    Silver Coated Endomegaprosthesis in Complex Surgery: Prosthetic Complications, Traumatology and Tumors

    Trabalho Final do Mestrado Integrado em Medicina apresentado à Faculdade de MedicinaWithin orthopedic surgery, both in oncology and traumatology, there are extreme clinical situations with the indication for amputation such as: prosthetic complications with massive osteolysis with or without infection, aggressive neoplasics lesions with significant bone infiltration and bone loss associated with complex trauma’s situations.The advent of using endomegaprosthesis as a solution in extreme orthopedic situations has relegated amputation in many of these situations.Within the scope of using these devices outside oncology, where they are usually used and which they were originally created for, the use of endomegaprostheses have extended to other areas of complex surgeries in orthopedics and traumatology.One of the worst complications that appears after the surgery, either short or long term, are periprosthetic infections.These infections arise around the prosthetic device, developing on account of bacteria colonization, along with biofilm formation, which compromise endomegaprosthesis’s viability.Over the past few years, preventive measures have been applied in order to minimize these severe complications. Due to effective bactericidal properties and low toxicity, silver has become a very interesting chemical element for prosthesis coating, in order to prevent periprosthetic infections.Even though the use of endomegaprosthesis’ silver coating is a comparatively new procedure in medicine, the scientific evidence reveals that these coating seems to be associated with a lower incidence of periprosthetic infection.Thus, the use of these devices, associated with lower periprosthetic infection rates, has allowed and encouraged its wider use in complex surgeries in Orthopedics, Oncology and Traumatology. In this sense, this article pretends go through the current status of using silver coated endomegaprosthesis in these procedures.Dentro da cirurgia do foro ortopédico, na área oncológica e na área traumatológica, há situações clínicas extremas com indicação para a amputação: complicações protésicas com osteólises maciças com ou sem infeção associadas, lesões neoplásicas agressivas com importante destruição óssea e situações de trauma complexo com perda de osso.O advento do uso das endomegapróteses como solução em situações ortopédicas extremas, veio relegar para segundo plano a amputação em muitas destas situações.No âmbito da utilização das endomegapróteses e dando relevo às áreas fora da oncologia onde habitualmente são usadas e para onde foram originalmente criadas, o uso destes dispositivos alargou-se para outras situações de cirurgias complexas em ortopedia e traumatologia.Uma das complicações mais graves que decorrem no pós-operatório das endomegapróteses, quer a curto, quer a longo prazo, são as infeções periprotésicas.Estas infeções desenvolvem-se devido à colonização bacteriana à volta da prótese, associadas à formação de biofilme, o que torna o seu tratamento muito difícil e, por consequência, compromete a viabilidade das endomegapróteses.No decorrer dos últimos anos foram implementadas medidas preventivas de forma a minimizar a incidência desta temível complicação. A prata, pelas suas propriedades bactericidas e pela sua baixa toxicidade, tornou-se um elemento muito interessante usado no revestimento das próteses, com intenção de prevenir o aparecimento dos processos infeciosos periprotésicos.Apesar da utilização de endomegapróteses revestidas a prata ser um procedimento relativamente recente na medicina, a evidência mostra que a sua implantação parece estar associada a uma menor incidência de infeção periprotésica.Assim, o uso destas próteses, associadas a menores taxas de infeção periprotésica, tem permitido e incentivado a sua maior utilização em cirurgias complexas de Ortopedia, Oncologia e Traumatologia. Neste sentido, pretende-se com este trabalho fazer o ponto de situação atual da utilização das endomegapróteses revestidas a prata nestas cirurgias

    Cannabis in Medicine: knowledge, stigma and attitude towards recreational and therapeutic Cannabis Use

    Trabalho Final do Mestrado Integrado em Medicina apresentado à Faculdade de MedicinaCannabis is recently growing in popularity, not only regarding its recreational use but also its medical use. With the recent approval of medical Cannabis use in Portugal in 2018, it is important to assess doctors' and medical students' attitudes, beliefs, and knowledge regarding recreational and medical use. Also, it is important to evaluate the educational needs of the medical population in Portugal.To do so, a questionnaire online was created based on studies done abroad. The questionnaire included all the objectives of the study and was, prior to its release, validated by experts and non-experts on Cannabis. The data was analyzed using descriptive and inferential statistics.In total, there were 585 responses, of which 281 were Doctors and 304 were Medical Students. The results found that the Portuguese medical community had very little contact with Cannabis and a low level of knowledge of Portuguese legislation on the matter, specifically regarding the indications and the substances that are currently approved. Also, knowledge was deficient in the theoretical aspects of Cannabis, which translated to discomfort in patient counseling. The majority of the population of the study saw Cannabis as a legitimate treatment and saw its potential to help their patients. However, a lot of doctors and medical students expressed concern towards the side effects and impact of use in clinical practice. Most participants reflected the need for more education on the subject, identifying legislation and prescription as the subjects they lacked the most. The participants' contact with the subject was very low during their medical education. However, it was evident an increase in its presence when doctors and medical students were compared (p<0.001).In conclusion, doctors and medical students are lacking in knowledge regarding Cannabis, so more education on this matter is needed through additional courses or its inclusion in formal medical education.A Canábis, quer medicinal quer recreativa, tem sido um tema com um aumento exponencial no seu mediatismo. E, com a aprovação da Lei da Canábis para fins medicinais em 2018, importa aferir