Neuropsychological and biopsychosocial evolution, therapeutic adherence and unmet care needs during paediatric transplantation: study protocol of a mixed-methods design (observational cohort study and focus groups)–the TransplantKIDS mental health project
The present article describes the protocol of a mixed-methods study (an observational cohort design and focus groups), aimed to examine neuropsychological functioning and other biopsychosocial outcomes, therapeutic adherence and unmet care needs in paediatric population undergoing solid organ or allogeneic hematopoietic transplant during the pre- and post-transplant phases. Following a multi-method/multi-source approach, neuropsychological domains will be comprehensively measured with objective tests (SDMT, K-CPT 2/CPT 3, TAVECI/TAVEC, WISC-V/WAIS-IV Vocabulary and Digit Span subtests, Verbal Fluency tests, Stroop, ROCF, and TONI-4); ecological executive functioning, affective and behavioral domains, pain intensity/interference, sleep quality and therapeutic adherence will be assessed through questionnaires (parent/legal guardians-reported: BRIEF-2 and BASC-3; and self-reported: BASC-3, BPI, PROMIS, AIQ and SMAQ); and blood levels of prescribed drugs will be taken from each patient’s medical history. These outcomes will be measured at pre-transplant and at 4-weeks and 6-months post-transplant phases. The estimated sample size was 60 patients (any type of transplant, solid organ, or hematopoietic) from La Paz University Hospital (Madrid, Spain). Finally, three focus group sessions will be organized with patients, parents/guardians, and transplant clinicians (n = 15, with 5 participants per group), in order to qualitatively identify unmet care needs during the pre-, and post-transplant stages of the process. Clinical Presentation Differences Among Four Subtypes of Femoroacetabular Impingement: A Case-Control Study
Objective
The aims of this study were to assess the psychological, functional, and physical differences between patients with femoroacetabular impingement (FAI) syndrome and individuals who were asymptomatic (controls) to analyze clinical differences among 4 subtypes of FAI (cam type, cam type with labral tear, pincer type, and pincer type with labral tear), to calculate the correlations among the variables described, and to identify risk factors contributing to the hip function variance.
Methods
A case–control study was conducted at a hospital rehabilitation unit with a cohort of individuals who were asymptomatic (controls) and a cohort of patients with FAI. Sociodemographic characteristics, pain intensity, psychological health status, hip range of motion (RoM), hip isometric strength, and hip function were assessed.
Results
A total of 69 controls and 69 patients with FAI were analyzed. Although sociodemographic characteristics were comparable between samples, poorer hip RoM, strength, and psychological health were found for the cases. FAI subtypes showed no significant pain intensity, psychological health, or RoM differences (except for hip adduction), but hip strength differed among FAI subtypes. Multiple significant correlations between psychological health, pain intensity, RoM, strength, and hip function were found. The variance values of the International Hip Outcome Tool, Hip Outcome Score (HOS) for daily living activities, and HOS for sports activities were partially explained (47.6%, 36.0%, and 21.6%, respectively) on the basis of kinesiophobia, hip strength, self-efficacy, and anxiety.
