    Oral healthcare access: self-perceived barriers faced during pregnancy - a systematic review.

Despite oral health care's importance, pregnant women underutilize these services due to various barriers. This systematic review aims to assess the self-reported barriers that hinder pregnant women's access to oral healthcare worldwide. METHODS: The PRISMA checklist was followed and PubMed, Web of Science, and Scopus were screened for articles. The barriers that prevent pregnant women from accessing oral healthcare were analysed based on three dimensions - availability, affordability, and acceptability. RESULTS: From a total of 942 articles, 31 were selected. The predominant barriers were the costs of dental care, lack of literacy, namely the misconception about dental care safety to the foetus, and limited oral health awareness. CONCLUSION: Addressing these barriers requires a comprehensive policy framework and resource allocation, integrating oral health into perinatal care, and raising awareness among non-dental healthcare professionals

    Multidimensional labeling of gesture in communication: The M3D proposal

Recent insights suggest that gestural behavior has a strong association with prosodic structure and that a single gesture can communicate various semantic and pragmatic meanings. This highlights the importance of developing a comprehensive, flexible, and transparent approach to gesture annotation that accounts for multiple dimensions of gesture, including a gesture's form, prosodic properties, and semantic and pragmatic contributions. To address this need for an increasingly dimensionalized approach to multimodal data annotation, the main goal of this paper is to present and describe a novel labeling system for manual gestures. The MultiModal MultiDimensional (M3D) system consists of an open access package that has been developed in coordination with five different labs working on gesture and its interaction with speech. The package includes a set of reliable annotation guidelines, a validated training program, and two annotated audiovisual corpora that represent over 60 minutes of lecture-style speech.23 p

    An Epidemiological Survey of Sepsis in a Tertiary Academic Hospital from Southwestern Romania.

This study is part of the FUSE (Functional Genomics in Severe Sepsis) project under the Human Functional Genomics Romania initiative. Our aim was to assess the epidemiology of sepsis in a tertiary academic hospital in southwestern Romania. Materials and methods: The study enrolled 184 patients with severe infections between May 2017 and November 2019, following the Sepsis-2 guidelines (SIRS criteria). Results: The present cohort of community-acquired severe infections shows respiratory and urinary tract as main sites of severe infection. The demographic and clinical characteristics of this Romanian study group are in line with those of other severe infection European cohorts. However, the predominance of confirmed Clostridium difficile cases represents a strong deviation, raising significant concerns for the communities to which the patients belong. Conclusions: Sepsis, with its complex pathophysiology and clinical presentation, remains one of the most daunting global health issues. In our cohort, the high number of Clostridium difficile cases prompts high vigilance and immediate intervention

    Uncovering the Advantages of Foam Dressings with Active Ingredients.

    Contains fulltext : 320528.pdf (Publisher’s version ) (Open Access)Background/Objectives: Foam dressings are designed for their ability to manage exudate and are selected to optimize wound repair. Various foam dressings are available, ranging from basic polyurethane to more sophisticated options, incorporating active components to combat infections or foster healing. This study investigates the requirements for the most suitable foam dressing through a combination of field research, laboratory testing, and clinical evaluation. Methods: We tested 17 foam dressings commonly used by wound care professionals while attending an international conference. An effective foam dressing should absorb wound fluid for several days, as wound care professionals value absorption and retention capacity, often favoring less frequent changing dressings, preferably twice a week or even weekly. Results: The foam dressings tested can absorb the expected amount of exudate typically produced by different wound types. There is some variability in retention capacity and product prices, resulting in differences in cost-effectiveness among products. In addition, some dressings are enriched with active ingredients that can accelerate healing through their antimicrobial and anti-inflammatory properties, such as foam dressings infused with silver or honey. A honey-based foam dressing was evaluated in a clinical survey involving eight wound care specialists, and four clinical cases with varying wound pathologies were discussed in more detail to highlight its key properties. Conclusions: Ideally, a foam dressing should have adequate absorption and retention capacities, effectively resolve and prevent infections, protect against external trauma, ensure optimal patient comfort without damaging newly formed granulation tissue, accelerate wound healing processes, and reduce wound care time (e.g., remaining in place for 7 days). Together, these factors make honey- or silver-loaded foam dressings more cost-effective than plain dressings due to their antimicrobial activities and ability to nourish tissues

    The effect of 12-weeks Nutritional supplementation on Nutritional Intake and Status among Indonesian Older Outpatients with Malnutrition Risk, the Prolansia study: a randomized controlled trial.

