障害者の口内法X線撮影に対する適応性に関与する要因の検討
application/pdfThis study was performed to make clear factors relating with adaptation to intraoral radiograph in the handicapped. We observed behaviors of the handicapped as taking roentgen photographs intraorally and analyzed. The subjects were 24 mentally retarded, but without motor disability patients, average age 23.3±13.3 years old. They were taken roentgen photograph of their upper and lower anterior teeth intraorally, and their behavior were recorded with VTR and evaluated. Each subject was examined his/her developmental level with Enjoji's Infant Analytic Development Test. We also surveyed their type of handicap, behavior management method in previous dental situation, number of visit, chronological age and sex. Those factors were analyzed according to the Akaike's Information Criterion (AIC). The results were as follows: (1) 15 patients (62.5% of the subjects) were able to accept without any restraint, and 5 patients (20.8% of the subjects) accepted by holding the patient's head. But, 4 patients (16.7% of the subjects) showed maladjusted behaviors, and it was impossible to take a roentgen photograph intraolly. (2) The most participate factor in adaptation were behavior management methods in previous dental situation, Those who had accepted dental treatment without any special behavior management methods or with only diluted nitrous oxide, were apt to accept intraoral radiograph. (3) Also, there were close correlation between the developmental level and the adaptability. It was that evaluating developmental level of the patient, especially in understanding of speech, personal relationship or speech ability should be useful means to make an estimate his/her adaptability. The readiness for intraoral radiograh might be expected at 3 years and 10 months in understanding of speech, 3 years and 6 months in personal relationship, or in speech ability. Those observation suggest that a handicapped patient who possesses a cirtain readiness, even if displayes maladjusted behaviors at the biginning, should be induced to accept intraoral radiograph with positive approach.journal articl
松本歯科大学病院小児歯科における外来患者の実態調査 : 第2報 新患児の18年間の推移について
application/pdfA total of 10401 children (5312 male and 5089 female) who first visited the Pedodontic clinic of Matsumoto Dental College Hospital from 1976 to 1994 were surveyed with regard to age, regional difference, incidence of caries and the nursing environment. The results were as follows. 1: 3-5 year old children occupied the largest percentage of first visit outpatients annually. The majority of all new patients were between this age. 2: The number of new patients coming from Matsumoto city, Higashi-Chikuma county and Minami-Azumino county showed an increase in number every year. 3: The new patient's chief complaints of tooth ache and caries treatment decreased every year. On the contrary, oral health management as the chief complaint did gradually increase. 4: While analyzing the incidence of caries, it was found that the df index of the deciduous and the permanent teeth decreased annually. 5: With regards to the nursing environment, an increase in the ratio of breastfeeding and regularity of eating between meals was observed. 6: Brushing habits and the application of topical fluoride increased over time.journal articl
A Stress Analysis on Abutment Teeth of Dowel Cemented Fixed Partial Dentures : On the effect of the resorption of alveoar bone
application/pdfStress in abutment teeth with cemented dowels and those with crown margins in fixed-fixed and cantilever bridges that resorp alveolar bone were analyzed by a two dimensional finite element method. Stress in all parts of the dentin in both bridges increased with the resorption of alveolar bone. Moreover, in th dentin and apex of the dowel in cantilever bridges, extreme stress when there were large amounts of resorption of alveolar bone, was seen. Therefore, in cantilevers bridges, cemented dowels must be created by large materials of Young's modulus. Furthermore, as much dentin as possible must be retained and the lever effect must be reduced.journal articl
上顎洞内に発症した歯原性角化囊胞に開窓療法を施した1例 : 口腔上顎洞瘻孔が鼻腔気流へ与える影響
application/pdfIt is difficult to choose treatment of odontogenic keratocyst occurred in the maxillary sinus, because it exhibit frequently recurrence. We extirpated large odontogenic keratocyst filled in the maxillary sinus, which was marsupialized, and removable acrylic resin plate was held in place for an oro-antral fistula caused by the marsupialization. We analyzed effect of the nasal airflow on oro-antral fistula. The results were that airflow through nostril of the same side as oro-antral fistula was decreased when airflow was much more than 250 ml/sec. This indicates that oro-antral fistula patent, because of occlusion by buccal mucosa, does not affect air flow associated with respiration at rest, while affect high airflow rate and an increase of airflow volume accompanied with hard physical training and blowing her nose due to an increase of nostril resistance.journal articl
