Effect of high bicarbonate hemodialysis solution on biochemical parameters and anthropometric indices
Introduction: Protein energy wasting is an adverse consequence of renal failure, which is correlated with increased mortality and morbidity. Metabolic acidosis has a major role in the development of protein energy wasting in hemodialysis patients. Every effort that could ameliorate this catabolic state would be beneficial to stabilize body composition. The aim of this study was to investigate the possible beneficial effects of high bicarbonate dialysis on anthropometric indices and biochemical parameters of nutrition. Methods: Fifty-six hemodialysis patients were randomly enrolled in two groups: an intervention group that underwent hemodialysis for 6 months with high bicarbonate dialysate concentration (36 mmol/L, N = 26) and a control group that underwent hemodialysis using a bicarbonate dialysate concentration of 30 mmol/L (N = 30). Biochemical parameters of nutrition and weight, body mass index (BMI), total body water, percent body fat, and other anthropometric indices were measured at the beginning and the end of the trial. Findings: At the end of the 6 month evaluation period, plasma levels of albumin, phosphorus, K, calcium, and bicarbonate showed no significant changes. Body weight and BMI increased significantly in high bicarbonate arm but did not change significantly in the control group. Percent body fat in the arms and legs did not change in intervention arm, but decreased significantly in the controls. Discussion: The results suggest that higher bicarbonate dialysis can have beneficial effects on nutritional status and might protect against loss of fat mass
Developing novel thin film composite membrane on a permeate spacer backing fabric for forward osmosis
A repeatable method was introduced for developing liable flat sheet thin-film forward osmosis (FO) membranes on spacer backing fabric support. Pasting of a water-soluble polymer (Polyvinylpyrrolidone (PVP)) on the backside of the fabric was investigated to prevent the penetration of the polymer dope (Polyethersulfone (PES)) into the backing fabric during phase inversion. All samples were prepared on spacer fabric with a 250 μm in thickness. First, PVP as a water-soluble polymer was cast in 100 μm thickness on a glass plate and then the spacer fabric was pulled from the top on the pasted PVP to fill the bottom side of the fabric. The lower bottom side of the fabric was filled with PVP. This helped to limit PES polymer penetration and to achieve a thinner polymeric support layer during phase inversion. After interfacial polymerization and forming rejection, thin-film composite (TFC) FO membranes were obtained. An investigation into the effect of multimedia training on the knowledge and self-efficacy of children with asthma
Introduction: Asthma is one of the most common chronic diseases among children is a global health issue and its rising trend has caused concern about the health system. Therefore, this study was conducted to investigate the effect of multimedia education on the knowledge and self-efficacy of children with asthma in Bushehr. Method: semi-experimental study. The research community included all children under the age of 12 years with asthma. The samples were selected by available method of 50 people and then divided into two experimental and control groups by simple random method. The data collection tools included questionnaires of demographic data, knowledge and self-efficacy in children with asthma. Validity and reliability of the instruments were measured and then multimedia training was run for the experimental group. Data analysis was performed by SPSS 18 and Mann–Whitney U test. Findings: the mean scores of knowledge and self-efficacy of the research samples in the experimental group increased after multimedia training, so there was a statistically significant difference between the two groups (p <0.001). Conclusion: Education of children with asthma through multimedia has led to increasing knowledge and self-efficacy. Therefore, considering the effectiveness of multimedia education in children training with asthma, it is recommended that this educational method are used for other chronic childhood diseases
Evaluation of internet addiction and mental health status among medical sciences students in Bushehr, southwest of Iran
Introduction: Overuse of the Internet has significant impact on human life by causing psychological and social problems. This study is an attempt to describe the prevalence of addictive Internet use and mental health status among students in a public university in Iran. The relationship between Internet use and socio-demographic as well as mental health dimensions was explored. Methods: The present study relied on a cross-sectional design and 400 students were selected through proportional stratified random sampling. The data were collected using validated and reliable scales, Young Internet Addiction Test (YIAT) and Goldberg General Health Questionnaire (GHQ-28). Descriptive statistics, Independent Sample t-test, Chi- square test, and Binary Logistic Regression were conducted for data analysis. Results: The mean age of participants was 22.06. The overall prevalence of Internet addiction among the students was 34.5%. Logistic regression analysis indicated that the age of starting Internet use [OR: 0.92, 95% CI: 0.87,0.99; p=0.028], daily time spent on the Internet [OR: 1.12, 95% CI: 1.05,1.21; p <0.0001], purpose of Internet use [OR: 3.32, 95% CI: 2.08,5.48; p <0.0001] and anxiety/insomnia [OR: 1.20, 95% CI: 1.07,1.28; p <0.0001] are the significant predictors of Internet addiction. Conclusion: The Internet addiction among the students was high. The purpose of Internet use, experiencing a higher level of anxiety/insomnia, surfing on the Internet for long time are related to the Internet addiction. Organizing cognitive behavioral intervention programs may be effective in developing self-regulation skills of students in order to control their Internet use
