Simultaneous cytomegalovirus and herpesvirus meningitis in an adolescent immunocompetent patient

    No full text
Background: Viral meningitis is an inflammatory condition of the meninges that manifests as neurological dysfunction with variable clinical signs, ranging from mild manifestations (such as headache) to seizures and intracranial hypertension. The clinical presentation can vary with the patient’s age, immune status, and etiology of the disease, with many symptoms being nonspecific. Coinfections in central nervous system (CNS) infections are rare, more commonly observed in immunocompromised patients. We report the first known case of viral meningitis due to concurrent cytomegalovirus (CMV) and herpes simplex virus type 2 (HSV-2) in an immunocompetent adolescent, highlighting a significant clinical consideration in the management of similar cases. Case Description: We present a case from March 2021 involving a 16-year-old previously healthy Latin male adolescent with no known immunity-related abnormalities, who presented diarrhea with macroscopic blood, headache, and fever. He was diagnosed with a rare viral coinfection of CMV and HSV-2 meningitis. Investigations to rule out primary or secondary immunodeficiency showed no abnormalities. The patient received antiviral therapy for 21 days with ganciclovir (5 mg/kg/dose), leading to a favorable outcome without subsequent neurological deficit or adverse events. Conclusions: This case enriches clinical practice by demonstrating effective management in an unlikely host, thereby broadening our understanding of viral meningitis in immunocompetent patients and informing future diagnostic and treatment strategies

Exploring Trypanosoma cruzi transmission dynamics in an acute Chagas disease outbreak using next-generation sequencing

Background: Chagas disease (CD), caused by Trypanosoma cruzi, poses a major global public health challenge. Although vector-borne transmission is the primary mode of infection, oral transmission is increasingly concerning. Methods: This study utilized long-amplicon-based sequencing (long-ABS), focusing on the 18S rRNA gene, to explore T. cruzi’s genetic diversity and transmission dynamics during an acute CD outbreak in Colombia, an area without domestic infestation. Results: Analyzing samples from five patients and five T. cruzi-positive marsupial samples, we identified coinfections between T. cruzi and Trypanosoma rangeli, mixed T. cruzi DTUs, suggesting possible links between human and marsupial T. cruzi infections. Coexistence of TcI, TcIV and T. rangeli suggests marsupial secretions as the possible source of T. cruzi transmission. Our investigation revealed diversity loss in DTUs TcIV and T. rangeli in humans after infection and in marsupial samples after culture. Conclusion: These findings provide significant insights into T. cruzi dynamics, crucial for implementing control and prevention strategies. Graphical abstract: (Figure presented.

Colombian surgical outcomes study insights on perioperative mortality rate, a main indicator of the lancet commission on global surgery – a prospective cohort study

    No full text
Background: Surgical care holds significant importance in healthcare, especially in low and middle-income countries, as at least 50% of the 4.2 million deaths within the initial 30 days following surgery take place in these countries. The Lancet Commission on Global Surgery proposed six indicators to enhance surgical care. In Colombia, studies have been made using secondary data. However, strategies to reduce perioperative mortality have not been implemented. This study aims to describe the fourth indicator, perioperative mortality rate (POMR), with primary data in Colombia. Methods: A multicentre prospective cohort study was conducted across 54 centres (hospitals) in Colombia. Each centre selected a 7-day recruitment period between 05/2022 and 01/2023. Inclusion criteria involved patients over 18 years of age undergoing surgical procedures in operating rooms. Data quality was ensured through a verification guideline and statistical analysis using mixed-effects multilevel modelling with a case mix analysis of mortality by procedure-related, patient-related, and hospital-related conditions. Findings: 3807 patients were included with a median age of 48 (IQR 32–64), 80.3% were classified as ASA I or II, and 27% of the procedures had a low-surgical complexity. Leading procedures were Orthopedics (19.2%) and Gynaecology/Obstetrics (17.7%). According to the Clavien–Dindo scale, postoperative complications were distributed in major complications (11.7%, 10.68–12.76) and any complication (31.6%, 30.09–33.07). POMR stood at 1.9% (1.48–2.37), with elective and emergency surgery mortalities at 0.7% (0.40–1.23) and 3% (2.3–3.89) respectively. Interpretation: The POMR was higher than the ratio reported in previous national studies, even when patients had a low–risk profile and low-complexity procedures. The present research represents significant public health progress with valuable insights for national decision-makers to improve the quality of surgical care.

    No full text
There is an imminent need for Bogota to implement a change in its entire waste management policy. Poor waste management is the third cause of greenhouse gases in Bogota. On the other hand, the Doña Juana landfill is already classified as a critical site, with a useful life of less than 1 year. Therefore, this waste disposal site represents an environmental hazard for the community and the planet. The city has a very low recycling rate of only 17%. This figure demonstrates the current situation and the importance of rethinking the projects and strategies that are being implemented. Bogota's recycling process is made up of 7 phases. Each of these has different challenges to address, and in general, there is unsustainability along the whole chain. However, it is important to start solving the problem at the root, by reducing the amount of waste that is generated. This degree work seeks to recommend strategies from European Union countries that promote recycling among citizens and households. Guided by a circular economy policy and a change of mentality about waste. Seeing it as a resource, rather than as something to be disposed of that has no value. However, to achieve this, the willingness of citizens to improve their waste separation habits is very important. It has been shown that if household members are surrounded by individuals who have good household solid waste management practices, they are more likely to adopt the same behaviors. This study uses a quantitative methodology to demonstrate that the main factor contributing to the city's low recycling rates is the lack of a clear understanding or knowledge of the waste separation system. And that the implementation of a combination of EU-inspired strategies would significantly increase recycling rates and improve overall household waste management in Bogotá

An integrated analysis of the academic literature for comprehending and advancing in the human resource digital transformation is required. In this sense, this study aims to analyze the development, current status, and trends of the literature of human resource management research regarding its digital transformation. A total of 284 academic documents published between 2001 and 2022 in all fields of knowledge were retrieved from the Web of Science (WoS) database. Data was analyzed through VOSViewer software and bibliometrix R-package. According to the bibliometric analyses, from 2015 digital transformation of HRM had a sustained crescent interest being more remarkable since 2020. Foundational papers, the most influential documents, journals and authors were identified. Through a bibliographic coupling analysis, we identify five current mainstreams: 1) Impact, challenges and out-comes of e-HRM, 2) e-HRM and organizational strategy, 3) Conceptualization of strategic human resource management, 4) Effectiveness of e-HRM: productivity and performance, and 5) e-HRM and innovation. Co-word analysis shows clusters oriented to 1) digital platforms, 2) data analytics and 3) organizational strategy. All the conducted analyses show that human factors have received less attention, a finding confirmed by the thematic map. “Management, strategy, information” and “antecedents, quality and economic growth” are acquiring special relevance in structuring the research field of digital HRM according to the thematic map. These findings and their implications are discussed