アパタイト含有キトサンフイルムの諸性質について
application/pdfChitosan is a polysaccharide extracted from Crustacea. Chitosan sol is produced by dissolving chitosan in a solution of organic acids. Chitosan film can be fabricated from chitosan sol, which has been neutralized with 5% polyphosphate solution and gelatinized. Chitosan film is characterized by both its elasticity and in vivo absorbtion capacity. Although the chitosan film become inelastic when dried, they will return to an elastic state when soaked in water. It is of practical importance to determine the amount of calcium ions dissolved from the hydroxyapatite which is kneaded with the chitosan film. In the present study, the relationship of various amounts of added hydroxyapatite and chitosan to shrinkage, hardness, tensile strength and elongation of the chitosan film was determined in addition to the amount of dissolved calcium ions. The following results were obtained. The shrinkage during drying was decreased by increaseing the amount of hydroxyapatite added. The hardness showed a maximum value at the ratio of hydroxyapatite: chitosan sol= 4 : 11. The hardness was increased by increasing the amount of chitosan. The tensile strength of the chitosan films kneaded with hydroxyapatite exhibited a higher value than those without haydroxyapatite. Elongation showed the highest value when the ratio of hydroxyapatite:chitosan sol=2 : 11. The tensile strength and elongation were increased by incresing the amount of chitosan. The amount of calcium ion dissolution showed the highest value when the ratio of hydroxyapatite:chitosan sol=4 : 11. When chitosan film was soaked in physiological saline, the original level of elasticity was restored.journal articl
レーザ溶接したチタン鋳造体の疲労特性について
application/pdfUntil now, clinical applications in dentistry for the joining of pure titanium have not been able to be carried out. At long last, this has been accomplished by degrees. In this study fatigue properties for cast titanium samples laser welded were examined. Fatigue tests were carried out under bending from one direction on the cantilever. The main results obtained are as follows; Under repeated strain of 0.25mm, the welded samples never ruptured and showed greater resistance to repeated strain of 0.5mm. Under repeated strain of 0.5mm, the number of stress cycles to rupture was somewhat increased as the distance from fulcrum to welded portion increased. Furthermore, for the most part, the number of stress cycles for samples that did not rupture at the fulcrum point were greater than the number of stress cycles for ruptured fulcrum point samples. A brittle fracture image with a pattern like a shell shape was shown at the fractured surface. There wasn't a distinct difference between the shape of the welded fractured surface samples and unwelded fractured surface samples. It was considered that the distance between the laser welding portion and the fulcrum portion should be somewhat longer to create conditions for greater fatigue properties of laser welds for case of clinical application.journal articl
幼若永久歯の口腔清掃に関する研究 : 第2報 第1大臼歯のプラーク除去効果について
application/pdfToothbrushing to physically remove plaque is one of the most commonly recommended and reliable means to achieve good oral hygiene. However, good brushing skills are needed particularly for the newly erupted first permanent molars because the anatomical structures and eruption processes of the first permanent molars are very complicated. In the study, thirty children aged 5-9 years participated, and OHI (oral hygiene index) for each child was assessed before and after by using two types of powered toothbrushes (arcuate and vibratory motions) and a regular manual toothbrush in removing supragingival plaque from the first permanent molars. Results were as follows: 1) In all surfaces of the maxillary and mandibular first permanent molars, the effects of the powered toothbrushes in removing supragingival plaque were found to be better than the manual toothbrush. 2) On right side of first permanent molars, a significant difference was found among arcuate powered toothbrush, vibratory powered toothbrush and the manual toothbrush. On the contrary, on the left side of first permanent molars, a significant difference was only found between the arcuate and the manual toothbrush. 3) In all different eruption stages of the first permanent molar, the effects of powered toothbrushes in removing supragingival plaque were found to be better than the manual toothbrush.journal articl
診断および予後観察にCT像が有用であったGarré骨髄炎の1例
application/pdfWe reported the Garré's osteomyelitis caused by infection from the distolingual root apex of the lower first molar, which was successfully managed by both root treatment and antibiotics therapy. After seven months the periosteal lesion resolved without bone contouration. Usefulness of CT for diagnosis and prognostic observation in the management of the Garré's osteomyelitis was discussed.journal articl
