    Biblicality of Early Medieval Canon Law through the Lens of Language Modeling

    Contains fulltext : 325625.pdf (Publisher’s version ) (Open Access)Using language modeling and the information-theoretic concept of perplexity, this study explores the influence of the Bible on early medieval canon law. We demonstrate that calculating perplexity of a corpus of canon law under a language model trained exclusively on Scripture can serve as a reliable proxy for accessing the overall linguistic similarity—stylistic, semantic, and syntactic—of canon law to the Bible. The paper presents the measured of various canon law texts, explores its chronological development, and delves into the linguistic and stylistic meaning of higher or lower by observing correlations between perplexity under the biblical language model and various linguistic features of canon law texts which can be extracted from rich morpho-styntactic annotation. We hypothesize that changes in the levels of , clearly observable across the chronological subdivisions of the corpus, suggest that the imitation of scriptural language may have been a deliberate strategy reflecting evolving views on the role and place of Scripture in legislation. Further research is needed to trace in more detail the connection between the authoritative status of canon law collections and the use of the Bible.20 p

    Self-reported Clinical Outcomes and Quality of Life in Agammaglobulinemia: the Importance of an Early Diagnosis.

    Contains fulltext : 322528.pdf (Publisher’s version ) (Open Access)PURPOSE: Patients with (X-linked) agammaglobulinemia (XLA) suffer from severe, recurrent infections potentially leading to life-threatening complications such as sepsis, meningoencephalitis and chronic lung disease. Early diagnosis and timely treatment can prevent infections and secondary complications, emphasizing a role for early detection of XLA via newborn screening (NBS). Our international multicenter survey study aimed to evaluate self-reported outcomes and parental perspectives in XLA patients to determine whether an early diagnosis is associated with better quality of life (QoL). METHODS: QoL-questionnaires included the PedsQL for children and SF-36, CVID_QOL, PADQOL-16 for adults. A new questionnaire was specifically developed for parents about an early diagnosis of XLA. RESULTS: In total, 88 adult and 65 pediatric XLA patients, and 69 parents from 14 countries completed the survey. Patients with an early diagnosis reported less severe, recurrent infections and less hospitalization (p < 0.05). QoL was significantly lower in multiple health domains for pediatric and adult patients with a late diagnosis compared to the general population. Patients with an early diagnosis reported similar QoL outcomes compared to the general population. Parents showed immense support for NBS for XLA stating that an early diagnosis prevents emotional insecurity, health damage, unnecessary diagnostics and allows early access to medical care and informed family planning. CONCLUSION: Our study has shown supportive evidence to pursue an early diagnosis of XLA from both a self-reported clinical, health related QoL and parental perspective. The main plea from patients and parents is to achieve an early diagnosis for XLA and severe B-lymphocyte deficiencies with NBS

    Contains fulltext : 324175.pdf (Publisher’s version ) (Open Access)Face-to-face communication relies on the seamless integration of multisensory signals, including voice, gaze, and head movements, to convey meaning effectively. This poses a fundamental computational challenge: optimally binding signals sharing the same communicative intention (e.g., looking at the addressee while speaking) and segregating unrelated signals (e.g., looking away while coughing), all within the rapid turn-taking dynamics of conversation. Critically, the computational mechanisms underlying this extraordinary feat remain largely unknown. Here, we cast face-to-face communication as a Bayesian Causal Inference problem to formally test whether prior expectations arbitrate between the integration and segregation of vocal and bodily signals. Specifically, we asked whether there is a stronger prior tendency to integrate audiovisual signals that convey the same communicative intention, thus establishing a crossmodal pragmatic correspondence. Additionally, we evaluated whether observers solve causal inference by adopting optimal Bayesian decision strategies or non-optimal approximate heuristics. In a spatial localization task, participants watched audiovisual clips of a speaker where the audio (voice) and the video (bodily cues) were sampled either from congruent positions or at increasing spatial disparities. Crucially, we manipulated the pragmatic correspondence of the signals: in a communicative condition, the speaker addressed the participant with their head, gaze and speech; in a non-communicative condition, the speaker kept the head down and produced a meaningless vocalization. We measured audiovisual integration through the ventriloquist effect, which quantifies how much the perceived audio position is misplaced towards the video position. Combining psychophysics with computational modelling, we show that observers solved audiovisual causal inference using non-optimal heuristics that nevertheless approximate optimal Bayesian inference with high accuracy. Remarkably, participants showed a stronger tendency to integrate vocal and bodily information when signals conveyed congruent communicative intent, suggesting that pragmatic correspondences enhance multisensory integration. Collectively, our findings provide novel and compelling evidence that face-to-face communication is shaped by deeply ingrained expectations about how multisensory signals should be structured and interpreted.27 p