Assessing the non-carcinogenic risk due to the intake of fluoride from fruit juice available in the market in Bushehr
In the present study, the fluoride (F) concentration, estimated daily intake (EDI), and non-carcinogenic risk of different fruit juices were determined. A total of 135 fruit juice samples from different brands and fruit types (cherry, orange, grape, apple, pineapple, and peach) were analyzed for fluoride concentration levels by using an ion selective electrode. The range of the fluoride concentration of the fruit juice samples was 0.00–0.444 mg.L-1 with a mean value of 0.069 mg.L-1. The results indicated that there were statistically significant differences between the fluoride concentration levels of some examined fruit juice brands as well as the fruit types. There was no statistically significant difference between the pH and F concentrations in the juice samples. The hazard quotient (HQ) for the different age groups showed that there was no significant risk for consumers from consuming fruit juice. However, the fluoride intake from other drinks containing a high level of fluoride should be considered when estimating the total daily fluoride intake from drinking, especially for those in warm regions with a high level of drink consumption
The effect of egg and its derivatives on vascular function: A systematic review of interventional studies
Background & aims: Cardiovascular diseases (CVDs) are considered as a major cause of health loss for all regions of the world. Atherosclerosis is one of the most important underlying causes of CVDs. Vascular dysfunction is the primary marker of atherosclerosis. Various studies have investigated the effect of egg intake on CVDs. This study aimed to determine the effect of egg and its derivatives consumption on vascular function. Methods: Using a comprehensive search strategy, the searching was conducted on PubMed, Scopus and Google Scholar search engines resources (from inception up to Feb 2020). Intended exposure and outcome were egg or its derivatives intake, and vascular function measuring methods, respectively. Results: A total of 35 papers were found through search databases. Finally, seven trials were included, which were published between 2005 and 2018. Results showed that acute and chronic consumption of whole eggs did not have a significant adverse effect on flow-mediated dilatation (FMD). Also, lutein-enriched egg yolk and egg ovalbumin-derived protein hydrolysate did not have significant positive effects on FMD and pulse wave velocity (PWV), respectively. However, one month egg-yolk-derived phospholipid (PL) preparation intervention increased FMD by 3.4% (p < 0.05), and replacing a portion of a glucose challenge with whole eggs or egg whites improved postprandial FMD (p < 0.05). Conclusions: We concluded that consumption of whole egg, despite being rich in cholesterol, has no adverse effect on vascular function, and even some of egg derivatives may improve vascular function. Further research is needed to justify the potential effects of egg or its derivatives on vascular function
The reference value of trabecular bone score (TBS) in the Iranian population
Trabecular bone score (TBS), as a tool for measurement of bone microarchitecture, represents fracture risk independently of bone density. The aim of this study was to estimate the reference values of TBS in both genders among the Iranian population to evaluate osteoporotic fractures in the future. Methods: The study was performed on healthy Iranian subjects who live in urban areas of Bushehr city, the capital of Bushehr province in southwestern Iran. The participants in this study were selected through a multistage, age and sex stratified, cluster random sampling. The TBS of L1-L4 was assessed by spine DXA images using TBS iNsight software (Discovery WI, Hologic Inc, USA). Age-related models of TBS were constructed using piecewise linear regression analysis. Results: In total, 691 participants aged ≥ 18 years (381 men and 310 women) were selected for the study. The mean and standard deviation (SD) of TBS value for men was 1.420 ± 0.094 and the age at the peak TBS was 30.0 years. Among women, the corresponding value for the mean of TBS was 1.428 ± 0.070 and the age at the peak TBS was 24.5 years. Two SDs below the mean of TBS were 1.326 in men and 1.357 in women. Therefore, the following normal range for TBS values has been proposed: Among men, TBS ≥ 1.326 is considered to be normal; TBS between 1.231 and 1.326 is considered to be partially degraded microarchitecture; and TBS ≤ 1.231defined degraded microarchitecture. Among women, TBS categories are defined as normal ≥ 1.357, partially degraded between 1.287 and 1.357 and degraded ≤ 1.287. Conclusions: This was the first study to propose evaluation of the normal range for TBS values in both genders in the Middle- East and Iran. According to our results: TBS ≤ 1.231 in men and TBS ≤ 1.287 in women is considered to be degraded microarchitecture among the Iranian population