Conclusion
Patients with FAI showed poorer psychological health, hip RoM, and strength than individuals who were asymptomatic and had similar sociodemographic characteristics. FAI subtype influenced the patients’ clinical presentations. Hip strength, kinesiophobia, anxiety, and self-efficacy should be targeted for improving hip function, as they have been demonstrated to be important contributors to hip disability.Sin financiación3.8 Q1 JCR 20221.193 Q1 SJR 2023No data IDR 202
Facework in teacher-student email interactions
Two trends have become prominent in higher education worldwide. On the one hand, globalization has favored the expansion and influence of the English language. On the other hand, email has become one of the main forms of communication in academic settings, especially in teacher-student out-of-class correspondence. While these facts have increasingly attracted scholarship attention, studies in education seem to focus more on the students’ display of face(work) alone, while neglecting the teacher’s counterpart. To redress this imbalance, the present study aims to examine face(work) as displayed in students’ email requests for consideration (e.g., on late assignments submission and class attendance) and teachers’ responses. A qualitative analysis of 20 sets of teacher-student interactions reveals different strategies opted for by the students and the teacher in face(work) management. Drawing on face-constituting theory, the findings show that whilst the students are concerned with their own face alone, the teacher is concerned with how to avoid classroom conflicts in the way that attends to one’s own face, the students’ face and the classroom harmony. In this way, the management of face(work) operates in a complex and dynamic way that allows the co-construction and reaffirmation of their respective identities. For example, whilst the students’ actions overlook the relevance of the interdependence relation between them, their peers and the teacher, which is critical for the harmony of the classroom, the teacher’s actions privilege connectedness over separateness. Retrospective, Real Life Study on the Effectiveness and Safety of a Depigmented-Polymerized Subcutaneous Vaccine Containing a Mixture of Grasses and Olea europaea
Introduction: Allergen immunotherapy is the only modifying treatment of the natural course of respiratory allergic diseases; however, the lack of evidence leads to little inconclusive results. Real life studies are on the rise and are becoming a valuable tool to confirm and complement findings from clinical trials. The objective of this study was to evaluate the effectiveness and safety of a depigmented-polymerized undiluted subcutaneous extract of grass and olive pollen, under routine clinical practice conditions. Methods: This was an observational, retrospective, longitudinal, single-center study on the use of a 2-pollen (grass mix and Olea europaea) undiluted subcutaneous extract over at least 3 consecutive years. Data were collected from 76 patients (n = 44 female; median age: 12.5 years old) diagnosed with allergic rhinoconjunctivitis with/without allergic asthma due to sensitization to both grasses and O. europaea. Primary and secondary effectiveness endpoints were symptom severity, concomitant medication, and immunological profile before and after completing the immunotherapy. A 2-year follow-up of patients’ symptoms and medication history after completing the subcutaneous immunotherapy (SCIT) was performed. Results: There was a significant improvement of symptoms and medication consumption after 3 years of SCIT treatment, and a significant decrease in specific IgE levels for grasses and O. europaea was observed after finishing the treatment. Conclusion: Three years treatment of allergic patients using an undiluted mixture of two allergen extracts was shown to be safe and effective for rhinitis and asthma, with efficacy maintained for at least 2 years after finishing SCIT. Utilizando una metodología científica asentada en el análisis de contenido
de las interacciones de usuarios, se propone un estudio exploratorio
compuesto por una etnografía virtual que identifica los contenidos
generados en materia de audio que han suscitado modificaciones en la
producción de las actualizaciones del videojuego analizado, estableciendo
así una correlación efectiva con la evolución tecnológica del título en
cuestión.In today's digital society, user interactions in collaborative virtual spaces on the internet are transforming the audiovisual design of big-budget digital products, such as AAA video game productions. The virtual communities of fans of a popular digital product are highly relevant for their online presence, due to the significant influence of user-generated contributions. These fandom communities are collaborative virtual spaces where users share knowledge and experiences on a topic or audiovisual product of interest.This research focuses on the virtual community of fans of the procedural video game No Man's Sky in 2016 and how the contributions of the wiki fandom are reflected in the official updates published until February 2024. An exploratory study is proposed, which uses a scientific methodology based on the content analysis of registered users' interactions. The study consists of a virtual ethnography that identifies the audio content generated by the virtual community. Real-Life Impact of Hypofractionated IMRT in Early Breast Cancer on Cardiovascular Risk
Various meta-analyses and studies based on historical registries have shown an excess of mortality and significant cardiovascular (CV) morbidity in breast cancer (BC) patients who receive adjuvant radiotherapy (RT) on the left side. However, these patients were treated with obsolete RT techniques, with conventional fractionation and without a precise knowledge of the heart dose-volume histograms (DVH) or CV-risk factors. We now know that the use of ultra-conformal techniques (IMRT) has led to a significant decrease in the heart dose, as reflected in the DVHs. In parallel, hypofractionated protocols -which result in lower EQD2 in organs at risk- have become the standard of care. The aim of our study was to assess whether the combination of both could reduce CV events compared to historical series and to identify clinical or dosimetric factors that could have a negative impact.
Materials/Methods
All BC patients at our institution who received hypofractionated (40.05Gy/15sessions) adjuvant forward-planned IMRT after conserving surgery were included. Patients in indication for elective lymph node RT and those who had received previous mediastinal RT were excluded.
