Study of psycho-emotional state of patients with acne depending on the severity and hereditary factors
Acne is one of the most common skin disease in young adults and occurs in 85% of boys and girls. The disease is characterized by progressive course of the formation of psycho-emotional disorders, mainly depression row in about half of patients. Installed heredity is almost 80% of first-degree relatives of patients with acne. In patients with positive family history of acne the disease occurs earlier and course harder. The aim of the study – is to determine the impact of acne on psycho-emotional status in patients. Materials and methods. We examined 78 patients with severe and moderate acne. All patients were questioned by questionnaire CADI (Cardiff Acne Disability Index). Results. Among the surveyed men with moderate acne average value was 6, median – 6, mode – 6; in men with severe acne course average value was 8.6, median – 9.5, mode – 10. Among women surveyed the results of the form were somewhat different from data that were shown by male patients. Women with moderate acne had average value 7 points, the median – 7.5 mode – 8; among women who had severe acne course the average value was – 8, median – 9. Conclusions. As can be seen from the results of the questionnaire, respondents in all groups were shown a significant effect of disease on the psycho-emotional state of patients.
Changes in Local Immunity Factors in Women with Pre-Existing Cervical Disease and Benign Ovarian Tumors
The objective of the research was to study the indices of local immunity in relation to the hormonal state of the body in women with benign ovarian tumors and pre-existing cervical diseases.Materials and methods. The concentration of major gonadotropic and steroid hormones during the female ovarian and menstrual cycle, as well as individual local immunity factors of proinflammatory cytokines and secretory immunoglobulin A was studied in 40 patients with benign ovarian tumors and pre-existing cervical diseases.Results and discussion. In the second phase of the ovarian and menstrual cycle, a significant reduction (by 3.0 times) in the concentration of progesterone and relative hypoestrogenism (by 1.5 times as compared to the control data) were found. Anovulation was observed in 40.00% of cases and corpus luteum deficiency syndrome was diagnosed in 62.5% of women. The assessment of secretory immunoglobulin A concentration allowed us to note (along with the hypersecretion of proinflammatory cytokines) an increase in this marker in half of the examined women in the presence of chronic cervicitis and vaginitis, candidiasis and viral lesion - along with the activation of proinflammatory cytokine depression of secretory immunoglobulin A synthesis as compared to the control data.Conclusions. Local immunity changes in women with benign ovarian tumors and pre-existing cervical diseases are accompanied by abnormalities of the hormonal profile and the association of maladaptive changes in the system of local immunity manifested by fluctuations in the level of secretory immunoglobulin A during the secretory phase of the menstrual cycle and an increase in proinflammatory cytokine synthesis. The results of the conducted study allowed us to note a significant increase in the concentration of secretory immunoglobulin A along with the activation of proinflammatory cytokine synthesis in most women with benign ovarian tumors and pre-existing cervical diseases which confirms the development of acute inflammatory reactions on the background of the existing hormonal imbalance. In patients with pre-cancerous cervical diseases, there was a tendency toward depression of secretory immunoglobulin A synthesis, a significant inhibition of local immunity, especially typical for cervical intraepithelial neoplasia on the background of human papillomavirus infection. All the aforementioned data confirm the formation of secondary immunodeficiency in this category of patients.
