    The Speaker’s Method: A Novel Release Method for Offspring Mammals and 5-year Study on Three Costa Rican Mammals

    Nowadays, wild animals are threatened by humans, with the number of species and individuals decreasing during recent years. Wildlife rescue centers play a vital role in the conservation of wildlife populations. This study aims to describe a new release technique, the Speaker’s Method, to rescue and facilitate the reunion of different baby mammals that arrived at a wildlife rescue center with their mothers within their natural habitat, avoiding the need for captivity. This method is based on a recorded baby’s cry played on a speaker to make a “call effect” in the mother. The efficacy of the Speaker’s Method for babies’ reunion with their mothers was 45.8% in Hoffmann’s two-toed sloths (Choloepus hoffmanni) and 91.9% in brown-throated sloths (Bradypus variegatus). Among the mantled howler monkeys (Alouatta palliata), 50% of the babies could be released using this new technique. The findings suggest that the method could be helpful in the early release of young individuals, highlighting higher release outcomes in these three species compared to traditional nursery care provided by human caretakers, who face inherent difficulties in raising young animals without their mothers.Sin financiación3.0 Q1 JCR 20220.684 Q1 SJR 2022No data IDR 2022UE

    Levosimendán ambulatorio repetitivo como puente al trasplante cardiaco

    Introduction and objectives Repetitive ambulatory doses of levosimendan are an option as a bridge to heart transplantation (HT), but evidence regarding the safety and efficacy of this treatment is scarce. The objective of the LEVO-T Registry is to describe the profile of patients on the HT list receiving levosimendan, prescription patterns, and clinical outcomes compared with patients not on levosimendan. Methods We retrospectively reviewed all patients listed for elective HT from 2015 to 2020 from 14 centers in Spain. Results A total of 1015 consecutive patients were included, of whom 238 patients (23.4%) received levosimendan. Patients treated with levosimendan had more heart failure (HF) admissions in the previous year and a worse clinical profile. The most frequent prescription pattern were fixed doses triggered by the patients’ clinical needs. Nonfatal ventricular arrhythmias occurred in 2 patients (0.8%). No differences in HF hospitalizations were found between patients who started levosimendan in the first 30 days after listing and those who did not (33.6% vs 34.5%; P=.848). Among those who did not, 102 patients (32.9%) crossed over to levosimendan after an HF admission. These patients had a rate of 0.57 HF admissions per month before starting levosimendan and 0.21 afterwards. Propensity score matching analysis showed no differences in survival at 1 year after listing between patients receiving levosimendan and those who did not (HR, 1.03; 95%CI, 0.36-2.97; P=.958) or in survival after HT (HR, 0.97; 95%CI, 0.60-1.56; P=.958). Conclusions Repetitive levosimendan in an ambulatory setting as a bridge to heart transplantation is commonly used, is safe, and may reduce HF hospitalizations.Introducción y objetivos El levosimendán ambulatorio repetitivo es una opción como puente al trasplante cardiaco (TxC), aunque la evidencia sobre su eficacia y su seguridad es escasa. El objetivo del registro LEVO-T es describir a los pacientes en lista de TxC que reciben levosimendán, sus pautas y los eventos clínicos durante el seguimiento, en comparación con los que no lo reciben. Métodos Se revisó en retrospectiva a los pacientes en lista de espera para TxC electivo de 14 centros españoles desde 2015 hasta 2020. Resultados Se incluyó a 1.015 pacientes consecutivos; los 238 (23,4%) que recibieron levosimendán mostraron más ingresos por insuficiencia cardiaca (IC) el año anterior y peor perfil clínico. Las dosis fijas por necesidades clínicas fueron la pauta más frecuente. Dos pacientes (0,8%) presentaron arritmias ventriculares no mortales. No hubo diferencias en hospitalizaciones por IC entre los que comenzaron levosimendán en los primeros 30 días después de inclusión y los que no (el 33,6 frente al 34,5%; p=0,848). De estos últimos, 102 (32,9%) pasaron a levosimendán después de un ingreso por IC, y la tasa de ingresos por IC/mes varió de 0,57 antes del levosimendán a 0,21 después. El análisis mediante emparejamiento por puntuación de propensión no mostró diferencias entre los pacientes con y sin levosimendán en la supervivencia a 1 año tras la inclusión en lista (HR=1,03; IC95%, 0,36-2,97; p=0,958) ni en la supervivencia tras el TxC (HR=0,97; IC95%, 0,60-1,56; p=0,958).     Radiographic Progression in Sacroiliac Joints in Patients With Axial Spondyloarthritis: Results From a Five-Year International Observational Study