quais as crenças, atitudes e conhecimento dos Médicos e Estudantes de Medicina sobre a Canábis, quer na vertente recreativa, quer na vertente medicinal, sendo este o objetivo do presente estudo. Além disso, este estudo tem também como objetivo averiguar quais as necessidades formativas dessa população, em Portugal.Para isto, elaborou-se um questionário, tendo por base estudos realizados no estrangeiro, que foi divulgado em formato digital no dia 28 de dezembro 2022. Para efeitos deste estudo, o questionário foi encerrado no dia 15 de fevereiro de 2023. O questionário incidiu sobre cada um dos objetivos e antes da sua divulgação, foi testado e validado por peritos e não peritos na área. Os dados obtidos foram analisados de forma descritiva e inferencial.No total, o inquérito teve 585 respostas, com 281 Médicos e 304 Estudantes de Medicina. Nos resultados obtidos foi possível observar que o contacto prévio dos inquiridos com a Canábis era muito reduzido, assim como o conhecimento da legislação portuguesa, das formulações e indicações aprovadas em Portugal. Foi também evidente um baixo nível de conhecimento teórico, e pouca confiança em transmitir informações aos doentes. Apesar disto, os inquiridos consideram estas terapêuticas algo legítimo e com utilidade para muitos doentes. Ainda assim, muitos expressaram preocupações com possíveis efeitos adversos destas terapêuticas. No que toca às necessidades formativas, a grande parte dos inquiridos referiu necessidade de formação adicional, identificando-se a prescrição e a legislação em vigor em Portugal como os temas mais solicitados. O contacto dos inquiridos com o tema da Canábis, durante o currículo médico, foi reduzido, apesar de ser evidente uma melhoria recente, ao serem comparados os Médicos com os Estudantes de Medicina (p<0,001). Assim, pode-se concluir que existem lacunas no conhecimento da população médica portuguesa no tema da Canábis, sendo necessária uma aposta na formação dos profissionais de saúde e na inclusão do tema no currículo médico

    Serum GFAP and NfL value in Dementia with Lewy Bodies

    Trabalho Final do Mestrado Integrado em Medicina apresentado à Faculdade de MedicinaIntroduction: The Dementia with Lewy Bodies (DLB) is frequently underdiagnosed or misdiagnosed as Alzheimer Disease (AD). The existence of a spectrum of DLB with AD co- pathology hinders the differential diagnosis. Current methods used in diagnosis are either invasive, expensive or nonspecific, thus emerging the need to establish biomarkers of easy access and specific for DLB. The aim is to study the role of serum biomarkers glial fibrillary acidic protein (GFAP) and neurofilament light chain (NfL) in the evaluation of DLB patients. Methods: Our study included 3 groups, enlisted from the Neurology Department of Coimbra Hospital and University Centre: 66 patients diagnosed with DLB, 52 with AD and 29 controls without Central Nervous System disease and cognitive deterioration. The groups were previously submitted to lumbar and venous puncture, in the context of their health care, and had AD biomarkers in cerebrospinal fluid (CSF) available, which allowed us to classify them, according to the ATN classification system, as to their evidence of amyloid pathology (A), tauopathy (T) and neurodegeneration (N). GFAP and NfL levels were quantified using Single Molecule Array ultrasensitive immunoassay methodology. Results: We found that serum GFAP and NfL had significantly higher levels in DLB patients comparing to controls (p < 0,001), without statistically significant differences with AD patients. In DLB patients, GFAP showed a positive correlation with serum NfL (p < 0,001), CSF biomarker t-Tau (p = 0,010) and negative with MMSE score (p = 0,047). When we analyzed separately DLB patients with evidence (A+; n = 38) and without evidence of amyloid co- pathology (A-; n = 28), we found that these correlations persisted only in the A+ subgroup. No statistically significant differences were found in GFAP and NfL serum levels between DLB patients with and without biological evidence of amyloid pathology. However, we observed higher levels of GFAP in DLB patients with biological evidence of neurodegeneration (N+; p = 0,014). Discussion and Conclusion: Serum GFAP and NfL showed limited usefulness in the differential diagnosis between DLB and AD and in identifying AD co-pathology in DLB patients. However, GFAP revealed an association with neurodegeneration and the cognitive deterioration in the presence of amyloid co-pathology.Introdução: A Demência com Corpos de Lewy (DCLewy) é frequentemente subdiagnosticada ou erradamente diagnosticada como Doença de Alzheimer (DA). A existência de um espetro de DCLewy com co-patologia de DA dificulta o diagnóstico diferencial. Os métodos atualmente usados para diagnóstico são invasivos, dispendiosos ou inespecíficos, surgindo a necessidade de estabelecer biomarcadores de fácil acesso e específicos para a DCLewy. O objetivo é estudar o papel dos biomarcadores séricos proteína glial fibrilar acídica (GFAP) e neurofilamento de cadeia leve (NfL) na avaliação de doentes com DCLewy. Métodos: O nosso estudo incluiu 3 grupos, recrutados no Serviço de Neurologia do Centro Hospitalar Universitário de Coimbra: 66 doentes diagnosticados com DCLewy, 52 com DA e 29 controlos sem doença do Sistema Nervoso Central e deterioração cognitiva. Os grupos foram previamente submetidos a punção lombar e venosa, no contexto dos seus cuidados de saúde, tendo disponíveis biomarcadores de DA no líquido cefalorraquidiano (LCR), o que permitiu a sua classificação, segundo o sistema ATN, quanto à existência de evidência biológica de patologia amilóide (A), taupatia (T) e neurodegeneração (N). Os níveis de GFAP e NfL no soro foram quantificados por um imunoensaio ultrassensível, por metodologia de Single Molecule Array. Resultados: Verificou-se que a GFAP e NfL no soro apresentaram níveis significativamente mais elevados nos doentes DCLewy relativamente ao grupo controlo (p < 0,001), sem diferenças relativamente à DA. Nos doentes com DCLewy, a GFAP apresentou uma correlação positiva com os NfL no soro (p < 0,001), a t-Tau no LCR (p = 0,010) e negativa com a pontuação no MMSE (p = 0,047). Quando analisados separadamente os doentes DCLewy com (A+; n = 38) e sem evidência de patologia amilóide (A-; n = 28), verificou-se que estas correlações se mantiveram apenas no subgrupo A+. Não se encontraram diferenças nos níveis de GFAP e NfL no soro entre doentes DCLewy com e sem evidência biológica de patologia amilóide. Contudo, observaram-se níveis superiores de GFAP em doentes DCLewy com evidência biológica de neurodegeneração (N+; p = 0,014). Discussão e Conclusão: A GFAP e NfL no soro não demonstraram utilidade no diagnóstico diferencial entre DCLewy e DA, nem na identificação de co-patologia amilóide nos doentes DCLewy. Todavia, a GFAP revelou uma associação à neurodegeneração e deterioração cognitiva em doentes com co-patologia amilóide

