    The relationships between body mass index, reciprocal ponderal index, wais-to-height ratio, and fitness in young adult males

    Background: Anthropometric measures such as the body mass index (BMI), reciprocal ponderal index (RPI), and waist-to-height ratio (WHR) have been proposed as predictors of physical fitness. This study aimed to identify the differences in explanatory capacity and fit of BMI, RPI, and WHR on physical fitness, which involves jumping, sprinting, change of direction, and aerobic capacity, by adjusting the polynomial regression. Methods: A sample of 297 healthy, recreationally active male university students between 18 and 20  years old was recruited for this study. Anthropometric measurements (height: 174.09  ±  6.27  cm, weight: 78.98  ±  20.27  kg, waist circumference: 93.74  ±  14.56  cm) were taken for each participant. Jumping tests (squat jump, countermovement jump), sprinting tests (20  m sprint), agility tests (agility T-test), and aerobic/endurance tests (6  min walk test, VAM-EVAL test) were performed. Nonlinear quadratic regression models were used to assess the relationship between the jump, sprint, and fitness test scores and the anthropometric indices. The models were compared based on R-squares and Bayesian Information Criterion (BIC). The significance level was set at p <  0.05. Results: The results showed that all the indices predicted a portion of the variance because all variables and index relationships were significant. Regarding the fitted models, the Bayesian Information Criterion showed that BMI was the best indicator of performance, although the RPI was better for VO2max. Conclusion: These findings may be of great interest to practitioners because it appears that anthropometric measures can be used to predict physical fitness in certain tests although the accuracy raises any concerns.Sin financiación2.6 Q2 JCR 20230.8 Q2 SJR 2023No data IDR 2023UE

    Cardiovascular disease in women: Do we need new diagnostic and therapeutic strategies?

    Cardiovascular disease (CVD) is the leading cause of death worldwide affecting both sexes equally. However, in comparison to men, in women, it often is underrecognized and undertreated in both primary and secondary prevention settings. It is clear, that in the healthy population, there are profound differences both anatomically and biochemically between women and men, and this may impact how both groups present when they become ill. Moreover, some diseases affect more frequently women than men such as myocardial ischemia or infarction without obstructive coronary disease, Takotsubo syndrome, some atrial arrhythmias, or heart failure with preserved ejection fraction. Therefore, diagnostic and therapeutic strategies that have been established largely on the basis of clinical studies with a predominantly male population must be adapted before being applied to women. There is a paucity of data regarding cardiovascular disease in women. It is inadequate to only perform a subgroup analysis evaluating a specific treatment or invasive technique when women constitute fifty percent of the population. In this regard, this may affect the time of clinical diagnosis and severity assessments of some valvulopathies. In this review, we will focus on the differences in the diagnosis, management, and outcomes for women with the most frequent cardiovascular pathologies including coronary artery disease, arrhythmia, heart failure, and valvopathies. In addition, we will describe diseases that exclusively affect women that are related to pregnancy, and some of them are life-threatening. Although the lack of research on women plays a role in the poorer outcomes in women, especially in ischemic heart disease, some techniques such as transcatheter aortic valve implantation and transcatheter edge-to-edge therapy seem to have better outcomes in women.Sin financiación3.3 Q2 JCR 20220.4 Q3 SJR 2022No data IDR 2022UE

    Position statement on infection screening, prophylaxis, and vaccination of pediatric patients with rheumatic diseases and immunosuppressive therapies, part 3: precautions in situations of surgery, fever, and opportunistic infections