Clinical Evaluation of Periodontal Tissue Status in Prosthodontic Treatment of Patients with Partial Tooth Loss and Generalized Periodontitis
The problem of selection and application of dental prostheses in periodontal disease is especially relevant in case of severe generalized periodontitis, which is accompanied by mobile tooth removal resulting in overloading the periodontium of the remaining teeth as well as the increase in tooth mobility. Therefore, in generalized periodontitis, it is important to apply the method of direct dental prosthetic rehabilitation since, in case of partial tooth loss, it will prevent the development of generalized periodontitis complications.The objective of the research was to improve the effectiveness of combination therapy for patients with generalized periodontitis and partial tooth loss applying the developed method of direct fixed dental prosthetic rehabilitation based on the study of the periodontal status.Materials and methods. The study included 129 patients with general periodontitis, II-III degree and partial tooth loss over the age of 45 years. According to prosthodontic treatment, all the patients were divided into three groups: Group I consisted of 42 (20 women and 22 men) patients who immediately after tooth extraction were rehabilitated with the application of direct plastic laminar immediate prosthesis and selective tooth grinding; permanent dental prosthetic rehabilitation was performed 6 weeks after tooth extraction; Group II included 43 (21 women and 22 men) patients who underwent traditional permanent dental prosthetic rehabilitation using fixed dental bridges 6 weeks after mobile tooth removal and wound healing; Group III comprised 44 (21 women and 23 men) patients who immediately after mobile tooth removal were rehabilitated with the application of direct fixed sectional dental bridge (Ukrainian patent UA 20995. 2007 Feb 15) and selective tooth grinding; permanent dental prosthetic rehabilitation was performed 6 months after tooth extraction. The control group consisted of 26 people with intact dentitions over the age of 45 years.Results and discussion. The application of direct dental prosthetic rehabilitation method after mobile tooth removal in combination treatment of generalized periodontitis allows stabilizing degenerative and inflammatory processes in the periodontium in the initial stages, as well as increasing the resistance of the capillaries and bone mineral density of the interdental septa. The obtained data indicated that the application of the proposed direct fixed sectional dental bridge was more effective as compared to conventional prosthetics methods.Conclusions. 1. Direct fixed prosthodontics using direct fixed sectional dental bridge is essential in combination treatment of generalized periodontitis, especially if mobile tooth extraction is needed. It allows increasing the effectiveness of treatment in comparison with direct removable prosthodontics using direct plastic laminar immediate prosthesis (Group I) and traditional treatment methods (Group II). 2. We have proved the advantages of direct fixed prosthodontics in treatment of patients with generalized periodontitis, II-III degree based on the improvement of oral hygiene indices, the gingival bleeding index, the papillary-marginal-alveolar index, the periodontal index and the depth of periodontal pockets 6-12 months after treatment
Criteria for Determining the Adaptive Capacity of Students of Higher Medical Institutions
The article focuses on the importance of the adaptive capacity of students of higher medical institutions in the process of their professional training. The criteria for evaluating the adaptive capacity of students depending on their age were determined. The main mechanisms of forming the adaptive capacity at the organism level were found to be: 1) biochemical mechanisms which manifest themselves in the intracellular processes, e.g. the change in enzyme activity or their number; 2) physiological mechanisms, e.g. increased sweating as the body temperature rises; 3) morphofunctional mechanisms, i.e. the features of body structure and functions which are associated with lifestyle; 4) behavioural mechanisms – e.g. the creation of comfortable living conditions, etc.; 5) ontogenetic mechanisms, i.e. the acceleration of individual development or its slowdown contributing to the survival when conditions change
Questioning of Dental Students as a Means of Internal Monitoring of the Educational Process at the Department of Clinical Anatomy and Operative Surgery
The article has analyzed a questionnaire survey of second-year dental students of Ivano-Frankivsk National Medical University who completed the study course “Operative Surgery and Topographic Anatomy”. The questioning of students was conducted to assess the independent preparation of students for practical training as well as to clarify the students’ attitude to both the educational process at the Department of Clinical Anatomy and Operative Surgery and the mastering of practical skills. The dialogue with students through questioning allows to find out ways for the improvement of teaching the discipline “Operative Surgery and Topographic Anatomy” in the credit-transfer system of the educational process

The evaluation of psycho-emotional state in junior students of medical university