    Mecanismos moleculares de resistência a imunomoduladores no Mieloma Múltiplo

    Dissertação de Mestrado em Bioquímica apresentada à Faculdade de Ciências e TecnologiaO Mieloma Múltiplo (MM) é uma neoplasia hematológica caracterizada pela presença de plasmócitos malignos monoclonais na medula óssea e pela presença dos denominados eventos definidores de mieloma. A introdução de novas classes de fármacos, nomeadamente inibidores do proteasoma (PI), imunomoduladores (IMiDs) e anticorpos monoclonais (AcMo), no tratamento do MM tem-se acompanhado de um aumento da sobrevivência dos doentes. Associados em esquemas terapêuticos com os PI e AcMo, atualmente são utilizados diferentes imunomoduladores no tratamento do MM, como a Lenalidomida (LEN) e a Pomalidomida (POM), sendo que esta classe de fármacos tem como alvo principal a proteína ligase E3 cereblon (CRBN). No entanto, esta patologia continua incurável sendo um grande obstáculo no tratamento, o desenvolvimento de resistência a fármacos. Existem vários mecanismos que têm vindo a ser associados com esta resistência à terapêutica, nomeadamente alterações genéticas e epigenéticas, desregulação dos transportadores de glucose e de efluxo e influxo de fármacos, alterações ao nível do alvo molecular, e a desregulação de vias de sinalização intracelulares de sobrevivência, de regulação do stresse oxidativo e da apoptose. Contudo, apesar do crescente conhecimento do mecanismo de resistência a fármacos em particular aos PIs, são necessários mais estudos na área da resistência aos IMiDs.O foco deste projeto foi avaliar os mecanismos moleculares de resistência a IMiDs, LEN e POM, in vitro, e averiguar o desenvolvimento de resistência cruzada ao Bortezomib (BTZ).Desta forma, foram desenvolvidas linhas resistentes a IMiDs por exposição contínua a LEN e POM, a partir das linhas sensíveis NCI-H929 e U-266 (resistentes à LEN, as células H929RL e U-266RL; resistentes à POM, as células U-266RP). A validação dos modelos de resistência e a avaliação de resistência cruzada ao PI foi avaliada através do ensaio de resazurina. As alterações genómicas foram avaliadas recorrendo ao Multiplex Ligation-dependent Probe Amplification (MLPA). A expressão dos genes dos transportadores de efluxo da família ABC (ABCB1, ABC2 e ABCG2), dos transportadores de influxo (SLC2A1 e SLC2A11), do alvo principal dos IMiDs (CRBN), da via NF-κB (NFKB1 RELA, RELB, REL e IKBKB), da via KEAP1/NRF2 (KEAP1 e NFE2L2), de MYC, da proteína anti-apoptótica pertencente à família BCL-2 (BCL2) e de uma enzima metabolizadora de fármaco (NQO1) foi analisada por qPCR nas linhas celulares sensíveis e resistentes. A expressão proteica dos transportadores de efluxo, P-gp, MRP1 e BCRP, e do factor de transcrição NF-κB foi avaliada por citometria de fluxo recorrendo à marcação membranar e intracelular, respetivamente, com anticorpos conjugados com fluorocromos. A atividade dos transportadores foi também analisada por citometria de fluxo. Por fim, os níveis de espécies reativas de oxigénio foram avaliadas por fluorimetria com recurso às sondas fluorescentes. Os resultados foram analisados estatisticamente (nível de significância de 95%).Os resultados obtidos demonstram que os três modelos de resistência desenvolvidos apresentam diferentes graus de resistência de acordo com o IMiD e a linha celular em questão. Assim, as linhas celulares resistentes à LEN apresentaram valores de IC25 6,5 vezes e mais de 1470,6 vezes superiores ao estimado nas linhas sensíveis aos IMiDs (nas NCI-H929 o IC25 é de 65,8 µM e nas U-266 de 0,3 µM). De facto, nas células H929RL o IC25 é de é 427,3 µM e nas células U-266RL é ˃ 500,0 µM, evidenciando resistência à LEN. Na linha celular resistente a POM, as células U-266RP, apresentaram um IC25 de 390,0 µM ou seja cerca de 557,1 vezes superior ao calculado para a linha sensível U-266 (IC25=0,7 µM). Todos os modelos estudados resistentes aos IMiDs evidenciaram sensibilidade ao BTZ.Nas linhas resistentes observou-se um aumento na percentagem de células que expressam P-gp e MRP1 (P-gp: H929RL 52,7 ± 5,5 %; U-266RL 22,0 ± 2,0 % e U-266RP 24,3 ± 2,9 %; e MRP1: H929RL, 59,7 ± 0,6 %; U-266RL, 81,0 ± 2,0 % e U-266RP, 96,7 ± 0,6 %) comparativamente com as linhas sensíveis (P-gp: NCI-H929 5,0 ± 1,4 %; U-266 5,5 ± 0,7 %; e MRP1: NCI-H929 51,0 ± 2,8 %; U-266 77,0 ± 4,2 %). Adicionalmente, os estudos de expressão génica demonstraram um aumento da expressão dos genes SLC2A1, CRBN, RELA, RELB, KEAP1 e MYC, e diminuição da expressão génica de REL, NFE2L2, BCL2 e NQO1 nas linhas resistentes. Relativamente às alterações genómicas, as linhas resistentes apresentavam amplificações e deleções, sendo que a linha U-266RL foi a que apresentou mais alterações. Em suma, a resistência aos IMiDs nos três modelos de resistência desenvolvidos pode ser mediada pelo aumento da expressão da P-gp e MRP1. Além disso, as células resistentes aos IMiDs não apresentam resistência cruzada ao BTZ. Contudo, os resultados necessitam de ser validados através de uma análise molecular mais extensa.Multiple myeloma is a haematological malignancy characterized by the presence of abnormal clonal plasma cells in the bone marrow and by the presence of defining myeloma events. In MM treatment, the introduction of new pharmacological classes, such as Proteasome Inhibitors (PI), Immunomodulators (IMiDs) and Monoclonal Antibodies (mAbs), has been increasing the patients' survival. In combination with PI, different immunomodulators are currently used, such as Lenalidomide (LEN) and Pomalidomide (POM) in MM treatment which has cereblon as the main target. Nonetheless, this disease remains incurable, and resistance development is still a concern. Several mechanisms have been associated with resistance, including genetic alterations, glucose and drug efflux transporters dysregulation, alteration in molecular target, and dysregulation of intracellular signalling pathways of survival, oxidative stress and apoptosis.This project's focus