松本歯科大学病院矯正科開設後15年間に来院した患者の実態調査 : その3 昭和57年~昭和61年
application/pdfFor Part 3 this report, we supplemented the material given in Parts 1 and 2 by surveying data on patients who visited our departmental clinic during the five year period from January 1982 through December 1986. The results of this survey are as follows: 1. The number of patients totaled 954, consisting of 333 males and 621 females. The ratio of males to females was roughly 1:2. 2. More patients came from areas closer to the clinic in terms of distance or commuting time. 3. Although the average annual rate of increase observed was small, the total number of patients increased over the five year period. 4. The highest monthly averages for numbers of patients were observed in October, March, and June, in descending order. 5. With regards to the distribution of malocclusion type, anterior crowding was observed to increase for both male and female patients. 6. When classified by age, more than 40% of the total number of patients were school children between 9 and 11 years old. 7. In the present survey, there were more female patients than males in all age groups. Also, patients 15 years or older showed pronounced increases for both males and females. 8. For all cases of Class II and Class III malocclusion and anterior crowding, the number of patients reached a peak in the 9-11 year age group for both males and females. 9. In the same manner as reported in Parts 1 and 2, increases were observed for both males and females with Class III malocclusion, and for males aged 15 years or more with anterior crowding.journal articl
原因が判明し難かった急性根尖性歯周炎の1例
application/pdfA maxillary second premolar, which had been treated with pulpectomy of an intact pulp about 8 years ago, flared up suddenly with spontaneous pain and pain on percussion. The condition was diagnosed as acute apical periodontitis of unknown cause. When the root canals were obturated and a radiograph was taken, it became clear for the first time that pulp gangrene in an apical ramification had caused first the acute apical periodontitis, and subsequently, a lateral lesion.journal articl
歯科教育機関における診断学の現況
application/pdfIn a previous investigation of the historical development of Japanese dental colleges, I have shown how each specialty has developed its own specific therapeutic methodologies, making it difficult to establish a specialty dealing exclusively with diagnosis. But oral diagnosis has been in existence since the Meiji period, and I believe it is necessary to create a single specialty called "Oral Diagnostics." Here I report the investigation of the current condition of oral diagnosis in 29 dental curricula in Japan. The results are as follows. 1. Lectures and preliminary medical examinations in oral diagnosis are given in 11 colleges. 2. Lectures are not given, but preliminary medical examinations are conducted in 14 colleges. 3. Lectures are not given, but preliminary examinations are given by each separate clinic within the institution in 4 colleges. In conclusion, I feel that in order to establish a more standard and functional dental education, it is necessary to create a field such as "Oral Diagnostics," in which the dental student approaches the oral region as a whole as the main object of study.journal articl
幼児期のむし歯減少についての疫学的分析
application/pdfIt is evident that the prevalence of tooth decay in infancy has been gradually declining throughont Japan. But the cause of this reduction has not been established scientifically. Accordingly, I tried to find some cause of this reduction in tooth decay by comparing the patterns of reduction for several areas with differing populations. It is clear that the reduction rate was very different according to the size of population. The inhabitants living in cities can generally receive good health services, but it is difficult to obtain good services for those living in rural areas. As a result, I belive that dental public services and care system have been important factors in the reduction of tooth decay prevalence. Under the public health care system establisled by the 1965 Maternity Health Care Law, all 3 year old children must receive a health examination which includes a dental check, and their parents receive dental health instruction from dentists, dental hygienists, and public health nurses. The prevalence of tooth decay declined slowly over the first 10 years after the enforcement of the Maternity Health Care Law. Subsequently, the Ministry of Welfare required that all local goverments administer health examinations, including a dental check, for 1 and a half year old children in 1977. Since that time tooth decay prevalence has declined remarkably. With regard to the dental care system, it has become easier to get dental care for children, because the number of dental clinics has increased since the second half of the 1970s. This agrees with the time of the beginning of the reduction in tooth decay prevalence, and coincides with the time of the establishment of a better dental public health care system in Japan.journal articl
齲蝕免疫に関する基礎的研究
application/pdfThe mouth, which is an open organ, is constantly covered with secretory and serum antibodies. These antibodies, which are resistant to protein-dissolving enzymes and bacterial protein-dissolving enzymes, are believed to prevent the attachment of oral bacteria such as Str. mutans on the tooth surface, and to control their multiplication by working as neutralizing or cohesive antibodies. In a basic study of anticaries immunity, Taubman, Michaleck and others suggested the existance of secretory type IgA acted clearly as an anticaries agent. From a clinical viewpoint, Arnold and others suggested the existence of secretory type IgA which acted against Str. mutans in the colostrum and saliva. In this study we investigated the relationship between Ig in the colostrum and the amount of Ig and pH in the infant saliva, as a preparatory study for examining the signifcance of the amount of Ig in controling the activity of caries and the adhesion of bacteria in the mouth. The results obtained are as follows. 1) The SRID (Single Radial Immunodiffuson) method was valuable for measuring the amount of Ig in saliva. 2) In ragard to the amount of Ig in the saliva, no significant difference was recognized between batches prior to processing and those receiving no processing. 3) No significant difference was recognized in the amount of Ig within natural saliva and saliva subjected to stimulus. 4) Sexual difference had no significant effect on the amount of Ig in the saliva.journal articl
歯槽堤造成法を補綴前処置として応用したブリッジ2例
application/pdfHydroxyapatite is well accepted for replacing deficiencies in alveolar bone and a considerable number of clinical cases have reported on its efficacy. It has been shown that hydroxyapatite, which has wide applications, is an effective preprosthetic preparation for the augmentation of alveolar ridges. This paper is a report of observations on two cases: the extraction of the 23 teeth on a 40-year-old woman, and the extraction of the 1 tooth in a 38-year-old woman.journal articl
A Clinical Evaluation of a Plastic Temporary Filling Material (Dura Seal®)
application/pdfRecently Dura Seal, a kind of plastic, has been marketed as a temporary filling material. This clinical study was done to determine whether or not Dura Seal has the properties advertised by the manufacturer. A total of 135 teeth, including 110 vital teeth and 25 pulpless teeth, were prepared for metal inlay cavities. The prepared cavities were sealed with Dura Seal for an average of 12.0 days. Dura Seal was found to be easily removable, to show no pulp damage, and to have considerable mechanical strength.journal articl
ポリオレフィン系軟質裏装材の基礎的検討 : 再加圧による色彩の変化について
application/pdfThe purpose of this study was to analyze the effect of re-pressing on denture base material made of resilient polyolefin, in terms of its resistance to color change and to swelling. Seven samples each of re-pressed polyolefin were immersed in three solutions (olive oil, olive oil plus β-Carotene, and iron hematoxylin) at 37℃ for seven days. Equal numbers of once-pressed samples were immersed in the same solutions for control. For both the re-pressed and the once-pressed samples, immersion was found to produce the following statistically significant chages: (1) swelling as observed in olive oil alone, (2) moderate color change in the olive oil plus β-Carotene solution, (3) slight color change in the iron hematoxylin solution.journal articl
