    Work that can be done from home: evidence on variation within and across occupations and industries

    Using large, geographically representative surveys from the US and UK, we docu ment variation in the percentage of tasks workers can do from home. We highlight three dimensions of heterogeneity that have previously been neglected. First, the share of tasks that can be done from home varies considerably both across as well as within occupations and industries. The distribution of the share of tasks that can be done from home within occupations, industries, and occupation-industry pairs is systematic and remarkably consistent across countries and survey waves. Second, as the pandemic has progressed, the share of workers who can do all tasks from home has increased most in those occupations in which the pre-existing share was already high. Third, even within occupations and industries, we find that women and workers with less stable work arrangements can do fewer tasks from home. Using machine-learning methods, we extend our working-from-home mea sure to all disaggregated occupation-industry pairs

    Probing neutrino emission at GeV energies from compact binary mergers with the IceCube Neutrino Observatory

    The advent of multi-messenger astronomy has allowed for new types of source searches by neutrino detectors. We present the results of the first search for 0.5 - 5 GeV astrophysical neutrinos emitted from all compact binary mergers, i.e., binary black hole, neutron star black, mass gap and binary neutron star mergers, detected by the LIGO and Virgo interferometers during their three first runs of observation. We use an innovative approach that lowers the energy threshold from ~10 GeV to ~0.5 GeV and searches for an excess of GeV-scale events during astrophysical transient events. No significant excess was found from the studied mergers, and there is currently no hint of a population of GeV neutrino emitters found in the IceCube data

    Patch augmentation surgery for rotator cuff repair: the PARCS mixed-methods feasibility study

    Background A rotator cuff tear is a common, disabling shoulder problem. Symptoms may include pain, weakness, lack of shoulder mobility and sleep disturbance. Many patients require surgery to repair the tear; however, there is a high failure rate. There is a need to improve the outcome of rotator cuff surgery, and the use of patch augmentation (on-lay or bridging) to provide support to the healing process and improve patient outcomes holds promise. Patches have been made using different materials (e.g. human/animal skin or tissue and synthetic materials) and processes (e.g. woven or mesh). Objectives The aim of the Patch Augmented Rotator Cuff Surgery (PARCS) feasibility study was to determine the design of a definitive randomised controlled trial assessing the clinical effectiveness and cost-effectiveness of a patch to augment surgical repair of the rotator cuff that is both acceptable to stakeholders and feasible. Design A mixed-methods feasibility study of a randomised controlled trial. Data sources MEDLINE, EMBASE and the Cochrane Library databases were searched between April 2006 and August 2018. Methods The project involved six stages: a systematic review of clinical evidence, a survey of the British Elbow and Shoulder Society’s surgical membership, a survey of surgeon triallists, focus groups and interviews with stakeholders, a two-round Delphi study administered via online questionnaires and a 2-day consensus meeting. The various stakeholders (including patients, surgeons and industry representatives) were involved in stages 2–6. Results The systematic review comprised 52 studies; only 15 were comparative and, of these, 11 were observational (search conducted in August 2018). These studies were typically small (median number of participants 26, range 5–152 participants). There was some evidence to support the use of patches, although most comparative studies were at a serious risk of bias. Little to no published clinical evidence was available for a number of patches in clinical use. The membership survey of British Elbow and Shoulder surgeons [105 (21%) responses received] identified a variety of patches in use. Twenty-four surgeons (77%) completed the triallist survey relating to trial design. Four focus groups were conducted, involving 24 stakeholders. Differing views were held on a number of aspects of trial design, including the appropriate patient population (e.g. patient age) to participate. Agreement on the key research questions and the outline of two potential randomised controlled trials were achieved through the Delphi study [29 (67%)] and the consensus meeting that 22 participants attended. Limitations The main limitation was that the findings were influenced by the participants, who are not necessarily representative of the views of the relevant stakeholder groups. Conclusion The need for further clinical studies was clear, particularly given the range and number of different patches available. Future work Randomised comparisons of on-lay patch use for completed rotator cuff repairs and bridging patch use for partial rotator cuff repairs were identified as areas for further research. The value of an observational study to assess safety concerns of patch use was also highlighted. These elements are included in the trial designs proposed in this study.     An evaluation of the effectiveness of perampanel in people with epilepsy who have previously undergone resective surgery and/or implantation of a vagal nerve stimulator

    About 30% of people with epilepsy (PWE) are drug-resistant. Those with focal seizures may be suitable for epilepsy surgery. Those not amenable to resective surgery can be considered for vagus nerve stimulation (VNS). However, after operative procedures, around 50% of patients continue to experience seizures. A multi-center retrospective study assessing perampanel effectiveness and tolerability for PWE who have undergone surgical resection and/or VNS implantation was performed. The primary outcome was ≥50% reduction in seizure frequency while secondary outcomes included side effects (SEs), dose-related effectiveness, and toxicity. The median perampanel dose was 6 mg.     Formation of singularities and existence of global continuous solutions for the compressible euler equations