was to evaluate the molecular mechanism underlying the IMiDs, LEN and POM, resistance in vitro and investigate the cross-resistance to Bortezomib (BTZ).Therefore, resistant cell lines to LEN and POM were developed from two in vitro cell models sensitive to these IMiDs, NCI-H929 and U-266, through continuous exposure to non-lethal doses. These resistant models were denominated according to the origin sensible cell line and the drug that they developed resistance to (LEN resistant cell lines: H929RL and U-266RL; POM resistant cell line: U-266RP). The three cell lines were maintained at their optimal growth conditions, which included the administration of 100 µM of LEN or POM. The effect of different drug concentrations, as well as the validation of resistant models and evaluation of cross-resistance to PI, were evaluated through the resazurin assay. Genomics alterations were evaluated using the MLPA assay. The gene expression of efflux transporters of the ABC family (ABCB1, ABC2 and ABCG2), of the solute transporters (SLC2A1 and SLC2A11), main IMiDs target (CRBN), of the NF-κB pathway (NFKB1 RELA, RELB, REL and IKBKB), of the KEAP1/NRF2 pathway (KEAP1 and NFE2L2), of the MYC, of the anti-apoptotic protein BCL2 (BCL2), and the metabolizing-drug enzyme (NQO1) were analysed by qPCR in sensitive and resistant cell lines. Besides that, protein expression of efflux transporters, P-gp, MRP1 and BCRP, and NF-κB was evaluated by flow cytometry using membrane and intracellular labelling, respectively, conjugated antibodies with fluorochrome. The activity of these transporters was also analysed by flow cytometry using Verapamil (P-gp inhibitor), MK-571 (MRP1/2 inhibitor) and Novobiocin (BCRP inhibitor). Lastly, the levels of intracellular peroxide, peroxide anion, nitric oxide, mitochondrial reactive oxygen species and reduced glutathione were evaluated using fluorescent probes by fluorometry, DCFH2-DA, DHE, DAF-FM, DHR-123 and MO. The results were analysed statistically (significance level of 95%).The three developed resistant models showed the IC25 values were 6.5 times (in H929RL cells is 427.3 µM), more than 1470.6 times (in U-266RL cells is ˃ 500 µM) and 557.1 times (U-266RP, 557.12 µM) higher than what was estimated for sensitive cell lines (in NCI-H929 cells is 65.78 µM; in U-266 cells is 0.34 µM). The resistant studied models to IMiDs exhibit sensitivity to BTZ, suggesting that there is no cross-resistance to this drug. The evaluation of the molecular mechanism associated with IMiDs resistance in the three developed models suggests that efflux transporters of the ABC family could have an important role since, to P-gp and MRP1, there were more positive cells (P-gp: H929RL 52.7 ± 5.5 %; U-266RL 22.0 ± 2.0 % e U-266RP 24.3 ± 2.9 %; and MRP1: H929RL, 59.7 ± 0.6 %; U-266RL, 81.0 ± 2.0 % e U-266RP, 96.7 ± 0.6% ) than in sensitive cell lines (P-gp: NCI-H929 5.0 ± 1.4 %; U-266 5.5 ± 0.7 %; e MRP1: NCI-H929 51.0 ± 2.8 %; U-266 77.0 ± 4.2). Relative to gene and protein expression of these three transporters, there were no observed coherent differences between the resistant models to the same drug. Besides that, the gene expression results showed an increase in SLC2A1, CRBN, RELA, RELB, KEAP1 e MYC genes, and a decrease in gene expression of REL, NFE2L2, BCL2 and NQO1 in resistant cell lines. Relative to genomics alterations, the resistant cell lines exhibit the presence of amplification and deletions genes, and U-266RL present more alterations. In short, the resistant cell lines to LEN and POM do not present cross-resistance to BTZ. Besides that, the increase in efflux transporters at the surfacing of cells, namely P-gp and MRP1, can be related to the IMiDs resistance. Therefore, the modulation of these transporters can contribute to overcoming the IMiDs resistance. The rest of the results should be posteriorly verified through extensive molecular analysis

    Regulação da resposta ao tratamento com Taxanos em Organóides de Cancro da Mama Triplo Negativo

    Dissertação de Mestrado em Investigação Biomédica apresentada à Faculdade de MedicinaTriple Negative Breast Cancer (TNBC) is the most aggressive and invasive type of breast cancer. TNBC is defined by the absence of hormone receptors. Therefore, it cannot be treated with targeted therapy available for breast cancer, which is mostly hormone therapy. Thus, to develop targeted therapies, there is a dire need to understand the molecular mechanisms regulating TNBC’s metastatic behavior.During the metastatic cascade in cancer, the tumor cells undergo various changes in their behavior and in their interactions with the microenvironment. One of the processes regulating these alterations is the Endosomal Trafficking (ET). It is the highly dynamic character of ET that allows migration and invasion of surrounding tissue and distant organs. Although it has been shown that cancer cell invasion relies on ET, the underlying molecular mechanisms are still unknown. It has been shown that TNBC relies on endosomal sorting/recycling (ESR) to promote and sustain its invasiveness. Moreover, the Feline sarcoma-related non-receptor kinase (FER) has been identified as a key regulator of ESR and invasion in TNBC. Although its potential to be used as a biomarker for TNBC therapeutic strategies has been suggested, it has not yet been fully assessed. Taxanes, used for cancer therapy, affect the microtubule stabilization. Since endosomal recycling uses microtubules for cargo transport to the membrane, we hypothesize that TNBC cells upregulating microtubule-dependent processes, would be highly sensitive to taxane-based treatments. To test our hypothesis, we characterized the presence of FER in TNBC invasive cells and assessed the response of Patient Derived Xenograft Organoids (PDXOs) to taxanes. To do so, we characterized PDXOs invasive behaviour. In invasive PDXOs, we identified two different populations: a proliferative/non-invasive cell population, and a FER-enriched/non-proliferative/invasive one. Moreover, Rab4, a marker of endosomal recycling, was enriched in invasive cells. Then, we assessed the response of PDXOs to docetaxel treatment. Docetaxel led to a decrease in invasion with non-significant effects on proliferation, suggesting that taxane could be used to target the invasive cells specifically.By understanding the effect of FER protein levels in the response to taxanes, this may aid in patients’ stratification and reduction of the administrated