    Objective: To evaluate progression from nonradiographic (nr-) to radiographic axial spondyloarthritis (r-axSpA) over 5 years in patients with recently diagnosed (≤1 year) axSpA fulfilling the Assessment of SpondyloArthritis international Society (ASAS) classification criteria. Methods: A prospsective, observational study (Patients with Axial Spondyloarthritis: Multi-Country Registry of Clinical Characteristics) was conducted in rheumatology practices in 29 countries. Baseline and follow-up radiographs of sacroiliac joints were centrally evaluated by three readers according to the grading system of the modified New York criteria for patients initially classified as nr-axSpA. Radiographic progression from nr-axSpA to r-axSpA was evaluated by Kaplan-Meier analysis. Cox proportional regression analyses for progression from nr-axSpA to r-axSpA were also conducted. Results: Among 2,165 patients with axSpA, 1,612 (74%) were classified as having r-axSpA (1,050 [65%]) or nr-axSpA (562 [35%]) by central reading. Of 246 patients with nr-axSpA (mean [SD] symptom duration: 4.4 [6.2] years) who had at least one follow-up sacroiliac joint radiograph, progression from nr-axSpA to r-axSpA at any follow-up visit was observed in 40 patients (16%) over 5 years. Mean time to radiographic progression was 2.4 years (ranging from 0.9 to 5.1 years). Progression to r-axSpA was associated with male sex (hazard ratio [HR] 3.16 [95% CI 1.22-8.17]), fulfillment of the imaging arm of the ASAS classification criteria (HR 6.64 [1.37-32.25]), and good response to nonsteroidal anti-inflammatory drugs (HR 4.66 [1.23-17.71]). Conclusion: 16% of patients with nr-axSpA progressed to r-axSpA within 5 years.     Differences between Novice and Expert Raters Assessing Trunk Control Using the Trunk Control Measurement Scale Spanish Version (TCMS-S) in Children with Cerebral Palsy

    The Trunk Control Measurement Scale (TCMS) is a valid and reliable tool to assess static and dynamic trunk control in cerebral palsy. However, there is no evidence informing about differences between novice and expert raters. A cross-sectional study was conducted with participants between the ages of 6 and 18 years with a CP diagnosis. The TCMS Spanish version (TCMS-S) was administered in-person by an expert rater, and video recordings were taken for later scoring by the expert and three other raters with varying levels of clinical experience. The intraclass correlation coefficient (ICC) was used to evaluate reliability between raters for the total and subscales of the TCMS-S scores. Standard Error of Measurement (SEM) and Minimal Detectable Change (MDC) were also calculated. There was a high level of agreement between expert raters (ICC >= 0.93), while novice raters demonstrated good agreement (ICC > 0.72). Additionally, it was observed that novice raters had a slightly higher SEM and MDC than expert raters. The Selective Movement Control subscale exhibited slightly higher SEM and MDC values compared to the TCMS-S total and other subscales, irrespective of the rater's level of expertise. Overall, the study showed that the TCMS-S is a reliable tool for evaluating trunk control in the Spanish pediatric population with cerebral palsy, regardless of the rater's experience level.La Escala de Medición del Control del Tronco (TCMS) es una herramienta válida y confiable para evaluar el control estático y dinámico del tronco en la parálisis cerebral. Sin embargo, no hay evidencia que informe sobre las diferencias entre evaluadores no-expertos y expertos. Se realizó un estudio transversal con participantes entre 6 y 18 años con diagnóstico de parálisis cerebral. La versión en español de la TCMS (TCMS-S) fue administrada en persona por un evaluador experto, y se tomaron grabaciones de video para su posterior puntuación por el experto y otros tres evaluadores con diferentes niveles de experiencia clínica. El coeficiente de correlación intraclase (CCI) se utilizó para evaluar la fiabilidad entre los evaluadores para las puntuaciones totales y de subescalas de la TCMS-S. También se calcularon el Error Estándar de Medición (EEM) y el Cambio Mínimo Detectable (CMD). Hubo un alto nivel de acuerdo entre los evaluadores expertos (CCI ≥ 0.93), mientras que los evaluadores no-expertos demostraron un buen acuerdo (CCI > 0.72). Además, se observó que los evaluadores no-expertos tenían un EEM y CMD ligeramente más altos que los evaluadores expertos. La subescala de Control de Movimiento Selectivo exhibió valores de EEM y CMD ligeramente más altos en comparación con el total de la TCMS-S y otras subescalas, independientemente del nivel de experiencia del evaluador.     The impact of ADHD on reading

