Clinical Aspects of Combination of Aesthetic Fixed Prosthetic Appliances
The objective of the research was to study the peculiarities of the combination of zirconia and ceramic occlusal surfaces when constructing aesthetic fixed prosthetic appliances.Materials and methods. The study included 70 patients with zirconia and ceramic occlusal surfaces of aesthetic fixed dental prostheses. Group I included 24 patients with a combination of zirconia and ceramic occlusal surfaces. Group II included 30 patients with a combination of ceramic occlusal surfaces. Group III included 16 patients with a combination of zirconia occlusal surfaces. All the patients were observed 12 and 24 months after prosthetic repair.Results. 12 and 24 months after prosthetic repair, the occlusal contact surface area was the largest in Group II (8.18±0.16 mm2 and 9.17±0.1 mm2, respectively). In Group I, where only one occlusive surface was made of zirconium dioxide, significantly reduced levels of abrasion were observed as compared to Group II – 8.07±0.21 mm2 and 8.65±0.23 mm2, respectively. 36 months after denture wearing, in Group III, the smallest contact surface area – 7.84±0.15 mm2as well as the lowest growth of the surface area was observed – 8.07±0.13 mm2.Conclusions. Dental prostheses with at least one ceramic occlusal surface exhibit a strong tendency to abrasion and, consequently, to an increase in the occlusal surface area resulting in an excessive load on prosthetic appliance. Moreover, functional and aesthetic values of prosthetic prosthesis sharply decrease. Therefore, we recommend to produce zirconia occlusal surface or at least to combine the same materials, as it will increase the longevity of prosthetic appliance
Dynamics of Gestational Weight Gain in Women with Different Pre-pregnancy Body Weight
International and national guidelines regulate the gestational weight gain (GWG), depending on the pre-pregnancy body mass index (BMI). Insufficient weight gain increases rate of delivery of small weight babies, and excessive GWG is associated with macrosomia, postpartum weight retention and obesity. Objective: Our objective was to assess the dynamics of GWG in women of different pre-pregnancy body weight.Material and methods: 219 pregnant women with different pre-pregnancy BMI were examined. In 45 ((20.5±2.7) %) patients were diagnosed insufficient, in 100 ((45.7±3.4) %) – normal and in 74 ((33.8±3.2) %) – excessive weight gain during pregnancy period. BMI was calculated by using the standard formula of person’s weight in kilograms divided by the square of her height in meters (kg/m2). Total weight gain was calculated by subtracting the pre-pregnancy weight from the last measured weight before delivery. Statistical analyses were carried out using Statistical program “Statistica 5.5”.Results: It has been established that the women with higher pre-pregnancy BMI have the lower weight gain during the gestation period r = -0.25 (p<0.001). Normal weight women have the highest level of GWG, while pregnant women with obesity have minimal level. However, underweight patients are in the higher risk of pathologically low GWG while overweight and obese women have greater risk of excessive GWG. The dynamics of weight gain during pregnancy shows that deficiency or absent weight gain in the first trimester is associated with an insufficient weight gain to the end of pregnancy r=0.58 (p<0.001). Conversely, rapid onset of excessive weight gain up to 12 weeks of gestation leads to further excessive GWG r = 0.77 (p<0.001).Conclusions: Body weight before pregnancy is an independent determinant of GWG. Adequate weight gain during pregnancy does not lead to obesity in women of all groups, while pathologically low and high GWG leads to inadequate accumulation of adipose tissue, which has negatively affects on the metabolism of maternal and fetal organisms
The research of the state of prosthetic bed tissues in patients with pemphigus vulgaris.
The clinical examination of condition of tissues prosthetic bed was conducted in 20 patients with pemphigus vulgaris. The results are compared with survey data of 20 persons of control group without somatic pathology. It was conducted the analyzes of subjective and objective assessment of tissues prosthetic bed, the results of clinical examination and frequency of various pathologies of the oral mucosa membrane in the studied groups, conducted the comparative evaluation of dental indicators
Peculiarities of Hepatic Encephalopathy in Patients with Alcohol Liver Cirrhosis Against the Background of Chronic Bronchitis Exacerbation
Objective. To evaluate the clinical manifestations of hepatic encephalopathy (HE) in patients with alcohol liver cirrhosis (ALC) in exacerbation of chronic bronchitis (CB).Materials and methods. 100 patients with ALC class B and C according to Child-Pugh, 82.00% of men and 18.00% of women aged (47.2±3.9) years, were examined: 48 patients without combination with CB (group I); 52 patients on the background of CB exacerbation (group II). Clinical-laboratory and instrumental examination was performed. The West-Haven criteria were used to assess the HE.Results. In 54.17% of patients in group І, the I degree of HE was determined, in 35.42% – the ІІ degree, in 10.42% – the ІІІ degree. In patients of group ІІ the HE of the І degree was found in 17.31%, the ІІ degree – in 50.0%, the ІІІ degree – in 32.69% (p<0.05). In patients with ALC and ALC in combination with CB, respectively, clinically HE was manifested by drowsiness in the daytime and the inversion of sleep (58.33%; 75.00%), lengthening the duration of the TMT-time test (61.59±2.38 sec; 72.84±3.03 sec), mood disorders (20.83%; 28.85%), appearance of asterixis (54.17%; 76.92%), constructive apraxia (41.67%; 55.77%).Conclusions. 1. Exacerbation of CB in patients with ALC contributes to the increase in the degree of HE, worsening the prognosis. 2. The relationship between the hyponatremia and the degree of HE is revealed
Immunogenetic Status of Children with Mild Iodine Deficiency, Latent Iron Deficiency and Their Combination