    The objective of this study is to provide practical recommendations on the management of pediatric patients with immune-mediated rheumatic diseases receiving immunosuppressive therapies. The recommendations specifically address the cases of surgery, fever, and opportunistic infections (varicella, herpes-zoster, tuberculosis, invasive fungal disease). A qualitative approach was applied. A narrative literature review was performed via Medline. Primary searches were conducted using MeSH terms and free text to identify publications on infections and vaccinations in pediatric patients with immune-mediated rheumatic diseases receiving immunosuppressive therapies. The results were presented and discussed in a nominal group meeting, comprising a committee of 12 pediatric rheumatologists from the Infection Prevention and Treatment Working Group of the Spanish Society of Pediatric Rheumatology. Several recommendations were generated. A consensus procedure was implemented via a Delphi process; this was extended to members of the Spanish Society of Pediatric Rheumatology and Spanish Society of Pediatric Infectious Disease of the Spanish Association of Pediatrics. Participants produced a score ranging from 0 (totally disagree) to 10 (totally agree). Agreement was defined as a vote ≥ 7 by at least 70% of participants. The literature review included more than 400 articles. Overall, 63 recommendations (19 on surgery, fever, and opportunistic infections) were generated and voted by 59 pediatric rheumatologists and other pediatric specialists. Agreement was reached for all 63 recommendations. The recommendations on special situations cover management in cases of surgery, fever, and opportunistic infections (varicella, herpes-zoster, tuberculosis, and invasive fungal disease). Conclusions: Hereby, we provided consensus and updated of recommendations about the management of special situations such as surgery, fever, and opportunistic in children with immune-mediated rheumatic diseases receiving immunosuppressive therapies. Several of the recommendations depend largely on clinical judgement and specific balance between risk and benefit for each individual and situation. To assess this risk, the clinician should have knowledge of the drugs, the patient’s previous situation as well as the current infectious disease, in addition to experience. What is Known: • Infectious diseases and related complications are a major cause of morbidity and mortality in patients with immune-mediated rheumatic diseases. • Information on how to manage the treatment in situations of fever, opportunistic infections, and surgery in children is limited, and guidelines for action are often extrapolated from adults.     Efecto del cacao rico en flavonoides para la prevención de la presbiacusia: Estudio in vitro

