, age 19 years. While signs of abscess formation and sinusitis were not visible an clinical examination, an X-ray graph showed a spherical slightly radiopaque area of mucosal enlargement without destuction of bone in the right maxillary sinus. The clinical diagnosis was suspected radicular cyst, but due to the histopathological finding of a cavity lined by granulation tissue, an alveolar abscess was diagnosed. As is well known the maxillary sinus is easily affected by odontogenic disease, because of the very close anatomic relation between this sinus and dental apices. Therefore, we have to differentiate radicular cysts from mucous cysts, and from postperative maxillary cysts, which look like radicular cysts in the X-ray findings. Roentogenographic features and the formation of alveolar abscess in maxillary sinus are discussed.journal articl

    Streptococcus milleriのヒアルロニダーゼの精製とその性状

    application/pdfScreening tests of gingival crevice dental plaque conducted to detect the bacterial strains responsible for degradation of acid-mucopolysaccharides provided an adequate strain for the production of hyaluronidase. This strain was identified as Streptococcus milleri, based on its various biological properties. The enzyme produced in the culture supernatant of anaerobic culture was purified to homogeneity by sequential procedures including ammonium sulfate precipitation, ionexchange chromatography, and gel filtration. The specific activity increased 15,000 fold and the recovery of the enzyme activity was 21.3%. The molecular weight was 100,000 and its isoelectric point was 9.3. The optimum pH for the activity was 6.0. Heating of the enzyme at 60℃ for 5 minutes resulted in complete inactivation. The enzyme activity was inhibited strongly by Zn^, Hg^, and Cu^, while Ca^, Mg^, Fe^, Co^, Mn^, and EDTA had no effect. The enzyme was active against hyaluronic acid, however, chondroitin sulfate, chondroitin sulfate A, B, and C were not degraded by this enzyme. In the enzymatic products of hyaluronic acid, unsaturated disaccharides were detected by the methods of gel filtration and paper chromatography.journal articl