    Más allá de la frecuente coexistencia del trastorno por déficit de atención con hiperactividad (TDAH) y el trastorno específico del aprendizaje de la lectura, la presente revisión pretende examinar la evidencia empírica disponible sobre cómo el TDAH impacta negativamente sobre el aprendizaje de la lectura. Los datos existentes apuntan a que la presencia del trastorno (especialmente los síntomas de falta de atención), puede afectar a i) la correcta adquisición de lectura, ya sea de manera directa o a través de su influencia sobre los precursores de la lectura; ii) las propias habilidades de decodificación (precisión y fluidez lectora), tanto de manera directa como indirecta a través de su influencia sobre procesos cognitivos como la distracción o las funciones ejecutivas; y ii) la comprensión lectora, probablemente de manera indirecta por las dificultades ejecutivas y en la memoria de trabajo verbal características del TDAH. Estas conclusiones presentan importantes implicaciones para caracterizar e intervenir mejor sobre las dificultades lectoras en el TDAH, ya sean clínicas o subclínicas.Beyond the frequent coexistence of attention deficit hyperactivity disorder (ADHD) and reading disorder (dyslexia), the present review aims to examine the available empirical evidence on how ADHD negatively impacts on learning to read. Existing data suggest that the presence of the disorder (especially inattention symptoms), may affect i) the correct acquisition of reading, either directly or through its influence on the precursors to reading; ii) decoding skills themselves (reading accuracy and fluency), both directly and indirectly through its influence on cognitive processes such as distractibility or executive functions; and iii) reading comprehension, probably indirectly through the executive and verbal memory difficulties characteristic of ADHD.     Los procesos comunicativos entrópicos de las agrupaciones del carnaval gaditano

    La ingente cantidad de información que anualmente generan las agrupaciones del carnaval gaditano ha motivado a algunos investigadores (Sacaluga, 2013; Fernández, 2016; entre otros) a recoger y compartir la denominación de “periodismo cantado” asociada a la función comunicativa que realizan las agrupaciones del Carnaval de Cádiz. Las agrupaciones se configuran como el elemento diferenciador de la fiesta y “se caracterizan fundamentalmente por la acidez, la mordacidad y la crítica de sus letras” (Sacaluga, 2013, 451). Aunque no existen datos oficiales exactos, se estima que cada año participan unas cuatro mil personas cantando, lo que supone unas seiscientas músicas originales, y en torno a más de nueve mil composiciones literarias diferentes (Sacaluga, 2013). Este reflejo, escrito y cantado, de la realidad acaba por conformar una hemeroteca cultural y socialmente aceptada, reconocida y, a menudo, consultada.     Influence of psychological inflexibility and mindfulness on hypersexuality and sexual satisfaction in a Spanish sample