    Probiotic Lactobacillus Species Modulate Immune Responses During Vaginal Epithelial Cell Colonization.

    Contains fulltext : 324103.pdf (Publisher’s version ) (Open Access)BACKGROUND: Vulvovaginal candidiasis (VVC), mainly caused by Candida albicans, affects approximately 75% of women at least once during their lifetime. VVC is characterized by fungal colonization, which leads to inappropriate local hyperinflammation and symptoms. Although the trigger of C. albicans pathogenicity is often unknown, antibiotic use and vaginal dysbiosis are associated with increased susceptibility to VVC. A healthy vaginal microbiota is normally dominated by Lactobacillus species, which are believed to keep C. albicans infections at bay. Probiotic lactobacilli are, therefore, explored to treat symptomatic VVC episodes. However, the influence of probiotic lactobacilli on immune responses in the context of VVC remains underexplored. METHODS: We investigated how probiotic lactobacilli influence vaginal epithelial and downstream inflammatory responses during C. albicans infection, using in vitro vaginal epithelial infection models and stimulating primary human immune cells with supernatants from these vaginal epithelial cells. RESULTS: Our study shows that although most of the tested probiotic lactobacilli reduced C. albicans-induced vaginal epithelial cell damage, some species, particularly Limosilactobacillus fermentum and Lacticaseibacillus paracasei, elicited proinflammatory responses even in the absence of C. albicans. Probiotic lactobacilli also differentially modulated the C. albicans killing efficiency and production of reactive oxygen species by neutrophils. CONCLUSIONS: Overall, vaginal epithelial and downstream immune responses during co-cultivation with C. albicans and probiotic lactobacilli were mostly driven by specific bacterial species and their interactions with the vaginal epithelium. Therefore, the induction of "controlled" inflammation by probiotic lactobacilli may be beneficial to improve neutrophil function; however, whether this alleviates immunopathology warrants further investigation

    Total self-BAT scores associated with choice of heavy menstrual bleeding treatment.

    Contains fulltext : 324474.pdf (Publisher’s version ) (Open Access)BACKGROUND: Heavy menstrual bleeding (HMB) is frequently the indicating symptom of an underlying bleeding disorder. The self-administered bleeding assessment tool (self-BAT) provides an overview of the bleeding history and indication of a bleeding tendency. Research about the association between a bleeding tendency and choice of HMB treatment is scarce. METHODS: The self-BAT was sent to all patients ≥ 18 and < 53 years old with systematic causes of HMB seeking care at a gynecological clinic. Closed questions on medical and surgical HMB management were provided, as well as a free-text question to reflect on personal experiences and opinions concerning HMB management. For clinical assessment of bleeding tendency, the cut-off for an abnormal self-BAT score was ≥ 5 in young women (18–30 years) or ≥ 6 for women of 31–52 years. RESULTS: Overall, 36% (234/650) of eligible women completed the questionnaire. The median total self-BAT score was 6 (interquartile range: 0–17). An abnormal total self-BAT score was more prevalent in women who underwent a hysterectomy compared to those who had not for women age 31–52 years (OR 4.3; 95%CI,1.2; 15.3; p = 0.02). Medical therapy (hormonal, tranexamic acid or iron) was more prescribed in women with an abnormal total self-BAT score. The most frequently mentioned responses from patients were related to low awareness of the effects of HMB and the need for patient empowerment. CONCLUSIONS: Findings imply that there is a substantial group of patients with HMB who experience a bleeding tendency. This bleeding tendency may influence the choice of HMB treatment like a hysterectomy. Structured bleeding assessment tools could guide optimal diagnostics and treatment for women with HMB