Programa de Doctorado en Biomedicina y Ciencias de la SaludLa pérdida de audición relacionada con la edad (ARHL, Age-related hearing loss) o presbiacusia se caracteriza por una pérdida auditiva neurosensorial gradual y progresiva, que afecta la comunicación de las personas y conduce a la depresión clínica y al retraimiento social. Actualmente no existen tratamientos curativos para esta enfermedad. La senescencia celular se considera un mecanismo básico del envejecimiento que limita la proliferación de células dañadas o envejecidas. Aunque juega un papel esencial en la homeostasis y la fisiología de los tejidos en el desarrollo normal, es una respuesta al estrés asociado con el envejecimiento, como el desgaste de los telómeros y las alteraciones genómicas, que son características del envejecimiento. En los trastornos relacionados con la edad, las células senescentes se acumulan en los tejidos a través del fenotipo secretor asociado a la senescencia (SASP). A medida que avanza la enfermedad, se produce un aumento adicional de células senescentes en los sitios dañados. Además, las células senescentes inducen una alta producción de especies reactivas de oxígeno (ROS) derivadas de mitocondrias. El estrés oxidativo es un factor esencial en el desarrollo de ARHL; además, la senescencia prematura de las células auditivas inducida por el estrés oxidativo puede producir pérdida de audición. El peróxido de hidrógeno (H2O2) representa un método comúnmente utilizado para generar senescencia celular in vitro. Los datos de este estudio mostraron la capacidad del H2O2 para inducir la senescencia en tres líneas celulares auditivas (House Ear Institute-Organ of Corti 1, HEI-OC1; órgano de Corti, OC-k3 y stria vascularis, células SV-k1) en búsqueda de mecanismos intrínsecos responsables de ARHL. Las células auditivas fueron expuestas a H2O2 en diferentes concentraciones y tiempos. Los resultados obtenidos muestran diferentes respuestas de las células auditivas en cuanto al crecimiento celular, actividad de la β-galactosidasa, cambios morfológicos, activación mitocondrial, niveles de estrés oxidativo y otros marcadores de daño celular (Forkhead box O3a, FoxO3a y 8-oxoguanina, 8- oxoG). La comparación entre las respuestas de estas células auditivas al H2O2 es un método útil para evaluar los mecanismos moleculares responsables de la senescencia de estas células auditivas. Además, este modelo in vitro podría ayudar a desarrollar estrategias terapéuticas anti-senescentes para el tratamiento de la AHRL. Es por ello por lo que los datos de este estudio determinaron que el cacao presenta efecto protector a nivel celular y molecular en la presbiacusia. Nuevamente, el estudio in vitro se realiza en células HEI-OC1, OC-k3 y SV-k1. Cada línea celular se dividió en un grupo control (CTR) y un grupo H2O2 (inducción de la senescencia tras reaccionar con moléculas de oxígeno e inducir la formación de radicales libres). Además, cada grupo de cada línea celular fue tratado con extracto polifenólico de cacao (CPE), midiendo diferentes marcadores de apoptosis, viabilidad, actividad de enzimas antioxidantes y estrés oxidativo/nitrosativo. Los datos muestran un aumento de especies reactivas oxidativas y especies reactivas de nitrógeno (RNS) en células senescentes en comparación con las de control. El tratamiento con CPE redujo efectivamente estos altos niveles y se correlacionó con una reducción significativa en la apoptosis de las células al inhibir la vía mitocondrial-apoptótica. Además, en las células senescentes, la actividad de las enzimas antioxidantes (superóxido dismutasa, SOD, catalasa, CAT y glutation peroxidasa, GPx) se recuperó después del tratamiento con el CPE. La administración de CPE también disminuyó el daño oxidativo del ADN en las células auditivas senescentes. En conclusión, el CPE inhibe la activación de la señalización apoptótica relacionada con la senescencia al disminuir el estrés oxidativo en las células auditivas senescentes.Age-related hearing loss (ARHL) or presbycusis is characterized by a gradual and progressive sensorineural hearing loss, which affects people's communication leading to clinical depression and social withdrawal. There are currently no curative treatments for this disease. Cellular senescence is considered a basic mechanism of aging that limits the proliferation of damaged or aged cells. Although senescence plays an essential role in tissue homeostasis and physiology in normal development, senescence is a response to stress associated with aging, such as telomere attrition and genomic alterations, which are aging characteristics. In age-related disorders, senescent cells accumulate in tissues associated with the aging process through the senescence-associated secretory phenotype (SASP). As the disease advances, an additional surge of senescent cells is produced at the damaged sites. In addition, senescent cells induce high production of mitochondrial-derived ROS. Oxidative stress is an essential factor in developing ARHL; additionally, premature senescence of auditory cells induced by oxidative stress can produce hearing loss. Hydrogen peroxide (H2O2) represents a method commonly used to generate cellular senescence in vitro. The data of this study showed that H2O2 induced senescence in three auditory cell lines (House Ear Institute-Organ of Corti 1, HEI-OC1; organ of Corti, OC-k3, and stria vascularis, SV-k1 cells) to elucidate the intrinsic mechanisms responsible for ARHL. The auditory cells were exposed to H2O2 at different concentrations and times. The results obtained show different responses of the hearing cells concerning cell growth, β-galactosidase activity, morphological changes, mitochondrial activation, levels of oxidative stress, and other markers of cell damage (Forkhead box O3a, FoxO3a, and 8-oxoguanine, 8-oxoG). Comparison between the responses of these auditory cells to H2O2 is a helpful method to evaluate the molecular mechanisms responsible for these auditory cells' senescence. Furthermore, this in vitro model could help develop anti-senescent therapeutic strategies for the treatment of AHRL. It is known that cocoa products have high levels of polyphenol content (mainly flavonoids), that are potent anti-inflammatory and antioxidant agents with proven benefits for health. That is why the data from this study determined that cocoa present protective effect at the cellular and molecular levels in Presbycusis. Again, the in vitro study is performed in HEI-OC1, OC-k3 y SV-k1 cells. Each cell line was divided into a control group (CTR) and an H2O2 group (induction of senescence after reacting with oxygen molecules and inducing the formation of free radicals). Additionally, every group of every cell line was treated with the cocoa polyphenolic extract (CPE), measuring different markers of apoptosis, viability, the activity of antioxidant enzymes, and oxidative/nitrosative stress. The data show an increase of reactive oxidative and nitrogen species (ROS and RNS, respectively) in senescent cells compared to control ones. CPE treatment effectively reduced these high levels and correlated with a significant reduction in apoptosis cells by inhibiting the mitochondrial-apoptotic pathway. Furthermore, in senescence cells, the activity of antioxidant enzymes (Superoxide dismutase, SOD; Catalase, CAT; and Glutathione peroxidase, GPx) was recovered after CPE treatment. Administration of CPE also decreased oxidative DNA damage in the auditory senescent cells. In conclusion, CPE inhibits the activation of senescence-related apoptotic signaling by decreasing oxidative stress in auditory senescent cells.No data 2022UE