    We are concerned with the formation of singularities and the existence of global continuous solutions of the Cauchy problem for the one-dimensional nonisentropic Euler equations for compressible fluids. For the isentropic Euler equations, we pinpoint a necessary and sufficient condition for the formation of singularities of solutions with large initial data that allow a far-field vacuum---there exists a compression in the initial data. For the nonisentropic Euler equations, we identify a sufficient condition for the formation of singularities of solutions with large initial data that allow a far-field vacuum---there exists a strong compression in the initial data. Furthermore, we identify two new phenomena---decompression and de-rarefaction---for the nonisentropic Euler flows, different from the isentropic flows, via constructing two respective solutions. For the decompression phenomenon, we construct a first global continuous nonisentropic solution, even though the initial data contain a weak compression, by solving a backward Goursat problem, so that the solution is smooth, except on several characteristic curves across which the solution has a weak discontinuity (i.e., only Lipschitz continuity). For the de-rarefaction phenomenon, we construct a continuous nonisentropic solution whose initial data contain isentropic rarefactions (i.e., without compression) and a locally stationary varying entropy profile, for which the solution still forms a shock wave in a finite time

    Leveraging nature-based solutions for transformation: Reconnecting people and nature

    1. Nature‐based Solutions (NbS) have rapidly been gaining traction across the research, policy and practice spheres, advocated as transformative actions to jointly address biodiversity loss and climate change. However, there are multiple, alternative ways to conceptualize NbS across those three spheres. 2. To inform the NbS discourses in research, policy and practice, we critically reflect on the prevailing framing of NbS. Although the concept links environmental health to human well‐being, we argue that its current dominant framing reinforces a dichotomy between people and nature by highlighting one, external nature working for the benefit of society. For the NbS concept to support transformation, we believe it must embody a reframing of human–nature relationships towards regenerative relationships between humans and nature. 3. To support the transformative aspirations of NbS, we propose a novel core framing of NbS making explicit the co‐dependence of people and nature, which underpins human well‐being and environmental health. We highlight how such a framing can support a transformation through influencing beliefs and normative values, and second, through the communication and application of the NbS concept in research, policy and practice. 4. We then elaborate on how such a framing is key to support inclusivity and collaboration between diverse research perspectives, policy objectives across scales and implementation practices to deliver just and successful NbS

    Anomalous wave statistics following sudden depth transitions: application of an alternative Boussinesq-type formulation

    Recent studies of water waves propagating over sloping seabeds have shown that sudden transitions from deeper to shallower depths can produce significant increases in the skewness and kurtosis of the free surface elevation and hence in the probability of rogue wave occurrence. Gramstad et al. (Phys. Fluids 25 (12): 122103, 2013) have shown that the key physics underlying these increases can be captured by a weakly dispersive and weakly nonlinear Boussinesq-type model. In the present paper, a numerical model based on an alternative Boussinesq-type formulation is used to repeat these earlier simulations. Although qualitative agreement is achieved, the present model is found to be unable to reproduce accurately the findings of the earlier study. Model parameter tests are then used to demonstrate that the present Boussinesq-type formulation is not well-suited to modelling the propagation of waves over sudden depth transitions. The present study nonetheless provides useful insight into the complexity encountered when modelling this type of problem and outlines a number of promising avenues for further research

    Hydrocortisone as an adjunct to brief cognitive-behavioural therapy for specific fear: endocrine and cognitive biomarkers as predictors of symptom improvement

    Background: Glucocorticoid (GC) administration prior to exposure-based cognitive-behavioural therapy (CBT) has emerged as a promising approach to facilitate treatment outcome in anxiety disorders. Further components relevant for improved CBT efficacy include raised endogenous GCs and reductions in information-processing biases to threat. Aims: To investigate hydrocortisone as an adjunct to CBT for spider fear and the modulating role of threat bias change and endogenous short-term and long-term GCs for treatment response. Methods: Spider-fearful individuals were randomized to receiving either 20 mg of hydrocortisone (n = 17) or placebo (n = 16) one hour prior to single-session predominantly computerised exposure-based CBT. Spider fear was assessed using self-report and behavioural approach measures at baseline, 1-day and 1-month follow-up. Threat processing was assessed at baseline and 1-day follow-up. Cortisol and cortisone were analysed from hair and saliva samples at baseline. Results/outcomes: Self-report, behavioural and threat processing indices improved following CBT. Hydrocortisone augmentation resulted in greater improvement of self-report spider fear and stronger increase in speed when approaching a spider, but not on threat bias. Neither threat bias nor endogenous GCs predicted symptom change, and no interactive effects with hydrocortisone emerged. Preliminary evidence indicated higher hair cortisone as predictor of a stronger threat bias reduction. Conclusions/interpretation: Our data extend earlier findings by suggesting that GC administration boosts the success of exposure therapy for specific fear even with a low-level therapist involvement. Future studies corroborating our result of a predictive hair GC relationship with threat bias change in larger clinical samples are needed

    Men are from Mars, and women too: a Bayesian meta-analysis of overconfidence experiments

    Gender differences in self-confidence could explain women’s under representation in high-income occupations and glass-ceiling effects. We draw lessons from the economic literature via a survey of experts and a Bayesian hierarchical model that aggregates exper imental findings over the last twenty years. The experts’ survey indicates beliefs that men are overconfident and women under-confident. Yet, the literature reveals that both men and women are typically overconfident. Moreover, the model cannot reject the hypothe sis that gender differences in self-confidence are equal to zero. In addition, the estimated pooling factor is low, implying that each study contains little information over a common phenomenon. The discordance can be reconciled if the experts overestimate the pooling factor or have priors that are biased and precise
