Triplanar ensemble U-Net model for white matter hyperintensities segmentation on MR images
White matter hyperintensities (WMHs) have been associated with various cerebrovascular and neurodegenerative diseases. Reliable quantification of WMHs is essential for understanding their clinical impact in normal and pathological populations. Automated segmentation of WMHs is highly challenging due to heterogeneity in WMH characteristics between deep and periventricular white matter, presence of artefacts and differences in the pathology and demographics of populations. In this work, we propose an ensemble triplanar network that combines the predictions from three different planes of brain MR images to provide an accurate WMH segmentation. In the loss functions the network uses anatomical information regarding WMH spatial distribution in loss functions, to improve the efficiency of segmentation and to overcome the contrast variations between deep and periventricular WMHs. We evaluated our method on 5 datasets, of which 3 are part of a publicly available dataset (training data for MICCAI WMH Segmentation Challenge 2017 - MWSC 2017) consisting of subjects from three different cohorts, and we also submitted our method to MWSC 2017 to be evaluated on the unseen test datasets. On evaluating our method separately in deep and periventricular regions, we observed robust and comparable performance in both regions. Our method performed better than most of the existing methods, including FSL BIANCA, and on par with the top ranking deep learning methods of MWSC 2017
The relative and attributable risks of cardia and non-cardia gastric cancer associated with H. pylori infection in China: a case-cohort study
Executive function in HIV-affected children and adolescents: a systematic review and meta-analyses
This review aimed to determine: whether EF is affected in children and adolescents (2–24-years-old) with perinatal HIV infection, perinatal HIV exposure without infection, and behaviourally acquired HIV. A systematic review (PROSPERO number: CRD42017067813) was conducted using 11 electronic databases (01.01.1981–09.07.2019) and 8 conference websites. Primary quantitative studies with EF scores on cognitive tasks and/or behavioural report measures were included. Meta-analyses were performed by EF subtype and subpopulations compared. 1789 records were found. Sixty-one studies were included in the narrative synthesis; 32 (N = 7884 participants) were included in meta-analyses. There was a distinct pattern of reduced EF in those with perinatal HIV infection on antiretroviral therapy compared to controls: pooled effect sizes were largest for verbal and visuospatial working memory, with smaller effects on planning, inhibitory control and set-shifting. Data were limited for other HIV-affected subpopulations. Perinatal HIV infection is associated with reduced EF with varying effect sizes for the different EF subtypes
Using Primary Care Data to Report Real-World Pancreatic Cancer Survival and Symptomatology.
Pancreatic cancer is the 10th most common cancer diagnosed; despite recent advances in many areas of oncology, survival remains poor, in part owing to late diagnosis. Whilst primary care data are used widely for epidemiology and pharmacovigilance, they are less used for observing survival. In this study we extracted a pancreatic cancer cohort from a nationally representative English primary care database of electronic health records (EHRs) and reported on their symptom and mortality data. A total of 11, 649 cases were identified within the Oxford Royal College of General Practitioners (RCGP) Clinical Informatics Digital Hub network. All-cause mortality data was recorded for 4623 (39.69%). Mean age at recording of cancer diagnosis was 71.4 years (SD 12.0 years). 1-year and 5-year survival was 22.06% and 3.27% respectively. Within a multivariate model, age had a significant impact on survival; those diagnosed under the age of 60 had the longest survival, as compared to those age 60 - 79 (HR: 1.36, 95% CI: 1.20 - 1.54, p < 0.001) and 80+ (HR: 2.13, 95% CI: 1.86 - 2.44, p < 0.01). Symptomatology was examined; at any time point abdominal pain was the most commonly reported symptom present in 5271 cases (45.2%), but within the 12 months preceding diagnosis jaundice was the most common feature, present in 2587 patients (22.2%). Future studies clarifying other contributing factors on survival outcomes and patterns of symptomatology are needed; primary care EHRs provide an opportunity to evaluate real-world cancer patient cohort data
Development and validation of the Early Executive Functions Questionnaire: A carer-administered measured of executive functions suitable for 9- to 30-month-olds
Executive functions (EFs) enable us to control our attention and behavior in order to set and work toward goals. Strong EF skills are linked to better academic performance, and greater health, wealth, and happiness in later life. Research into EF development has been hampered by a lack of scalable measures suitable for infancy through to toddlerhood. The 31-item Early Executive Functions Questionnaire (EEFQ) complements temperament measures by targeting cognitive and regulatory capabilities. Exploratory Factor Analysis (n = 486 8- to 30-month-olds) and Confirmatory Factor Analysis (n = 317 9- to 30-month-olds) indicate Inhibitory Control, Flexibility, and Working Memory items load onto a common “Cognitive Executive Function (CEF)” factor, while Regulation items do not. The CEF factor shows strong factorial measurement invariance for sex, and partial strong factorial measurement invariance for age. CEF and Regulation scores show limited floor and ceiling effects, good internal consistency, short-term stability, and convergent validity with carer-report measures of attentional control. The EEFQ is sensitive to developmental change. Results indicate that the widely overlooked period between late infancy and early toddlerhood may be a sensitive period for EF development. The low-resource demands of the EEFQ afford the possibility to study emergent EFs at scale; opening up new opportunities in basic developmental and intervention research

associated to the genus-two, class superconformal field theory of
type . The free field realisation is in the style of recent
work by the authors, and is formulated in terms of a one-dimensional isotropic
lattice vertex algebra along with two pairs of symplectic fermions. Our
realisation makes manifest an enhanced outer automorphism group
of the VOA that is inherited from the symplectic fermion system. This extends
an outer automorphism that has been observed in recent work of
Kiyoshige and Nishinaka and significantly simplifies the structure of the
algebra. Along the way, we also produce a realisation of the generic subregular

Large-volume hyperthermia for safe and cost-effective targeted drug delivery using a clinical ultrasound-guided focused ultrasound device
Lyso-thermosensitive liposomes (LTSLs) are specifically designed to release chemotherapy agents under conditions of mild hyperthermia. Preclinical studies have indicated that magnetic resonance (MR)-guided focused ultrasound (FUS) systems can generate well-controlled volumetric hyperthermia using real-time thermometry. However, high-throughput clinical translation of these approaches for drug delivery is challenging, not least because of the significant cost overhead of MR guidance and the much larger volumes that need to be heated clinically. Effectively empowering: a different look at bolstering the effectiveness of global environmental assessments
Global environmental assessments are widely considered to play a prominent role in environmental governance. However, they are also criticised for a lack of effectiveness in informing policy and decision-making. In response, GEAs have adopted a number of strategies to bolster their effectiveness, including by orienting themselves towards solutions (solution-orientation), increasing the diversity of included experts (participation), and producing more targeted assessments (contextualisation). In this article, we analyse these strategies as attempts to be effective for multiple audiences while also identifying the limitations of these strategies. Based on this analysis, we propose to conceive of GEAs as processes that are able to empower diverse actors – ranging from diplomats in international negotiations to civil society activists, or indigenous and local knowledge holders – to act towards socio-environmental objectives. Seen in this light, the effectiveness of GEAs can be improved by reflecting on which actors can benefit from assessments and how assessments can contribute to their empowerment. This strategy goes beyond current proposals that aim to strengthen the authority of assessments by boosting the scientific quality and credibility of the reports. Indeed, it complements them with an explicitly political perspective. Using examples of empowerment in different phases of GEA production and use, we argue that this reconceptualisation of effectiveness requires assessments to reflect a diversity of problem and solution frames, thereby creating entry points for the empowerment of a broad range of actors. We conclude by providing three illustrative ideas to improve effectiveness for the design and execution of assessments