taxane dosages, potentially contributing to a decrease in secondary effects.O Cancro da Mama Triplo-Negativo (TNBC) é o tipo mais agressivo e invasivo de cancro da mama. O TNBC é definido pela ausência de recetores hormonais, portanto, não pode ser tratado com terapias direcionadas disponíveis para o cancro da mama, que são principalmente terapias hormonais. Sendo assim, para desenvolver terapias direcionadas, existe uma grande necessidade de entender os mecanismos moleculares que regulam o comportamento metastático do TNBC.Durante a cascata metastática no cancro, as células tumorais sofrem várias mudanças no seu comportamento e nas suas interações com o microambiente. Um dos processos que regulam essas alterações é o Tráfico Endossómico (ET). É o caráter altamente dinâmico do ET que permite a migração e invasão de tecidos envolventes e órgãos distantes.Embora tenha sido demonstrado que a invasão de células cancerígenas depende do ET, os mecanismos moleculares subjacentes ainda são desconhecidos. Foi demonstrado que o TNBC depende da triagem/reciclagem endossómica (ESR) para promover e sustentar invasão. Além disso, a proteína quinase não recetora relacionada com o sarcoma felino (FER) foi identificada como um regulador chave da ESR e da invasão no TNBC. Embora o seu potencial como biomarcador para estratégias terapêuticas do TNBC tenha sido sugerido, ainda não foi totalmente avaliado.Taxanos, usados para a terapia contra o cancro, afetam a estabilização dos microtúbulos. Uma vez que a reciclagem endossómica utiliza microtúbulos para o transporte de proteínas para a membrana, nós hipotetizamos que as células do TNBC com processos dependentes de microtúbulos aumentados seriam altamente sensíveis aos tratamentos à base de taxanos. Para testar a nossa hipótese, caracterizamos a presença de FER nas células invasivas do TNBC e avaliamos a resposta de Organóides Derivados de Pacientes (PDXOs) aos taxanos. Para isso, caracterizamos o comportamento invasivo dos PDXOs. Nos PDXOs invasivos, identificamos duas populações diferentes: uma população de células proliferativas/não invasivas e outra com elevados níveis de FER/não proliferativa/invasiva. Além disso, Rab4, um marcador de reciclagem endossómica, estava enriquecido nas células invasivas. Em seguida, avaliamos a resposta dos PDXOs ao tratamento com Docetaxel. O Docetaxel levou a uma redução na invasão, com efeitos não significativos na proliferação, sugerindo que os taxanos poderiam ser usados para atingir especificamente as células invasivas.Compreender o efeito dos níveis de FER na resposta aos taxanos poderá ajudar na estratificação dos pacientes e na redução das doses administradas de taxanos, contribuindo potencialmente para a diminuição dos efeitos secundários

    Tese de Programa de Doutoramento em Direitos Humanos nas Sociedades Contemporâneas apresentada ao Instituto de Investigação InterdisciplinarOs objetivos desta tese são (1) desenvolver uma abordagem interdisciplinar sobre a procura de saneamento básico no contexto do Direito Humano ao Saneamento (DHS) e do Objetivo de Desenvolvimento Sustentável (ODS) 6 e (2) analisar como ferramentas das Tecnologias da Informação e Comunicação (TIC) e informação recolhida por cidadãos podem contribuir para medir e monitorizar o acesso/a procura de saneamento básico. Esta análise estende-se através da Cadeia de Valor do Saneamento, incluindo a contenção, a coleta, o transporte, o tratamento e a reutilização/descarga dos efluentes e das águas residuais. O DHS salienta que casas de banho seguras, acessíveis e higiénicas protegem e apoiam a dignidade humana e são essenciais para a saúde humana. Cerca de 1,7 mil milhões de pessoas no mundo não têm acesso a serviços sanitários seguros, enquanto 494 milhões praticam defecação a céu aberto (WHO, 2022). Patógenos de dejetos humanos contribuem para doenças diarreic as que matam cerca de 700 crianças menores que cinco anos todos os dias (WHO/UNICEF, 2021). A lacuna que este estudo aborda reside na utilização de ferramentas TIC para medir o acesso e a procura de saneamento, através de uma abordagem de direitos humanos que inclui Ciência Cidadã. A pergunta de investigação pretende analisar 'até que ponto podem as ferramentas TIC e as novas tendências em informação obtida através de crowdsourcing e de cidadãos monitorizar a procura de casas de banho e contribuir para a satisfação do DHS?'. Para responder a esta questão, esta tese inclui uma revisão bibliográfica, inquéritos a profissionais da área do desenvolvimento, entrevistas com profissionais que trabalham na área da água, saneamento e higiene (WASH - Water, Sanitation and Hygiene) e especialistas em informação, e o mapeamento de ferramentas digitais de recolha de informação. A matriz elaborada constitui um recurso importante para a recolha de informação sobre ativ idades WASH. Esta investigação contribui com conhecimento na interseção do DHS, das ferramentas TIC e da informação recolhida por cidadãos. Os anexos incluem dez áreas para investigação futura baseadas nos resultados deste estudo.The objectives of this thesis are to (1) develop an interdisciplinary framework to situate demand for sanitation in context of the Human Right to Sanitation (HRtS) and Sustainable Development Goal (SDG) 6 and (2) understand how Information Communication Technology (ICT) tools and citizen-led data can contribute to measuring and monitoring access to and demand for sanitation, which extends throughout the Sanitation Value Chain (SVC) to include the containment, emptying, transport, treatment and reuse/disposal of human waste and wastewater. The Human Right to Sanitation (HRtS) emphasises how safe, accessible, and hygienic toilets protect and support human dignity and are essential to human health. An estimated 1.7 billion people worldwide do not have safe sanitation services, whilst 494 million practice open defecation (WHO, 2022). Pathogens from human faeces contribute to diarrheal diseases that kill around 700 children aged under five every day (WHO/UNICEF, 2021). The gap in research this study addresses is the potential use of ICTs to measure sanitation access and demand, through human rights lenses that consider Citizen Science. The primary research question driving this study is ‘to what extent can ICT tools and new trends in crowdsourced and citizen-generated data monitor demand for toilets and contribute to the attainment of the HRtS?’ To answer this question, this thesis includes a literature review, a survey of development professionals, interviews with Water, Sanitation and Hygiene (WASH) professionals and data experts and a mapping of digital data tools. The resource matrix provides a resource for WASH-related data gathering. This research generates new knowledge at intersections of the HRtS, ICT tools and citizen-led data.     Impacto Metabólico da Proteína de Endereçamento Associada a GPCR 2 na Fisiopatologia da Obesidade