Currently, psychosocial factors, stress and mental exhaustion are considered by WHO experts and the International Labour Organization as an important new factor affecting the health, working capacity, training and career. Young generation, especially students, who are an industrial and intellectual potential of the country can be attributed to the risk group because of the extremely high burden of adaptation. The intensity and tension of modern life at a psychological level preceding the occurrence of negative emotional experiences and stress reactions that, accumulating, cause the formation of depressions. The aim of the study was to examine the level of psycho-emotional stress in the junior students of medical university. The observations of the medical students of the second year of study were performed. To determine the psycho-emotional stress we have used adaptive varint of H.Ayzenko method that include an anxiety, frustration, aggressiveness and rigidity, through questionnaires. According to a survey results of the state of psycho-emotional state low levels of stress resistance were found in students in learning activities, indicating that students are exposed to the negative impact of the environment. It should also be noted that the level of emotional condition of students depends on the personal experience of stressful situations that arise in students’ lives
Influence of Glucosaminilemuramylpentapeptide on the Functional Peculiarities of the Respiratory System and the Dynamics of IL-6 in Patients with Infiltrative Tuberculosis Combined with Chronic Bronchitis
The aim of our study was to evaluate the functional peculiarities of the respiratory system and the dynamics of IL-6 in patients with infiltrative tuberculosis combined with chronic bronchitis.Materials and methods. There were examined 40 patients with infiltrative tuberculosis combined with chronic bronchitis: 18 patients receiving basic therapy according to the category and 22 patients who were added glucosaminilemuramylpentapeptide to basic therapy. Determination of IL-6 in bronchoalveolar content was performed by ELISA analysis.Results. The additional inclusion of glucosaminilemuramylpentapeptide into the complex therapy of patients with infiltrative tuberculosis combined with chronic bronchitis in acute phase manifests itself with positively aimed dynamic changes of the bronchial mucous membrane according to the data of endoscopy. The standard scheme of treatment combined with immune modulator affects the recovery of lung function in comorbidity of pathologies explaining it as the action of the means aimed at enhancing of sanogenetic mechanisms of liquidation of combination of specific and nonspecific inflammation in the broncho-pulmonary system, and thus – the reduction of the basis for secondary bronchial obstruction. Effects of Quercetin on Cardiac Fibrosis in Patients with Acute Myocardial Infarction and Arterial Hypertension
Arterial hypertension is an independent predictor of acute myocardial infarction. Nowadays, plasma levels of fibronectin and matrix metalloproteinase 9 are the markers of left ventricular remodeling.The objective of the research was to investigate potential antifibrotic effects of Quercetin in patients with acute myocardial infarction and arterial hypertension.Material and methods. 130 patients with myocardial infarction (63 individuals with concomitant arterial hypertension and 67 individuals without it) were observed. All the patients were divided into groups of basic treatment and additional prescription of Quercetin. Transthoracic echocardiogram was used. To evaluate plasma level of fibronectin and matrix metalloproteinase 9 the ELISA method was applied.Results. In all the patients, a significant decrease in fibronectin plasma levels was observed since the 28th day of treatment; however, it was more significant in group of additional prescription of Quercetin. Revascularization and pharmacological management of myocardial infarction resulted in the reduction in matrix metalloproteinase 9 plasma levels in all the patients since the 7th day of treatment; however, it was more significant in group of additional prescription of Quercetin. Conclusions. Quercetin possesses potential antifibrotic properties causing a reduction in plasma levels of fibronectin and matrix metalloproteinase 9 in patients with myocardial infarction and concomitant arterial hypertension
Changes of Prooxidant-Antioxidant Systems in Experimental Acute Pancreatitis
Mortality in acute destructive pancreatitis, despite the development and introduction of new methods of treatment, remains stable high and in severe forms reaches 25-85%. Activation of neutrophils and macrophages in acute pancreatitis leads to an "oxygen burst", which is closely linked with the activation of lipid peroxidation.Goals. The purpose is to establish dynamic changes in the indexes of prooxidant-antioxidant systems in acute L-arginine-induced pancreatitis.Materials and methods. The study was performed on 62 white male rats of Wistar line weighing 180-220g, with modeled acute pancreatitis. Blood for analysis have been taken: the blood serum on 12, 24, 48 and 72 hours of experiment to determine the activity level of thiobarbituric acid products, diene conjugates, catalase and lactate for assessment of the intensity of oxidative stress and antioxidant systems.Results. The obtained results of the study showed that acute L-arginine-induced pancreatitis is accompanied by an intensification of lipid peroxidation processes (LPO). Revealed that the most pronounced increase in all blood parameters is observed 24 hours after the beginning of the study. A significant increase in the active products of tiobarbituric acid (TBA-AP) and diene conjugates (DC) was detected - 1.98 and 2.7 times, respectively, and 2.2 times the growth of catalase (CT). At the next stage of the experiment there is a slowdown in the rate of LPO, as evidenced by the following values. Thus, for 48 years in the 3rd group: TBA-AP - they increased by 5.1% (p> 0.05), DC - by 3.3% (p> 0.05), and the level of CT - by 43.4% (P <0.05), compared with data for 24 hours. It is important to note that at 72 hours, the CT level decreased by 23.3% (p> 0.05), which may indicate an exhaustion of antioxidant systems. Indicators of LPO on 72 hours compared with 48 hours in group III: TBA-AP - increased by 1.7% (p> 0.05), DC - by 5.7% (p> 0.05).Conclusions. Acute L-arginine-induced pancreatitis is accompanied by an intensification of lipid peroxidation-oxidation processes that can potentiate the development of multiple organ failure in pancreatic inflammation. The most pronounced changes in lipid peroxidation-oxidation rates are observed for 24 hours of study