    Contains fulltext : 320547.pdf (Publisher’s version ) (Open Access)BACKGROUND: Standard care for older outpatients who are at risk of malnutrition in Indonesia is still based on the 2017's recommendations of the Indonesian Geriatrics Society. and does not provide nutritional supplementation as recommended by ESPEN guidelines 2019/2022. OBJECTIVE: We compared the effects of supplementation of at least 400 kcal/day including 30 g or more of protein/day as nutritional intervention as recommended by the ESPEN Guideline with standard care recommended by The Indonesian Geriatrics Society, in Indonesian older adults who are at risk of malnutrition. METHODS: Older outpatients (60 years or older) at a geriatric clinic of the national referral hospital with or at risk of malnutrition were recruited. They were randomly allocated to 12 weeks of supplementation with a nutrient dense drink twice a day on top of standard care compared to standard care only. We assessed energy and nutrient intake at baseline, after 6 and 12 weeks and nutritional status, physical performance, and vitamin D level at baseline and after 12 weeks. Data analyses were blinded. RESULTS: As many as 105 older outpatients (65 % women, mean age 72.5 years, SD = 6.3) were randomly assigned to the intervention (n = 54) and the control group (n = 51). One hundred and one participants completed the intervention, with an average compliance of 90% to the nutritional intervention. Nutritional supplementation significantly increased daily intake of energy, protein, total fat, vitamin D, vitamin B12, calcium (all p values <0.001) and carbohydrate (p = 0.002) in both men and women after 12 weeks. The intervention group showed an increase in vitamin D levels (p = 0.008). Furthermore, the intervention group gained more body weight than the control group did (p = 0.021)), especially in women (p = 0.017). Women in the intervention group also showed more increase in skeletal muscle mass (p = 0.023). Improvements in muscle strength and physical performance were not statistically different between the groups. No significant adverse effects were noted. CONCLUSION: Nutritional supplementation is effective in improving nutritional intake and status among Indonesian outpatients with malnutrition risk, which has the potential to support and enhance the standard of care.01 juni 202

    Impact of immunomodulating medication on maternal Tdap vaccination-induced antibodies in infants.

    Item does not contain fulltextBACKGROUND AND AIMS: Current international guidelines on inflammatory bowel disease (IBD) advise to continue immunomodulating medication during pregnancy. Data on the effect of this medication on maternal Tdap (Tetanus-Diphtheria-acellular pertussis) vaccination and transfer of antibodies to the infant are scarce. METHODS: Pregnant women with IBD receiving various immunomodulating medications and their infants were prospectively recruited in the Pregnancy Exposure to TNF alpha inhibitors and Immunological effect (PETIT) study cohort from 16 hospitals in the Netherlands between December 2018 and March 2023. All women were offered maternal Tdap vaccination according to the Dutch National Immunization Program. IgG concentrations against all Tdap components were measured at birth (maternal and cord blood) and at 2 months of age in the infant, preceding active immunization. We compared geometrical mean concentrations (GMCs) in IBD mother-infant pairs with healthy control mother-infant pairs. RESULTS: Geometrical mean concentrations of antibodies against all Tdap components were significantly higher in 135 maternally vaccinated mothers and their infants compared with 25 unvaccinated IBD women-infant pairs treated with immunomodulating medication during pregnancy, at all timepoints. However, GMC against pertactin (PRN) was significantly lower in vaccinated IBD mother-infant pairs compared to healthy vaccinated control mother-infant pairs, particularly in mothers treated with anti-tumor necrosis factor alpha and their infants. CONCLUSIONS: Maternal Tdap vaccination in women with IBD receiving immunomodulating medication during pregnancy results in significantly higher Tdap-antibody concentrations in their infants compared to infants of unvaccinated IBD mothers, although GMC was lower for PRN compared to healthy controls. These results support current recommendations to advise maternal Tdap vaccination in pregnant women receiving immunomodulating medication