    Tese de Doutoramento em Biologia Experimental e Biomedicina apresentada ao Instituto de Investigação InterdisciplinarOs GPCRs, juntamente com proteínas G e β-arrestinas, têm sido implicados numa miríade de funções celulares. Compreendendo a maior família de genes do genoma humano, os GPCRs regulam a maioria dos processos metabólicos e são vistos como uma opção terapêutica potencial para o tratamento da obesidade e comorbidades associadas, incluindo diabetes tipo 2. No entanto, o papel das vias de sinalização do GPCR na fisiopatologia da obesidade e doenças relacionadas ainda requer maior elucidação.Neste estudo, exploramos o papel de GPRASP2, uma proteína de endereçamento de GPCR, no metabolismo e na obesidade. Esta proteína é altamente expressa no SNC e está envolvida na determinação do destino destes recetores via tráfego endossomal-lisossomal. Curiosamente, mostramos que ratinhos knockout constitutivos para Gprasp2 sob uma dieta padrão exibem características clássicas da síndrome metabólica (incluindo excesso de peso, aumento da adiposidade, danos no fígado e clareamento dos adipócitos castanhos), de forma dependente da idade. Estas características foram observadas juntamente com o aumento da atividade lipogénica e níveis elevados de colesterol circulante. Concomitantemente com estes resultados na periferia, a GPRASP2 modula os circuitos hipotalâmicos, conforme mostrado pela regulação negativa dos principais neuropeptídeos hipotalâmicos relacionados com a fome.As nossas observações, juntamente com o que se sabe até agora sobre o impacto que membros da família GPRASP têm nas funções comportamentais e cognitivas, através da sua ação sobre GPCRs específicos enriquecidos no SNC, destacam o potencial da Gprasp2 como um alvo poderoso no desenvolvimento de intervenções terapêuticas relevantes para uma infinidade de condições.GPCRs, together with G proteins, and β-arrestins, have been implicated in a wide variety of cellular functions. Comprising the largest gene family of the human genome, GPCRs regulate most metabolic processes and are seen as a potential therapeutic option for the treatment of obesity and associated comorbidities, including Type 2 diabetes. However, the role of GPCR signaling pathways in the pathophysiology of obesity and related diseases still requires further elucidation.In this study, we have explored the role of GPRASP2, a GPCR sorting protein, in metabolism and obesity. This protein is highly expressed in the CNS and is involved in determining the fate of these receptors via endosomal-lysosomal trafficking. Interestingly, we show that constitutive knockout mice for Gprasp2 under a standard chow diet display classic features of metabolic syndrome (including overweight, increase adiposity, liver damaged and brown adipocyte whitening), in an age-dependent manner. These features were observed together with enhanced lipogenic activity and higher circulating cholesterol levels.     Implementação de estágios enzimáticos no branqueamento ECF industrial de pasta kraft de eucalipto

    Tese de Programa Doutoral em Biorrefinarias apresentada à Faculdade de Ciências e TecnologiaA produção de papel de impressão e escrita (P&W) requer a disrupção da estrutura da matéria fibrosa da madeira (cozimento) para produzir pasta. Visto possuir cor castanha, a pasta deve ser sujeita a um processo de branqueamento, para depois ser introduzida no processo de produção de papel. Tendo em vista a produção de pasta com propriedades óticas e de resistência de elevada qualidade, desejadas em papel P&W, o cozimento kraft é o processo de produção de pasta mais comum, enquanto o branqueamento elemental chlorine-free (ECF) é o processo de branqueamento predominante No entanto, o uso de ClO2 (D) como principal agente de branqueamento deste processo gera compostos organoclorados (AOX). Devido a regulamentações ambientais, os limites legais para a emissão destes compostos por parte da indústria têm sido progressivamente reduzidos, o que levou à procura de tecnologias alternativas, que pudessem contribuir para a redução ou eliminação da necessidade de reagentes à base de cloro, nomeadamente a implementação de um estágio de pré-deslenhificação com oxigénio. Uma outra tecnologia emergente é o branqueamento assistido por enzimas (biobranqueamento) focado principalmente na utilização de xilanase e/ou lacase. Até hoje, apenas o tratamento com xilanase alcançou com sucesso aplicação comercial, tendo permitido reduzir a quantidade de reagentes derivados do cloro e, consequentemente, as emissões de AOX. O principal objetivo deste trabalho foi o desenvolvimento de uma sequência de branqueamento modificada, pela incorporação de pelo menos um estágio de tratamento enzimático (com xilanase, lacase ou ambas) numa sequência ECF OD0EPD1D2, aplicada numa instalação fabril portuguesa, onde é utilizado eucalipto como principal matéria-prima. O objetivo final era a redução de custos e do impacto ambiental do branqueamento. As modificações introduzidas deveriam, no mínimo, manter a qualidade do produto final (papel P&W). Três endo-xilanases, uma lacase e dois produtos com atividade, quer de xilanase, quer de lacase, foram estudados. Na fase inicial do estudo, foi usada pasta lavada em laboratório para avaliar o efeito de pH, temperatura e carga de enzima no desempenho do estágio enzimático, considerando as condições fabris e as características da pasta. Apesar de exclusivamente conduzido à escala laboratorial, a segunda parte do estudo, ao invés da maior parte dos trabalhos publicados abordando este tema, teve em conta o impacto de condições industriais típicas (condições operatórias e circuito de filtrados) no desempenho do estágio enzimático, bem como os efeitos da adição deste tratamento ao nível do processo industrial completo. Utilizaram-se, por isso, pastas recolhidas em fábrica, sem lavagem laboratorial adicional. Os resultados mostraram que o tratamento de pasta com lacase não se apresentou viável industrialmente. O estágio com xilanase mostrou-se, por outro