Results
882 patients with invasive BC or DCIS were analyzed (450 right breast; 432 left). After a median follow-up of 8ys (maximum 14), 35 CV events were identified (5 myocardial infarction/angina, 20 arrhythmias, 2 severe valvular disease and 7 "other non-ischemic”). No statistically significant differences were observed according to breast laterality (P = 0.15). With regard to heart DVHs, none of those analyzed had a deleterious effect, although there was a slight and progressive trend towards statistical significance (Table). Patient-dependent CV risk factors that were found to have a significant impact on the incidence of late CV events were dyslipidemia, hypertension, family history of heart disease and overweight/obesity (P = 0.03, <0.001, <0.001 and 0,02 respectively).
Conclusion
Although the deleterious impact of thoracic RT on CV late events is well known, in our cohort it has been shown to be less determinant than patient-dependent baseline risk factors for any CV event. Further long-term follow-up will be necessary to determine whether these results are sustained over time and, if event curves continue to diverge, to assess the extent to which they do so. Exploring Biomechanical Correlates in Voice Analysis of Multiple Sclerosis Patients
The predominant alterations in voice of pa- tients with multiple sclerosis (MS) are phonatory instability, vocal asthenia and roughness, shortness of breath, hypo- phonia, and hypernasality. However, research on alterations of acoustic parameters has few studies and disparate results. The objective of this study was to investigate voice dis- turbances in patients with MS, both with objective measures (analysis of biomechanical) and subjective measures (scales and questionnaires). Methods: This is an experimental study with a total of 20 participants with MS. Voice samples were collected, and biomechanical correlates were analyzed through the Clinical Voice Systems program, Online Lab App. The VHI-30 (Voice Handicap Index) questionnaire, the GRBAS (grade, roughness, breathiness, asthenia, strain) scale, and the Hospital Anxiety and Depression Scale were used as subjective measures. Results: Ninety-!ve percentages of participants feel and describe dysphonic dif!culties. Selfperception of vocal disability correlated with auditory vocal perceptual analysis in the sample of women. Impact of Offensive Team Variables on Goal Scoring in the First Division of the Spanish Soccer League: A Comprehensive 10-Year Study
This study assessed the impact of team offensive variables on goals scored in Spain’s first-division soccer league from 2012–2013 to 2021–2022. A nomothetic multidimensional study was conducted, following the STROBE checklist. From 115 team variables available in the INSTAT database, 57 offensive metrics were selected. The selection was performed independently by five researchers to ensure comprehensive inclusion of relevant metrics. The data included 38 league matches each season, involving 30 teams. The study identified key offensive performance metrics strongly correlated with goals scored. These include shots on target (r = 0.898), chances created (r = 0.871), penalty box entries (r = 0.852), positional attacks with shots (r = 0.818), total shots (r = 0.807), central attacks with shots (r = 0.804), accurate passes (r = 0.760), and efficiency in positional attacks (r = 0.755). The findings underscore the importance of specific offensive metrics in enhancing goal-scoring capabilities. Coaches can improve their team’s offensive performance by focusing on passing accuracy, dribbling, and maximizing scoring chances. Successful teams consistently convert attacks into shots on goal through various situations, including central attacks, flanks, counterattacks, corners, and set pieces. IA generativa: política de uso ético por parte del alumnado universitario
A pesar del notorio esfuerzo por promulgar una legislación integral sobre la Inteligencia Artificial, existe una presión cada vez mayor sobre las universidades para abordar el uso ético de esta tecnología en la educación superior a través de sus propias normativas. Este estudio presenta las bases, implantación y resultados de una experiencia piloto de política de uso de la inteligencia artificial generativa (IAG) en las aulas de la Facultad de Ciencias Sociales de la Universidad Europea de Valencia. Analiza aspectos vinculados a la docencia y aprendizaje mediante IAG (uso, ética y regulación); desarrollo de competencias digitales; evaluación de aprendizajes; impacto social (Sostenibilidad curricular y Objetivos de Desarrollo Sostenible), relacionando los resultados con la educación de calidad y el fomento del pensamiento crítico.
