An agent-based model of Bayesian inference as an educational tool
The scientific and practical importance of correct Bayesian reasoning can hardly be overrated. But what is the best method to teach it and to fight widespread and harmful misconceptions like the false positive paradox? We studied the existing literature on the question and experimented with math cubes to find more intuitive visualizations. These inspired us to start experimenting with the programmable modeling environment NetLogo. The result is an agent-based model of Bayesian inference as an educational tool that combines the most successful teaching methods found so far: the use of natural frequencies, relatable visualization, interactive visualization, and visualization that can easily be reproduced with physical props. The model can be modified, improved, extended, and used in many ways. While testing in controlled experiments is still outstanding, the result thus far leads us to conclude that agent-based modeling is a most promising way of teaching Bayesian inference and illustrating it for practical purposes.
N-terminal pro-B-type natriuretic peptide post-discharge monitoring in the management of patients with heart failure and preserved ejection fraction - a randomized trial: The NICE study
Aims: There is a lack of specific studies assessing the impact of natriuretic peptide monitoring in the post-discharge management of patients with heart failure (HF) and preserved ejection fraction (HFpEF), throughout the vulnerable phase following acute HF hospitalization. The NICE study aims to assess the clinical benefit of incorporating N-terminal pro-B-type natriuretic peptide (NT-proBNP) into the post-discharge management of HFpEF patients.
Methods and results: Individuals admitted with HFpEF (left ventricular ejection fraction >50%) were included in a multicentre randomized controlled study employing an open-label design with event blinding (NCT02807168). Upon discharge, 157 patients were randomly allocated to either NT-proBNP monitoring (n = 79) or no access to NT-proBNP (control group, n = 78) during pre-scheduled visits at 2, 4 and 12 weeks. Clinical endpoints were evaluated at 6 months. The primary endpoint of HF rehospitalizations occurred in 12.1% patients, without significant differences observed between the NT-proBNP monitoring group (12.8%) and the control group (11.4%) (hazard ratio [HR] 1.15, 95% confidence interval [CI] 0.47-2.81, p = 0.760). Regarding secondary endpoints, the NT-proBNP monitoring group demonstrated a significantly lower risk of death (1.3% vs. 10.1%; HR 0.12, 95% CI 0.02-0.09), whereas non-HF hospitalizations (12.8% vs. 19.0%, p = 0.171) and any adverse clinical event (26.9% vs. 36.7%, p = 0.17) did not reach statistical significance. Awareness of NT-proBNP levels were associated with higher doses of diuretics and renin-angiotensin system inhibitors (angiotensin-converting enzyme inhibitors/angiotensin II receptor blockers) in the NT-proBNP monitoring group.
Conclusions: Post-discharge monitoring of NT-proBNP in HFpEF patients did not exhibit an association with reduced rates of HF hospitalization in this study. Nonetheless, it appears to enhance global clinical management by optimizing medical therapies and contributing to improved overall survival.Non\u2010conditional grant from Roche Diagnostics18.2 Q1 JCR 20225.919 Q1 SJR 2023No data IDR 2022UE
A New Clinical Examination Algorithm to Prescribe Conservative Treatment in People with Hip-Related Pain
Hip-related pain is a common issue in active adults affecting their quality of life, mobility, and overall function, and it can lead to persistent disability. However, diagnosing hip-related pain is challenging due to the many potential sources and causes, including intra-articular and extra-articular pathology, and referred pain from other areas (lumbar or groin related pain). To address this, there is a need for a clinical algorithm based on the best available evidence and expert consensus. This algorithm could guide healthcare professionals in assessing and managing patients with hip-related pain, during the diagnosis, test selection, intervention, monitoring, and promoting collaboration among various healthcare providers. This clinical algorithm for hip-related pain is a comprehensive, flexible, adaptable to different settings, and regularly updated to incorporate new research findings. This literature review aims to establish a clinical algorithm specifically for prescribing exercise treatment to patients with hip-related pain, addressing their individual needs and enhancing their overall care.Sin financiación4.0 Q2 JCR 20220.847 Q1 SJR 2023No data IDR 2022UE

"Democratic" and systematic violation of the fundamental right to active vote in case of request for registration in the registry of a municipality different from the current
The objective of this article is to empirically analyze the situation and treatment of
the fundamental right to active suffrage and, therefore, to participation in the political life of the
country by any citizen in cases of change of registration, an administrative procedure that involves
the immediate and automatic removal of existing census data, taking into account that registration
in the register is declarative and does not constitute the right to vote, constituting an essential
instrument for the exercise of the fundamental right to vote. The constitutional and jurisprudential
requirements for the development and limitation of the content of this fundamental right are
analyzed, as well as for its effective exercise in conditions of equality, transparency and equity.Sin financiación0.1 Q* JCR 2022No data SJR 20230,20 C2 IDR 2022UE
Addressing Challenges in Wildlife Rehabilitation: Antimicrobial-Resistant Bacteria from Wounds and Fractures in Wild Birds
Wildlife rescue centers frequently admit animals with injuries and bone fractures. Open fractures are common in birds due to their anatomy, and this can lead to complications like osteomyelitis, which implies a serious bone infection and often necrosis, or death of the affected bone tissue. Antibiotic therapy is crucial, but the rise in antimicrobial-resistant isolates in wildlife raises concerns about treatment efficacy. A study focused on isolating, identifying, and assessing antimicrobial resistance in bacteria from wounds and fractures in wild birds. Among 36 isolates, Staphylococcus spp. dominated (63.8%), with 82.6% exhibiting antimicrobial resistance, particularly to clindamycin, an antimicrobial key in the treatment of infected bone fractures. This escalating resistance poses a dual threat to wildlife—therapeutic failure and the spread of resistant bacteria in ecosystems.Ministry of Ecological Transition (MITECO)Complutense University of Madrid (Spain)3.0 Q1 JCR 20220.698 Q1 SJR 2023No data IDR 202
The Alternative Model of Personality Disorders: Assessment, Convergent and Discriminant Validity, and a Look to the Future
The Alternative Model of Personality Disorders (AMPD) is a dimensional, empirically based diagnostic system developed to overcome the serious limitations of traditional categories. We review the mounting evidence on its convergent and discriminant validity, with an incursion into the less-studied ICD-11 system. In the literature, the AMPD's Pathological Trait Model (Criterion B) shows excellent convergence with normal personality traits, and it could be useful as an organizing framework for mental disorders. In contrast, Personality Functioning (Criterion A) cannot be distinguished from personality traits, lacks both discriminant and incremental validity, and has a shaky theoretical background. We offer some suggestions with a view to the future. These include removing Criterion A, using the real-life consequences of traits as indicators of severity, delving into the dynamic mechanisms underlying traits, and furthering the integration of currently disengaged psychological paradigms that can shape a sounder clinical science.