lado, viável, se aplicado após o estágio O (sequência de branqueamento OXD0EPD1D2); contudo, nenhum benefício foi observado quando aplicado em pasta crua. A diferença observada no que respeita à eficácia da xilanase foi atribuída ao excesso de carryover e de pH na pasta crua. A sequência OXD0EPD1D2 otimizada permitiu reduzir as cargas de ClO2 e de NaOH nos estágios D0 e EP em 20 e 10 %, respetivamente, em comparação com a sequência de branqueamento convencional. Isto resultou na redução da quantidade de AOX no filtrado do estágio D0 até quase 30 %. Adicionalmente, as propriedades papeleiras da pasta branqueada não foram prejudicadas, e foram mesmo superiores em termos de estabilidade da brancura. No entanto, a ação da xilanase resultou na degradação e libertação de xilana da pasta, o que se traduziu em quebra de rendimento no branqueamento e em aumento da carência química de oxigénio (CQO) no filtrado do estágio enzimático, com consequências para a produtividade da fábrica e para o sistema de recuperação de energia. Por outro lado, o filtrado do tratamento enzimático mostrou ter valor comercial acrescentado, devido à libertação adicional de produtos secundários com atividade pré-biótica. O processamento desta corrente, através de um sistema de nanofiltração por membranas, resultou em concentração e purificação parcial desses compostos de valor acrescentado, apesar de perdas consideráveis terem aparentemente ocorrido. A comparação de diferentes produtos enzimáticos comerciais aplicados numa simulação rigorosa do processo industrial de branqueamento permitiu identificar diferenças significativas no que diz respeito aos mecanismos de ação de diferentes xilanases. A análise técnico-económica preliminar da tecnologia desenvolvida providenciou um resultado muito promissor. Considerando a fiabilidade dos resultados obtidos, bem como o grau de aproximação ao processo industrial, o passo seguinte para a validação desta tecnologia deveria ser um ensaio industrial.White printing and writing (P&W) paper production requires the destructuring of wood fibrous material (pulping), in order to produce pulp. Since it possesses dark brown colour, pulp must be subjected to a so-called bleaching process, in order to obtain bright pulp which can then be used in the paper manufacturing process. Concerning the production of pulp with very good optical and strength properties, desirable in white P&W paper, kraft cooking is the most common pulping process, whereas elemental chlorine-free (ECF) bleaching is the predominant pulp bleaching one. However, the use of ClO2 (D) as the main bleaching agent in this process generates organochlorinated compounds (AOX). Due to environmental regulations, the legal thresholds for the emissions of such compounds by the pulp industry have been progressively lowered, which drove the search for alternative bleaching technologies, that could contribute to reduce or eliminate the need for chlorine-based chemicals, namely the implementation of a pre-delignification stage with oxygen. Another emerging bleaching technology is enzyme-assisted bleaching (biobleaching), mainly focused on the use of xylanase and/or laccase. Until now, only the treatment with xylanase has found successful commercial application, having allowed to reduce the amount of chlorine-based chemicals and, consequently, AOX emissions. The main goal of this work was to develop a modified bleaching sequence by the incorporation of at least one enzymatic treatment stage (employing xylanase, laccase, or both) in an ECF-type OD0EPD1D2 sequence, applied at a portuguese mill that uses eucalypt wood as the main raw material. The final objective was the reduction of bleaching costs and environmental impact. The introduced modifications should be able to at least maintain the quality of the final product (white P&W paper). Three endo- xylanases, one laccase and two products exhibiting both xylanase and laccase activities were studied. In the initial part of the study, lab-washed pulp was used to evaluate the effect of pH, temperature and enzyme load on the enzymatic stage performance, taking into account mill conditions and pulp characteristics. Although exclusively conducted at the laboratory-scale, the second part of this study, unlike most published work on this subject, accounted for the impact of typical industrial conditions (operating conditions and filtrate circuit) on the enzymatic stage performance, as well as for the effects of the addition of such treatment on the overall industrial process. Pulps collected at the mill were thus used without further laboratory washing steps. Results showed that the laccase treatment of pulp was not viable at the mill. The xylanase stage was, on the other hand, shown to be viable, if applied following the oxygen (O) stage (OXD0EPD1D2 bleaching sequence); however, no benefits were observed when applied directly on unbleached pulp. The observed difference regarding xylanase efficacy was attributed to excess carryover and pH in unbleached pulp. The optimised OXD0EPD1D2 sequence allowed to reduce ClO2 and NaOH loads in the D0 and EP stages by 20 and 10 %, respectively, in comparison to the conventional bleaching sequence. This resulted in the reduction of the amount of AOX in the D0 stage filtrate by up to almost 30 %. Furthermore, the papermaking properties of the bleached pulp were not impaired and could even be improved in terms of brightness stability. On the other hand, xylanase action resulted in the degradation and release of pulp xylan, which translated into bleaching yield loss and chemical oxygen demand (COD) increase in the filtrate from the enzymatic stage, which might affect mill productivity, as well as the energy recovery system. On the other hand, the filtrate from the enzymatic stage showed added commercial value, due to additional release of by-products with pre-biotic activity. The processing of that stream through a membrane nanofiltration system resulted in partial concentration and purification of those added-value compounds, although substantial losses apparently occurred. The comparison of different commercial enzymatic products applied in an accurate simulation of the industrial bleaching process allowed to identify significant differences regarding the action mechanisms of different xylanases. The preliminary techno-economic analysis of the developed technology provided a very promising result.     