Due to the prevalence of microelementosis (including iodine and iron deficiencies), cytogenetic abnormalities in children with microelement imbalance were studied.The objective of the research was to assess the abnormalities in the immunogenetic status of the organism by the frequency and spectrum of chromosomal aberrations, associations of acrocentric chromosomes and to determine the frequency of micronuclei in peripheral blood leukocytes in children with mild iodine deficiency, latent iron deficiency and their combination.Materials and methods. There were examined 68 boys and 65 girls at the age of 6 to 18 years. In the analysis of indicators, the main attention was paid to the age- (6-11 and 12-18 years) and gender-related peculiarities.Results and discussion. In all the children, associations of acrocentric chromosomes of two chromosomes were most commonly observed: in the control group, this indicator was 73.74%; in iodine deficiency, it was 67.72%; in iron deficiency, it was 67.68%; in combined microelementosis, the indictor was 68.68%. Chromosomal abnormalities were recorded in 56.03% of children. However, in the control group, this indicator was 40.94%, while in microelement imbalance, it was 71.13%. The most significant changes in the spectrum of chromosomal aberrations were identified in iodine and iron deficiencies (increase in the frequency of paired fragments, dicentrics, translocations, and the presence of a ring chromosome).Conclusions. Changes in the frequency and characteristics of the number of chromosomes in associations of acrocentric chromosomes, the frequency and spectrum of chromosomal aberrations, and the number of micronuclei indicated genotype instability, especially in iodine deficiency and combined microelement imbalance
Effect of Urogenital Infections on the Development of Placentary Dysfunction in Women Belonging to Risk Groups
The study involved 36 pregnant women with inflammatory diseases of female genital organs and 40 placentas of women with the physiological course of pregnancy and childbirth. Immunohistochemical and histological methods were used. It has been established that pregnant women with urogenital infections compared to those with physiological pregnancy had a lower concentration of the placental lactogen hormone and more active proapoptic protein of the BAX against the background of the unchanged concentration of the anti-apoptotic protein Bcl-2 in the cytoplasm of the invasive cytotrophoblast of the basal lamina
Units of labor input of orthopedists during consultation for implants. Fixed restorations
Implant-supported fixed restorations are widely used in practice with the aim of prosthetic rehabilitation of edentulous patients; however, in Ukraine, the aforementioned methods are currently not completely lawful due to the absence of departmental standards of time needed for orthopedists to manufacture such types of dentures.In prosthetic dentistry, labor inputs are determined based on the average standard working time for professionals that not always correspond to the actual time expenditures which must be taken into consideration when determining the appropriate units of labor input. The objective of the research was to determine the units of labor input of orthopediststo manufacture implant-supported fixed restorations.Materials and methods. The object of the research involved clinical processes of manufacturing implant-supported fixed restorations.The units of labor input were determined based on the average time standards obtained in the result of timing measurements of clinical processes of providing appropriate care.The conversion of the obtained time standards (TS) to the units of labor input (ULI) was performed according to the formula: ULI = Tts / T1uliResults and discussion. Clinical processes of manufacturing 69 single implant-supported restorations by 15 orthopedists were studied. There were manufactured 13 crowns with screw-retained solid abutments, 21 crowns with screw-retained abutments, 17 single abutments, 18 screw-retained crowns with burn-out plastic abutments or abutments with a platform for attaching and pressing.In addition, the work of 18 orthopedists who manufactured 30 implant-supported bridges including 8 prosthetic dentures with screw-retained solid abutments, 10 prosthetic dentures with screw-retained abutments, 12 screw-retained dentures with burn-out plastic abutments or abutments with a platform for attaching and pressing was investigated and timed.The results of the research showed that the initial values of the labor input indicators when manufacturing single implant-supported fixed restorations were within the range of 1.5-1.5 ULI (the average values) and 3.3-3.5 ULI (the actual values) depending on the prosthetic construction. When manufacturing implant-supported bridges, the indicators were as follows: the average values – 2.1-2.3 ULI, the actual values – 3.6-3.9 ULI depending on the prosthetic construction as well. Conclusions. The results obtained due to timing measurements of clinical processes of manufacturing implant-supported fixed restorations, their analysis as well as the calculations of the standard time showed that the units of labor input of orthopedists determined on the basis of the average time standards did not coincide with the units of labor input of manufacturing these prostheses determined on the basis of the actual time standards

New approaches to obtaining scientific innovation in morphological studies of bladder transitional epithelium
Objective: To demonstrate the capabilities of cluster analysis in receiving scientific innovation results in morphological studies of cells of the bladder urothelium.Materials and methods.10 Wistar rats were used. Histological sections were stained with hematoxylin and eosin; electron microscope studies were conducted; morphometry was performed in ImageJ and statistics – in studio-R using nonparametric methods and multivariate statistics.Results. A brief description of the main stages of cluster analysis shows way to determine the most important features of uroteliocytes and to reveal their heterogeneity, algorithms of Euclidean metrics and methods of clustering were described, the features of the application of the analysis in morphological studies were presented, an example of using these methods in searching for new results was presented, the models of morphological substantiation of clustering results were showed. Conclusion: 1) cluster analysis provides a scientific novelty in studies of transitional epithelium of the bladder; 2) it is used in case of heterogeneity of cellular composition of urothelium that is detected with a help of coefficient of variation; 3) the most significant features of uroteliocytes are their cell area and their nuclei area; 4) new results on the number of clusters were obtained by method of Ward, and new data on their indicators – by k-means; 5) Euclidean metric is the best to use; 6) to assess the adequacy of the results pairwise comparisons between multiple clusters were carried out according to their indicators; 7) results are presented in dimentional projection and they characterize cellular composition of the urothelium as structural system and detect systemic effects