    Sexual satisfaction is tremendously relevant to wellbeing and quality of life. Inversely, hypersexuality may increase the risk of psychological distress, unprotected sex, and marital problems. This study attempts to explore the sociodemographic variables related to hypersexuality and the relationship between hypersexuality and sexual dissatisfaction with psychological inflexibility, cognitive fusion, body awareness, bodily dissociation, and mindfulness skills in the Spanish population. The sample was made up of 530 participants between 18 and 67 years of age. In total, 5.5% displayed hypersexuality, more likely in males, lesbians and gay men, singles, and students. An online survey was conducted including standardized questionnaires as follows: AAQ-II, CFQ, SBC, MAAS, NSSS-S, and HBI. Significant moderate positive correlations were found between hypersexuality and psychological inflexibility, cognitive fusion, and bodily dissociation, and an inverse moderate correlation with mindfulness skills. Moderate negative correlations were found between sexual satisfaction and bodily dissociation. Linear regression showed psychological inflexibility was the variable with the most weight on hypersexuality, while bodily dissociation was the variable with the most influence on sexual dissatisfaction. This study suggests lesbians and gay men, males, and singles may show an increased vulnerability toward engaging in hypersexual behavior and provides justification for interventions focused on psychological inflexibility and mindfulness skills to treat hypersexuality and improve sexual satisfaction.Sin financiación3.8 Q1 JCR 20220.891 Q2 SJR 2022No data IDR 2022UE

    Eficacia de la electrólisis percutánea aplicada en la fascia plantar y ejercicio excéntrico vs la neuromodulación percutánea aplicada en el nervio tibial y ejercicio excéntrico en sujetos militares con fascitis plantar