Relatório de Estágio do Mestrado Integrado em Ciências Farmacêuticas apresentado à Faculdade de FarmáciaThis work consists of a SWOT analysis of the internship in community pharmacy, hospital pharmacy and also the monograph entitled “Vitamin D between recommendations and controversy”. The pharmacist plays a fundamental role as a public health agent, his contribution to guidance and advice in this area is extremely important. It should be noted that before resorting to the pharmacological approach, there is a wide area of intervention. Vitamin D has been studied for several years and its importance in regulating bone metabolism is well known. This vitamin is different from the others, as its synthesis occurs predominantly through exposure of the skin to solar radiation. The diet is a smaller or less effective source, but the consumption of foods rich in vitamin D is important. Moderate sun exposure and the intake of foods rich in vitamin D are recommended as a preventive measure for vitamin D deficiency. These are simple measures; however, the high prevalence of vitamin D deficiency worldwide is known. Although vitamin D has been extensively studied, it is still the subject of controversy. The analytical methodologies currently used to quantify vitamin D are unreliable, the definition of vitamin D remains controversial, and there are differences in recommendations between scientific organizations. Only with the existence of validated reference laboratory methods will it be viable to develop scientific studies, allowing new advances in the area, clarifying controversial issues, and putting an end to discrepancies between scientific organizations.Este trabalho consiste na análise SWOT do estágio em farmácia comunitária, farmácia hospitalar e ainda na monografia intitulada “Vitamina D entre as recomendações e a controvérsia”. O farmacêutico, desempenha um papel fundamental como agente de saúde pública, a sua contribuição na orientação e aconselhamento nesta área é de extrema importância, salienta-se que antes de recorrer à abordagem farmacológica, existe uma ampla área de intervenção. A vitamina D é alvo de estudos há vários anos e a sua importância na regulação do metabolismo ósseo é bem conhecida. Esta vitamina distingue-se das restantes, uma vez que a sua síntese ocorre predominantemente pela exposição da pele à radiação solar, a dieta constitui uma fonte minor ou menos efetiva, mas consumo de alimentos ricos em vitamina D é importante. Recomenda-se a exposição solar moderada e a ingestão de alimentos ricos em vitamina D como medida preventiva para a deficiência de vitamina D. Estas são medidas simples, no entanto, é conhecida a elevada prevalência de deficiência em vitamina D a nível mundial. Apesar de a vitamina D ser amplamente estudada é ainda alvo de controvérsia. As metodologias analíticas atualmente utilizadas para a quantificação da vitamina D são pouco fiáveis, a definição de vitamina D permanece controversa, e existem diferenças nas recomendações entre as organizações científicas. Apenas com a existência de métodos laboratoriais de referência validados será viável o desenvolvimento de estudos científicos, permitindo novos avanços na área, o esclarecimento das questões controversas e acabar com as discrepâncias entre as organizações científicas

    Relatório de Estágio do Mestrado Integrado em Ciências Farmacêuticas apresentado à Faculdade de FarmáciaO presente documento integra uma análise dos estágios realizados na Farmácia dos Olivais e no INFARMED, I.P. com base na metodologia SWOT (Strengths, Weaknesses, Opportunities, Threats), bem como a monografia “Unleashing the Future of Pharmaceutical Industry: Embracing the Revolution of Continuous Manufacturing”. A indústria farmacêutica, num paradigma de constante evolução, tem alicerçado o seu progresso na inovação. À medida que novas tecnologias despoletam e que se abrem novos horizontes, as tradicionais práticas da indústria são confrontadas com oportunidades promissoras. Na última década, os avanços substanciais na ciência e na engenharia abriram caminho para a integração gradual do continuous manufacturing (CM) na indústria. Estes avanços, juntamente com a adoção da abordagem Quality-by-Design e a evolução do process analytical technology, impulsionaram o desenvolvimento científico e regulamentar, constituindo as bases para o CM. Não obstante os reconhecidos benefícios do CM, relacionados com a qualidade e o custo de fabrico, vários desafios dificultaram a sua introdução generalizada na indústria. Esta dissertação visa explorar a evolução dinâmica do fabrico de medicamentos, delineando a trajetória dos métodos convencionais até à atual mudança de paradigma do CM na indústria farmacêutica. Ao analisar os seus princípios, implicações tecnológicas, panorama regulamentar e perspetivas futuras, esta monografia aborda a forma como o CM muda o atual paradigma da produção de medicamentos para uma produção com mais qualidade e eficiência.This document includes an analysis of the internships carried out in Regulatory Affairs and Community Pharmacy based on the SWOT methodology (Strengths, Weaknesses, Opportunities, Threats), as well as the monograph untitled “Unleashing the Future of Pharmaceutical Industry: Embracing the Revolution of Continuous Manufacturing”. In the ever-evolving pharmaceutical industry landscape, innovation has been a cornerstone for progress. As technology advances and new horizons emerge, the industry's traditional practices are met with transformative opportunities. In the past decade, substantial strides in science and engineering have cleared the path for the gradual integration of continuous pharmaceutical manufacturing (CM) within the industry. These advancements, coupled with the adoption of the Quality-by-Design (QbD) framework and the evolution of process analytical technology (PAT), have propelled both scientific and regulatory readiness for CM. Although the benefits related to the quality and cost of CM are widely recognized, several challenges hampered the widespread introduction of this manufacturing approach. This dissertation aims to explore the dynamic evolution of pharmaceutical production, charting the trajectory from conventional methods to the paradigm-shifting realm of CM in the pharmaceutical industry. By examining its principles, technological implications, regulatory landscape, and future prospects, this study sheds light on how CM reshapes drug production paradigms for enhanced efficiency and superior product quality