Epigenetics in rheumatoid arthritis; fibroblast-like synoviocytes as an emerging paradigm in the pathogenesis of the disease
Rheumatoid arthritis (RA) is characterized by immune dysfunctions and chronic inflammation that mainly affects diarthrodial joints. Genetics has long been surveyed in searching for the etiopathogenesis of the disease and partially clarified the conundrums within this context. Epigenetic alterations, such as DNA methylation, histone modifications, and noncoding RNAs, which have been considered to be involved in RA pathogenesis, likely explain the nongenetic risk factors. Epigenetic modifications may influence RA through fibroblast-like synoviocytes (FLSs). It has been shown that FLSs play an essential role in the onset and exacerbation of RA, and therefore, they may illustrate some aspects of RA pathogenesis. These cells exhibit a unique DNA methylation profile in the early stage of the disease that changes with disease progression. Histone acetylation profile in RA FLSs is disrupted through the imbalance of histone acetyltransferases and histone deacetylase activity. Furthermore, dysregulation of microRNAs (miRNAs) is immense. Most of these miRNAs have shown an aberrant expression in FLSs that are involved in proliferation and cytokine production. Besides, dysregulation of long noncoding RNAs in FLSs has been revealed and attributed to RA pathogenesis. Further investigations are needed to get a better view of epigenetic alterations and their interactions. We also discuss the role of these epigenetic alterations in RA pathogenesis and their therapeutic potential
Role of glucose metabolism in aggressive phenotype of fibroblast-like synoviocytes: Latest evidence and therapeutic approaches in rheumatoid arthritis
Glucose metabolism is considerably increased in inflamed joints of rheumatoid arthritis (RA) patients at early stages. Fibroblast-like synoviocytes (FLSs) activation and subsequent joint damage are linked with metabolic alterations, especially glucose metabolism. It has been shown that glucose metabolism is elevated in aggressive phenotype of FLS cells. In this regard, glycolytic blockers are able to reduce aggressiveness of the FLS cells resulting in decreased joint damage in various arthritis models. Besides, metabolic changes in immune and non-immune cells such as FLS can provide important targets for therapeutic intervention. Glycolytic enzymes such as hexokinase 2 (HK2), phosphofructo-2-kinase/fructose-2,6-bisphosphatase (PFKFB), and phosphoglycerate kinase (PGK) play essential roles in aggressive behavior of FLS cells. It has been documented that the HK2 enzyme is significantly upregulated in RA FLS cells, compared with osteoarthritis (OA) FLS cells. The HK2 is expressed in a few tissues and upregulated in the inflamed synovium of RA patients that makes it a potential target for RA treatment. Furthermore, HK2 has different roles in each cellular compartment, which offers another level of specificity and provides a specific target to reduce deleterious effects of inhibiting the enzyme in RA without affecting glycolysis in normal cells. Thus, targeting the HK2 enzyme might be an attractive potential selective target for arthritis therapy and safer than global glycolysis inhibition. Therefore, this review was aimed to summarize the current knowledge about glucose metabolism of FLS cells and suggest novel biomarkers, which are potential candidates for RA treatment
Using STROBE checklist to assess the reporting quality of observational studies affiliated with Shiraz University of Medical Sciences, and its correlates: a scientometric study from Iran
The reporting quality of Observational Studies (OSs) is an important measure of their overall quality. We aim to assess the reporting quality of OSs of Shiraz University of Medical Sciences (SUMS) in Iran in the years 2012–2015, using Strengthening the Reporting of Observational Studies checklist. Systematic online search was performed. A random sample of SUMS affiliated published articles was selected. Articles were appraised and scored by two reviewers. Variables such as the study design, publication year, journals’ impact factor etc. were retrieved and their correlation with the articles’ scores was assessed. Out of 4297 published articles during 2012–2015, 1742 (40.5%) were OSs of which we assessed 171 (~ 10%) studies. Among these, 87 (50.9%), 74 (43.3%) and 10 (5.8%) articles had a cross-sectional, case–control and cohort design, respectively. Overall score of the reporting quality was 79% ± 0.01. It was at 81% ± 0.1, 77% ± 0.01 and 83% ± 0.02 for cross-sectional, case–control and cohort studies, respectively. A significant correlation was observed between the study design and the score for the reporting quality (P = 0.015). Reporting of “flow-diagram” (5%), “sources of bias” (28%) and “study size calculation” (30%) were the most missed items. Although the overall reporting quality of OSs was found to be at an acceptable rate, there are points of concern regarding some of the most important items that deserve the attention of authors as well as reviewers and editors