    Tesis inédita presentada en la Universidad Europea de Madrid. Escuela de Doctorado e Investigación. Programa de Doctorado en Biomedicina y Ciencias de la SaludLa fascitis plantar es una condición común en el miembro inferior que se caracteriza por cambios biomecánicos y estructurales en la fascia plantar, resultando en dolor intenso y discapacidad significativa. El 15% de población al año, recibe tratamientos ortopédicos o conservadores de larga duración, sin embargo, en muchos casos estos tratamientos no tienen efecto y terminan en cirugías. Además, en el ámbito militar las lesiones relacionadas con el entrenamiento, son la principal causa de discapacidad, rehabilitación a largo plazo, deterioro funcional y bajas prematuras. Estos hallazgos resaltan la necesidad de mejorar los enfoques de tratamiento. En este contexto, la presente investigación pretende esclarecer la eficacia en el tratamiento de fascitis plantar con dos técnicas mínimamente invasivas. Objetivo: Comprobar la eficacia de la técnica de electrólisis percutánea (EP) en la fascia plantar más un programa de ejercicios excéntricos vs la técnica de neuromodulación percutánea ecoguiada (NPE) en el nervio tibial más un programa de ejercicios excéntricos sobre la intensidad de dolor, grosor de la fascia plantar, grosor del tendón de Aquiles, grosor de la grasa talar, patrón ecogénico de la fascia plantar, flexibilidad de tobillo y satisfacción en sujetos militares con fascitis plantar. Metodología: Se llevó a cabo un ensayo clínico, aleatorizado, prospectivo y longitudinal, en dos grupos: grupo 1, EP (n=34), grupo 2, NPE (n= 33), con un periodo de intervención de 2 veces a la semana por 4 semanas y 1 mes de seguimiento, en las instalaciones del “Hospital Militar Central del Perú”. Resultados: La intensidad de dolor disminuyó (p < 0.001) en ambos grupos, y en comparación entre grupos, la EP (p = 0,023) presentó mejor eficacia; el grosor de la fascia plantar se redujo (p < 0.001) en ambos grupos, y al comparar entre grupos, la EP (p < 0.001) presentó un mayor efecto; el grosor del tendón de Aquiles disminuyó (p < 0.001), y al comparar entre ambos grupos fueron similares (p < 0.05); El patrón ecogénico de la fascia plantar mejoró (p < 0.001) en ambos grupos, y al comparar entre grupos, la EP fue más eficaz para un patrón ecogénico; la flexibilidad de tobillo mejoró (p < 0.001) en ambos grupos, y sin efecto en comparación de grupos; y en la variable satisfacción, la EP fue más eficaz (p < 0.01) en comparación con la NPE. Con respecto a las otras variables, no se observaron resultados estadísticamente significativos en cuanto a la toma, interacción y de grupo. Del análisis correlacional del grosor de la fascia plantar con el dolor, se ha observado una relación positiva entre el grosor de la fascia y la intensidad del dolor. Conclusiones: La técnica de EP en la fascia plantar más un programa de ejercicios excéntricos y la técnica de NPE más un programa de ejercicios excéntricos se han mostrado eficaces en el tratamiento de la fascitis plantar en sujetos militares. Al comparar la eficacia entre ambas técnicas, se encontró que la EP es más eficaz que la NPE en la disminución de la intensidad de dolor, disminución del grosor de la fascia plantar, patrón ecogénico en la fascia plantar y satisfacción de los sujetos.Plantar fasciitis is a common condition of the lower limb that is characterized by biomechanical and structural changes in the plantar fascia, resulting in severe pain and significant disability. 15% of the population each year receives long-term orthopedic or conservative treatments. However, in many cases these treatments have no effect and end in surgery. Additionally, in the military, training-related injuries are the leading cause of disability, long-term rehabilitation, functional impairment, and premature discharges. These findings highlight the need to improve treatment approaches. In this context, the present investigation aims to clarify the effectiveness in the treatment of plantar fasciitis with two minimally invasive techniques. Objective: To check the efficacy of the percutaneous electrolysis (PE) in plantar fascia plus an eccentric exercise program vs. the ultrasound-guided percutaneous neuromodulation (PEN) in the tibial nerve plus an eccentric exercise program on pain intensity, thickness of plantar fascia, Achilles tendon thickness, talar fat thickness, echogenic pattern of the plantar fascia, ankle flexibility and satisfaction in military subjects with plantar fasciitis. Methodology: A randomized, prospective and longitudinal clinical trial was carried out in two groups: group 1, PE (n=34), group 2, PEN (n= 33), with an intervention period of 2 times a week for 4 weeks and 1 month of follow-up, in the facilities of the “Central Military Hospital of Perú”. Results: Pain intensity decreased (p < 0.001) in both groups, and in comparison between groups, PE (p = 0.023) presented better efficacy; The thickness of the plantar fascia was reduced (p < 0.001) in both groups, and when comparing between groups, PE (p < 0.001) had a greater effect; The thickness of the Achilles tendon decreased (p < 0.001), and when compared between both groups they were similar (p < 0.05); The echogenic pattern of the plantar fascia improved (p < 0.001) in both groups, and when comparing between groups, PE was more effective for an echogenic pattern; ankle flexibility improved (p < 0.001) in both groups, and no effect in comparison of groups; And in the satisfaction variable, PE was more effective (p < 0.01) compared to PEN. With respect to the other variables, no statistically significant results were observed in terms of intake, interaction and group. From the correlational analysis of the thickness of the plantar fascia with pain, a positive relationship has been observed between the thickness of the fascia and the intensity of pain. Conclusions: The PE in plantar fascia plus an eccentric exercise program and the PEN in tibial nerve plus an eccentric exercise program have been shown to be effective in the treatment of plantar fasciitis in military subjects. When comparing the effectiveness between both techniques, it was found that PE is more effective than PEN in reducing pain intensity, decreasing the thickness of the plantar fascia, echogenic pattern in the plantar fascia and subject satisfaction.No data 2022UE

    Papel de la marcha y equilibrio en sujetos ancianos (70-79 años) y muy ancianos (>80 años)