    Differences in Malnutrition Inflammation Score of Hemodialysis Patients Associated With Hemodialysis Factors. A Spanish Multicenter Epidemiologic Study

    Objectives: The aims of this study are to analyze the prevalence of malnutrition in hemodialysis (HD) patients in Spain, and to assess the association of malnutrition in these patients with sociodemographic characteristics, comorbidity, and parameters related to HD. Design and Methods: A multicenter, retrospective, cross-sectional study in HD patients from centers all over Spain was conducted. Nutritional status of patients was assessed using Malnutrition Inflammation Score (MIS), and was stratified according to MIS values into 5 categories: ≤2, normal nutrition; >2 to ≤5, mild malnutrition or risk of malnutrition; >5 to ≤7, moderate malnutrition; >7 to ≤10, severe malnutrition, and >10, extreme malnutrition. Results: A total of 52 Spanish HD Units participated in the study enrolling 2,748 patients. Mean age of patients was 68.20 ± 14.24 years, 1,811 (65.9%) were men. Mean time on HD was 55.63 ± 63.25 months. Using an MIS cut-off point of 2 for malnutrition, 89% of patients were malnourished (MIS > 2). However, with a cut-off point of 5, more commonly described in the literature, the percentage of patients with malnutrition was reduced to 51.7%. Using this cut-off, we observed significant differences between patients with malnutrition and normo-nourished patients in biochemical parameters, age, Charlson Index, HD residual renal function, scheme, and vascular access (permanent catheter vs arteriovenous fistula). A multivariate regression analysis showed that age, sex, HD scheme, vascular access, residual renal function, and comorbidity index were predictive factors for malnutrition. We found that a high percentage of HD patients with malnutrition did not receive oral supplementation. Conclusions: The prevalence of malnutrition in HD patients in Spain, assessed using the MIS scale, was high. Higher malnutrition was associated with the use of catheter versus fistula, and standard HD versus online hemodiafiltration, and with the absence of residual renal function, older age, greater comorbidity, and male sex.     Evaluation of the efficacy of an antioxidant combination for the modulation of metabolic, endocrine, and clinical parameters in patients with polycystic ovary syndrome

    Objective: to evaluate the efficacy of dietary supplementation with a combination of antioxidants (lipoic acid, N-acetylcysteine, vitamin B6, and S-adenosyl-l-methionine) for the modulation of metabolic, endocrine, and clinical parameters in comparison with oral contraception in non-diabetic women newly diagnosed with polycystic ovary syndrome (PcOs). Methods: this was a prospective, partially randomized, multicenter study in which non-diabetic women with PcOs were recruited under routine clinical practice conditions and distributed in three groups to receive the following regimen for 6months: 1) antioxidant combination (MN group); 2) oral contraception (Oc group); or 3) oral contraception and the antioxidant combination (MN + Oc group). General recommendation of healthy diet and regular exercise was given to all patients. Metabolic, endocrine, clinical, and quality of life parameters were recorded at baseline and after 6months of therapy. Results: a total of 96 women with PcOs were included in the study. after 6months of treatment, the homeostasis model assessment-estimated insulin resistance (hOMa-iR) level was reduced only in the MN group, with a significant mean reduction of −0.92 points. androstenedione was significantly reduced in all groups. clinical parameters that significantly improved in all groups were hirsutism, acne, irregular menstruation, and quality of life, with no statistical differences between the groups. Conclusions: this study showed that the antioxidant combination might be a suitable therapy for patients with PcOs when oral contraceptive is not indicated, because in all groups clinical parameters, irregular menstruation as well as androstenedione and quality of life were significantly improved with no statistical difference between groups.Margan Biotech SL2.0 Q4 JCR 20220.59 Q2 SJR 2023No data IDR 2022UE