Programa de Doctorado en Medicina y CirugíaLa marcha y el equilibrio en las personas mayores son dos exploraciones básicas que cada día tienen más impacto en la valoración geriátrica integral y, por lo tanto, en la atención de las personas mayores. Especialmente la velocidad de la marcha, considerada una de las exploraciones con mayor poder predictivo de eventos adversos como ítem independiente. A pesar de ello, hay muchas lagunas de conocimiento en este tema y una gran escasez de estudios, particularmente sobre personas sanas, lo que impide establecer parámetros de referencia. En este sentido, no encontramos estudios españoles que analicen la marcha y el equilibrio en personas mayores de 70 años, y los estudios de otros países difieren mucho tanto en sus resultados como en sus criterios de selección de lo que consideran una muestra sana. Tampoco encontramos estudios de posturografía sobre personas mayores de 80 años, por lo que hay que extrapolar datos de poblaciones más jóvenes. Además, la mayoría de los estudios realizan la selección de una muestra considerada normal extrapolando criterios de exclusión de las personas jóvenes en lugar de tener en cuenta otros criterios propios de las personas mayores, como la fragilidad y las caídas. Por otro lado, la evaluación de la marcha y el equilibrio, como hemos mencionado, puede ser de ayuda para detectar distintas trayectorias de envejecimiento y predecir eventos adversos. El objetivo principal de este trabajo es establecer criterios de normalidad en los parámetros temporoespaciales de la marcha, equilibrio y postura en las personas mayores con marcha visual normal (definida como la ausencia de marcha patológica diagnosticada por un evaluador experto). Los objetivos secundarios son analizar si los criterios de exclusión e inclusión usados actualmente para seleccionar una muestra para explorar los parámetros de normalidad en la marcha son los adecuados en las personas mayores, así como analizar posibles trayectorias de envejecimiento tras dos años de seguimiento y posibles factores influyentes. Para responder a estas preguntas, se diseñó un estudio observacional, longitudinal, en población no institucionalizada que contaba con una valoración inicial y un seguimiento tras dos años. Los resultados muestran diferencias significativas en todos los parámetros temporoespaciales de la marcha que impiden extrapolar los datos de personas más jóvenes. Teniendo en cuenta las diferencias que hemos encontrado entre países en la primera parte de esta Tesis (revisión bibliográfica), explicables por la distinta morfología de los participantes, lo adecuado sería usar los parámetros españoles en lugar de las referencias internacionales. Para extrapolar estos resultados a distintas poblaciones, este trabajo ha generado una fórmula de determinación personalizada de los valores de normalidad para cada sujeto con marcha visual normal y para cada variable de la marcha considerados y propone usar un intervalo de confianza con nuestros datos y la mencionada fórmula, siempre que se pueda. En el caso de la posturografía, a excepción del test de carga de simetrías y la mayoría de los ítems de control de ritmo y control direccional, también hay evidencia de su relación con la edad. En la literatura disponible no hay datos de estudios en personas mayores de 79 años, por lo que proponemos valores de referencia. Aunque tienen menos potencia estadística, son los primeros analizados. Por otro lado, hemos encontrado que, aunque nuestra muestra tuviera una marcha visual normal, hay factores patológicos (como la fragilidad y las caídas) que modifican los parámetros de normalidad y pueden estar presentes en individuos con marcha visual normal. Los sujetos frágiles y los caedores con marcha visual normal tienen un mayor riesgo de presentar alteraciones en los parámetros de marcha que aumentan el riesgo de padecer consecuencias adversas en su evolución y que, por tanto, deben evaluarse sistemáticamente en este subgrupo de pacientes. Por todo ello, concluimos que no es correcto extrapolar ni datos ni criterios de selección de personas jóvenes a las mayores, ni siquiera los valores medios habitualmente aceptados. Aportamos con este trabajo tanto parámetros como fórmulas que pueden ser utilizados en la población española, especialmente para la marcha. La posibilidad de determinar los valores de normalidad para cada individuo puede tener gran relevancia clínica, dada la gran heterogeneidad de estos valores en la población mayor. Por tanto, cubrimos una brecha en el diagnóstico de la práctica clínica. Además, proponemos el uso de posturografía y Gait Rite como diagnóstico de cribado de fragilidad y caídas. Por un lado, puede ayudar a detectarlas antes que escalas de cribado ya validadas como el SPPB y la velocidad de la marcha y antes de que se detecten visualmente alteraciones en la marcha. Las variables clásicas en los diferentes modelos logísticos y en la mayoría de los casos no permiten construir modelos con gran capacidad explicativa, lo que avala su multicausalidad y la dificultad en un abordaje sistematizado, tanto desde el punto de vista diagnóstico como terapéutico. Aun así, la exploración por posturografía y Gait Rite puede servir para detectar trayectorias de envejecimiento independientemente de la fragilidad y las caídas (factores ya demostrados predictores de trayectorias) en los participantes considerados sanos. Con ello, este trabajo puede contribuir a disminuir las lagunas de conocimiento en el tema y a mejorar la práctica clínica diaria de las personas mayores.Gait and balance in the elderly are two basic examinations that each day have more impact on the comprehensive geriatric assessment and, therefore, on the care of the elderly. Especially gait speed, considered one of the examinations with the greatest predictive power of adverse events as an independent element. Despite this, there are many knowledge gaps on this subject and a great paucity of studies, particularly on healthy people, which prevents establishing reference parameters. In this sense, we did not find Spanish studies that analyze gait and balance in people over 70 years old, and studies from other countries differ greatly both in their results and in their selection criteria for what they consider to be a healthy sample. We also did not find posturography studies on people over 80 years old, so data from younger populations must be extrapolated. In addition, most studies select a sample considered normal by extrapolating exclusion criteria from young people instead of taking into account other typical of older people criteria, such as frailty and falls. On the other hand, the evaluation of gait and balance, as we have mentioned, can help to detect different aging trajectories and predict adverse events. The main objective of this work is to establish normality criteria in the temporospatial parameters of gait, balance and posture in older people with visually normal gait (defined as the absence of pathological gait diagnosed by an expert evaluator). The secondary objectives are to analyze whether the exclusion and inclusion criteria currently used to select a sample to explore the parameters of normal gait are adequate in the elderly, as well as to analyze possible aging trajectories after two years of follow-up and possible influential factors To answer these questions, an observational, longitudinal study was designed in a non-institutionalized population that included an initial assessment and a follow-up after two years. The results show significant differences in all gait temporospatial parameters that prevent extrapolating the data from younger people. Taking into accountthe differences we found among countries in the first part of this thesis (bibliographic review), can be explained through the different morphology of the participants, it would be more appropriate to use Spanish parameters instead of international references. In order to extrapolate these results to different populations, this work has generated a formula for personalized determination of normality values for each subject with visually normal gait and for each variable of the march considered and proposes using a confidence interval with our data and the aforementioned formula, whenever possible. In the case of posturography, with the exception of the symmetries load test and most of the rhythm control and directional control items, there is also evidence of its relationship with age. In the available literature there are no data from studies in people over 79 years old, so we propose reference values. Although they have less statistical power, they are the first to be reported. On the other hand, we have found that, even if our sample had a visually normal gait, there are pathological factors (such as frailty and falls) that can modify the normal parameters and may be present in individuals with visually normal gait. Frail and faller subjects with visually normal gait have a higher risk of presenting alterations in gait parameters that increase their risk of suffering adverse consequences in their evolution and, therefore, should be systematically evaluated in this subgroup of patients. For all these reasons, we conclude that it is not correct to extrapolate neither data nor selection criteria from young people to older people, not even the usually accepted average values. With this work we provide both parameters and formulas that can be used in the Spanish population, especially for gait. The possibility of determining normal values for each individual may have great clinical relevance, given the great heterogeneity of these values in the larger population. Thus, we fill a diagnostic gap in clinical practice. In addition, we propose the use of posturography and Gait Rite as diagnostic screening for frailty and falls. On the one hand, it can help to detect them before already validated screening scales such as the SPPB and gait speed and before gait alterations are visually detected. The classical variables in the different logistic models and in most cases do not allow the construction of models with great explanatory capacity, which supports their multicausality and the difficulty in a systematized approach, both from the diagnostic and therapeutic point of view. Even so, examination by posturography and Gait Rite may serve to detect aging trajectories independently of frailty and falls (factors already proven predictors of trajectories) in participants considered healthy.     Semi-guided learning tool as framework for STEM students learning: A case study for final year projects

    Communication and planning are skills expected among graduates. However, a skills deficit continues to be present among students. To address this problem, here we considered the final year project (FYP), a widely used way of working on skills in STEM degrees. Furthermore, we took into account recent research that highlights the importance of students' thoughts, beliefs, and emotions, in addition to characteristics such as self-efficacy or self-management within the learning process. Here a framework was designed around a semi-guided learning approach, with the aim of improving STEM students' skills and providing them with a work context that facilitates favorable behaviors and feelings associated with writing their FYP report.
